A drug allergy passport should at least contain information on the culprit drug(s) including international nonproprietary name, clinical manifestations including severity, diagnostic measures, potential cross-reactivity, alternative drugs to prescribe, and where more detailed information can be obtained from the issuer. It should be given to patients only after full allergy workup. In 2009, through Court Order 004, the Colombian Constitutional Court declared that thirty‐four separate groups of Indigenous peoples were at risk of extermination due to armed conflict, and it ordered the government to protect them. In 2013, the court then ordered the government to prove its compliance with Order 004. The government did so through a complex documentation process. The production of documents in Colombia not only records compliance but also constitutes compliance even if the actions are limited. In this article, I examine the role of documentation in mediating legal protection in a violent context. How does a governmental report obscure a persisting threat that is mostly ignored? Using participant observation at the Colombian Interior Ministry on the government's response, I illustrate how this documentation converts ordinary governmental actions into restitution of rights and manufactures compliance via the display—and concealment—of numbers through a focus on aesthetics. This generates the effect of sufficient state response while obfuscating the failure to achieve any intended changes: a phenomenon that I term complying incompliantly. Transforming negligence into a proper response through a paper trail outsmarts auditing and maintains the state's legitimacy by normalizing the government's shortcomings. The data paper, an emerging scholarly genre, describes research data sets and is intended to bridge the gap between the publication of research data and scientific articles. Research examining how data papers report data events, such as data transactions and manipulations, is limited. The research reported on in this article addresses this limitation and investigated how data events are inscribed in data papers. A content analysis was conducted examining the full texts of 82 data papers, drawn from the curated list of data papers connected to the Global Biodiversity Information Facility. Data events recorded for each paper were organized into a set of 17 categories. Many of these categories are described together in the same sentence, which indicates the messiness of data events in the laboratory space. The findings challenge the degrees to which data papers are a distinct genre compared to research articles and they describe data‐centric research processes in a through way. The amount of scientific papers is growing together with the development of science itself; but, although there is an unprecedented availability of large citation indexes, some daily activities of researchers remain time‐consuming and poorly supported. In this paper, we present Visual Bibliographies (VisualBib), a real‐time visual platform, designed using a zz‐structure‐based model for linking metadata and a narrative, visual approach for showing bibliographies. VisualBib represents a usable, advanced, and visual tool, which simplifies the management of bibliographies, supports a core set of bibliographic tasks, and helps researchers during complex analyses on scientific bibliographies. We present the variety of metadata formats and visualization methods, proposing two use case scenarios. The maturity of the system implementation allowed us two studies, for evaluating both the effectiveness of VisualBib in providing answers to specific data analysis tasks and to support experienced users during real‐life uses. Application programming interface (API) is an important form of software reuse. API documentations, such as API specifications, tutorials, and online forums, are valuable learning resources for reusing the APIs. In recent years, many data‐driven API documentation mining (ADM) methods have been proposed. These methods mine API documentations and return API‐related information to help developers better understand and reuse APIs. These methods treat documentations as unstructured data and apply various data mining techniques to analyze the documentation data. Currently, there is no comprehensive review of the data‐driven approach to API documentation mining. This review aims to fill in this gap by analyzing and discussing the state of the art ADM papers. We survey 32 representative papers published in prominent software engineering journals and conferences in recent 5 years (January 2014–July 2019). We analyze their mining tasks, mined data, problems, data mining techniques, and evaluation metrics. Based on the survey results, we point out research challenges and future research directions in this area. Aim To develop an Australian nursing documentation in aged care ( Quality of Australian Nursing Documentation in Aged Care ( QANDAC)) instrument to measure the quality of paper-based and electronic resident records. Methods The instrument was based on the nursing process model and on three attributes of documentation quality identified in a systematic review. The development process involved five phases following approaches to designing criterion-referenced measures. The face and content validities and the inter-rater reliability of the instrument were estimated using a focus group approach and consensus model. Results The instrument contains 34 questions in three sections: completion of nursing history and assessment, description of care process and meeting the requirements of data entry. Estimates of the validity and inter-rater reliability of the instrument gave satisfactory results. Background: Pelvic pain has cognitive, behavioral, sexual, and emotional consequences. Nurses involved in pelvic floor rehabilitation clinics have contacts with patients reporting chronic pain and should know the most appropriate service for patient referral, to submit the problem to professionals capable of correctly assessing and managing the condition. Furthermore, in some countries nurses can use conservative methods to treat the painful symptoms inside a multidisciplinary team such as breathing retraining, biofeedback, and noninvasive neuromodulation. This paper aims to provide an overview of the literature regarding the role of rehabilitation nurses in dealing with patients suffering from chronic urogenital pelvic pain or urogenital painful syndromes, inside a multidisciplinary team. Methods: Scoping review on Pubmed, CINAHL, Embase, Scopus, Web of Science including trials, reviews, case studies or series, and other descriptive studies regarding the role of nurses inside the multidisciplinary team in the management of males and females presenting chronic pelvic pain (CPP) or chronic pelvic pain syndrome (CPPS). Results: The 36 papers included in this review allowed answering research questions in four areas of nursing: collecting basic information, referring the person to appropriate services, evidence‐based nursing interventions for CPP and CPPS, and proper documentation. Clinical history and assessment of breathing pattern, Muscular assessment and research of trigger points are the main points of data collection. Techniques for muscular relaxation and breathing retraining are important aspects of treatment, as well as biofeedback and noninvasive neuromodulation where the law allows nurses to practice such techniques. The McGill pain questionnaire and the pain inventory of the International Pain Society allow systematic data collection and handover. Conclusion: Rehabilitation nurses work inside multidisciplinary teams when dealing with persons suffering from pelvic pain; further research is needed as our comprehension of the underlying pathophysiological mechanisms of CPP and CPPS evolve. Background: With the growing integration of technology in education, the adoption of blended learning (b‐learning) has gained attention. B‐learning combines traditional classroom teaching with online components, holding potential to enhance student outcomes and educational efficiency. Yet, current research predominantly concentrates on higher education institutions in urban areas, creating a void in understanding its impact on K‐12 education, particularly in rural settings. Objectives: This qualitative study aims to propose recommendations for successful implementation of blended learning in rural K‐12 areas by addressing the key stakeholders influencing its adoption and identifying the main factors affecting its success. Methods: We performed a content analysis of grey literature documents detailing the implementation of b‐learning in K‐12 education. Additionally, interviews with crucial stakeholders such as teachers, principals, and experts in rural schools in South America provide insights into the challenges and prospects of b‐learning adoption in these contexts. Results and Conclusions: The study identifies pivotal stakeholders for effective b‐learning implementation, outlining their roles and addressing challenges inherent in rural settings. Recommendations for enhancing b‐learning's implementation in developing countries are also proposed. Background: A gap in vancomycin therapeutic drug monitoring (TDM) electronic documentation, management, and related clinical pharmacist activities was identified by our clinical pharmacy team. In response, an electronic TDM (eTDM) template was planned, designed, and implemented by the pharmacy department team in collaboration with the electronic medical records team (EMR) and documentation committee. Aim: To assess the impact of an electronic therapeutic drug monitoring (eTDM) template to measure the impact of adherence to vancomycin guidelines. Guideline adherence indicators include the number of vancomycin levels within the therapeutic range and the number of appropriate concentrations taken. Method: An interventional, single‐centre study of vancomycin therapy in adult patients was performed from 2019–2021. Data were extracted from the electronic medical records and TDM paper‐based forms completed by clinical pharmacists. The number of concentrations within the therapeutic range and the number of appropriately taken levels were analysed by the chi‐square test. This study received an exemption from the Peninsula Health Human Research Ethics Commitee (QA/60523/PH‐2019/197291). Results: There was a total of 198 concentrations collected in the 'before' period and 125 concentrations collected in the 'after' period. The number of concentrations in therapeutic range increased from 34.8% to 43.2% (p = 0.132), not statistically significant. The number of concentrations taken appropriately increased from 33.8% to 55.2% (p < 0.001). The proportion of patients with pharmacist involvement increased from 43.0% to 57.0% (p = 0.868). Conclusion: The study shows that implementation of an electronic vancomycin monitoring template improved the proportion of appropriate levels taken and is the preferred method of documentation by clinical pharmacists. Future projects may benefit from analysing the cost associated with unnecessary vancomycin serum levels ordered or inclusion of pharmacists in the TDM of other narrow therapeutic medications. This study presents and explains the phenomenon of indexjacking, which involves the systematic infiltration of hijacked journals into international indexing databases, with Scopus being one of the most infiltrated among these databases. Through an analysis of known lists of hijacked journals, the study identified at least 67 hijacked journals that have penetrated Scopus since 2013. Of these, 33 journals indexed unauthorized content in Scopus and 23 compromised the homepage link in the journal's profile, while 11 did both. As of September 2023, 41 hijacked journals are still compromising the data of legitimate journals in Scopus. The presence of hijacked journals in Scopus is a challenge for scientific integrity due to the legitimization of unreliable papers that have not undergone peer review and compromises the quality of the Scopus database. The presence of hijacked journals in Scopus has far‐reaching effects. Papers published in these journals may be cited, and unauthorized content from these journals in Scopus is thus imported into other databases, including ORCID and the WHO COVID‐19 Research Database. This poses a particular challenge for research evaluation in those countries, where cloned versions of approved journals may be used to acquire publications and verifying their authenticity can be difficult. The paper reviews the evolution of Interactive Management, later referred to as Structured Democratic Dialogue, starting from the early 1970s up to this date. The authors propose a generational classification scheme consisting of five periods based primarily on whether some or all stages of the process were implemented synchronously or asynchronously and whether the participants' presence was physical, virtual or hybrid. Other aspects such as modifications in the steps of the process; the evolution of the software; domains of applications; file management; methods of collecting or recording contributions, votes, clarifications and preparation of reports; and key players are also considered and reported within the context of the primary scheme. The paper considers key advances achieved at each generational stage in terms of process or software, discusses associated challenges and concludes with a view towards the future of the emerging fifth generation. Background Study: The research discusses the need for a paradigm shift in engineering education current practices to accommodate the digital native students. The paper emphasizes the importance of integrating disruptive technologies, namely Virtual Reality (VR) through Head Mounted Displays VR (HMD VR) and Desktop Based VR (DB VR) and comparing it with the traditional teaching methods. The research focuses on second‐year undergraduate students enrolled in a concrete structures course. Objectives: The objective of the study is to analyse the impact of DB VR and HMD VR on students' motivation, performance and workload in comparison with traditional 2D drawings. The study also aims to investigate how simulation sickness and system usability could impact students' motivation and performance when interpreting concrete structures. Methods: The study divided second‐year civil engineering students into three groups, 2D, DB VR and HMD VR. The performance and motivation levels of students were assessed based on the number of mistakes identified and an intrinsic motivation survey. Furthermore, data on system usability and simulation sickness were collected. Spearman rank correlation was conducted to analyse the relationships between the variables tested. Results: The results have shown that students' motivation and performance were much higher when using VR in general and HMD VR specifically, students' efficiency and accuracy were also observed. On the other hand, usability was found to be moderately correlated with motivation, with students rating HMD VR as the most usable approach. A moderate inverse correlation was observed between simulation sickness and motivation, indicating that higher simulation sickness led to lower motivation levels. Conclusion: The study concludes that integrating VR in general, DB VR, or HMD VR into a concrete structures course for undergraduate students can significantly improve students' performance and motivation. Furthermore, enhancing usability of any of the methods used can also improve the performance and motivation. On the other hand, simulation sickness inversely affects motivation and performance, therefore, VR developers and educators should not neglect it. This paper tests the applicability of an unmanned aerial system (UAS) in obtaining spatial data from archaeological sites in forested areas. Our case study discusses the remains of the anti‐Turkish fortifications called 'Turške Šance'—the defensive ditches that are preserved in the area of the Preški vrh (Ravne na Koroškem, Slovenia). Up to 10 fortifications of various shapes and sizes were built in the last quarter of the 15th century. Today, they represent a unique archaeological site. A detailed geodetic survey of the site was performed and compared with results of photogrammetry using the DJI Phantom 4 PRO UAS drone and DJI Phantom camera. Furthermore, official Slovenian LiDAR data of the area were obtained, and archaeologic field surveys, in situ inspections and metal detector scanning were made. The points obtained from the total station were used for 3D modelling and taken as a reference. Drone photogrammetry was performed once per week to compare each point cloud with another using CloudCompare and to the reference one depending on the volume of the tree leaves which was estimated with green leaf index (GLI) and leaf area index (LAI). The increase of the tree leaves volume deteriorates the point cloud obtained with a UAS. The paper analyses how significant this influence is. This paper discusses the evolution of documentary culture in early medieval Tuscia by quantitatively examining the Latin spelling of charter scribes in relation to the following factors: time, the distinction between the formulaic and non‐formulaic parts of the document, the scribe's domicile, the scribe's professional status, and the document type. The paper asks what the spelling of charters tells us about administrative and socio‐cultural changes in charter production and in scribal education. Background: The Joint Commission lists improving staff communication (handoffs) as part of several National Safety Goals. In this study, we developed an electronic web‐based charting system for clinical pathology handoffs, which primarily consist of transfusion medicine calls, and evaluated the advantages over a paper‐based handwritten call log. Materials and Methods: A secure online web browser application using Research Electronic Data Capture (REDCap) was designed to document on‐call pathology resident consults. A year after implementation, an online survey was administered to our pathology residents in order to evaluate and compare the usability of the electronic application (e‐consults) to the previous handwritten call log, which was a notebook where trainees hand wrote different components of the consult. Results: The REDCap web‐based application includes discrete fields for patients' information, requesting physician contact, type of consult, action items for follow‐up and faculty responses, as well as other information. These components have eventually progressed to be an online consult call catalog. With approximately 1079 consults per year, transfusion medicine‐related calls account for ~90% of the encounters, while clinical chemistry, microbiology and immunology calls constitute the remainder. The overall response rate of the survey was 96% (29 of 30 participants). Of the 16 respondents who experienced both call log systems, 100% responded that REDCap was an improvement over the handwritten call log (P < 0·0001). Conclusion: E‐consult documentation entered into a web‐based application was a user‐friendly, secure clinical information access and effective handoff system as compared to a paper‐based handwritten call log. The safety of practitioners working in the area of domestic abuse is a current subject of research and practice concern, as services endeavour to better understand constantly evolving tactics of violence and abuse and respond appropriately. This paper reports on a subset of findings from a practice‐led research project focussed on capacity‐building workers and their organizations to address domestic abuse, which highlighted the ongoing concerns expressed by practitioners—particularly female practitioners—about their own physical, emotional, and professional safety. Ethnographic notes were taken of Community of Practice discussions in four regions of New South Wales, Australia, involving 69 senior health practitioners from a range of service streams: specialist domestic abuse, mental health, substance use, and child protection. Themes identified through thematic analysis included the following: a tailored approach to practising safely; attending to physical safety; threats to professional identity; promoting emotional well‐being; documentation to support worker safety; and attending to cultural safety. These themes are encapsulated in the practitioner question: Who's got my back?—highlighting the importance of legislation, policy, and practices that provide an environment in which safety and support are embedded in a culture of care at every organizational level. Introduction: Systematic reviews are considered the highest level of evidence that can help guide evidence‐informed decisions in nursing practice, education, and even health policy. Systematic review publications have increased from a sporadic few in 1980s to more than 10,000 systematic reviews published every year and around 30,000 registered in prospective registries. Methods: A cross‐sectional design and a variety of data sources were triangulated to identify the journals from which systematic reviews would be evaluated for adherence to Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) 2020 reporting guidelines and scope. Specifically, this study used the PRISMA 2020 reporting guidelines to assess the reporting of the introduction, methods, information sources and search strategy, study selection process, quality/bias assessments, and results and discussion aspects of the included systematic reviews. Results: Upon review of the 215 systematic reviews published in 10 top‐tier journals in the field of nursing in 2019 and 2020, this study identified several opportunities to improve the reporting of systematic reviews in the context of the 2020 PRISMA statement. Areas of priority for reporting include the following key areas: (1) information sources, (2) search strategies, (3) study selection process, (4) bias reporting, (5) explicit discussion of the implications to policy, and lastly, the need for (6) prospective protocol registration. Discussion: The use of the PRISMA 2020 guidelines by authors, peer reviewers, and editors can help to ensure the transparent and detailed reporting of systematic reviews published in the nursing literature. Clinical Relevance: Systematic reviews are considered strong research evidence that can guide evidence‐based practice and even clinical decision‐making. This paper addresses some common methodological and process issues among systematic reviews that can guide clinicians and practitioners to be more critical in appraising research evidence that can shape nursing practice. There is a statutory duty for all practitioners to record information in child protection work in England. Case recording is a daily task for practitioners, yet an under-researched area of practice. This continuing professional development (CPD) paper will consider the context in which case recording takes place and highlight messages from child protection reviews and enquiries before exploring learning from contemporary research. The complexities of case recording will be considered and how practitioners can reflect upon and improve their daily case recording skills. Practitioners are encouraged to keep the child's views and experiences central and consider the long-term impact of case recording on adults who have experienced abuse and neglect in childhood. While there is growing recognition of the importance of meaningful youth participation in activities and decisions which impact their lives, facilitating such engagement within different forums requires varied approaches. A programme of youth research at one Western Australian university has sought to embed pathways for ongoing youth participation in longitudinal research. In particular, we discuss Navigating Through Life, a large‐scale WA research project comprising three sub‐studies exploring young people's transitions from out of home care (OOHC) to independence. We established a youth advisory group to run alongside the project and employed peer researchers to conduct research tasks. In this paper, we seek to explicate the development and various iterations of youth participation in the project. We utilise our reflections, interview data and documentation from YAG meetings and feedback sessions with the peer researchers to illustrate the processes of transformation and flexibility required to develop and maintain youth participation in various ways across the project's life. Drawing on our findings and current literature, we highlight how funding bodies and researchers must consider the additional needs, unique and changeable life circumstances of young people to best facilitate their involvement in research. How do visual methods, particularly the practice of photography, help us to visualise and understand absence in deathscapes? In this paper I will argue that photographs, with their ability to freeze moments, are able to capture what I term points of praxis – moments in which practices by individuals inscribe meaning onto deathscapes, and in that intense and captured presence, evoke the feeling of absent individuals. Such points of praxis can exist in two ways – the praxis of the body and the praxis of objects – both of which I will illuminate with the use of visual ethnographic methods. To support my argument, I present work taken from a three‐year visual ethnographic study of Bukit Brown Cemetery, specifically drawing on the social and cultural documentation of the Hungry Ghost Festival. Background: Phonetic transcription is recognized in regulatory standards as an essential skill for Speech and Language Therapists (SLTs) in the assessment, diagnosis and management of clients with speech difficulties. Previous research has identified that approaches to phonetic transcription vary, and that SLTs often lack confidence in transcribing. However, SLTs' views and working practices have not been investigated in detail, particularly in terms of whole service approaches and following the recent increase in telehealth. Aims: To investigate SLTs' views about phonetic transcription, their working practices at both individual and service levels, and the factors that influence these. Methods & Procedures: A total of 19 SLTs from the UK were recruited to online focus groups via social media and local networks. Participants discussed their views of, and practices in, phonetic transcription. Themes were identified using reflexive thematic analysis. Outcomes & Results: Three broad themes were generated division and unity; one small part of a big job; and fit for purpose. SLTs were uniformly proud of their ability to phonetically transcribe and viewed this as a unique skill, but clear differences existed between different groups of SLTs in their views and practices. Investing in phonetic transcription was not always a priority for SLTs or services, and although many felt under‐confident in their skills they considered these to be adequate for the populations they usually encounter. SLTs make an early judgement about possible therapy targets, which influences the level of detail used in their phonetic transcription. Practical barriers are often not addressed at service level, and assessment via telehealth poses some specific challenges. Conclusions & Implications: SLTs and services would benefit from increased investment in phonetic transcription in terms of time, opportunities for continuing professional development (CPD) and initiatives such as electronic patient records (EPRs) which support the use of phonetic symbols. Identifying target sounds at an early stage raises questions about the implications of disregarding other features of speech, and the selection of appropriate intervention approaches. Further research is needed to analyse actual rather than reported practices, and to consider the relationship between phonetic transcription and intervention approaches. Future studies could also identify precise CPD requirements and evaluate the effectiveness of CPD. What this paper adds: What is already known on the subject: Previous research has demonstrated that SLTs often lack confidence in phonetic transcription and that practices are varied, with relatively little use of narrow transcription. SLTs are interested in opportunities to maintain and develop transcription skills but do not often undertake CPD for transcription. What this paper adds to existing knowledge: By using focus groups as a forum for discussions, this study provides a rich and detailed insight into SLTs' views about clinical transcription and their working practices, with previously unreported details about the reasons for these practices in a clinical context and at a service‐wide level. What are the potential or actual clinical implications of this work?: Transcription is often de‐prioritized in non‐specialist contexts, with practical barriers and a lack of clear and consistent protocols at a whole‐service level. There is an opportunity for service managers to address the systemic difficulties in using transcription effectively by raising the profile and value of transcription amongst clinicians, and promoting CPD opportunities, using the findings of this study as a rationale for funding this. Introduction: People living with dementia experience communication difficulties. Personal information documents, or healthcare passports, enable communication of information essential for the care of a person with dementia. Despite the potential for providing person‐centred care, personal information documents are not ubiquitously used. The Capability Opportunity Motivation—Behaviour (COM‐B) model can be used to understand factors determining individuals' behaviours. Objectives: This study aimed to identify the barriers to and facilitators of the use of healthcare passports for people living with dementia through a systematic review methodology. Methods: A systematic search of six electronic databases was undertaken. Grey literature was searched using three databases. All study types reporting barriers to or facilitators of the use of personal information documents in the care of adults living with dementia in high‐income countries were included. Study quality was assessed using the NICE Quality Appraisal Checklist. Thematic synthesis was used to develop descriptive themes, which were subsequently mapped to the COM‐B framework. Results: Nineteen papers were included. Themes included training, awareness, embedding the process in norms and appreciating the value of the personal information documents. A broad range of barriers and facilitators was identified within each COM‐B domain. Conclusion: This framework provides a starting point for evidence‐informed initiatives to improve the use of personal information documents in the care of people with dementia. Transracial adoption was legalized in South Africa through legislative reform as South Africa moved into a post‐apartheid era of non‐racialism. Transracial adoption offers one option of placement for Black children in need of alternative care. However, adoption continues to face obstacles to implementation, including cultural obstacles which impact on placement of Black African children. This paper reports on adoption statistics on national adoption in the period 2013–2021 and a documentary analysis of the current legal and policy framework governing transracial adoption. We cover international instruments, local laws, including the Children's Act of 2005, and relevant policy documents designed to facilitate the implementation of adoption. We show that the law is supportive of transracial adoption, but that policy documents offer a somewhat contradictory stance to the practice. The lack of success of transracial adoption as a child protection strategy does not lie within the legal framework but may lie in the difficulties in interpreting and implementing the range of different policies by those implementing these policies. More research into the implementation practice of these bureaucrats is required to understand the underutilization of transracial adoption as a placement option for Black children in South Africa. Purpose: The purpose of this paper is to describe the implementation of evidence‐based, individualized coping plans and to compare the rates of child anxiety, child cooperation, and parent anxiety before and after implementation of these coping plans for children receiving recurring BoNT‐A injections emphasizing the role of Certified Child Life Specialists (CCLSs) within the interdisciplinary team approach. Design and Methods: A retrospective chart review was conducted for children receiving BoNT‐A injections at a Physical Medicine clinic pre‐ and post‐implementation of the coping plan. Descriptive statistics were used to evaluate care plan implementation, child cooperation, child anxiety, and parent anxiety. Results: Post‐implementation of coping care plans, children experienced improved cooperation during BoNT‐A injections. Parental anxiety decreased once coping plans were implemented. Documentation improved after the implementation of coping plans specific to areas involving cooperation, child's anxiety and distress, and parental anxiety and distress. It was also noted that there was an increase in child anxiety assessments within documentation. Practice Implications: CCLS were able to utilize their expertise to collaboratively create individualized coping care plans to increase child's cooperation and decrease parental anxiety during BoNT‐A injections. Rationale, aims and objectives Structuring the diagnostic section of the medical record ( MR) improves diagnosis and communication between doctors. However, little is known about the therapeutic section of the MR. The aim of this study was to gain insight into the extent to which MRs are structured for therapeutic information, to determine which therapeutic data registrars and clinical consultants consider should be recorded in the MR and to what extent registrars record this information themselves. Methods A multicentre observational study was carried out in the internal medicine outpatient clinics of five teaching hospitals in the Netherlands. Preformatted structure, importance and actual recording of therapeutic information was compared with a reference list of 35 therapeutic items based on the WHO Guide to Good Prescribing (e.g. drug name, indication for drug). Results The preformatted structure of four paper MRs and one electronic MR was assessed. Eight of the 35 therapeutic items were listed in the paper MRs and 18 items in the electronic MR. Registrars and consultants agreed on the importance of recording most of the therapeutic items in the MR, 25 and 27 out of the 35 items, respectively; however, registrars recorded only 11 of the 35 items in the paper MR and 20 of the 35 items in the electronic MR. Conclusions The structure and content of paper and electronic MRs are not adequate. While both registrars and consultants agree on the importance of recording therapeutic items in the MR, registrars fail to record most of this information in practice. The results of this study can be used as starting point for the discussion regarding the necessity of structured recording of therapeutic information in the MR and its possible benefits with regard to medication safety and training of the new generation of prescribers. Introduction: Critically ill patients are at high risk of developing pressure ulcers resulting in serious untoward patient and health care system outcomes. Pressure ulcer prevention is therefore an important patient safety priority and establishing a structured approach to pressure ulcer risk assessment to identify patients at risk is a critical first step. Methods: The literature was searched using three electronic databases from 2000 to 2011 to identify papers reporting on pressure ulcer risk factors and assessment in adult critical care. The review and appraisal of papers were conducted by two critical care nurses. Papers underwent detailed review if they met inclusion criteria where they identified pressure ulcer assessment scores, scales or risk factors and related to adult critical care patients Findings: Seven papers were reviewed. No single assessment tool was sufficiently validated for critically ill patients and seven key critical care risk factors were identified. These risk factors were: mechanical ventilation, impaired circulation, dialysis, long surgery, low protein and too unstable to turn. Tool development: The tool Critical Care Pressure Ulcer Assessment Tool made Easy (CALCULATE) was developed utilizing the risk factors from the literature and expert critical care nursing consensus decision-making. Discussion: In the absence of current consensus, valid assessment scales and limited evidence for the most appropriate pressure ulcer assessment for critically ill patients, this assessment tool offers an easy, appropriate alternative for critically ill patients than existing tools primarily validated for acute care wards. Conclusions: 'CALCULATE' offers an important contribution towards the advancement and development of critical care pressure ulcer risk assessment. Future research is needed to further enhance and inform pressure ulcer risk assessment of the critically ill patients. In the European Union Trademark law (EUTM), clear and precise subject‐matter of a trade mark is a prerequisite of proper filing and determines the conditions and scope of protection. Registration rules have evolved together with the meaning of this standard. The paper examines the legal status of position marks, which lacked their own registration specifics within initial EUTM, and thus were usually filed through other categories (figurative, colour, three‐dimensional), and contrasts this practice with the registration scheme of the new law. The analysis looks into the effects of Court of Justice's guidance when retroactively applied to trade marks registered upon earlier law. The exemplary case of a blade of grass inside a bottle is used to map the meaning of 'unaligned' description and/or indication vis‐à‐vis the prime relevance of (graphic) representation. The final part explores how identifying the subject‐matter of a position mark has an impact on assessing its distinctiveness, engaging within functionality objections and proving genuine use. The paper advocates for refraining from applying a fixed understanding of the requirement of clear and precise subject‐matter, especially with regard to old registrations, and, a fortiori, to flexible subject‐matter, such as position signs. To a great extent, the effectiveness of a government may be traced to its constituting documents which serve as a guide or map for how it is supposed to function. From a systems perspective, we may expect a more systemic Constitution to inform the operation of a government that is more systemic and so more effective. This paper uses Integrative Propositional Analysis (IPA) to conduct a preliminary evaluation of the systemic structure of the US Constitution. The analysis finds that the US Constitution has a vanishingly low level of structure. This previously unrecognized lack of structure provides a new explanation for why the nation is increasingly misgoverned (and/or more difficult to govern) and why divisions are deepening between partisan groups, leading to argument and conflict. A path forward is suggested to develop a more systemic Constitution and more effective governance; one supporting a more just, prosperous, and sustainable society. [ABSTRACT FROM AUTHOR]

Background: An innovative and interactive real‐world environment can be presented with augmented reality (AR) that comprises digital visual elements, audio, or other sensory information delivered via technology to enhance one's experience. AR has numerous potential applications in various everyday fields. The education sector is one such arena where AR has been implemented prominently globally. AR in education can impact active students' learning to memorize effectively and can persuade them to engage in a meaningful process of learning. Objectives: This article presents a bibliometric analysis of some recent applications of AR, emphasizing the potential effects, challenges, and trends of AR in the education sector for the past 15 years. Methods: A total of 3909 documents published during 2006–2020 were retrieved from the Scopus database for analysis. The bibliometric analysis included document types, subject categorization, document growth, as well as top contributing sources, countries, authors, and funding sponsors. It also analyses keywords, abstracts, titles, and characteristics of the most cited documents. Additionally, it provides a brief account of the challenges for AR adaptation in education. Results and conclusions: The findings uncover an increased use of AR to promote enhanced learning in the education field. This work will provide the survey, limitations, and drawbacks, as well as open usability and technical issues that may represent new challenges for future research in this field. Background: Current methods of antimicrobial usage surveillance have limited efficacy in changing practice due to delayed reporting to clinicians and the inability to stratify by medical specialty. This study was undertaken in a tertiary teaching hospital with a well established antimicrobial stewardship (AMS) programme and electronic medicines management (eMM) system in Sydney, Australia. Aims: To describe and analyse the implementation of a novel AMS audit and feedback method, in the context of an eMM system. Methods: The AMS team conducted the audit weekly, and the study design was a prospective, observational study. All acute, adult inpatients were included in this intervention. All active systemic antimicrobial prescriptions on the day of the rounds were included. Results: The prevalence of patients on antimicrobial therapy was 37%. The median time taken per round was 44 min for eMM compared to 58 min for paper. All key performance indicators improved over the study period. Appropriateness compared to guidelines increased from 55% to 71%, and documentation of an indication increased from 75% to 98%. There were 1413 recommendations made, with the most common being to cease an antimicrobial agent. The recommendation uptake rate was 47% at 24 h post-round. Conclusions: AMS rounds are an effective tool for auditing and providing feedback on antimicrobial use and should include all antimicrobials rather than solely 'restricted' agents. These rounds had a high uptake rate, improvements in the appropriateness of antimicrobial use, and a planned duration or review date. Background: A lack of standardisation of documentation accompanying older people when transferring from residential to acute care is common and this may result in gaps in information and in care for older people. In Ireland, this lack of standardisation prompted the development of an evidence based national transfer document. Objectives: To pilot a new national transfer document for use when transferring older people from residential to acute care and obtain the perceptions of its use from staff in residential and acute care settings. Methods: This was a pre‐ and post‐study design using purposive sampling following the STROBE guidelines. The pilot was conducted in 26 sites providing residential care and three university hospitals providing acute care. Pre‐pilot questionnaires focused on current documentation and were distributed to staff in residential care (n = 875). A pilot of the new paper‐based transfer document was then conducted over three months and post‐pilot questionnaires distributed to staff from both residential and acute care settings (n = 1085). The findings of the pilot study were discussed with multidisciplinary expert advisory and stakeholder groups who recommended some revisions. This consensus informed the development of the final design of the new revised transfer document. Results: Pre‐pilot: 23% response rate; 83% (n = 168) participants agreed/strongly agreed that existing documentation was straightforward to complete but could be more person‐centred. Post‐pilot: 11% response rate; 75% (n = 93) of participants agreed/strongly agreed that the new transfer document promoted person‐centred care but recommended revisions to the new document regarding layout and time to complete. Conclusions: This study highlighted some of the challenges of providing safe, effective and relevant transfer information that is feasible and usable in everyday practice. Implications for practice: Standardisation and being person‐centred are important determining factors in the provision of relevant up to date information on the resident being transferred. Aim: The aim of the paper is to compare the quality of nursing documentation in the Children's Hospital before and after the NANDA‐I nursing diagnoses training. Methods: Research employed the interventional study design, and pre–post study design. Before and after the NANDA‐I nursing diagnoses training, 50 nursing records were analyzed in the interventional pre–post study, using D‐Catch instrument. Results: The most often documented problem‐centered nursing diagnosis before training was anxiety and after the training, hyperthermia. The most common risk diagnoses before and after the training was risk of infection. Before the training, one health promotion diagnosis was determined in the nursing records, and after the training the number increased to four. The highest value was given to readability of the nursing documentation both before and after the training. The lowest score before the training was given to the quality determiners of the accurate nursing diagnoses and after the training given to the determiners of the results' quantity. The sum score of documenting the nursing interventions was the most inconsistent before the training and after the training. The most consistent was the readability of the nursing records before and after the training. Statistically significant differences in the improvement of quality were revealed in all areas except for the readability of the nursing documentation and the quantity of nursing assessment. Conclusions: The results of the study revealed that following the training, the quality of nursing documentation improved, the wording of the nursing diagnoses improved, and the number of accurate nursing diagnoses had increased. Implications for Nursing Practice: Results of the research provide an overview of the importance of the training in improving the quality of nursing documentation and aid the educators in planning the trainings, focusing more on the challenges in the documentation. Detailed and accurate documentation of the reagents, tools and methods used in a study is key for reproducible science. However, the information provided in the Materials and Methods section is not always sufficiently detailed to allow for the adoption of methodologies across laboratories. Substantial time and effort, as well as extensive correspondence with the authors of a published paper, is often required in order to obtain all the relevant information related to a particular technique. Even after following a trail of references that frequently lead to a paper published decades ago, it is sometimes impossible to find a sufficiently detailed description of a technique “performed as described before”. This article draws on 2 years' worth of ethnographic observation of team meetings to explore decision‐making in an NHS clinical genomics service. The focus of discussions was on ambiguous genomic results known as VUS or Variants of Uncertain Significance, which may be pathogenic but which also may turn out to be benign. In examining decision‐making around such results, we note how, in contrast to much policy and promotional material in this area, clinicians in these meetings (clinical geneticists and genetic counsellors) place great emphasis on parental phenotypes and whether the parents of a patient share the symptoms and signs of the suspected condition. This information is then combined with the result of genomic tests to decide whether the variant a patient has is responsible for their condition. This article explores the way in which clinicians attempt to flexibly enrol parents into genomic explanations through informal diagnosis of their possible phenotypes and the way in which actually meeting parents allows some clinicians to trump explanations based on documentary or photographic data. The paper sheds light on the way that earlier scholarly understandings of such decisions (around, say dysmorphology) remain relevant and explores claims that laboratory tests overrule clinical decision‐making. Introduction: Deaths in care homes and "at home" are anticipated to account for a third of UK deaths by 2040. Currently, palliative and end of life care are not part of statutory training in care homes. Reflective practice is a tool that can facilitate practice‐based learning and support. Following a feasibly study to test "online" supportive conversations and reflection sessions (OSCaRS) to support care home staff in relation to death/dying during the first months of the COVID pandemic, a one‐year practice development follow‐up project was undertaken with the aim to create a team of NHS/specialist palliative care (SPC)‐based facilitators to lead and support OSCaRS provision in up to 50 care homes in one region in Scotland—the focus of this paper. Methods: Forty care home managers attended an on‐line session explaining the project, with a similar session held for 19 NHS/SPC‐based nurses external to care homes. Those interested in facilitating OSCaRS then attended three education sessions. Data collected: records of all activities; reflective notes on OSCaRS organised/delivered; a summary of each OSCaRS reflection/learning points; final interviews with NHS/SPC trainee facilitators. Results: A total of 19 NHS/SPC facilitators delivered one or more OSCaRS in 22 participating care homes. However, as of January 2022 only six trained facilitators remained active. Out of the 158 OSCaRS arranged, 96 took place with a total of 262 staff attending. There were three important aspects that emerged: the role, remit, and resources of NHS/SPC supporting OSCaRS; requirements within care homes for establishing OSCaRS; and, the practice‐based learning topics discussed at each OSCaRS. Conclusion: Attempts to establish a team of NHS/SPC facilitators to lead OSCaRS highlights that end of life care education in care homes does not clearly fall within the contractual remit of either group or risks being missed due to more pressing priorities. Records are persistent representations of activities created by partakers, observers, or their authorized proxies. People are generally willing to trust vital records such as birth, death, and marriage certificates. However, conspiracy theories and other misinformation may negatively impact perceptions of such documents, particularly when they are associated with a significant person or event. This paper explores the relationship between archival records and trustworthiness by reporting results of a survey that asked genealogists about their perceptions of 44th U.S. President Barack Obama's birth certificate, which was then at the center of the "birtherism" conspiracy. We found that although most participants perceived the birth certificate as trustworthy, others engaged in a biased review, considering it not trustworthy because of the news and politics surrounding it. These findings suggest that a conspiracy theory can act as a moderating variable that undermines the efficacy of normal or recommended practices and procedures for evaluating online information such as birth certificates. We provide recommendations and propose strategies for archivists to disseminate correct information to counteract the spread of misinformation about the authenticity of vital records, and we discuss future directions for research. Midwives and nurses play a critical role in safeguarding the lives of women in resource‐constrained African countries. Working within the context of scarce resources may undermine their moral agency and hinder their ability to care. The purpose of this paper is to understand the influence of resource scarcity on midwifery and nursing care and practice. A critical ethnography was conducted in the obstetric department of three tertiary‐level facilities in Ghana. Purposive sampling was used to recruit 30 midwives and nurses and semistructured interviews, field notes and documentary materials were used to generate in‐depth understanding. Ethical approval was granted from Canada and Ghana and written, and ongoing informed consent was obtained from the participants. Five conceptual themes depicting the impact of scarce resources on midwifery and nursing care were discovered: compromised care, constrained care, dehumanized care, missed care and disengaged care. Improving the maternal health of women and averting avoidable maternal morbidity and mortality require governments and institutions to invest in health infrastructure that will support the delivery of ethical and safe midwifery care for women in their most vulnerable period. In this paper, we report findings from a qualitative, multidisciplinary analysis of 20 Child Practice Reviews (CPRs) (previously Serious Case Review) in Wales. The reviews were analysed from three different disciplinary perspectives: law, criminology and practice (social work). The following cross‐cutting themes were identified from our coding of the reviews: (i) hierarchy of knowledge, where certain sources of knowledge were privileged over others; (ii) information sharing/recording, where deficiencies of sharing or recording of information were evident; (iii) partial assessment, where certain assessments were not always holistic; and lastly, (iv) voice of the child, where the experience or perspective of the child was not always considered. In addition, our analysis revealed a range of challenges to the CPR process as implemented. This paper is the first to explore themes emerging from (Welsh) CPRs and is also the first to do so from a multidisciplinary perspective. We conclude by highlighting the recommendations for practice, such as the need for greater learning pan‐Wales, regular publication of the major themes emerging from CPRs and the need for deposit of CPRs in a central repository to facilitate learning from reviews. Background: There is very little awareness of the speech, language and communication needs (SLCN) of rough sleepers. The small amount of documentation that does exist involves a wider group of homeless adults (not just rough sleepers), and reports that communication needs are an area of concern. Aims: To investigate: (1) the reported prevalence of SLCN amongst UK nationals recorded on the Combined Homeless and Information Network (CHAIN) as sleeping on the streets of London; (2) whether rough sleepers with reported SLCN differ from those without; and (3) what factors best predict patterns of rough sleeping and accommodation stays. Methods & Procedures: A data set of 513 participants was provided by CHAIN, which contained information relating to all new rough sleepers and people with long‐term histories of rough sleeping (UK nationals only) recorded by street outreach teams in London between 1 April and 30 June 2013. Also included was data about UK nationals provided with support by the Homelessness and Brain Injury Project. The data set contained information including basic demographics, communication skills, health and social care needs, and institutional background and extended to a 5‐year period. Outcomes & Results: (1) SLCN data were often not recorded with data available for only 62% of individuals on the CHAIN databases. However, for those with SLCN data, the prevalence of SLCN was significantly higher than for the general population (17.1%; p < 0.001). (2) There were no significant differences between those with and without SLCN on additional risk factors, quarters rough sleeping, accommodation stays or staff‐recorded alerts. (3) There was a positive correlation between rough sleeping and additional risk factors for those with SLCN (r = 0.32, p < 0.001) and for those without (r = 0.25, p < 0.001). Regression analysis indicated that additional risk factors were more predictive than SLCN in explaining the number of quarters rough sleeping and accommodation stays. Conclusions & Implications: SLCN are highly prevalent amongst rough sleepers and significantly greater than for the UK general population. SLCN are not clearly related to rough sleeping behaviour, but the presence of additional risk factors is highly significant in this regard. Homelessness organizations should provide training for staff in SLCN in order to promote better recording of SLCN, inclusive communication and appropriate support to people who are homeless. Background: Paperwork is a key tool that transforms organizational intentions into actions in group homes, although prescriptive procedures may limit how frontline staff use it in practice. The aim of this study was to explore how frontline staff use paperwork in group homes for people with intellectual disability and identify practice implications. Method: Constructivist grounded theory methodology guided the research. Data collection included semi‐structured interviews and participant observations. Coding, comparison and sorting methods were adopted to analyse how staff used paperwork. Results: Staff followed organizational paperwork rules when they aligned with their resident‐focused approach to work. When they perceived rules to misalign with this approach, they managed paperwork by adjusting the time and place of completion, managing content, creating alternative tools and refusing completion. Conclusions: Staff purposefully managed paperwork rather than simply following procedures. Disability service organizations could develop flexible paperwork procedures and include frontline perspectives in paperwork development. [ABSTRACT FROM AUTHOR]

Reverse engineering of sequence diagrams refers to the process of extracting meaningful information about the behavior of software systems in the form of appropriately generated sequence diagrams. This process has become a practical method for retrieving the behavior of software systems, primarily those with inadequate documentation. Various approaches have been proposed in the literature to produce from a given system a series of interactions that can be used for different purposes. The reason for such diversity of approaches is the need to offer sequence diagrams that can cater for the users' specific goals and needs, which can vary widely depending on the users' perception and understandability of visual representations and the target application domains. In this paper, we systematically review existing techniques in this context while focusing on their distinct purposes and potentials of providing more understandable sequence diagrams. In addition, a qualitative evaluation of such techniques is conducted to expose their adequacy and applicability for effective program comprehension. Finally, we list a set of possible extensions to the unified modeling language sequence diagram standard that we anticipate will enhance its versatility and understandability of program control flow, followed by a number of concluding remarks. Background: Person‐centred care is a growing imperative in healthcare, but the documentation of person‐centred care is challenging. According to the Gothenburg Framework of Person‐centred Care, care should be documented in continuously revised care plans and based on patients' personally formulated goals and resources to secure a continuous partnership. Objectives: This study aimed to examine care plans produced within a randomised controlled trial that tested a person‐centred care intervention in older people with acute coronary syndrome. Nurses with training in the theory and practice of person‐centred care had written the care plans. Methods: We conducted a secondary analysis of care plans developed in a randomised controlled trial for assessing person‐centred care in patients with acute coronary syndrome (Myocardial Infarct [MI] or unstable angina pectoris). The study sample included 84 patients, with three care plans for each patient from inpatient (T1), outpatient (T2) and primary care (T3), that is, a total of 252 care plans. We conducted a descriptive quantitative content analysis of the care plans to examine the reported patients' life‐world and medical/health resources and goals. Results: The analysis illustrates the differences and overlaps between life‐world and medical/health goals and resources. The documented goals and resources change over time: life‐world goals and resources decreased with time as medical/health goals and resources documentation increased. Conclusions: This paper illustrates that in the setting of a randomised controlled trial, nurses with training in person‐centred care recorded fewer life‐world and more medical/health goals over time. Placing life‐world goals at the top of the goal hierarchy enables alignment with medical/health goals. Further research should explore whether the goals and resources documented in care plans accurately reflect patients' wishes as they transition along the care chain. This paper presents a contribution to the study of bibliographic corpora through science mapping. From a graph representation of documents and their textual dimension, stochastic block models can provide a simultaneous clustering of documents and words that we call a domain‐topic model. Previous work investigated the resulting topics, or word clusters, while ours focuses on the study of the document clusters we call domains. To enable the description and interactive navigation of domains, we introduce measures and interfaces that consider the structure of the model to relate both types of clusters. We then present a procedure that extends the block model to cluster metadata attributes of documents, which we call a domain‐chained model, noting that our measures and interfaces transpose to metadata clusters. We provide an example application to a corpus relevant to current science, technology and society (STS) research and an interesting case for our approach: the abstracts presented between 1995 and 2017 at the American Society of Clinical Oncology Annual Meeting, the major oncology research conference. Through a sequence of domain‐topic and domain‐chained models, we identify and describe a group of domains that have notably grown through the last decades and which we relate to the establishment of "oncopolicy" as a major concern in oncology. Background: Emergency physicians on-scene provide highly specialized care to severely sick or injured patients. High-quality research relies on the quality of data, but no commonly accepted definition of EMS data quality exits. Glasgow Coma Score (GCS) and Systolic Blood Pressure (SBP) are core physiological variables, but little is known about the quality of these data when reported in p-EMS research. This systematic review aims to describe the quality of pre-hospital reporting of GCS and SBP data in studies where emergency physicians are present on-scene.Methods: A systematic literature search was performed using CINAHL, Cochrane, Embase, Medline, Norart, Scopus, SweMed + and Web of Science, in accordance with the PRISMA guidelines. Reported data on accuracy of reporting, completeness and capture were extracted to describe the quality of documentation of GCS and SBP. External and internal validity assessment was performed by extracting a set of predefined variables.Results: We included 137 articles describing data collection for GCS, SBP or both. Most studies (81%) were conducted in Europe and 59% of studies reported trauma cases. Reporting of GCS and SBP data were not uniform and may be improved to enable comparisons. Of the predefined external and internal validity data items, 26%-45% of data were possible to extract from the included papers.Conclusions: Reporting of GCS and SBP is variable in scientific papers.
