The physiological and biochemical demands of intense exercise elicit both muscle-based and systemic responses. The main adaptations to endurance exercise include the correction of electrolyte imbalance, a decrease in glycogen storage and the increase of oxidative stress, intestinal permeability, muscle damage, and systemic inflammatory response. Adaptations to exercise might be influenced by the gut microbiota, which plays an important role in the production, storage, and expenditure of energy obtained from the diet as well as in inflammation, redox reactions, and hydration status. A systematic and comprehensive search of electronic databases, including MEDLINE, Scopus, ClinicalTrials.gov, ScienceDirect, Springer Link, and EMBASE was done. The search process was completed using the keywords: "endurance", "exercise", "immune response", "microbiota", "nutrition", and "probiotics". Reviewed literature supports the hypothesis that intestinal microbiota might be able to provide a measureable, effective marker of an athlete's immune function and that microbial composition analysis might also be sensitive enough to detect exercise-induced stress and metabolic disorders. The review also supports the hypothesis that modifying the microbiota through the use of probiotics could be an important therapeutic tool to improve athletes' overall general health, performance, and energy availability while controlling inflammation and redox levels. The present review provides a comprehensive overview of how gut microbiota may have a key role in controlling the oxidative stress and inflammatory responses as well as improving metabolism and energy expenditure during intense exercise. Les demandes fisiològiques i bioquímiques d'un exercici intens provoquen respostes musculars i sistèmiques. Les principals adaptacions a l'exercici de resistència inclouen la correcció del desequilibri electrolític, la disminució de l'emmagatzematge de glucogen i l'augment de l'estrès oxidatiu, la permeabilitat intestinal, el dany muscular i la resposta inflamatòria sistèmica. Les adaptacions a l'exercici poden estar influenciades per la microbiota intestinal, que té un paper important en la producció, emmagatzematge i despesa d'energia obtingudes de la dieta, així com en la inflamació, reaccions redox i estat d'hidratació. S'ha realitzat una cerca sistemàtica i exhaustiva de bases de dades electròniques, incloses MEDLINE, Scopus, ClinicalTrials.gov, ScienceDirect, Springer Link i EMBASE. El procés de cerca es va completar amb les paraules clau: "resistència", "exercici", "resposta immune", "microbiota", "nutrició" i "probióticos". La literatura revisada recolza la hipòtesi que la microbiota intestinal podria proporcionar un marcador mesurable i eficaç de la funció immune d'un atleta i que l'anàlisi de la composició microbiana també podria ser prou sensible per detectar l'estrès induït per l'exercici i els trastorns metabòlics. La revisió també recolza la hipòtesi que la modificació de la microbiota a través de l'ús de probiòtics podria ser una eina terapèutica important per millorar la salut general dels atletes, el rendiment i la disponibilitat d'energia mentre controlen els nivells d'inflamació i redox. La present revisió proporciona una visió general de com la microbiota intestinal pot tenir un paper clau en el control de l'estrès oxidatiu i les respostes inflamatòries, així com millorar el metabolisme i la despesa energètica durant un exercici intens. Las demandas fisiológicas y bioquímicas de un ejercicio intenso provocan respuestas musculares y sistémicas. Las principales adaptaciones al ejercicio de resistencia incluyen la corrección del desequilibrio electrolítico, la disminución del almacenamiento de glucógeno y el aumento del estrés oxidativo, la permeabilidad intestinal, el daño muscular y la respuesta inflamatoria sistémica. Las adaptaciones al ejercicio pueden estar influenciadas por la microbiota intestinal, que tiene un papel importante en la producción, almacenamiento y gasto de energía obtenidos de la dieta, así como en la inflamación, reacciones redox y estado de hidratación. Se ha realizado una búsqueda sistemática y exhaustiva de bases de datos electrónicas, incluidas MEDLINE, Scopus, ClinicalTrials.gov, ScienceDirect, Springer Link y EMBASE. El proceso de búsqueda se completó con las palabras clave: "resistencia", "ejercicio", "respuesta inmune", "microbiota", "nutrición" y "probióticos". La literatura revisada apoya la hipótesis de que la microbiota intestinal podría proporcionar un marcador medible y eficaz de la función inmune de un atleta y que el análisis de la composición microbiana también podría ser lo suficientemente sensible para detectar el estrés inducido por el ejercicio y los trastornos metabólicos. La revisión también apoya la hipótesis de que la modificación de la microbiota a través del uso de probióticos podría ser una herramienta terapéutica importante para mejorar la salud general de los atletas, el rendimiento y la disponibilidad de energía mientras controlan los niveles de inflamación y redox. La presente revisión proporciona una visión general de cómo la microbiota intestinal puede tener un papel clave en el control del estrés oxidativo y las respuestas inflamatorias, así como mejorar el metabolismo y el gasto energético durante un ejercicio intenso.

14. A Systematic Review of Validity and Reliability of Perceived Exertion Scales to Older Adults.

Rating of perceived exertion (RPE) scales have been used to monitor and to prescribe exercise intensity of older adults, although many scales have not determined the validity and reliability to this population. The aim of this study was to evaluate the cross-cultural adaptation, validity and reliability of RPE scales applied to older adults. We performed the systematic search of the articles in March 2020 and included Web of Science, Scopus, Psyclnfo, PubMed, Virtual Health Library and Science Direct databases. We limited the search to English language, with no year restriction. A modified McMaster (1998) critical appraisal tool determined the level of quality assessment in each included study. Forty-four papers met the inclusion criteria and there were 5 different RPE scales that had been used with older adults. Lower score in methodological quality was found to scale application (i.e., definition, instruction or familiarization) and there was no consensus about the instructions to use RPE scales. The Borg 6-20 was the more suitable tool for controlling exercise in older adults, due to its validation and cross-cultural adaptation for older adults. Thus, the results of this systematic review demonstrate the need for studies performing cross-cultural adaptation of other RPE scales to different languages and their validity and reliability under different exercise conditions when considering older adults' evaluations. 19. Identifying patterns of unhealthy diet and physical activity in four countries of the Americas: a latent class analysis.

Objectives. To determine clusters of individuals who present similar health behaviors in terms of diet, physical activity, and sedentarism, in four countries of the Americas: Brazil (2013), Chile (2009), Mexico (2012), and the United States of America (2013). This makes it possible to determine which of these behaviors occur simultaneously, as well as the demographic and sociodemographic characteristics associated with each cluster. Methods. The individual-level data analyzed were drawn from national health interviews and health examination surveys in Brazil, Chile, Mexico, and the United States, for different time periods. Using international physical activity guidelines and national dietary guidelines, the health behaviors of each individual were assessed. A latent class analysis was conducted to classify individuals into clusters based on these behaviors, and was followed by multinomial regressions to determine the characteristics of those in each class. Results. Overall, most individuals belonged to the classes characterized by average or unhealthy diets but sufficient amounts of physical activity. However, large differences exist across countries and population groups. Men with higher socioeconomic characteristics were globally more likely to belong to the least healthy class in each country. Conclusions. Findings from this analysis support the implementation of more refined policy actions to target specific unhealthy behaviors in different population groups, defined by gender, age group, socioeconomic status, and, to some extent, place of residence. Objective: The aim of this paper is to compare the state of Cardiac Rehabilitation Programs (CRP) in 2009 with 2015. Focus is directed on health care, training of health-providers, research, and the barriers to their implementation. Methods: All authors of RENAPREC-2009, and other cardiac rehabilitation leaders in Mexico were requested to participate. These centres were distributed into two groups: RENAPREC-2009 centres that participated in 2015, and the new CRP units. Results: In 2009 there were 14 centres, two of which disappeared and another two did not respond. CRP-units increased by 71% (n = 24), and their geographic distribution shows a centripetal pattern. The coverage of CRP-units was 0.02 centres per 100,000 inhabitants. Only 4.4% of eligible patients were referred to CRP, with a rate of 10.4 patients/100,000 inhabitants in 2015. The ratio of Clinical Cardiologists to Cardiac Rehabilitation Specialists was 94:1, and the ratio of Intervention Specialists to cardiac rehabilitation experts was 16:1. Cardiac rehabilitation activities and costs varied widely. Patient dropout rate in phase II was 12%. Several barriers were identified: financial crisis (83%), lack of skilled personnel (67%), deficient equipment (46%), inadequate areas (42%), and a reduced number of operating centres (38%). Conclusions: CRPs in Mexico are still in the process of maturing. Mexican CRP-centres have several strengths, like the quality of the education of the professionals and the multidisciplinary programs. The aim of this paper was to analyse the levels of personal and social responsibility and physical activity participation according to students’ gender and Education Stage, and to know the relationship between these two variables. The sample consisted on 344 students (152 boys y 192 girls), between 11 and 17 years old (M = 14.03; SD= 2.15), corresponding to Primary Education Stage (N = 174) and Secondary Education Stage (N = 170), who were administered the Personal and Social Responsibility Questionnaire and the Physician-based Assessment and Counseling for Exercise. The results showed significantly higher values of personal responsibility and physical activity in boys. Thus, primary stage students showed significant higher values on personal responsibility, helping others, and physical activity than secondary stage students. Finally, the practice of physical activity was positively and significantly associated with the personal and social responsibility of the students. These data can help teachers and coaches to promote values through physical activity.

La migraña es un tipo de cefalea primaria causada por la activación de un mecanismo encefálico que conlleva la liberación de sustancias inflamatorias, que acaba provocando dolores de cabeza. La alimentación y el ejercicio físico influyen en los síntomas provocados por la migraña. A partir de la evidencia científica encontrada se ha elaborado un protocolo de investigación que consiste en un ensayo clínico aleatorizado y que es complementario a dos estudios previos. La idea del estudio es comprobar cómo afecta la combinación de suplementos de vitamina D y distintos tipos de ejercicio en mujeres con normopeso de Lleida entre 18 y 50 años que padecen migrañas. Para la elaboración del protocolo se realizó una búsqueda bibliográfica en Pubmed, con el objetivo de investigar la relación entre migrañas y nutrición; suplementación; y ejercicio físico. Como resultado del análisis de datos bibliográficos se obtuvieron efectos beneficiosos en diversos grupos de población que padece de migraña tanto con ejercicio, nutrición y suplementación alimentaria. La recogida de datos se gestionará mediante un formulario en papel donde se recogerán tanto datos personales como datos necesarios para relacionar las diferentes variables, estos resultados serán trasladados a Excel para su análisis. Se espera obtener evidencia científica de este protocolo complementario a los dos estudios en los que se basa. L’alimentació i l’exercici físic influeixen en els símptomes provocats per la migranya. A partir de l’evidència científica trobada, s’ha elaborat un protocol d’investigació que consisteix en un assaig clínic aleatoritzat i que és complementari a dos estudis previs. La idea de l’estudi és comprovar com afecta la combinació de suplements de vitamina D i diferents tipus d’exercici en dones amb normopes de Lleida entre 18 i 50 anys que pateixen migranyes. Per l’elaboració del protocol es va realitzar una cerca bibliogràfica en Pubmed, amb l’objectiu d’investigar la relació entre migranyes i nutrició; suplementació; i exercici físic. Migraine is a type of primary headache caused by the activation of a brain mechanism that involves the release of inflammatory substances, which ends up causing headaches. Diet and physical exercise are strictly related to the symptoms caused by migraine. From the scientific evidence found an investigation protocol based on a randomized clinical trial that complements two previous studies has been elaborated. The idea of the research is to prove how vitamin D supplements combined with a different type of physical exercise affect women of a range of 18 to 50 years with normal weight from Lleida who suffer from migraine. To prepare the protocol, a bibliographic search was carried out in Pubmed, with the aim of investigating the relationship between migraines and nutrition; supplementation; and physical exercise. As a result of the analysis of bibliographic data, beneficial effects were obtained in various population groups suffering from migraine both with exercise, nutrition and food supplementation. Data collection will be managed through a paper form where both personal data and data necessary to relate the different variables will be collected. These results will be transferred to Excel for analysis. It is expected to obtain scientific evidence of this complementary protocol to the two studies on which it is based.

Resumen La sarcopenia se caracteriza por la pérdida de la masa muscular y la fuerza. El objetivo fue determinar el tamaño del efecto global del tratamiento con ejercicio y/o suplementación sobre la masa muscular de personas adultas mayores diagnosticadas con sarcopenia, así como identificar las variables que moderan el efecto del tratamiento. Con respecto al método, se realizó una búsqueda de literatura en las bases de datos EBSCOhost, Ovid, ProQuest, PubMed, ScienceDirect, SpringerLink, Embase. De 4770 estudios recuperados, se incluyeron los 14 que cumplieron los criterios de inclusión: analizar la masa muscular, emplear diseños experimentales, aportar la estadística descriptiva, administrar un programa de ejercicio, suplementación o combinado de estas intervenciones e incluir personas mayores de 60 años diagnosticadas con sarcopenia. Los estudios debían estar publicados en texto completo en inglés o español. El cálculo del tamaño de efecto global se siguió utilizando el modelo de efectos aleatorios. En los resultados, fueron analizados 818 sujetos pertenecientes al grupo experimental y 284 al grupo control. Se obtuvo un tamaño de efecto global de 0.16 (p=0.005), procedente de 38 tamaños de efecto. El gráfico de funnel plot y la prueba de Egger no evidenciaron la presencia de sesgo general, ni publicación. Se necesitan 23 estudios para que el tamaño de efecto global sea no significativo. En la conclusión, se evidencia un cambio significativo en la masa muscular de personas adultas mayores diagnosticadas con sarcopenia, utilizando el tipo de intervención de ejercicio contra-resistencia con o sin suplementación; asimismo, se establece una guía de recomendaciones sobre prescripción de ejercicio y suplementación. The Pilates Method (PM) is a method of physical and mental conditioning, which proposes movements involving several groups of muscle. PM may cause physiological and body changes for its practitioners. The aim was to identify the effects of PM in body composition of healthy adults through a systematic review of randomized studies. In order to find studies on the topic, systematic searches were conducted in six electronic databases between February and April, 2015. The terms used during the search were: Pilates, Pilates trainning, Pilates-based exercises, adultos, adults, composição corporal, body composition, somatotipo, somatotype, and combinations of such words. The methodological quality of the papers was assessed by PEDro scale. Three papers were selected for analysis by the end of the search, one of them had a good methodological quality (80%); and, the other two, a low methodological quality (50% and 40%). It was concluded that there is moderate scientific evidence about the effect of Pilates Method in body composition of healthy adults practitioners. It is suggested that more studies in this area should be done. El Método Pilates (MP) se caracteriza por ser un método de acondicionamiento físico y mental, que propone movimientos que involucran diferentes grupos musculares, los cuales pueden generar cambios fisiológicos y corporales en sus practicantes. El objetivo de este estudio fue identificar los efectos de PM en la composición corporal de sujetos adultos sanos a partir de una revisión sistemática de ensayos aleatorios. Para identificar los estudios que abordaron el tema, se realizaron búsquedas sistemáticas en seis bases de datos electrónicas entre febrero y abril de 2015. Los términos de búsqueda utilizados en las bases de datos fueron: Pilates, Pilates Training, Pilates-Based ejercicios, adultos, adultos, composición corporal, composición corporal, somatotipo, somatotipo, y las combinaciones entre dichas palabras. La calidad metodológica de los artículos se evaluó mediante la escala PEDro. Después de evaluar los criterios de inclusión y exclusión para este estudio, se seleccionaron tres artículos para el análisis, uno con buena calidad metodológica (80%); y, dos, una baja calidad metodológica (50% y 40%). Se concluye, al final de esta revisión sistemática, que existe evidencia moderada sobre el efecto de la PM en la composición corporal de adultos sanos. Se sugiere realizar más estudios en la zona. Il Metodo Pilates (MP) si caratterizza per essere un metodo di condizionamento fisico e mentale, che propone movimenti che coinvolgono diversi gruppi muscolari, che possono generare cambiamenti fisiologici e corporei nei suoi praticanti. Lo scopo di questo studio era di identificare gli effetti del PM sulla composizione corporea di soggetti adulti sani da una revisione sistematica di studi randomizzati. Per identificare gli studi che hanno affrontato l'argomento, tra febbraio e aprile 2015 sono state effettuate ricerche sistematiche in sei banche dati elettroniche. I termini di ricerca utilizzati nelle banche dati sono stati: Pilates, Pilates Training, Esercizi Pilates-Based, adulti, adulti, composizione corporea, composizione corporea, somatotipo, somatotipo e le combinazioni tra tali parole. La qualità metodologica degli articoli è stata valutata utilizzando la scala PEDro. Dopo aver valutato i criteri di inclusione ed esclusione per questo studio, sono stati selezionati tre articoli per l'analisi, uno di buona qualità metodologica (80%); e, due, una bassa qualità metodologica (50% e 40%). Si conclude, al termine di questa revisione sistematica, che vi sono prove moderate sull'effetto del PM sulla composizione corporea di adulti sani. Si suggerisce di effettuare ulteriori studi sul territorio. Fatigue, mood disturbances, under performance and gastrointestinal distress are common among athletes during training and competition. The psychosocial and physical demands during intense exercise can initiate a stress response activating the sympathetic-adrenomedullary and hypothalamus-pituitary-adrenal (HPA) axes, resulting in the release of stress and catabolic hormones, inflammatory cytokines and microbial molecules. The gut is home to trillions of microorganisms that have fundamental roles in many aspects of human biology, including metabolism, endocrine, neuronal and immune function. The gut microbiome and its influence on host behavior, intestinal barrier and immune function are believed to be a critical aspect of the brain-gut axis. Recent evidence in murine models shows that there is a high correlation between physical and emotional stress during exercise and changes in gastrointestinal microbiota composition. For instance, induced exercise-stress decreased cecal levels of Turicibacter spp and increased Ruminococcus gnavus, which have well defined roles in intestinal mucus degradation and immune function. Diet is known to dramatically modulate the composition of the gut microbiota. Due to the considerable complexity of stress responses in elite athletes (from leaky gut to increased catabolism and depression), defining standard diet regimes is difficult. However, some preliminary experimental data obtained from studies using probiotics and prebiotics studies show some interesting results, indicating that the microbiota acts like an endocrine organ (e.g. secreting serotonin, dopamine or other neurotransmitters) and may control the HPA axis in athletes. What is troubling is that dietary recommendations for elite athletes are primarily based on a low consumption of plant polysaccharides, which is associated with reduced microbiota diversity and functionality (e.g. less synthesis of byproducts such as short chain fatty acids and neurotransmitters). As more elite athletes suffer from psychological and gastrointestinal conditions that can be linked to the gut, targeting the microbiota therapeutically may need to be incorporated in athletes' diets that take into consideration dietary fiber as well as microbial taxa not currently present in athlete's gut. La fatiga, las alteraciones del estado de ánimo, el bajo rendimiento y el malestar gastrointestinal son comunes entre los atletas durante el entrenamiento y la competición. Las demandas psicosociales y físicas durante el ejercicio intenso pueden iniciar una respuesta al estrés activando los ejes simpático-adreno-medular e hipotálamo-hipófiso-suprarrenal (HPA), lo que resulta en la liberación de estrés y hormonas catabólicas, citoquinas inflamatorias y moléculas microbianas. El intestino es el hogar de trillones de microorganismos que tienen un papel fundamental en muchos aspectos de la biología humana, incluyendo el metabolismo, la función endocrina, neuronal e inmune. Se cree que el microbioma intestinal y su influencia en el comportamiento del huésped, la barrera intestinal y la función inmune son un aspecto crítico del eje cerebro-intestino. El presente trabajo reflexiona sobre la visión biológica del concepto energía que se aplica actualmente en las aulas a partir del análisis de diversos libros de texto, en papel y digitales. Se pretende analizar cuál es el concepto de energía que aparece en los textos y en los ejercicios del final de la unidad didáctica, así como identificar una posible relación entre la demanda cognitiva de los ejercicios de final de unidad y la manera de presentar el concepto de energía. Los resultados muestran una multiplicidad de ideas respecto al concepto de energía tanto en los textos como en los ejercicios analizados, siendo las más comunes las relacionadas con la idea de flujo y almacenaje, llegando a detectar diferentes ideas del concepto de energía dentro de una misma unidad didáctica. Respecto al análisis de la demanda cognitiva de los ejercicios de final de unidad, los resultados muestran un claro dominio de las actividades de menor demanda cognitiva, destacando el hecho de que mayoritariamente en ellos no se trabaja el concepto de energía sino que, por ejemplo, se limitan a solicitar al alumnado definiciones o la elaboración de listas de órganos y orgánulos. El present treball reflexiona sobre la visió biològica del concepte energia que s'aplica actualment a les aules a partir de l'anàlisi de diversos llibres de text, en paper i digitals. Es pretén analitzar quin és el concepte d'energia que apareix en els textos i en els exercicis del final de la unitat didàctica, així com identificar una possible relació entre la demanda cognitiva dels exercicis de final d'unitat i la manera de presentar el concepte d'energia. Els resultats mostren una multiplicitat d'idees respecte al concepte d'energia tant en els textos com en els exercicis analitzats, sent les més comunes les relacionades amb la idea de flux i magatzematge, arribant a detectar diferents idees del concepte d'energia dins d'una mateixa unitat didàctica. Respecte a l'anàlisi de la demanda cognitiva dels exercicis de final d'unitat, els resultats mostren un clar domini de les activitats de menor demanda cognitiva, destacant el fet que majoritàriament en ells no es treballa el concepte d'energia sinó que, per exemple, es limiten a sol·licitar a l'alumnat definicions o l'elaboració de llistes d'òrgans i orgànuls. This paper considers the biological approach of the concept of energy that is currently used in the classroom from the study of different textbooks and digital books. The purpose is to find out the energy concept set out in textbooks and in the end of unit exercises, and also identify a possible relationship between the cognitive demand of the end of unit exercises and the approach to the concept of energy. Results show a multiplicity of ideas about the concept of energy, not only in texts but also in exercises, being the ideas of flow and storage the most common ones and having also detected different ideas about the concept of energy within a teaching unit. Regarding the cognitive demand of the exercises, results show the prevalence of lower cognitive demand activities, highlighting the fact that most of them do not work the concept of energy at all but instead pupils are just asked, for example, to give definitions or to create a list of organs and organelles.

40. Métodos de entrenamiento de fuerza alternativos para incrementar la activación muscular en ejercicios de empuje

Introducción. Los movimientos en los que se empuja o presiona con los brazos a una resistencia externa constituyen un elemento clásico dentro de los programas de entrenamiento de la fuerza. Tradicionalmente se han empleado ejercicios convencionales con pesos libres y máquinas con los que se conseguía alcanzar intensidades de activación muscular altas. Sin embargo, tratando de incrementar en mayor medida la intensidad en este tipo de ejercicios, durante los últimos años se han comenzado a utilizar nuevos medios y métodos alternativos (por ejemplo utilizando dispositivos de suspensión, resistencia elástica o tratando de activar músculos de forma selectiva). No obstante, es escasa la evidencia científica que avala dichos recursos materiales y métodos alternativos para incrementar la activación muscular en extremidades superiores y tronco al compararlos con métodos más tradicionales. Los objetivos de la presente tesis fueron: 1) Comparar la activación muscular al realizar flexiones suspendidas con cuatro elementos de suspensión diferentes (Artículo I). 2) Evaluar la activación muscular durante diferentes variaciones de flexiones de brazos y comparar su activación con la del press de banca y el press polea en bipedestación, ambos realizados al 50%, 70% y 85% de 1 repetición máxima (RM) (Artículo II). 3) Evaluar la activación muscular en 6RM de press de banca y en 6RM de flexiones de brazos en el suelo realizadas con resistencia elástica añadida y a continuación evaluar las ganancias de fuerza tras utilizar un ejercicio u otro (Artículo III). 4) Evaluar si al focalizarse en el pectoral mayor o el tríceps braquial durante el press de banca se puede incrementar la activación de forma selectiva. Se presta especial atención a la relación entre la intensidad del ejercicio y la magnitud de la activación selectiva (Artículo IV). Métodos. Para investigar el primer objetivo de la tesis (Artículo I), 29 varones, estudiantes universitarios sanos participaron de manera voluntaria realizando 3 flexiones de brazos con cada uno de los 4 dispositivos de suspensión. La velocidad del ejercicio fue controlada utilizando un metrónomo y el orden de los ejercicios fue aleatorio. Los niveles de activación muscular fueron adquiridos utilizando electromiografía (EMG) en la porción larga del tríceps braquial, trapecio porción superior, porción anterior del deltoides, porción clavicular del pectoral mayor, recto anterior del abdomen, recto femoral y erector lumbar espinal. Para investigar el segundo objetivo (Artículo II), 29 varones, estudiantes universitarios y sanos participaron de manera voluntaria realizando 3 repeticiones en todas las condiciones bajo los mismos procedimientos estandarizados. Se analizó la señal de EMG de los músculos recto anterior del abdomen, oblícuo externo, porción esternocostal del pectoral mayor, porción anterior del deltoides, porción larga del tríceps braquial, trapecio porción superior, serrato anterior y la porción posterior del deltoides. Para abordar el tercer objetivo de la presente tesis (Artículo III), 30 estudiantes universitarios con experiencia de entrenamiento de fuerza participaron voluntariamente en un estudio de dos partes. Cada sujeto participó en 16 sesiones diferentes realizadas en el siguiente orden: dos sesiones de familiarización, evaluación de 1RM en el press de banca, dos test de 6RM con evaluación de EMG en la porción esternocostal del pectoral mayor y la porción anterior del deltoides, 10 sesiones de entrenamiento durante 5 semanas (usando las mismas cargas y variables que fueron usadas durante la evaluación EMG), medición de 1RM y 6RM en press de banca. Por último, un total de 18 adultos hombres con experiencia de entrenamiento de fuerza participaron voluntariamente para evaluar el cuarto objetivo de la tesis (Artículo IV). Los sujetos realizaron 3 repeticiones en 3 diferentes condiciones de press de banca (press de banca tradicional y press de banca tratando de utilizar de forma selectiva los músculos pectoral mayor o tríceps braquial) a intensidades del 20%, 40%, 50%, 60% y 80% de 1RM. Se evaluó la señal de EMG de los músculos mencionados. Resultados y conclusiones. En el Artículo I, las flexiones suspendidas con el dispositivo con polea generaron mayor actividad en el tríceps braquial, el trapecio porción superior, el recto femoral y el erector lumbar espinal que el resto de dispositivos de suspensión y que las flexiones tradicionales. El dispositivo con dos anclajes generó mayor activación del pectoral mayor que el resto de dispositivos y las flexiones tradicionales. Sin embargo, las flexiones tradicionales proporcionaron mayor activación muscular de la porción anterior del deltoides que el resto de dispositivos de suspensión, exceptuando el de dos anclajes. Todos los dispositivos de suspensión incrementaron de forma efectiva la activación muscular en el recto anterior del abdomen. En el Artículo II, las flexiones realizadas con resistencia elástica generaron similar activación muscular en los principales movilizadores que el press de banca realizado a alta intensidad, mientras que generaron mayor activación en la musculatura abdominal. Las flexiones suspendidas fueron muy efectivas para estimular la musculatura abdominal. La activación del pectoral mayor, porción anterior del deltoides y serrato anterior fue mayor durante condiciones más estables, mientras que los músculos del abdomen, el tríceps braquial y la porción posterior del deltoides se activaron más durante los ejercicios realizados en situación de inestabilidad. En el Artículo III, 6RM en press de banca y 6RM en flexiones con resistencia elástica generaron comparables valores de EMG durante la sesión de evaluación y también provocaron ganancias de fuerza similares después del periodo de intervención, mientras que el grupo control permaneció igual. Así, cuando los niveles de activación muscular son comparables y los ejercicios de empuje sean biomecánicamente y cinemáticamente similares, las ganancias de fuerza serán similares. En el Artículo IV, sujetos con experiencia de entrenamiento de fuerza fueron capaces de incrementar la activación del tríceps braquial o el pectoral mayor cuando se focalizaban en uno u otro músculo durante el press de banca realizado hasta intensidades del 60% de 1RM. Parece existir un umbral entre 60% y 80% de 1RM. En conclusión, existen cantidad de alternativas a los métodos convencionales para alcanzar niveles altos de actividad muscular (y por tanto ganancias de fuerza). Ello proporciona un mayor grado de variación en ejercicios para incrementar la fuerza muscular de lo que se pensaba. Introduction. Pushing or pressing movements with the arms against external resistance are classic elements of strength training programs. Traditionally, conventional strength exercises using free weights and machines have been used for achieving high intensities of muscle activity. However, new ways and alternative training methods (e.g. by using suspension training devices, elastic resistance or trying to selectively activate muscles) have increased during recent years. Nevertheless, scientific evidence supporting the use of such training methods and devices to achieve high intensities of upper-body muscle activity is scarce. The main purposes of this PhD thesis were: 1. To compare muscle activity during suspended push-ups using different suspension training systems (Paper I). 2. To compare muscle activity during push-up variations with the bench press exercise and the standing cable press exercise both performed at 50%, 70%, and 85% of the 1-repetition maximum (Paper II). 3. To compare muscle activity during 6 repetition maximum (6RM) bench press and added-resistance push-up, and subsequently to evaluate the strength gains after a training period with each of these exercises (Paper III). 4. To evaluate whether focusing on using the pectoralis major and triceps brachii muscles, respectively, during bench press can selectively activate these muscles. We were especially interested in measuring the relationship between exercise intensity and the magnitude of selective activation (Paper IV). Methods. To investigate the first purpose of the thesis (Paper I), 29 fit male university students voluntarily participated performing 3 push-ups each with 4 different suspension systems. Push-up speed was controlled using a metronome. Testing order was randomized. The level of muscle activity was recorded using electromyography (EMG) for the triceps brachii, upper trapezius, anterior deltoid, clavicular pectoralis, rectus abdominis, rectus femoris, and lumbar erector spinae. To investigate the second purpose (Paper II), 29 fit male students voluntarily participated performing 3 repetitions in all conditions under the same standardized procedures. EMG signals were recorded for the rectus abdominis, external oblique, sternocostal head of the pectoralis major, anterior deltoid, long head of the triceps brachii, upper trapezius, anterior serratus and posterior deltoid. To address the third aim of the present thesis (Paper III), 30 university students with resistance training experience voluntarily participated in a two-part study. Each participant took part in 16 sessions in the following order: two familiarization sessions, a 1RM bench press test session, two 6RM tests with EMG data collection on the sternocostal head of the pectoralis major and anterior deltoid, 10 training sessions during 5 weeks (using the same loads and variables that were used during the EMG data collection), a post-1RM bench press estimation session, and lastly a post-6RM bench press estimation session. Finally, to evaluate the fourth purpose of this thesis, 18 resistance-trained adult men voluntarily participated. Subjects performed 3 repetitions on 3 different bench press conditions with intensities of 20%, 40%, 50%, 60% and 80% of 1RM: regular bench press, and bench press focusing on selectively using the pectoralis major and triceps brachii, respectively. EMG signals were recorded for the triceps brachii and pectoralis major muscles. Results and conclusions. In Paper I, the suspended push-up with a pulley system provided greater triceps brachii, upper trapezius, rectus femoris and lumbar erector spinae muscle activity compared with the other suspension systems and the standard push-up. The twoanchor system provided greater pectoralis major activity than the other systems and the standard push-up. However, the standard push-up provided greater anterior deltoid activity than the other suspension systems except the two-anchor system. All the suspension systems effectively enhanced rectus abdominis activity.In Paper II, elastic-resisted push-ups induced similar levels of muscle activity the prime movers as the bench press performed at high load while also providing a greater core activity. Suspended push-ups were highly effective to induce high levels of abdominal muscle activity. Muscle activity of the pectoralis major, anterior deltoid and anterior serratus were higher during more stable pushing conditions, whereas abdominal muscles, triceps brachii, posterior deltoid and upper trapezius were more active during the unstable exercises. In Paper III, the 6RM bench press and the 6RM elastic-resisted push-ups induced comparable levels of muscle activity as assessed by EMG during the baseline session, and provided comparable muscle strength gains after the intervention period, whereas control group remained unchanged. Thus, when the levels of muscle activity are comparable and the kinematics and biomechanics of the push exercises are broadly similar, the gain in muscle strength will be similar. In Paper IV, resistance-trained participants were able to increase triceps brachii or pectoralis major muscle activity during the bench press when focusing on using either of these specific muscle at intensities up to 60% of 1RM. A threshold between 60% and 80% where selective activity no longer was possible appeared to exist. In conclusion, several alternatives to conventional methods for reaching high levels of muscle activity – and thus strength gains – appear to exist. This provides a larger degree of variability for exercises to increase muscle strength than previously thought.

El presente trabajo analiza la definición de la categoría posición socioeconómica (PSE) y las variables con las cuales se representa en los productos académicos del campo de la actividad física, además de su relación con la categoría de imagen corporal. Para lograr el objetivo, se rastrean elementos que permiten dar cuenta si los documentos de investigación se abordan desde alguno de los dos contextos: determinantes (DDSS) o determinación social de la salud (DSS). Se inicia con un rastreo global por medio de los motores de búsqueda, las bases de datos y los repositorios institucionales. Posteriormente se parametriza la ruta, desde las categorías imagen corporal (IC) y PSE. Las investigaciones pretenden dar cuenta de la evaluación a 15 años del programa "Salud para Todos" de la ONU de 2001, en el marco de los Objetivos Del Milenio. Se revisaron resúmenes de los productos, descartando aquellos donde la categoría PSE o sus descriptores asociados tuvieran un papel secundario. Se limitó a Latinoamérica y España por su tradición histórica colonizadora; con el ánimo de conocer la postura de esta comunidad frente al proceso globalizado de la salud en el mundo. Al grupo final se le aplican criterios parametrizados a partir de la revisión teórica, para responder los interrogantes basados en las implicaciones que tiene la PSE en el pensamiento actual de la producción científica en el campo de la actividad física; y cómo las otras categorías de análisis se ven o no manifiestas. El índice de calidad científica CASPe, determina la pertinencia de los textos. En el aspecto teórico, se encuentra que la categoría PSE, a pesar de ser muy utilizada, tiene una conceptualización difusa. Por tal motivo, se propone una definición de PSE sustentada en el pensamiento sociológico. En el aspecto empírico, al rastrear las variables con que se reemplaza la PSE en las investigaciones, se encuentran grandes diferencias y el uso de múltiples y disímiles subcategorías. This paper analyzes the definition of the category socioeconomic status (PSE) and the variables which is represented in academic products in the field of physical activity in addition to its relationship with body image category. determinants (DDSS) or social determinants of health (DSS): To achieve the objective, elements that allow account if research papers are addressed from one of the two contexts are tracked. It starts with a global tracking through search engines, databases and institutional repositories. Then the path is parameterized, from body image categories (IC) and PSE. The research aims to account for the assessment to 15 years of Health for All program 2001 UN within the framework of the Millennium Development Goals. summaries of products were reviewed, discarding those where the PSE category or its associated descriptors have a secondary role. The scientific quality index CASPe, determines the relevance of the texts. On the theoretical side, we find that PSE category, despite being widely used, has a fuzzy conceptualization. Therefore, a definition of PSE supported by the sociological thought is proposed. Antecedents i objectius: La malaltia pulmonar obstructiva crònica (MPOC) és una malaltia respiratòria de les vies aèries inferiors que es caracteritza per la presència de limitació al flux aeri poc reversible. Com a conseqüència de la malaltia els pacients amb MPOC presenten una reducció de la seva capacitat d’exercici. Així mateix, sovint s’observa una pèrdua progressiva de la capacitat d’exercici al llarg del temps, tal i com es descriu a la teoria del cercle viciós de la dispnea-inactivitat en la MPOC. És sabut que la pèrdua longitudinal de capacitat d’exercici és un important factor predictiu de mortalitat en aquets pacients. Per aquest motiu, és important identificar factors de risc associats amb aquesta pèrdua. L’objectiu d’aquesta tesi és aprofundir en el coneixement sobre els determinats i els efectes de la pèrdua de capacitat d’exercici en els pacients amb MPOC. Mètodes: La recerca portada a terme en el marc d’aquesta tesi doctoral es basa en el projecte “Caracterització fenotípica i evolució de la MPOC (Estudi PAC-MPOC)”. Es van recollir variables clíniques i funcionals en un total de 342 pacients amb MPOC en fase estable. La distància caminada a la prova de marxa de 6 minuts (6MWD) es va mesurar en el moment basal i 18-24 mesos més tard. La informació relacionada amb els ingressos hospitalaris esdevinguts durant el seguiment es va obtenir de registres centralitzats. Es van portar a terme anàlisis de regressió lineal múltiple per tal d’estudiar els factors predictius de la pèrdua en la 6MWD. Així mateix, es va portar a terme una revisió sistemàtica de la literatura per tal d’identificar models conceptuals per al cercle viciós de la dispnea-inactivitat a la MPOC i es van utilitzar models estructurals per tal de validar-los amb dades longitudinals de l’estudi PAC-MPOC. Resultats: Vam observar que el quocient entre la capacitat inspiratòria i la capacitat pulmonar total (IC/TLC) i la dispnea eren les variables basals que predeien la pèrdua de capacitat d’exercici a la nostra cohort; i que els ingressos hospitalaris que esdevenien durant el seguiment s’associaven amb una major pèrdua de capacitat d’exercici. D’altra banda, vam identificar 9 diagrames que expliquen el cercle viciós de la dispnea-inactivitat a la MPOC, però cap d’ells semblava ser vàlid a partir de les nostres dades. Proposem un model alternatiu que té present la informació esmentada en models conceptuals previs, contempla el paper central de la limitació en la capacitat d’exercici i presenta una adequada bondat d’ajust. Conclusions: A partir de la recerca portada a terme a aquesta tesi doctoral, es conclou que la hiperinsuflació pulmonar estàtica, la dispnea i els ingressos hospitalaris són importants factors predictius de la pèrdua de capacitat d’exercici a pacients amb MPOC. A més, la limitació de la capacitat d’exercici sembla que té un paper important en el cercle viciós de la dispnea-inactivitat descrit a la MPOC. Aquests resultats poden ser d’interès a l’hora de guiar intervencions terapèutiques, com la rehabilitació pulmonar, destinades a prevenir l’empitjorament de la capacitat d’exercici i frenar el cercle viciós de la malaltia en els pacients amb MPOC., Background and objective: Chronic obstructive pulmonary disease (COPD) is characterised by poorly reversible airflow limitation and reduced exercise capacity. Moreover, exercise capacity is a strong predictor of mortality and it often declines over time, as theoretically described in the vicious circle of dyspnoea-inactivity. Therefore, it is important to identify risk factors for exercise capacity decline in COPD. The objective of this thesis is to understand the determinants and effects of exercise capacity decline in patients with COPD. Methods: The research conducted in the frame of this thesis is based on the population of the Phenotype and Course of COPD (PAC-COPD) study cohort. Clinical and functional variables were collected for 342 clinically stable COPD patients. The 6-minute walk distance (6MWD) was determined at baseline and at a second visit 18 to 24 months later. Information on hospitalisations during follow-up was obtained from centralised administrative databases. Multivariate linear regression was used to assess factors related to 6MWD decline. We also conducted a systematic review to identify previously published conceptual models for the dyspnoea-inactivity vicious circle in COPD and used structural equation models to validate them with longitudinal PAC-COPD data. Results: We observed that inspiratory capacity-to-total lung capacity (IC/TLC) ratio and dyspnoea were the baseline variables that predicted exercise capacity decline in our COPD cohort; and that all-cause hospital admissions during follow-up, particularly if frequent, were significantly related to a higher decline in exercise capacity. None of the 9 diagrams identified in the literature explaining the dyspnoea-inactivity vicious circle in COPD were supported by our data. We propose an alternative model, taking into account previous models and evidence from the literature. This model, with a central role of exercise capacity limitation, provides significant associations among variables and appropriate goodness of fit when validated with repeated PAC-COPD data. Conclusions: From the research conducted in the present thesis, we conclude that static lung hyperinflation, dyspnoea and hospital admissions are important predictors of exercise capacity decline in COPD patients. Exercise capacity limitation has a central role in the dyspnoea-inactivity vicious circle proposed in COPD. These results may help to guide early therapeutic interventions, such as pulmonary rehabilitation, to prevent exercise capacity deterioration and interrupt the disease vicious circle in COPD patients.

Este estudio pretende analizar la literatura científca publicada referente a los efectos cognitivos de la actividad física en personas con enfermedad de Alzheimer (EA). Se buscaron artículos que trataran principalmente los efectos cognitivos de la actividad física en personas con EA, encontrando fnalmente 35 artículos. En cuanto a la evolución temporal, se observa una tendencia creciente de publicaciones con el paso de los años. Cabe destacar la gran variedad y heterogeneidad de autores principales y revistas. Geográfcamente, los países con mayor nú- mero de publicaciones son Estados Unidos y Brasil, destacando sobre el resto. Teniendo en cuenta estos resultados, podemos concluir que el estudio de los efectos en aspectos cognitivos y propios de la enfermedad propiciados por la actividad física es un campo de estudio relativamente nuevo en el que debemos profundizar Abstract: Tis study pretend to analyse the scientifc literature published regarding cognitive effects of physical activity on people with alzheimer`s disease (AD). Papers which treat mainly the cognitive efects of physical activity on people with AD were searched for, fnally founding 35 papers. Concerning the temporal evolution, a growing trend of publications is observed. It is noteworthy the variety and heterogeneity of main authors and journals. Geographically, the countries with more publications are United States and Brazil towering above the rest. Taking these results into account, we can conclude the study of the efects produced on by physical activity is a feld relatively new we must go deeply into it

Wheat-dependent exercise-induced anaphylaxis (WDEIA) is increasing. In vitro test such as ω-5-gliadin levels are useful in the diagnosis, while oral single blind challenge tests (OCT) with wheat plus exercise continuous being the gold standard diagnostic method. This paper reports the case of a 38-year-old woman, with several episodes of anaphylaxis after eating different foods and doing exercise after ingestion. An allergy study was performed with positive skin prick tests for wheat, barley and rye. Total IgE 238.0KU/L, positive specific IgE (>100KU/L) to wheat, barley and rye, and negative to rTri-a-19 ω-5 gliadin. OCT with bread and exercise was positive. In this case of wheat-dependent exercise-induced anaphylaxis (WDEIA) with negative serum specific IgE to ω-5 gliadin, negative results with γ, α, β y ω-gliadin doesn’t exclude the diagnosis of WDEIA.
