Brazilian mental health focuses on the social determination model within mental health policies and interventions to address mental and social suffering as resulting from social exclusion and structural violence. Structural concerns focused on class stratification and socioeconomic inequalities overweight cultural ones in Brazilian mental health. In this article we examine, first the role of culture and interculturality in mental health practices and mental health guidelines for health professionals continuing training in Brazil. Second, we explore interviews with health professionals around Continuing Education in Health provided by mental health collaborative care initiatives within primary health care as illustration of a local practice of structural competency in mental health. Finally, we examine the central role played by the practices of territorialisation in shaping health care training and care interventions. The focus on the territory can promote critical apprehensions of the intercultural and structural factors that shape health practices, enrich health professional education oriented towards the basic tenets of social medicine and provide global discussions around structural competency with innovative ways of thinking about health education, health care and spatiality. Objective: to analyze nurses' discourse about the potentialities in using information technologies as organizational support for the COVID-19 coping actions in Primary Health Care. Method: a qualitative and exploratory study conducted in the Family Health Strategy units from the city of João Pessoa, Paraíba, Brazil. Data collection was carried out from September to November 2021 with 26 nurses selected through the snowball technique, resorting to a semi-structured interview script. The empirical material was organized in the Atlas.ti 9 software and grounded on the theoretical- methodological contribution of Discourse Analysis, French Line. Results: three discursive blocks were evidenced: innovation based on social media; health education actions; resoluteness in organizational actions, presenting the relevance of the WhatsApp®, Instagram® and Facebook® apps as strategic resources, in order to collaborate in the Primary Health Care are with the organization of health actions against COVID-19 by nurses. Conclusion: health units have the potential to strengthen the assistance provided through digital organizational devices; however, they need political support that invests in the structure and strategies to enhance organization of the health actions. Objective: highlight health inequalities of homeless adolescents based on the conceptual framework of social determinants of health. Method: this is a convergent parallel mixed-methods study. The population consisted of adolescents who are homeless, with purposeful sampling and data saturation. Quantitative data were obtained with a questionnaire and qualitative data through semi-structured interviews. Results: 19 homeless adolescents participated in the study, 13 (68.4%) aged 16 to 19 years; 11 (57.9%) were male, 3 (15.8%) were female, and 5 (26.3%) were transgender adolescents. Participants experienced different levels of exposure and vulnerabilities to conditions that affect health and were directly related to intermediate determinants of health. Conclusion: this study provides an understanding of the inequalities related to the health of homeless adolescents and shows evidence that supports strategies to promote equity and dignity in health care. Objective: to understand the perspective of health professionals on the concepts of Clinical Economics and Patient Advocate. Method: a qualitative study with health professionals. Data were collected in June/2017 from the question: What is your perception of the concept of Clinical Economics and Patient Advocate? Qualitative data were analyzed using the Collective Subject Discourse. Results: the speeches pointed out that it deals with innovative, challenging and applicable proposals for nursing and health care. Patient Advocate and the Clinical Economics concept can support the decisions of the health team/service managers. The ideas can enhance the integration of the teams, resolve conflicts, bring together different professional categories and mitigate the risks of legal, financial and ethical responsibilities regarding health care. Violent extremism is commonly conditioned by a variety of psychological processes and mechanisms that when activated or deactivated aid implication in extreme behavior, including destructive actions with a large dose of cruelty against people and groups. One of those processes is moral disengagement, which was originally postulated by Bandura. To test this relationship, the present research focused on studying these mechanisms in members of Colombian illegal armed groups. Total sample size was 18 (14 males and four females) demobilized members of the Self-Defense Forces of Colombia (Autodefensas Unidas de Colombia [AUC]) and guerrilla organizations (Revolutionary Armed Forces of Colombia [FARC], National Liberation Army [ELN], among others), which had participated directly or indirectly in violent actions against people and groups, which included murders, tortures, and massacres. Qualitative methodology was used, specifically in-depth interviews and content analysis. This analysis led to the verification in the narratives of the participants of the use of all the mechanisms of moral disengagement described by Bandura aiming to justify their behavior within the armed group. The most noteworthy mechanisms were those that minimized participation (especially, attributing behavior to obeying orders: displacement of responsibility) and moral justification, especially, the context of confrontation. Moral disengagement processes are found in armed group members (such as insurgency, terrorist organizations, or militias). These mechanisms cancel ordinary psychological reactions of rejection, fear, and moral controls that oppose the carrying out of cruelty and extreme violence. [ABSTRACT FROM AUTHOR]

Objective: Describe the use of Yoga in breastfeeding support, from the perspective of nursing mothers. Methods: This is an exploratory, descriptive study with a qualitative approach, with ten lactating women, from the municipality of Niterói - RJ, in the period between May and September 2018, using the structured interview, workshop as data production tool Yoga and field diary. The data were submitted to discourse analysis in the thematic modality. Results: Three categories were identified: Yoga as emotional and physical support for mothers; The assistance of the health team, and the meanings of the experience during the parturition and breastfeeding process; and The mother-baby relationship as a basis for maternal well-being and continuity of breastfeeding. Conclusion: The convergence between Yoga and breastfeeding is identified, since the physiological and psychic processes are manifested in the health of the physical body, holistically interconnected. The senses and negative emotions, reported in the breastfeeding process were minimized with the use of yoga as a therapeutic resource. [ABSTRACT FROM AUTHOR]

Objective: to analyze how the social, historical, economic and professional elements influence the institutionalization process of cesarean childbirth in Brazil and France. Method: a qualitative study grounded on the theoretical framework of the institutional socioclinic. The data were produced through focus groups and individual interviews, supported by semi-structured scripts, with health professionals, mothers and fathers and a research diary. The study had 83 participants. The analysis was based on the theoretical framework and thematic analysis. Cross analysis was developed between the different types of data, services and groups, as well as between countries. Results: the participants were characterized in terms of schooling, employment, level of training and workplace. The countries researched have their own peculiar health and educational structure and are marked by a social-historical-economic construction outlined by patriarchy. The New Public Management influences both contexts and makes health care precarious. The technical and organizational dimensions reinforce an intellectual and social division of work and knowledge in health. Conclusion: the institutionalization process of childbirth in the countries is marked by social, economic and professional aspects. Objective. To understand the meaning of the nurse’s commitment to nursing care. Method. Phenomenological study, developed in seven Family Health Units in Salvador, Bahia, with twenty-one nurses, through the phenomenological interview. For the understanding of the discourses the Humanistic-Existential-Personalist Triad Configuration was used. Results. Nurses’ commitment has dimensions, their meaning represents the very essence of the profession, is directed to nursing care based on satisfaction, recognition, motivation, ethics and the search for a change in the health standards of the population. It was revealed in resource constraints in health services, in the limitations of professional competencies, in daily assignments, in the process of humanizing care, in solving user problems, in monitoring families and in linking with the community. Objective: to analyze the main challenges of nursing in facing Coronavirus Disease-19 under the perspective of nurse managers in the west macro-region of Santa Catarina. Method: it consists of a qualitative study, whose data collection was done through interviews with nurses who represent the management of health care network in the region. The analysis technique used was the Discourse of the Collective Subject (DCS). Results: the legacy of Florence Nightingale to contemporary nursing practice; the weaknesses and the technical operational capacity with which nursing faces in the Unified Health System (Sistema Único de Saúde - SUS); the strategies for strengthening the Unified Health System and qualification of nursing practices; and the potentialities identified in the pandemic scenario were the main ideas that emerged. In the bicentennial year of Florence Nightingale, nurses recognize her legacy to public health practice and management. Several variables interfere in professional practice, such as epidemiological aspects, working conditions, and care management in a pandemic. Conclusion: the pandemic scenario has taken nursing to a position of practical and scientific protagonism as a result of its proactivity and leadership in the search for knowledge based on scientific evidence. Introduction: This paper argues the role of Discourse Analysis - DA (Michel Pêcheux, Eni Orlandi) as a theoretical-methodological contribution to the evaluation of health campaigns. Objectives: It shows how DA can be applied to identify and understand discourses of resistance to health promotion campaigns. Methodology: It analyses the HIV prevention campaign of the Brazilian Ministry of Health designed for the carnival of 2010, as well as individual interviews with two people living with HIV. Results: By comparing the discourses mapped in the campaign and those present in the interviews, some dissonance of senses, understood as forms of resistance, are identified. Discussion: Such results also lead to the discussion regarding biopower issues (Michel Foucault) and the relationship between power and resistance in historical processes of subjectivation. Conclusion: It is concluded that, before trying with campaigns to change individual's practices towards behaviors that promote health, institutional practices must be changed in order to prioritize listening to people when designing such campaigns, by applying Discourse Analysis's theoretical-methodological contributions. Background. Occupational therapy practice is enhanced through clear understanding of its epistemological foundations. In the area of substance use, practices are strongly functionalist. Purpose. This study unearths epistemologies through analyzing practices addressing the social needs of clients with problematic substance use. Method. This study used an exploratory qualitative approach and was based on Marxist philosophies. Qualitative interviews were conducted with nine occupational therapists experienced in working with substance users. Data were analyzed through a dialectic approach to content analysis based on the Marxist theory of labour process. Findings. The occupational therapy practices were found to follow common principles: clients are agents of transformation in health processes; creativity is a strong element of practice and underlies the whole therapeutic process; and conditions of social reproduction compose the object of occupational therapy (i.e., what is transformed through occupational therapy practices). Two different approaches using these principles were uncovered according to the labour-process purposes: inclusive and emancipatory. Implications. In 2006, the committee that developed the National Health Policy for the Black Population (NHPBP) chose sickle cell disease as their “flag to demand health rights.” The drafting of this policy was official recognition from the Ministry of Health for racial differences of its citizens in order to address certain inequalities in the form of racial health reparations. Through an ethnographic study which consisted of participant observation, life-story and semi-structured interviews, and surveys in the urban centers of Rio de Janeiro, São Paulo, Salvador, Belo Horizonte, and Brasília between November 2013 and November 2014, I introduce a new conceptual approach called biocultural citizenship. It is a flexible mode of enacting belonging that varies depending on disease status, skin color, social class, recognition of African lineage, and other identifiers. Using empirical evidence, this article explores how people living with sickle cell disease (SCD), civil society, and the Brazilian government—at state and federal levels—have contributed to the discourse on SCD as a “black” disease, despite a prevailing cultural ideology of racial mixture. Specifically, I demonstrate that the SCD movement strategically uses Blackness to make claims for health rights. Biocultural citizenship is dependent on the idea of biological and cultural difference that is coproduced by the State and Afro-Brazilian citizens. The use of biology to help legitimate cultural claims, especially in the Black Atlantic, contributes a new and distinct way to think about how race and skin color are used as tools of agency for diasporic communities. Objective: to identify nurses' social representations about the relationship among prostitution, health and the Family Health Strategy. Method: qualitative study supported by the Theory of Social Representations, carried out with 12 nurses from the Family Health Strategy. Data collection used the Free Words Association technique and structured interviews. The data were organized using an association of the lexical content and the Collective Subject Discourse, presented in a figure and tables, analyzed in a descriptive and interpretative way. Results: the social representations of the nurses influence the actions and behaviors offered insofar as they are restricted to the sexual and reproductive sphere, to the detriment of the contextual aspects of everyday life, work and health. Conclusion: nurses' social representations link sexual practices to the risk of catching sexually transmitted infections and lacks the articulation of health promotion and the principle of integrality. Purpose: to present the opinion of professionals about street dwellers undergoing treatment of tuberculosis and identify strategies of control of tuberculosis in this population. Method: an exploratory and descriptive study involving 17 health professionals working in street clinics. A semi-structured study composed of closed questions and a guiding question. The statements were analyzed using the discourse analysis technique, resulting in the identification of two analytical categories: 1. Meanings attributed to street dwellers with tuberculosis, and 2. Control of tuberculosis in homeless people. Results: the analysis identified situations that limited adherence to tuberculosis treatment, including the reasons for staying in the streets, living conditions, and risk factors (dependence on alcohol and other drugs, short-sightedness, constant relocations, and lack of perspectives). Street dwellers were knowledgeable about the disease. Furthermore, there were difficulties in solving several problems of people living in the streets, including living conditions and lifestyle, social stigma, relocations, drug abuse, and lack of life project. Conclusion: coping with the complexity of situations related to living in the streets limits to the work of health professionals because these situations go beyond health care and require intersectoral actions. Objective: to investigate the planning pathways in the transfer of Directly Observed Treatment of tuberculosis. Method: a qualitative study conducted using interviews and a semi-structured guide, administered to five subjects who were among the coordinators and managers of the tuberculosis control programs, and the secretary of health of a municipality in the south of Brazil. Situational Strategic Planning and Discourse Analysis of the French matrix were the theoretical and analytical references used, respectively. Results: three reflexive axes were identified: weaknesses in the process of planning the Directly Observed Treatment transfer, antagonism between planning and daily requirements and formulation of planning and execution. Lack of systematization regarding the planning and execution for transfer the Directly Observed Treatment policy, demonstrates the fragility and incipience of this activity, and the possibility of its non-existence. Conclusion: the urgent need for managers and coordinators to better appropriate the theoretical framework for changing public policies, and the related planning mechanisms, includes a proposal for reorganization and qualification of the diffusion process, both practical-operative and political-organization. Objective: to know the historical process of the creation of the Midwifery Technical Chamber and its determinants; identifying the reasons that gave rise to the institutional support offered to Midwifery by the Technical Chamber of Minas Gerais Regional Council of Nursing (COREN-MG). Methodology: social-historic study, from secondary data and semi-structured interviews, which were analyzed based on discourse analysis. Results: midwifery Technical Chamber is a landmark in support to Midwifery; their actions provided sociopolitical advances in the field of autonomy. Objective: to understand how nurses understand the applicability of early intension directives in the care of the last phases of life. Methodology: qualitative, descriptive and exploratory research, carried out with eight nurses from a university hospital in the south of Brazil, in December 2014, based on semi-structured interviews, submitted to discursive textual analysis. Results: three categories emerged: conflicts in patient care in the final phase; Advantages and difficulties in the applicability of antecipated directives of intention; Benefits in the applicability of the antecipated intension directives by the nursing. Objective: describe the perception of workers of the Family Health Strategy teams in relation to teamwork. Methodology: a descriptive and qualitative research, performed between April and June 2014, through semi-structured interviews with 54 professionals working in 10 Family Health Strategy units in a city in southern Brazil. Data interpretation was by means of thematic category analysis. Results: from discourse analysis emerged categories: multi-professionality; sharing decisions and responsibilities; respect and appreciation of the work of others; division of duties, in which elements were identified that promote the two models that permeate the current professional practice, fragmented and focused on the disease and integral. Background: Since the publication of the reports by the Commission on Social Determinants of Health (CSDH), many research papers have documented inequities, explaining causal pathways in order to inform policy and programmatic decision-making. At the international level, the sustainable development goals (SDGs) reflect an attempt to bring together these themes and the complexities involved in defining a comprehensive development framework. However, to date, much less has been done to address the monitoring challenges, that is, how data generation, analysis and use are to become routine tasks. Objective: To test proposed indicators of social determinants of health (SDH), gender, equity, and human rights with respect to their relevance in tracking progress in universal health coverage and population health (level and distribution). Design: In an attempt to explore these monitoring challenges, indicators covering a wide range of social determinants were tested in four country case studies (Bangladesh, Brazil, South Africa, and Vietnam) for their technical feasibility, reliability, and validity, and their communicability and usefulness to policy-makers. Twelve thematic domains with 20 core indicators covering different aspects of equity, human rights, gender, and SDH were tested through a review of data sources, descriptive analyses, key informant interviews, and focus group discussions. To test the communicability and usefulness of the domains, domain narratives that explained the causal pathways were presented to policy-makers, managers, the media, and civil society leaders. Results: For most countries, monitoring is possible, as some data were available for most of the core indicators. However, a qualitative assessment showed that technical feasibility, reliability, and validity varied across indicators and countries. Producing understandable and useful information proved challenging, and particularly so in translating indicator definitions and data into meaningful lay and managerial narratives, and effectively communicating links to health and ways in which the information could improve decision-making. Conclusions: This exercise revealed that for monitoring to produce reliable data collection, analysis, and discourse, it will need to be adapted to each national context and institutionalised into national systems. This will require that capacities and resources for this and subsequent communication of results are increased across countries for both national and international monitoring, including the successful implementation of the SDGs. Background: Psychosocial care centers (CAPS) are a type of service that redirects care to people with mental health problems in Brazil, de-emphasizing the asylum model and promoting the psychosocial model. Aim: To understand and analyze how CAPS contribute to the possibilities of occupational engagement of users. Methods: A qualitative research methodology was used and semi-structured interviews were conducted with 17 users of CAPS and 12 people from their social networks, totaling 29 interviews. Discourse analysis was used as a reference for data analysis. Results: CAPS provide opportunities, and, as users engage in the activities offered by CAPS, they can find the types of occupations that they want to perform and what they like to do, as their range of opportunities and access are expanded. However, promoting these occupational opportunities and choices in the community remains a challenge. Conclusions: CAPS have been able to help users improve their occupations in everyday life. A descriptive qualitative research conducted with 20 drug addicts during treatment at a Center of Psychosocial Attention for Alcohol and other Drugs, aimed to identify intra and interpersonal determinants of relapse perceived by the drug addict. The data were collected through semi-structured interviews, submitted to Content Analysis, and organized into categories following predictive determinants for relapse. The relapse occurred by intrapersonal determinants, as self-efficacy expressed by self-confidence in interrupting the drug consumption; the result expectation by anticipation of pleasurable drug effects; the motivation by the absence of volition to interrupt the consumption; coping with the difficulty to confront daily problems; negative and positive emotional states; and craving. Interpersonal determinants expressed by social support were related to the influence of thirds. Objetivo: compreender os limites e as possibilidades avaliativas da Estratégia Saúde da Família no que tange ao reconhecimento de necessidades em saúde de mulheres que vivenciam violência. Método: trata-se de um estudo de abordagem qualitativa, fundamentado na perspectiva de gênero e que adotou necessidades em saúde como categoria analítica. Os dados foram coletados por entrevistas com profissionais de saúde e mulheres usuárias de um serviço de saúde e analisados a partir do método de análise do discurso. Resultados: o encontro entre os discursos de mulheres usuárias e de profissionais do serviço de saúde revelaram como interface necessidades propriamente humanas, a exemplo da autonomia e do vínculo. A compreensão sobre necessidades limitada ao reconhecimento de agravos de ordem física ou psíquica, assim como a predominância do reconhecimento de necessidades de manutenção da vida à frente das necessidades propriamente humanas revelaram-se nos discursos dos profissionais como importantes limitações das práticas. Conclusão: destaca-se a perspectiva de gênero enquanto ferramenta que deve ser agregada ao cotidiano das práticas profissionais em saúde para confirmar ou negar o caráter transformador da assistência em vigor no que concerne ao reconhecimento e enfrentamento das necessidades em saúde das mulheres. We aimed to detect the photographers' criteria of how to make the portrayal of male and female athletes and analyze their speech regarding the photos registration based on gender hierarchy. Participated ten sports photographers from different medias and both sexes, randomly selected. The instrument was a semi structured interview and as a technique, it was used the discourse analysis.
