Aim: In the literature, there is evidence suggesting an association between substance use and psychosis. However, little is known about substance use in those who may be in the pre-psychotic phase, that is, those who are putatively prodromal are considered to be at clinical high risk (CHR) of developing psychosis., Methods: We conducted a review of publications measuring patterns and rates of substance use in CHR for psychosis individuals and the effects on the transition to psychosis., Results: Of 5527 potentially relevant research papers, 10 met inclusion criteria of CHR subjects and specifically mentioned substance use in the sample. The results of these studies varied. Cannabis, alcohol and tobacco/nicotine were reported as the most commonly used substances. There was limited information on the changes in patterns of use over time. Two out of the ten studies found a significant association between the use of substances and subsequent transition to psychosis. In one of these studies, substance abuse was a predictor of psychosis when included as a variable in a prediction algorithm. In the other study, the abuse of cannabis and nicotine was associated with transition to psychosis., Conclusions: We found limited evidence to suggest that increased rates of substance use may be associated with transition to psychosis. Unlabelled: Oppositional defiant disorder (ODD) or conduct disorder (CD) occurs when children's disruptive and antisocial behaviours start to interfere with their academic, emotional and/or social development. Recently, there has been a considerable investment to implement national school-based early intervention programs to help prevent the onset of ODD/CD., Aim: This paper describes the delivery of the Royal Children's Hospital, Child and Adolescent Mental Health Service and Schools Early Action Program: a whole school, multi-level, multidisciplinary approach to address emerging ODD/CD and pre- versus post-delivery assessment in 40 schools over a 4-year period (2007-2010)., Methods: All children from preparatory to grade 3 (ages 4-10 years) were screened for conduct problems (n = 8546) using the Strengths and Difficulties Questionnaire. Universal, targeted and indicated interventions were delivered in school settings. In total, 304 children participated in the targeted group program where the Child Behaviour Checklist was used as a pre- and post-intervention measure. Cohen's d effect sizes and a reliability change index were calculated to determine clinical significance., Results: Significant reductions in both parent- and teacher-reported internalizing and externalizing symptoms were noted. Aims: Historically, the Australian health system has failed to meet the needs of young people with mental health problems and mental illness. In 2006, New South Wales (NSW) Health allocated considerable funds to the reform agenda of mental health services in NSW to address this inadequacy. Children and Young People's Mental Health (CYPMH), a service that provides mental health care for young people aged 12-24 years, with moderate to severe mental health problems, was chosen to establish a prototype Youth Mental Health (YMH) Service Model for NSW. This paper describes nine key principles developed by CYPMH to guide the development of YMH Service Models in NSW., Methods: A literature review, numerous stakeholder consultations and consideration of clinical best practice were utilized to inform the development of the key principles., Results: Subsequent to their development, the nine key principles were formally endorsed by the Mental Health Program Council to ensure consistency and monitor the progress of YMH services across NSW. As a result, between 2008 and 2012 YMH Services across NSW regularly reported on their activities against each of the nine key principles demonstrating how each principle was addressed within their service., Conclusions: The nine key principles provide mental health services a framework for how to reorient services to accommodate YMH and provide a high-quality model of care. While qualitative methods have gained considerable recognition in medical education research, employing multiple qualitative data sources in assessing long-term educational impact is rare. Utilising in-depth data analysis method to six cross-sectional cohorts (2004-2009) of students' reflection papers (n = 213), this article demonstrates how students experienced subtle but important shifts in their attitudes (including personal, professional and spiritual domains) after making field visits to a hospice centre as part of the Special Needs Dentistry module. For retrospective assessment of learning retention, a pilot focus group was conducted with three junior faculty members who participated in the field visits to a hospice during their own undergraduate training. A subsequent focus group was conducted with graduates of the 2008 (n = 8) cohort using a refined discussion guide arising from the analysis of pilot group results. Graduates were unanimous in stating that the visits had sown 'seeds' in their minds and hearts, seeds which started to grow after they completed dental school and began to practice. Objectives: In studies involving theoretical constructs, it is important that measures have good content validity and that there is not contamination of measures by content from other constructs. While reliability and construct validity are routinely reported, to date, there has not been a satisfactory, transparent, and systematic method of assessing and reporting content validity. In this paper, we describe a methodology of discriminant content validity (DCV) and illustrate its application in three studies., Methods: Discriminant content validity involves six steps: construct definition, item selection, judge identification, judgement format, single-sample test of content validity, and assessment of discriminant items. In three studies, these steps were applied to a measure of illness perceptions (IPQ-R) and control cognitions., Results: The IPQ-R performed well with most items being purely related to their target construct, although timeline and consequences had small problems. By contrast, the study of control cognitions identified problems in measuring constructs independently. In the final study, direct estimation response formats for theory of planned behaviour constructs were found to have as good DCV as Likert format., Conclusions: The DCV method allowed quantitative assessment of each item and can therefore inform the content validity of the measures assessed. The methods can be applied to assess content validity before or after collecting data to select the appropriate items to measure theoretical constructs. Further, the data reported for each item in Appendix S1 can be used in item or measure selection. Statement of contribution What is already known on this subject? There are agreed methods of assessing and reporting construct validity of measures of theoretical constructs, but not their content validity. Content validity is rarely reported in a systematic and transparent manner. What does this study add? The paper proposes discriminant content validity (DCV), a systematic and transparent method of assessing and reporting whether items assess the intended theoretical construct and only that construct. In three studies, DCV was applied to measures of illness perceptions, control cognitions, and theory of planned behaviour response formats. Objectives: Kidney donation from a living donor to an unknown recipient has been legal in the UK since 2006. Yet there is little research into the experiences of unspecified kidney donors (UKDs) in interaction with the health care systems., Design: This article explores the experiences of 14 UKDs recruited through four regional transplant co-ordinating centres in England. At interview, they were invited to share their donation stories and discuss the antecedents, social, and psychological processes involved. Interviews were audio-taped and transcribed., Methods: Transcripts were analysed using a grounded theory approach employing a constant comparison methodology. Themes emerging from the data were named to form categories organized around the central focus of the research, forming an analytical story of UKDs' experiences., Results: Two major categories emerged: 'connected to others' and 'uneasy negotiations with others'. 'Connected to others' encompasses the motivations and psychological and social consequences of UKD. 'Uneasy negotiations with others' refer to the concerns and conflicts that arose during the donation process., Conclusions: This study highlights the importance of social relationships on the process and outcomes of UKD. These UKDs report both intra- and interpersonal benefits from donation. The donation process, however, also created interpersonal stress, and conflicting messages about the acceptability of their donation were experienced in UKDs' personal lives and in their interactions with health care services. Findings are discussed with reference to the wider literature on UKD and altruism and in relation to implications for clinical practice., Statement of Contribution: What is already known on this subject? Unspecified living kidney donation is an under-researched area with only three research papers published worldwide that report on the motivations and experiences of donors. These studies indicate that donors endorse pro-social values and receive positive interpersonal and intrapersonal benefits from donation. What does this study add? UKDs' experiences are made explicit and provide a framework for future research. Social connections (capital) are an important precursor to and outcome from donation. Pruritus is a symptom that significantly affects the patient's quality of life in cutaneous T cell lymphoma (CTCL). The most effective treatments are those that address the condition itself; however, it is often not possible to control this symptom. Lymphoma-related pruritus normally becomes more severe as CTCL progresses, constituting an important factor for quality of life in these patients. Substance P is a neuromodulator which appears to play a key role in pruritus. Aprepitant is a neurokinin-1 receptor antagonist affecting the substance P receptor. So far, several cases have been documented with an antipruritic response to the drug aprepitant in advanced-stage mycosis fungoides (MF). In this paper, we describe an excellent response to aprepitant in a female patient with severe pruritus secondary to hypopigmented stage I MF. We would also like to stress the absence of nausea and vomiting of this combined therapy of interferon and aprepitant. In my commentary on Francesca Minerva's article 'New Threats to Academic Freedom', I agree with her contention that the existence of the Internet has given rise to new and very serious threats to academic freedom. I think that it is crucial that we confront those threats, and find ways to eliminate them, which I believe can be done. The threats in question involve both authors and editors. In the case of authors, I argue that the best solution is not anonymous publication, but publication using pseudonyms, and I describe how that would work. In the case of editors, my proposal is a website that a number of journals would have access to, where papers that editors judge to be clearly worthy of publication, but whose publication seems likely to set off a firestorm of public and media protest, could be published without any indication of the journal that had accepted the paper for publication., (© 2014 John Wiley & Sons Ltd.)

Philosophy has long been concerned with 'moral status'. Discussions about the moral status of children, however, seem often to promote confusion rather than clarity. Using the creation of 'savior siblings' as an example, this paper provides a philosophical critique of the moral status of children and the moral relevance of parenting and the role that formative experience, regret and relational autonomy play in parental decisions. We suggest that parents make moral decisions that are guided by the moral significance they attach to children, to sick children and most importantly, to a specific sick child (theirs). This moral valorization is rarely made explicit and has generally been ignored by both philosophers and clinicians in previous critiques. Recognizing this, however, may transform not only the focus of bioethical discourse but also the policies and practices surrounding the care of children requiring bone marrow or cord blood transplantation by better understanding the values at stake behind parental decision making., (© 2012 John Wiley & Sons Ltd.)

Clearance of aggregates during protein purification is increasingly paramount as protein aggregates represent one of the major impurities in biopharmaceutical products. Aggregates, especially dimer species, represent a significant challenge for purification processing since aggregate separation coupled with high purity protein recovery can be difficult to accomplish. Biochemical characterization of the aggregate species from the hydrophobic interaction and cation exchange chromatography elution peaks revealed two different charged populations, i.e. heterogeneous charged aggregates, which led to further challenges for chromatographic removal. This paper compares multimodal versus conventional cation exchange or hydrophobic chromatography methodologies to remove heterogeneous aggregates. A full, mixed level factorial design of experiment strategy together with high throughput experimentation was employed to rapidly evaluate chromatographic parameters such as pH, conductivity, and loading. A variety of operating conditions were identified for the multimodal chromatography step, which lead to effective removal of two different charged populations of aggregate species. Introduction and Aims: This paper examines the level of public support for alcohol control policies during a period of policy evolution in Ireland., Design and Methods: A comparison of attitudes to alcohol policy at three points in time was undertaken. The first survey took place in 2002 and was repeated in 2006 and 2010 using a national quota sample of 1000 adults with face-to-face interviews. Policy areas examined were drunk-driving, price, availability and promotion., Results: In 2002, the highest level of public support was for drink-driving countermeasures (84% favoured random breath testing) and measures to restrict alcohol promotions (67% favoured restrictions). Support for stricter measures on price and availability was lower. Trends showed a decline in support for tax increases and for early closing time, whereas support for fewer outlets (off-trade) increased., Discussion and Conclusion: When public concerns (level of support) for stricter alcohol policies were seen to be meet, as occurred with price and hours through increased tax and reversal of opening hours, public concern/support declined. When concerns were not met, as with outlets, support continued to rise most likely influenced by greater number of off-trade outlets and cheaper alcohol. Support for liberal alcohol policies was low and the majority favoured the status quo on price and availability. Introduction and Aims: The aim of our paper is to present trend data concerning public opinion on alcohol policy in the Canadian province of Ontario over a 16-year period (1996-2011), to assess if the level of support for alcohol control policies changed over this period and if any changes in public support for alcohol policy parallel real changes in alcohol distribution in Ontario., Design and Methods: Selected policy-related items from 10 probability surveys of Ontario adults were analysed by means of logistic regression., Results: A significant decline was found for attitudes supporting restricting corner stores sales and government control of liquor stores. A weaker decline was seen for attitudes towards reducing the number of places to buy alcohol. However, an increasing trend for attitudes favouring the status quo or greater control through taxes and hours of sale was found. There was no significant trend for attitudes towards maintaining the current level or reducing the number of liquor or beer stores. While there was some variation in trends in support by demographic characteristics and drinking level, not one sector stood out., Discussion and Conclusions: A gradual erosion of support is evident and concurrent with gradual increase in access to alcohol in Ontario during the time-period under study. The decline in support for alcohol control measures seems to be a general rather than a focused development. Introduction and Aims: Regulatory and collaborative intervention strategies have been developed to reduce the harms associated with alcohol consumption on licensed venues around the world, but there remains little research evidence regarding their comparative effectiveness. This paper describes concurrent changes in the number of night-time injury-related hospital emergency department presentations in two cities that implemented either a collaborative voluntary approach to reducing harms associated with licensed premises (Geelong) or a regulatory approach (Newcastle)., Design and Methods: This paper reports findings from Dealing with Alcohol-Related problems in the Night-Time Economy project. Data were drawn from injury-specific International Classification of Disease, 10th Revision codes for injuries (S and T codes) presenting during high-alcohol risk times (midnight--5.59 am, Saturday and Sunday mornings) at the emergency departments in Geelong Hospital and Newcastle (John Hunter Hospital and the Calvary Mater Hospital), before and after the introduction of licensing conditions between the years of 2005 and 2011. Time-series, seasonal autoregressive integrated moving average analyses were conducted on the data obtained from patients' medical records., Results: Significant reductions in injury-related presentations during high-alcohol risk times were found for Newcastle since the imposition of regulatory licensing conditions (344 attendances per year, P < 0.001). None of the interventions deployed in Geelong (e.g. identification scanners, police operations, radio networks or closed-circuit television) were associated with reductions in emergency department presentations., Discussion and Conclusions: The data suggest that mandatory interventions based on trading hours restrictions were associated with reduced emergency department injury presentations in high-alcohol hours than voluntary interventions., (© 2014 Australasian Professional Society on Alcohol and other Drugs.)

Hidradenoma papilliferum (HP) is a benign adnexal neoplasm which preferentially develops in the anogenital region of women. Although the origin of HP was previously thought to be an apocrine sweat gland, recent studies have suggested that it may derive from the anogenital mammary-like gland (MLG). In this paper, we present a 43-year-old Japanese woman with hidradenoma papilliferum of the vulva. The lesion developed 7 years prior to her visit, and clinically appeared as a skin-colored cystic nodule. Histopathological examination revealed that the neoplasm was formed by the tubular structures consisting of two types of pleomorphic cells, columnar cells in the luminal layer and cuboidal cells in the basal layer. Further, the surgical specimen contained a wide, divergent, lobular ductal structure located in the vicinity of the neoplastic lesion, which was consistent with MLG., (© 2014 Japanese Dermatological Association.)

Background: This paper synthesizes research on the contribution of workplace injustices to occupational health disparities., Methods: We conducted a broad review of research and other reports on the impact of workplace discrimination, harassment, and bullying on workers' health and on family and job outcomes., Results: Members of demographic minority groups are more likely to be victims of workplace injustice and suffer more adverse outcomes when exposed to workplace injustice compared to demographic majority groups. A growing body of research links workplace injustice to poor psychological and physical health, and a smaller body of evidence links workplace injustice to unhealthy behaviors. Although not as well studied, studies show that workplace injustice can influence workers' health through effects on workers' family life and job-related outcomes., Conclusion: Injustice is a key contributor to occupational health injustice and prospective studies with oversample of disadvantaged workers and refinement of methods for characterizing workplace injustices are needed., (© 2013 Wiley Periodicals, Inc.)

Background: Manual labor employment occurs in environments with exposures likely to impact skin-related quality of life (SRQOL)., Objectives: The objectives of this paper are to (1) document the dimensions of SRQOL, (2) examine its association with skin symptoms, and (3) identify the predictors of SRQOL in Latino manual workers., Methods: A population-based survey of 733 Latino manual workers obtained Dermatology Life Quality Index (DLQI) and skin symptoms in the prior year., Results: Two-thirds of workers were employed in production. Skin symptoms in prior year were reported by 23%. Impaired SRQOL was reported by 23%. In multivariate analyses, reduced SRQOL was associated with age, occupation, childhood indigenous language use, and experience of skin symptoms in the prior year., Conclusions: Despite overall high SRQOL exposures in some immigrant occupational groups produce reduce SRQOL. Safe and reliable venous access is mandatory in modern health care, but central venous catheters (CVCs) are associated with significant morbidity and mortality, This paper describes current Swedish guidelines for clinical management of CVCs The guidelines supply updated recommendations that may be useful in other countries as well. Literature retrieval in the Cochrane and Pubmed databases, of papers written in English or Swedish and pertaining to CVC management, was done by members of a task force of the Swedish Society of Anaesthesiology and Intensive Care Medicine. Consensus meetings were held throughout the review process to allow all parts of the guidelines to be embraced by all contributors. All of the content was carefully scored according to criteria by the Oxford Centre for Evidence-Based Medicine. We aimed at producing useful and reliable guidelines on bleeding diathesis, vascular approach, ultrasonic guidance, catheter tip positioning, prevention and management of associated trauma and infection, and specific training and follow-up. A structured patient history focused on bleeding should be taken prior to insertion of a CVCs. The right internal jugular vein should primarily be chosen for insertion of a wide-bore CVC. Catheter tip positioning in the right atrium or lower third of the superior caval vein should be verified for long-term use. Ultrasonic guidance should be used for catheterisation by the internal jugular or femoral veins and may also be used for insertion via the subclavian veins or the veins of the upper limb. The operator inserting a CVC should wear cap, mask, and sterile gown and gloves. For long-term intravenous access, tunnelled CVC or subcutaneous venous ports are preferred. Intravenous position of the catheter tip should be verified by clinical or radiological methods after insertion and before each use. Simulator-assisted training of CVC insertion should precede bedside training in patients. Units inserting and managing CVC should have quality assertion programmes for implementation and follow-up of routines, teaching, training and clinical outcome. The family of (non-parametric, fixed-step-size) adaptive methods, also known as 'up-down' or 'staircase' methods, has been used extensively in psychophysical studies for threshold estimation. Extensions of adaptive methods to non-binary responses have also been proposed. An example is the three-category weighted up-down (WUD) method (Kaernbach, 2001) and its four-category extension (Klein, 2001). Such an extension, however, is somewhat restricted, and in this paper we discuss its limitations. To facilitate the discussion, we characterize the extension of WUD by an algorithm that incorporates response confidence into a family of adaptive methods. This algorithm can also be applied to two other adaptive methods, namely Derman's up-down method and the biased-coin design, which are suitable for estimating any threshold quantiles. We then discuss via simulations of the above three methods the limitations of the algorithm. In this paper we implement a Markov chain Monte Carlo algorithm based on the stochastic search variable selection method of George and McCulloch (1993) for identifying promising subsets of manifest variables (items) for factor analysis models. The suggested algorithm is constructed by embedding in the usual factor analysis model a normal mixture prior for the model loadings with latent indicators used to identify not only which manifest variables should be included in the model but also how each manifest variable is associated with each factor. We further extend the suggested algorithm to allow for factor selection. We also develop a detailed procedure for the specification of the prior parameters values based on the practical significance of factor loadings using ideas from the original work of George and McCulloch (1993). A straightforward Gibbs sampler is used to simulate from the joint posterior distribution of all unknown parameters and the subset of variables with the highest posterior probability is selected. Parameters in structural equation models are typically estimated using the maximum likelihood (ML) approach. Bollen (1996) proposed an alternative non-iterative, equation-by-equation estimator that uses instrumental variables. Although this two-stage least squares/instrumental variables (2SLS/IV) estimator has good statistical properties, one problem with its application is that parameter equality constraints cannot be imposed. This paper presents a mathematical solution to this problem that is based on an extension of the 2SLS/IV approach to a system of equations. We present an example in which our approach was used to examine strong longitudinal measurement invariance. We also investigated the new approach in a simulation study that compared it with ML in the examination of the equality of two latent regression coefficients and strong measurement invariance. Overall, the results show that the suggested approach is a useful extension of the original 2SLS/IV estimator and allows for the effective handling of equality constraints in structural equation models., (© 2013 The British Psychological Society.)

Hypersensitivity diseases are not adequately coded in the International Coding of Diseases (ICD)-10 resulting in misclassification, leading to low visibility of these conditions and general accuracy of official statistics. To call attention to the inadequacy of the ICD-10 in relation to allergic and hypersensitivity diseases and to contribute to improvements to be made in the forthcoming revision of ICD, a web-based global survey of healthcare professionals' attitudes toward allergic disorders classification was proposed to the members of European Academy of Allergy and Clinical Immunology (EAACI) (individuals) and World Allergy Organization (WAO) (representative responding on behalf of the national society), launched via internet and circulated for 6 week. As a result, we had 612 members of 144 countries from all six World Health Organization (WHO) global regions who answered the survey. ICD-10 is the most used classification worldwide, but it was not considered appropriate in clinical practice by the majority of participants. The majority indicated the EAACI-WAO classification as being easier and more accurate in the daily practice. They saw the need for a diagnostic system useful for nonallergists and endorsed the possibility of a global, cross-culturally applicable classification system of allergic disorders. This first and most broadly international survey ever conducted of health professionals' attitudes toward allergic disorders classification supports the need to update the current classifications of allergic diseases and can be useful to the WHO in improving the clinical utility of the classification and its global acceptability for the revised ICD-11., (© 2014 John Wiley & Sons A/S. Wild primates face significant challenges associated with locating resources that involve learning through exploration, encoding, and recalling travel routes, orienting to single landmarks or landmark arrays, monitoring food availability, and applying spatial strategies that reduce effort and increase efficiency. These foraging decisions are likely to involve tradeoffs between traveling to nearby or distant feeding sites based on expectations of resource productivity, predation risk, the availability of other nearby feeding sites, and individual requirements associated with nutrient balancing. Socioecological factors that affect primate foraging decisions include feeding competition, intergroup encounters, mate defense, and opportunities for food sharing. The nine research papers in this Special Issue, "Primate Spatial Strategies and Cognition," address a series of related questions examining how monkeys, apes, and humans encode, internally represent, and integrate spatial, temporal, and quantity information in efficiently locating and relocating productive feeding sites in both small-scale and large-scale space. The authors use a range of methods and approaches to study wild and captive primates, including computer and mathematical modeling, virtual reality, and detailed examinations of animal movement using GPS and GIS analyses to better understand primate cognitive ecology and species differences in decision-making. The attribution of goal-directed behavior to observations of primate foraging and ranging requires that simpler explanations for observed behavior patterns be eliminated. Computer-generated simulations of non-goal-directed foraging behavior can be used as null models for higher complexity cognitive foraging, and can provide quantifiable data against which to compare the observed behavioral patterns in wild primates. In this paper, we compare the results of two variations of computer simulated null models with observed foraging behavior of wild spider monkeys (Ateles belzebuth). One model simulates monkeys searching using a modified random-walk model in which monkeys alternate 100-m steps with turn angles derived from observed behavior. The second model constrains travel to an observed route system derived from observations of wild spider monkeys. Simulated monkeys in each model searched among increasing densities of feeding trees ranging from 10 to 1,000. We compared travel distance, travel directness, and accuracy of starting direction for each feeding tree discovered for the two models. We then compared these results with those derived from observations of wild spider monkeys. Route-model monkeys traveled shorter distances and more directly to feeding trees than did randomly foraging monkeys, and discovered trees in the direction they started more often. A half-century of research in behavioral pharmacology leaves little doubt that behavior-environment contingencies can determine the behavioral effects of drugs. Unfortunately, a coherent behavior-analytic framework within which to characterize the myriad ways in which contingencies interact with drugs, and to predict effects of a given drug under a given set of conditions, still has not developed. Some behavioral pharmacologists have suggested the concept of behavioral mechanisms of drug action as a foundation for such a framework. The notion of behavioral mechanisms, however, does not seem to have been fully embraced by behavioral pharmacologists. It is suggested here that one reason for this is that the concept itself has not been sufficiently clarified (i.e., stimulus control over use of the phrase is not sufficiently precise). Furthermore, early behavioral pharmacologists may not have possessed an adequate set of analytic tools to develop a viable framework based upon behavior mechanisms. In the first part of this paper, the notion of behavioral mechanisms of drug action is explored, and the sort of data that might provide evidence of a behavioral mechanism is considered. In the second part, it is suggested that the increased availability of quantitative models in behavior analysis may help provide the tools needed for elucidating behavioral mechanisms of drug action. Aims: This paper provides a historical review of gambling in Canada and examines the benefits and shortcomings of present-day Canadian gambling policies and practices. This includes a discussion of provincial and federal government roles in gambling regulation and an overview of problem gambling prevention and treatment initiatives., Methods: The gambling studies literature was probed for pertinent information on factors such as historical development, legislative changes, economic conditions and cultural influences that have affected gambling participation and social responsibility strategies in Canada., Results: Two major Criminal Code of Canada amendments (in 1969 and 1985) were pivotal in Canadian gambling expansion. The first decriminalized lotteries and casinos, while the second allowed electronic gambling devices and authorized provinces to operate and regulate gambling. These changes resulted in a radical gambling expansion which, in addition to raising provincial revenues, created public policy concerns. Varying provincial government interpretations of the ambiguous Criminal Code gambling statutes led to a lack of uniformity in how provinces regulate and operate gambling; when gambling expanded, there were no legislative safeguards in place to deal with the personal and societal effects of problem gambling. Subsequent programs designed to prevent and treat problem gambling have not been overly effective., Conclusions: Canadian provinces have a monopoly on gambling within their borders and treat the activity as a profit-driven business enterprise. Problems associated with widespread gambling such as addiction, increased crime, bankruptcy and suicide are seen as minor concerns and not addressed in an aggressive fashion. Objective: At least 50% of bipolar disorder (BD) patients have an additional diagnosis, one of the most difficult to manage being obsessive-compulsive disorder (OCD). Defining the nosology of BD-OCD comorbidity has important clinical implications, given that treatments for OCD can worsen BD outcomes., Method: A systematic review was conducted on: i) BD-OCD comorbidity lifetime prevalence and ii) on standard diagnostic validators: phenomenology, course of illness, heredity, biological markers, and treatment response. Relevant papers published through March 30th 2013 were identified searching the electronic databases MEDLINE, Embase, PsycINFO, and the Cochrane Library., Results: Sixty-four articles met inclusion criteria. Lifetime comorbidity prevalence was 11-21% in BD patients and 6-10% in OCD patients. Compared to non-comorbid subjects, BD-OCD has a more episodic course of OC symptoms (up to 75% vs. 3%), typically with worsening during depression (78%) and improvement during mania/hypomania (64%), as well as a higher total mean number of depressive episodes (8.9±4.2 vs. 4.1±2.7) and perhaps more antidepressant-induced mania/hypomania (39% vs. 9%)., Conclusion: In this first systematic review of BD-OCD comorbidity, it appears that OC symptoms are usually secondary to BD, rather than representing a separate disease., (© 2014 John Wiley & Sons A/S. IDEAS (intellectual disabilities evaluation and advice system) provides the opportunity for physicians who are sending samples for the Greenwood Genetic Center (GGC) 92-gene X-linked intellectual disability (XLID) (formerly X-linked mental retardation) panel to have their male patient's clinical features reviewed by an experienced panel of six Clinical Geneticists. They were asked to obtain parental consent, complete a one-page information form, and provide A-P and lateral photographs. The panel members independently reviewed the material and forwarded comments about clinical features, possible diagnoses, and/or further testing for the patient. We present the results of the first 55 patients evaluated. In only a single case did all panelists agree on a non-XLID diagnosis, later proven by genetic testing. The XLID gene panel diagnosed an additional five (9%) cases, but in only two cases did one panelist suggest the correct gene, which was one of four they suggested. This paper examines the possible reasons for the low rate of clinical diagnosis and suggests that, while the data received were often incomplete, the most important reasons for lack of diagnoses were the source of referral and selection of patients for review. We did note that there were a number of instances where we disagreed with the submitted information as to whether the individual was dysmorphic and with the stated presence of certain physical signs, most often downslanted palpebrae and posterior ear angulation. These differences in assessment of clinical signs and the general lack of completeness and detail provided in the standard data sheet, including that regarding the extended family history, lead us to raise concerns regarding the feasibility of establishing high quality central clinical databases designed to aid in the interpretation of exomic/genomic variants., (© 2014 Wiley Periodicals, Inc.)

The article offers the author's insights regarding the epidemic of the Copenhagen poliomyelitis from 1952-1953. The author says that during the first four weeks of the epidemic 27 out of the 31 patients with bulbar-spinal paralysis have died. He mentions several topics persons who helped save lives of the during the epidemic, including Department of Communicable Diseases professor Henry Cai Alexander Lassen, freelance anasthetist Bjørn Ibsen, and doctor Mogens Bjørneboe.

Background This prospective, observer-blinded, randomised, multicentre study aimed at determining the non-inferiority of 50 mg of plain 1% 2-chloroprocaine vs. 10 mg of 0.5% plain bupivacaine in terms of sensory block onset time at T10 after spinal injection. The study hypothesis was that the difference in onset times of sensory block to T10 between the two drugs is ≤ 4 min. Methods One hundred and thirty patients undergoing lower abdominal or lower limb procedures (≤ 40 min) were randomised to receive one of two treatments: 50 mg of plain 1% 2-chloroprocaine ( Group C, n = 66) or 10 mg of plain 0.5% bupivacaine ( Group B, n = 64). Times to sensory and motor block onsets, maximum sensory block level, readiness for surgery, regression of sensory and motor blocks, first analgesic requirements, unassisted ambulation, home discharge, and side effects after 24 h and 7 days were registered blindly. Results Chloroprocaine was comparable with plain 0.5% bupivacaine in terms of time to sensory block at T10 level. Group C showed faster onsets of motor block (5 vs. 6 min), maximum sensory block level (8.5 vs. 14 min), resolution of sensory (105 vs. 225 min) and motor (100 vs. 210 min) blocks, unassisted ambulation (142.5 vs. 290.5 min), first analgesic requirement (120 vs. 293.5 min), and home discharge (150 vs. 325 min) (all comparisons, P < 0.05). No chloroprocaine patient developed transient neurological symptoms. Conclusion Spinal anaesthesia with 50 mg of plain 1% 2-chloroprocaine is similar to 10 mg of plain 0.5% bupivacaine in terms of onset of sensory block at T10 but shows quicker recovery from anaesthesia than with 0.5% bupivacaine. Background Fluid resuscitation is still a major challenge. We aimed to describe changes in central venous oxygen saturation ( ScvO2) and venous-to-arterial carbon dioxide gap ( dCO2) during an experimental stroke volume ( SV) index ( SVI)-guided hemorrhage and fluid resuscitation model in pigs. Methods Twelve anesthetized, mechanically ventilated pigs were bled till baseline SVI ( Tbsl) dropped by 50% ( T0), thereafter fluid resuscitation was performed with balanced crystalloid in four steps until initial SVI was reached ( T4). Statistical analysis was performed with Statistical Program for Social Sciences version 18.0; data are expressed as mean ± standard deviation. Results After bleeding, ScvO2 dropped ( Tbsl = 78 ± 7 vs. T0 = 61 ± 5% P < 0.05) and oxygen extraction ratio increased ( Tbsl = 0.20 ± 0.07 vs. T0 = 0.36 ± 0.05, P < 0.05). By T4 the ScvO2 normalized, but on average it remained 5% lower than at Tbsl ( T4 = 73 ± 9% P < 0.05) and oxygen extraction also remained higher as compared with Tbsl ( T4 = 0.24 ± 0.09 P = 0.001). ScvO2 showed significant correlation with SVI ( r = 0.564, P < 0.001). dCO2 increased during hypovolemia ( Tbsl:5.3 ± 2.0 vs. T0:9.6 ± 2.3 mmHg, P = 0.001), then returned to normal by T4 = 5.1 ± 2.6 mmHg, and it also showed significant correlation with SVI ( R = −0.591, P < 0.001) and oxygen extraction ( R = 0.735, P < 0.001). Conclusions In this SV-guided bleeding and fluid resuscitation model, both ScvO2 and dCO2 correlated well with changes in SV, but only the dCO2 returned to its baseline, normal value, while ScvO2 remained significantly lower than at baseline. These results suggest that dCO2 may be a good hemodynamic endpoint of resuscitation, while ScvO2 is not strictly a hemodynamic parameter, but rather an indicator of the balance between oxygen delivery and consumption. Reported here are the single-crystal X-ray structure analyses of bis-μ-methanol-κ4 O: O-bis{[hydrotris(3-phenyl-2-sulfanylidene-2,3-dihydro-1 H-1,3-imidazol-1-yl)borato-κ3 H, S, S′](methanol-κ O)sodium(I)}, [Na2(C27H22BN6S3)2(CH4O)4] (NaTmPh), bis-μ-methanol-κ4 O: O-bis{[hydrotris(3-isopropyl-2-sulfanylidene-2,3-dihydro-1 H-1,3-imidazol-1-yl)borato-κ3 H, S, S′](methanol-κ O)sodium(I)}-diethyl ether-methanol (1/0.3333/0.0833), [Na2(C18H28BN6S3)2(CH4O)4]·0.3333C4H10O·0.0833CH3OH (NaTmiPr), and a novel anhydrous form of sodium hydrotris(methylthioimidazolyl)borate, poly[[μ-hydrotris(3-methyl-2-sulfanylidene-2,3-dihydro-1 H-1,3-imidazol-1-yl)borato]sodium(I)], [Na(C12H16BN6S3)] ([NaTmMe] n). NaTmiPr and NaTmPh have similar dimeric molecular structures with κ3 H, S, S′-bonding, but they differ in that NaTmPh is crystallographically centrosymmetric ( Z′ = 0.5) while NaTmiPr contains one crystallographically centrosymmetric dimer and one dimer positioned on a general position ( Z′ = 1.5). [NaTmMe] n is a one-dimensional coordination polymer that extends along the a direction and which contains a hitherto unseen side-on η2-C=S-to-Na bond type. An overview of the structural preferences of alkali metal soft scorpionate complexes is presented. In the coordination polymer catena-poly[[[diaqua[5-carboxy-2-(pyridin-3-yl)-1 H-imidazole-4-carboxylato-κ2 N3, O4]lead(II)]-μ-5-carboxy-2-(pyridin-3-yl)-1 H-imidazole-4-carboxylato-κ3 N3, O4: N2] dihydrate], {[Pb(C10H6N3O4)(H2O)2]·2H2O} n, the two 5-carboxy-2-(pyridin-3-yl)-1 H-imidazole-4-carboxylate ligands have different coordination modes, one being terminal and the other bridging. The bridging ligand links PbII cations into one-dimensional coordination polymer chains. The structure is also stabilized by intra- and interchain π-π stacking interactions between the pyridine rings, resulting in the formation of a two-dimensional network. Two new diamagnetic coordination compounds, [Fe(phen)3][C(CN)3]2, (I), and [Fe(bpy)3][C(CN)3]2·1.5H2O, (II), have been synthesized and characterized by single-crystal X-ray diffraction analysis, and IR and UV-Vis spectroscopy (phen is 1,10-phenanthroline, C12H8N2, and bpy is 2,2′-bipyridine, C10H8N2). Both compounds are ionic with distorted octahedral [Fe(phen)3]2+ or [Fe(bpy)3]2+ complex cations, with average Fe-N distances of 1.977 (2) and 1.971 (3) Å, respectively, and two uncoordinated planar tricyanomethanide, or [C(CN)3]−, counter-anions balancing the positive charges of the cations. Solvent water molecules and tcm anions in (II) are linked via O-H...N hydrogen bonds into negatively charged layers and the interlayer space is filled by [Fe(bpy)3]2+ cations. The structures of (I) and (II) are stabilized by π-π interactions between the stacked aromatic rings of the phen ligands of two adjacent cations and by O-H...N hydrogen bonds, respectively, and also by π-π stacking interactions between phen and tcm units in (I). The CoII cation in poly[[aqua(μ-benzene-1,2-dicarboxylato-κ3 O1, O2: O1)(μ-4,4′-bipyridine-κ2 N: N′)cobalt(II)] trihydrate], {[Co(C8H4O4)(C10H8N2)(H2O)]·3H2O} n, is octahedrally coordinated by two N atoms of two 4,4′-bipyridine ligands, three O atoms from phthalate anions and a fourth O atom from a coordinated water molecule. The packing consists of planes of coordination polymers linked by hydrogen bonds mediated by three solvent water molecules; the linkage is achieved by the water molecules forming intricate oligomeric clusters which also involve the O atoms of the phthalate ligands. The crystal structure of the title compound {(C5H14N2)2[Cd2Cl8]} n, (I), consists of hydrogen-bonded 2-methylpiperazinediium (H2MPPA2+) cations in the presence of one-dimensional polymeric {[CdCl3(μ3-Cl)]2−} n anions. The CdII centres are hexacoordinated by three terminal chlorides and three bridging chlorides and have a slightly distorted octahedral CdCl3(μ3-Cl)3 arrangement. The alternating CdCl6 octahedra form four-membered Cd2Cl2 rings by the sharing of neighbouring Cd-Cl edges to give rise to extended one-dimensional ladder-like chains parallel to the b axis, with a Cd...Cd distance of 4.094 (2) Å and a Cd...Cd...Cd angle of 91.264 (8)°. The H2MPPA2+ cations crosslink the [CdCl3(μ3-Cl)] n chains by the formation of two N-H...Cl hydrogen bonds to each chain, giving rise to one-dimensional ladder-like H2MPPA2+-Cl2 hydrogen-bonded chains [graph set R42(14)]. The title complex salt, (C16H36N)[MnBr(C32H16N8)] or (TBA)[MnIIBr(Pc)] (TBA is tetrabutylammonium and Pc is phthalocyaninate), has been obtained as single crystals by the diffusion technique and its crystal structure was determined using X-ray diffraction. The high-spin ( S = ) [MnIIBr(Pc)]− macrocycle has a concave conformation, with an average equatorial Mn-N(Pc) bond length of 2.1187 (19) Å, an axial Mn-Br bond length of 2.5493 (7) Å and with the MnII cation displaced out of the 24-atom Pc plane by 0.894 (2) Å. The geometry of the MnIIN4 fragment in [MnIIBr(Pc)]− is similar to that of the high-spin ( S = ) manganese(II) tetraphenylporphyrin (TPP) in [MnII(1-MeIm)(TPP)] (1-MeIm is 1-methylimidazole). A novel nitride, Sr2− yEu yB2−2 xSi2+3 xAl2− xN8+ x ( x≃ 0.12, y≃ 0.10) (distrontium europium diboron disilicon dialuminium octanitride), with the space group P2 c, was synthesized from Sr3N2, EuN, Si3N4, AlN and BN under nitrogen gas pressure. The structure consists of a host framework with Sr/Eu atoms accommodated in the cavities. The host framework is constructed by the linkage of MN4 tetrahedra ( M = Si, Al) and BN3 triangles, and contains substitutional disorder described by the alternative occupation of B2 or Si2N on the (0, 0, z) axis. The crystal structures of two crown-ether-coordinated caesium halogen salt hydrates, namely di-μ-bromido-bis[aqua(1,4,7,10,13,16-hexaoxacyclooctadecane)caesium(I)] dihydrate, [Cs2Br2(C12H24O6)2(H2O)2]·2H2O, (I), and poly[[diaquadi-μ-chlorido-μ-(1,4,7,10,13,16-hexaoxacyclooctadecane)dicaesium(I)] dihydrate], {[Cs2Cl2(C12H24O6)(H2O)2]·2H2O} n, (II), are reported. In (I), all atoms are located on general positions. In (II), the Cs+ cation is located on a mirror plane perpendicular to the a axis, the chloride anion is located on a mirror plane perpendicular to the c axis and the crown-ether ring is located around a special position with site symmetry 2/ m, with two opposite O atoms exactly on the mirror plane perpendicular to the a axis; of one water molecule, only the O atom is located on a mirror plane perpendicular on the a axis, while the other water molecule is completely located on a mirror plane perpendicular to the c axis. Whereas in (I), hydrogen bonds between bromide ligands and water molecules lead to one-dimensional chains running along the b axis, in (II) two-dimensional sheets of water molecules and chloride ligands are formed which combine with the polymeric caesium-crown polymer to give a three-dimensional network. Although both compounds have a similar composition, i.e. a Cs+ cation with a halogen, an 18-crown-6 ether and a water ligand, the crystal structures are rather different. In our ongoing development of ferrocene ligands, 1-dimethylamino-2-(diphenylphosphinothioyl)ferrocene is being used as a convenient building block to obtain racemic or enantiomerically pure ligands. Using this building block in large excess allowed the formation of several by-products, two of which have already been reported; the structure of a third by-product, namely 1-(diphenylphosphinothioyl)-2-{[(diphenylphosphinothioyl)sulfanyl]methyl}ferrocene, [Fe(C5H5)(C30H25P2S3)], is presented here. The crystal structure is built up from a ferrocene unit, with one of the cyclopentadienyl (Cp) rings substituted in the 1- and 2-positions by a protected diphenylphosphinothioyl group and a [(diphenylphosphinothioyl)sulfanyl]methyl fragment, -CH2SP(=S)Ph2. There are C-H...S interactions which result in the formation of chains parallel to the c axis. After desulfurization, the crude material was then reacted with Pd and Pt ( M) precursors [ MCl2(CH3CN)2] to yield two isostructural dinuclear complexes arranged around twofold axes, namely ( R, R/ S, S)-bis{μ-[2-(diphenylphosphanyl)ferrocen-1-yl]methanethiolato-κ3 P, S: S}bis[chloridopalladium(II)] pentane disolvate, [Pd2{Fe(C5H5)(C18H15PS)}2Cl2]·2C5H12, and the platinum(II) analogue, ( R, R/ S, S)-bis{μ-[2-(diphenylphosphanyl)ferrocen-1-yl]methanethiolato-κ3 P, S: S}bis[chloridoplatinum(II)] toluene monosolvate, [Pt2{Fe(C5H5)(C18H15PS)}2Cl2]·C7H8, in which the two metal atoms present a slightly distorted square-planar geometry formed by two bridging S atoms and P and Cl atoms. The P, S-chelating ligand results from the rupture of one of the P-S bonds in the starting ligand. These dinuclear complexes display a butterfly geometry.
