Background: People with a learning disability experience challenges accessing primary health care services, including eye care services. Methods: Eye care needs of people with a learning disability, and how well they are met by existing services in England, were explored. Barriers and enablers to accessing these services were investigated. This was informed by a scoping review of the literature and a historic literature library. Findings: Adults with a learning disability are 10 times more likely than other adults to have a serious sight problem and children with learning disabilities are 28 times more likely. There is good evidence of high levels of unmet eye care need special schools in England with over 4 in 10 children attending having no history of any eye care. Conclusion: The authors discuss possible systemic changes to address these inequalities in England. These include automatic entitlement to an NHS sight test annually, specialist pathways in community opticians, eye care services in special schools, and peer to peer and peer to professional promotion of services by people with lived experience. Dedicated care pathways have improved uptake of services in other areas of primary care. Research into the effectiveness of lived experience eye health advocacy is needed. Purpose: Research suggests that there are challenges in the accessibility of eye care for children in England. This study explores the barriers and enablers to eye examinations for children under 5 years of age from the perspective of community optometrists in England. Methods: Optometrists working in community settings were invited to participate in virtual focus group discussions using an online platform based on a topic guide. The discussions were audio‐recorded, transcribed and thematically analysed. Themes were derived from the focus group data based on the study aim and research question. Results: Thirty optometrists participated in the focus group discussions. The overarching themes identified as barriers to eye examinations for young children in a community setting were as follows: 'Time and Money', 'Knowledge, Skills and Confidence', 'Awareness and Communication', 'Range of Attitudes' and 'Clinical Setting'. The key themes for enabling eye examinations for young children were as follows: 'Improving behaviour', 'Enhancing training and education', 'Enhancing eye care services', 'Raising awareness', 'Changes in professional bodies' and 'Balancing commercial pressures and health care'. Conclusion: Time, money, training and equipment are perceived by optometrists as key factors in providing an eye examination for a young child. This study identified a need for improved training and robust governance related to eye examinations for young children. There is a need for change within eye care service delivery such that all children, regardless of age and ability, are examined regularly, and by conducting these examinations, optometrists remain confident. BACKGROUND: In 2012, Chicago Public Schools and the Chicago Department of Public Health partnered together to create the Chicago School‐Based Vision Program (CSBVP). This ongoing, city‐wide program provides school‐based vision services (eye examinations, eyeglasses provision, and eye care referrals) to students with limited access. METHODS: Descriptive analysis of the program operations from 2012 to 2020, including number of students served and exam findings during 2017‐2020, as well as lessons learned and recommendations for reproducing the successes of the CSBVP in other contexts. RESULTS: During its first 8 years, the CSBVP provided eye examinations to over 350,000 students. During the years 2017‐2020, this included 126,238 eye exams and provision of eyeglasses to 73,751 students. CONCLUSIONS: The CSBVP is built upon strong community and school relationships. The program offers vision services, equitably, to all students across the district. The lessons learned through the implementation of the CSBVP may provide guidance for future district‐wide vision programs. [ABSTRACT FROM AUTHOR]

Purpose: Good vision during childhood is vital for visual, educational, and social development. Previous research highlights challenges in the accessibility of eye care for children. This study investigates the accessibility of primary eye care for young children with typical development and those with autism in England. Methods: A telephone survey was conducted using four hypothetical scenarios (a child aged 1, 3, 5 years and a 13‐year‐old with autism). Four hundred community optometric practices (100 different practices for each scenario) were contacted to explore the availability of an eye examination. The caller acted as a parent, asking about the availability of an eye examination for their child and raising concerns regarding the child. Key barriers and enablers to the accessibility of primary eye care were identified through an analysis of qualitative information. Results: Of the 400 practices, only three (<1%) stated that they do not perform eye examinations on children. Fifty‐six practices (14%) stated that they would examine a child at any age, the remainder (n = 341, 85%) specifying a minimum age at which they would perform eye examinations on children. Lack of 'communication' from the child and 'equipment' were identified as barriers to accessing eye care for young children. Eye care for children with autism was enabled by factors related to adaptability and appointment time. Conclusion: Our results suggest that communication and a lack of appropriate equipment for examining children are potential barriers to accessing primary eye care. No clear barriers were identified for an older child with autism. Eye examinations are more accessible for older children in these scenarios (aged 5 with typical development and 13 years with autism) than younger children (aged 1 and 3 years old). While the UK General Ophthalmic Services (GOS) terms do not permit contract holders to exclude categories of patients from a GOS eye exam, our findings suggest this is taking place. Importance Screening for diabetic retinopathy for early detection and treatment can prevent vision loss. Background: We aimed to assess rates of eye examination of people with diabetes, adherence with national guidelines and investigate characteristics of those who do not adhere. Design We used data from the 45 and Up Study, a cohort study of 267 153 randomly selected residents aged ≥45 years from New South Wales, Australia. Individual survey data collected in 2006 to 2009 were linked to corresponding national Medicare Benefits Schedule claims data for 2006 to 2016. Participants: The study sample included 24 832 participants who reported having diabetes and at least 5 years of observation. Methods: Claims for visits to optometrists or ophthalmologists were assessed to estimate rates of eye examination. Poisson regression models were used to investigate factors associated with non‐adherence. Main outcome measure: Participants were classified as adherent if the average time between eye care claims was consistent with national guideline of having an eye exam every 2 years. Results: Of 50% to 75% people with diabetes met the biennial eye examination guidelines and only 21% to 28% with diabetes duration ≥10 years were adherent to the annual eye examination guideline. Characteristics associated with greatest (~1.3‐fold) risk of non‐adherence were smoking, age <60 years and higher income. Conclusions and relevance: There is a clear need to improve rates of adherence to eye examination guidelines among people with diabetes to reduce the personal and societal burden of diabetic retinopathy. Problem: In the Katherine region, Northern Territory, barriers to eye care for Aboriginal and Torres Strait Islander people include unclear eye care referral processes, challenges coordinating patient eye care between various providers, complex socioeconomic determinants and a lengthy outpatient ophthalmology waiting list. Design: Mixed methods participatory approach using a regional needs analysis, clinical file audit and stakeholder survey, to develop, implement and monitor quality improvement strategies. Setting: Collaboration with Aboriginal Community Controlled Health Services and regional eye care stakeholders in the Katherine region. Key measures for improvement: Clinical audit data captured frequency and rates of primary eye checks, ophthalmology referrals and spectacle prescriptions. A survey was developed and applied to assess stakeholder perspectives of regional eye care systems. Strategy for change: Quality improvement strategies informed by regional data (clinical audits and survey) included increasing service delivery to match eye care needs, primary eye care training for Aboriginal Community Controlled Health Services staff, updating Aboriginal Community Controlled Health Services primary care templates and forming a regional eye care coalition group. Effects of change: Post‐implementation, rates and frequency of recorded optometry examinations, number of spectacles prescribed and rates of annual dilated fundus examinations for patients with diabetes increased. There was a decrease in the number of patients with diabetes who had never had an eye examination. Eye care stakeholders perceived a marked improvement in the effectiveness of the regional eye care system. Lessons learnt: Our findings highlight the importance of engaging services and stakeholders to ensure a systems approach that is evidence‐informed, contextually appropriate and reflects commitment to improved eye health outcomes. Importance Provision of refractive changes is important to predict eye care needs for aging population. Background: To provide 5‐year refractive changes in a rural Chinese adult population. Design Population‐based longitudinal study. Participants: At baseline, 6830 subjects aged 30+ years took part in the Handan Eye Study. A total of 5394 of the 6323 survivors (85.3%) participated in the 5‐year follow‐up. Methods: Ocular examinations including standardized refraction were performed according to the same protocol at both baseline and follow‐up. Main Outcome Measures: Change in spherical equivalent (SE; sphere + 1/2 cylinder) and astigmatism. Results: A total of 3970 right eyes were available for refraction analysis. The 5‐year change in SE for all subjects was +0.17 diopters (D), and was −0.21D, +0.14D, +0.40D and +0.08D for subjects aged 30–39, 40–49, 50–59, and 60–69 years, respectively. By binary regression analysis, myopic shift was associated with severe nuclear opacity, longer axil length, diabetes and large change of lens power, while hyperopic shift was associated with older age and ocular hypertension at baseline. There was a mean change of 0.18D in the against‐the‐rule astigmatism. Conclusions and Relevance: There was a myopic shift for those 30–39 years old and a hyperopic shift from 40 to 69 years old in a rural Chinese adult population. Those with severe nuclear opacity, longer axil length, diabetes and large change of lens power tended to have a myopic shift, while those being older and having ocular hypertension at baseline were prone to have a hyperopic shift. Abstract: Purpose: To learn more about the perceptions and attitudes toward eye care of members of the public — in particular, their knowledge of the purpose, components and providers of eye examinations, their ability to self‐monitor ocular status and their awareness of major sight threatening eye conditions. Methods: This study used the qualitative research strategy, grounded theory. Participants were recruited via poster and social media. Two researchers, one with optometric knowledge and one without, moderated seven 60‐ to 90‐min, semi‐structured focus group discussions, which were audiotaped and transcribed. Participants also completed a short demographic questionnaire. Three researchers employed constant comparative coding strategies to identify common themes within the focus group transcripts. The number of participants represented by identified themes were tracked and themes were listed in order of decreasing frequency. Results: Focus groups included 25 participants (nine male, 16 female), ranging in age from 18 to 71 years (mean: 41.7). Five themes related to eye care awareness were identified: eye examination purpose, test procedure identification, eye care professional roles, asymptomatic eye disease awareness, and significant eye disease awareness. Perceived eye examination purposes were vision/prescription priority with some eye health knowledge, comprehensive evaluation of visual system, and vision/prescription only. Most participants who responded could correctly identify the use of an eye chart and a phoroptor; fewer were able to do the same for a direct ophthalmoscope and slit lamp biomicroscope. Less than a quarter of participants could accurately identify the roles of all three major eye care providers. Trauma was the most commonly mentioned risk for vision loss, followed by diabetes and infection. Participants’ knowledge appeared most often to have been obtained from personal experience rather than as the result of any systematic educational initiative. Conclusions: This study found notable gaps in knowledge of eye care and sight risks. If these gaps result in fewer eye examinations, they potentially contribute to increased risk of vision loss due to later stage detection. The results provide motivation for further study and development of effective public health education strategies. Background To assess the proportion of Australian Indigenous adults who require eye care services (separately among those with and without diabetes) and determine implications for eye care service planning. Design The National Indigenous Eye Health Survey ( NIEHS) was a population-based study of 30 randomly selected geographical areas. Participants The NIEHS included 1189 Indigenous adults aged 40-80 years. Methods A standardized eye examination was performed. Main Outcome Measure The number requiring eye care services by diabetes status. Results Those with diabetes were older (median 53 years) than those without diabetes (median age 50 years), P < 0.001. The total estimated population-based need for annual eye care in the NIEHS population was 52% ( n = 615), and of those 72% were people with diabetes. Among those with diabetes, 29% required further primary referral for diabetic retinopathy, 12% for cataract, 1% for trachomatous trichiasis and 5% for uncorrected distance refractive error. Among those without diabetes 13% required further primary referral for cataract, 0% for trachomatous trichiasis and 5% for uncorrected distance refractive error. Conclusion This study has shown that among Indigenous adults, those with diabetes form 72% of those requiring an eye examination in any year. A key strategy to close the gap for vision for Australia's Indigenous population is to ensure those with diabetes undergo annual eye screening, have clearly defined care pathways and receive timely treatment. Abstract: Importance: National data on eye health‐care service utilization will inform Australia's eye health policy. Background: To investigate the utilization of eye health‐care services by Australians. Design: Cross‐sectional survey. Participants: Indigenous Australians aged 40 years and older and non‐Indigenous Australians aged 50 years and older. Methods: One thousand seven hundred thirty‐eight Indigenous Australians and 3098 non‐Indigenous Australians were recruited from 30 randomly selected sites, stratified by remoteness. Sociodemographic, ocular history and eye health‐care service utilization data were collected, and an eye examination was conducted. Main outcome measures: Recentness of eye examinations, types of providers used and associated risk factors. Results: Approximately 67.0% of Indigenous Australians and 82.5% of non‐Indigenous Australians underwent an eye examination within the previous 2 years. Indigenous status (P < 0.001), male gender (P < 0.001), Outer Regional (P < 0.001) and Very Remote (P < 0.001) residence were associated with less recent examinations. Participants with >self‐reported eye disease or diabetes were most likely to have been examined within the past year (P < 0.001). For Indigenous Australians, older age was associated with recent eye testing (P = 0.001). Those with retinal disease and cataract were more likely to see an ophthalmologist (P < 0.001), and those with refractive error were more likely to see an optometrist (P < 0.001). In Regional Australia, non‐Indigenouspeople were more likely to see optometrists (P < 0.001), and Indigenous Australians were more likely to utilize other, non‐specialistservices (P < 0.001). Conclusions and relevance: Eye examination frequency has improved in Indigenous and non‐Indigenous Australians compared with previous population‐based research. The article presents a brief report of the results of an equity profile conducted for a semi-rural, relatively affluent population to establish the differences in eye health care and enhanced optometry scheme. The study involved 2,794 patients in the CHANGES glaucoma referral refinement scheme between August 200-June 2011. It finds that equity profiling is a quick and inexpensive way for health professionals and commissioners to understand and meet the needs of these patients.
