Assessments of river condition are needed to guide all aspects of river management. Such assessments have evolved over three decades from simply capturing the mosaic of river physical habitats to recognizing that habitat mosaics are dynamic, driven mainly by physical processes and modified by human (indirect) pressures and (direct) interventions. To embrace these broader aspects, riparian as well as in‐stream environments need to be evaluated, going beyond subjective assessments to incorporate observations that support understanding of both physical habitat structure and cause‐effect relationships. This paper reports on an operational approach to assessing the physical condition of rivers, which attempts to bridge the gap between a physical habitat and a geomorphic condition assessment. The approach forms part of Biodiversity Metric 2.0, a habitat‐based methodology for measuring and accounting for biodiversity losses and gains resulting from development or land management change at individual project sites across England. The river condition assessment component adopts a bottom‐up multi‐scale approach that integrates field observations of physical habitats and of features indicative of geomorphic processes to deliver assessments of longer subreaches, whose condition is then evaluated within the context of the reach‐scale geomorphological type of river. By applying the assessment before, immediately after, and following recovery from project implementation, changes in condition and their causes can be evaluated. The assessment method is presented to an international audience, outlining its structure, application and testing, and critically discussing its strengths and weaknesses, because the methodological approach could be helpful for devising methods for application in other environmental contexts. There is an assumption that marine activities, particularly seafood-based activities, are an important source of employment and income in poor coastal areas. Linking the 2010 Index of Multiple Deprivation ( IMD) for England with the 2010 Business Structure Dataset ( BSD), this paper examines whether employment in the marine sector is concentrated in areas of high deprivation as is assumed. Merging the IMD 2010 with the BSD allows us to examine rates of marine-based employment and their location both across (inter) and within (intra) regions. This analysis provides the first empirical evidence that employment in the marine sector, particularly fishing and seafood processing, are located in areas with higher than average deprivation, even if the overall regional rate of deprivation is relatively low (the South East and the South West of England). Methodologically, this analysis indicates that an index such as the IMD based on a wide range of social and economic data can be used to understand the socioeconomic impacts of a sector within an area. Whilst most importantly this paper further demonstrates that intra-regional analysis is more insightful in exploring the geographical socioeconomic impact of the marine sector than inter-regional level analysis. With regard to the academic contribution, this paper also aims to conceptualise marine activity as a core economic activity located both in peripheral and non-peripheral areas. Teaching about the nature of science (NOS) is seen as a priority for science education in many national contexts. The present paper focuses on one central issue in learning about NOS: understanding the nature and status of scientific theories. A key challenge in teaching about NOS is to persuade students that scientific knowledge is generally robust and reliable, yet also in principle always open to challenge and modification. Theories play a central role, as they are a form of conjectural knowledge that over time may be abandoned, replaced, modified, yet sometimes become well established as current best scientific understanding. The present paper reports on findings from interviews with 13–14 year olds in England where target knowledge presents theories as ‘consistent, comprehensive, coherent and extensively evidenced explanations of aspects of the natural world’. Student thinking reflected a two-tier typology of scientific knowledge in which largely unsupported imaginative ideas (‘theories’) became transformed into fairly definitive knowledge (such as laws) through relatively straightforward testing. These results are considered in relation to research into intellectual development which indicates that effective teaching in this area requires careful scaffolding of student learning, but has potential to contribute to supporting intellectual development across the curriculum. This is an era of significant government involvement in schools in England, despite consistent rhetoric from the Department of Education to the contrary. In such a period, signs can be detected of the juncture between a postmodern identity and post-professional status, two models of teacher professionalism supposed in Hargreaves’ work on the fourth age of professionalism. This paper focuses on the former as the nature of the contemporary context. Defining features of postmodernity are considered through a focus on the broad issue of teacher professionalism. For this paper, this is embodied by teachers’ involvement with the development of the Primary National Curriculum for England. Evidence will be drawn from just before its 1988 introduction until 2013 in order to critique several of the key tenets of postmodernity as identified by Hargreaves. This is of particular importance in a context where a strong defence is necessary to enable the sustainment, and growth, of teachers’ professional autonomy. Evidence from an ethnographic study of three secondary school geography departments in England is drawn on to describe aspects of the relationships between examination boards and school subjects. This paper focuses on one department, in ‘Town Comprehensive’, and the argument is illustrated through a discussion of observed lessons with a teacher in this department. Ofqual (Office of Qualifications and Examinations Regulation) have recently announced that examination boards may continue to endorse commercially available teaching resources. The argument presented in this paper extends possible areas of ‘risk’ identified beyond those they currently consider. Specifically, it is argued that chief examiners play multiple roles in the recontextualisation of knowledge, holding substantial power over school subjects. The strong role of accreditation as a rationale is argued to restrict knowledge taught in school geography to horizontal discourses, limiting students’ access to powerful knowledge. This paper assesses the nature of parental participation and legal representation in pre-proceedings meetings in England and Wales. These are called when a local authority is considering care proceedings on a child. The parent(s) are invited to a meeting to discuss the concerns, and are entitled to attend with a lawyer. The paper draws on findings from a study of the process which included a file survey of over 200 cases, observations of 36 meetings and interviews with more than 90 key informants, including parents. The aim of the process is (usually) to reach an agreement to prevent the case going to court, but the families are usually well known to children's services, and have been through many meetings and agreements before. What then are the possibilities for parental participation and legal representation in the meetings? The study shows that they may help bring a greater degree of clarity to the local authority's proposals, but are not expected to challenge them. Paradoxically, they serve to reinforce the authority's position. The meetings can help divert cases, but it is important to be realistic about the chances of change in these often long-standing 'edge of care' cases. [ABSTRACT FROM AUTHOR]

This paper examines the parallel discourses of 'lost' local flood expertise and the growing use of commercial consultancies to outsource aspects of flood risk work. We critically examine the various claims and counter-claims about lost, local and external expertise in flood management, focusing on the aftermath of the 2007 floods in East Yorkshire, England. Drawing on interviews with consultants, drainage engineers and others, we caution against claims that privilege 'local' floods knowledge as 'good' and expert knowledge as somehow suspect. This paper urges carefulness in interpreting claims about local knowledge, arguing that it is important always to think instead of hybrid knowledge formations. We conclude by arguing that experiments in the co-production of flood risk knowledge need to be seen as part of a spectrum of ways for producing shared knowledge. [ABSTRACT FROM AUTHOR]

This paper explores religious adolescents' reported experiences of secondary schools. Fifty-four qualitative interviews were conducted in places of worship in three cities in England with Christians (n=46), Jews (n=38) and Muslims (n=15). Secondary schools of a religious and non-religious character were reported as not providing a suitable environment for religious observances, nor as a place to act and behave according to participants' religious principles. Religious adolescents reported prejudice and criticism of their beliefs or religious affiliations from their peers and sometimes from teachers. They also perceived their religious traditions to be distorted, inaccurately or unfairly represented in some lessons. The focus of this paper is the identity choices religious adolescents reported in response to these challenges. Three groups of identity choices are theorised and explored: religious identity seeking, religious identity declaration and religious identity masking. This paper provides an overview of research about adolescent neglect funded by the Department of Children, Schools and Families (now the Department for Education) and the Department of Health. The paper examines what is known from literature about the causes of adolescent neglect, together with its potential consequences for well-being. Drawing on qualitative data, the concept of neglect is explored from the perspectives of young people themselves and the professionals who work with them. Consideration is given to examples of interventions and preventative models geared towards addressing adolescent neglect. Key findings indicate that there is a need for both a re-examination of current definitions of neglect in the light of age-related distinctions and perspectives, and a fuller understanding of the particular needs of adolescents who are experiencing neglect. Additionally, the research highlights that there is a lack of research knowledge about neglectful parenting and the behaviour of young people, as well as limited understanding of interventions with neglected adolescents. Key themes from the research are discussed in terms of their implications for future practice, policy and research in relation to working to improve the welfare of neglected young people. At the small area scale simple methods for forecasting total populations are often employed because of a lack of data for cohort-component models, concerns about the reliability of these models for forecasting small population totals, and resource constraints. To date, a select number of authors have assessed the forecast accuracy of several individual, averaged, and composite models. This paper extends this stream of work by evaluating a large number of models on new datasets. The aims of the paper are to examine the performance of (a) 10 individual forecasting models (some of which are well known; others less so); (b) averages of every combination of 2, 3, 4, and 5 of the individual models (627 in total); and (c) composite models based on population size and growth rates (200,000 in total). Do averaged and composite models outperform individual models? Using new small area population datasets, forecasts from 2001 to 2031 were produced for three case study countries, Australia, New Zealand, and England & Wales. Both forecast accuracy and credibility (avoidance of negatives; degree of constraining to state populations) were assessed in 2011; for 2031, just credibility was evaluated. Of the individual models, constant share of growth (positive shares only) and constant share of population performed the best. A small proportion of averaged and composite models outperformed the best individual models in forecast accuracy. This paper examines two potential lacunae in understanding low-income residents' experiences of contemporary state-led gentrification via a study of neighbourhood restructuring in Salford, UK, 2004-2014. The first is the localised politics preparing the ground for neighbourhood 'redevelopment' and housing demolition. The second is the blighted social landscape which emerges with the subsequent stalling of this project. A focus on 'before' and 'after' is adopted in order to disrupt the linear policy and 'effects' temporalities that much qualitative gentrification research tends to inhabit. We see how state-led neighbourhood restructuring does not simply displace, but carries residents from 'empowerment' to abandonment and transfers them from active struggle into devitalised limbo. As such, the paper demarcates challenges and opportunities for resident mobilisation inherent in a vacillating urban renewal programme, powerful in its inception but which has since 'hit the buffers' (Lees 2014, Antipode 46(4):921-947) in light of global and municipal fiscal crises. Legal action to separate newborn babies from their parents is regarded as 'draconian' by the courts and subject to intense scrutiny. This paper outlines current legal and social work issues relating to such intervention and discusses the potential benefits of the 'pre-proceedings process' to address them. This process enables parents to have legal representation in a discussion with children's services when care proceedings are contemplated. The paper reports the findings of recent socio-legal research by the authors into the use of the process in England and Wales. In six local authorities, 30 per cent of 120 cases where the pre-proceedings process was used related to unborn babies; a pre-proceedings letter was sent in 75 per cent of unborn baby cases where care proceedings were considered, compared with 57 per cent of cases overall. The process could help to secure parental cooperation for pre-birth assessments and short-term protective arrangements at birth, thus avoiding emergency intervention. Families were diverted from care proceedings in 30 per cent of the cases where this was a possibility. Furthermore, the process was valued by social workers and lawyers as being fairer to parents. This paper reports on doctoral research carried out in the UK into the experience of complexity in child protection work. The study explored what complexity meant for practitioners, and examined its significance for interprofessional working. A qualitative, case-based approach was adopted, drawing on the experiences of 17 practitioners working on two separate child protection cases. Participants were from a range of agencies, including children's social care, education, youth offending and child/adolescent mental health. Two rounds of semi-structured interviews were carried out over a period of four months. Interview transcripts were analysed using interpretative phenomenological analysis, a research methodology derived from qualitative psychology. The findings revealed complexity to be a multifaceted phenomenon, driven by the dynamics of cause and effect in open social systems. Practitioners' perception of unpredictability and volatility in these cases fed into various aspects of collaborative work: the experience of relationships, the process of assessment, efforts at intervention and concerns about risk. The paper concludes by exploring some implications for policy and practice in terms of how interprofessional networks are set up to manage the needs of vulnerable children and families. In England, pupils aged 16 take the General Certificate of Secondary Education (GCSE) examinations for a range of subjects. The current assessment models for GCSE include a two-tier structure for some subjects and a non-tier model for the others. The tiered subjects have a higher tier designed for high achieving pupils and a lower tier for low achieving pupils. The higher tier paper is targeted at grades A*–D (with A* the highest grade available), while the lower tier paper at grades C–G (with G the lowest grade). The UK government has proposed a comprehensive reform of GCSEs. It suggested that, with tiered papers, pupils are forced to choose between higher and lower tier papers, which will place a cap on the ambition of those entering for the lower tier. The government therefore suggests avoiding tiering in the reformed GCSEs when possible. This paper discusses the technical and equity issues with the use of tiered examinations in current GCSEs and reviews potential alternative assessment approaches for effective differentiation between pupils for the reformed GCSEs. Solar energy is the most abundant of all renewable energy sources, and the development pressures for solar farms have grown rapidly in the last 5 years within the UK. With this in mind, this paper outlines the characteristics of solar farms, describes their development within the UK, examines some of the issues raised by these developments and offers a concluding discussion of the contributions that public and media relations firms can make to the development of solar farms. The paper reveals that solar farms have been developed on both agricultural land and brownfield sites and that the development pressures are greatest in the South West and South East of England. The findings reveal that proposals to develop solar farms have generated a wide range of environmental, community and economic issues and that public relations companies have an important role to play in fostering the transition from fossil fuels to renewable sources of energy. Emotions such as guilt and blame are frequently reported by non-breastfeeding mothers, and fear and humiliation are experienced by breastfeeding mothers when feeding in a public context. In this paper, we present new insights into how shame-related affects, cognitions and actions are evident within breastfeeding and nonbreastfeeding women's narratives of their experiences.As part of an evaluation study of the implementation of the UNICEF UK Baby Friendly Initiative CommunityAward within two primary (community based) care trusts in North West England, 63 women with varied infant feeding experiences took part in either a focus group or an individual semi-structured interview to explore their experiences, opinions and perceptions of infant feeding. Using a framework analysis approach and drawing on Lazare's categories of shame, we consider how the nature of the event (infant feeding) and the vulnerability of the individual (mother) interact in the social context to create shame responses in some breastfeeding and non-breastfeeding mothers.Three key themes illustrate how shame is experienced and internalised through 'exposure of women's bodies and infant feeding methods', 'undermining and insufficient support' and 'perceptions of inadequate mothering'. The findings of this paper highlight how breastfeeding and non-breastfeeding women may experience judgement and condemnation in interactions with health professionals as well as within community contexts, leading to feelings of failure, inadequacy and isolation. The Coalition Government's ' Green Paper' (DfE 2011) proposes a systemic overhaul of services for pupils with special educational needs in England, with increased parental choice of provision and 'sharper accountability' (p. 67) in schools. Deadlines for various stages of this reform have not been met, and its final nature remains uncertain. This paper reveals SENCOs' insights into their changing role in this turbulent policy context. This is achieved through the thematic analysis of 227 responses to an 'open-ended' question in the national Special Educational Needs Coordinator ( SENCO) Survey 2012. Findings from this sample indicate that SENCOs predict that schools in England will become more inclusive, with greater shared responsibility for achievement for all, and SENCOs' increased involvement in staff training and other whole school capacity-building activities. Respondents predict a greater partnership with parents, for whom they will provide advice and links to other services. They foresee their reduced involvement in direct teaching and an intensification of their work in other ways, especially in terms of paperwork associated with pupil tracking and other accountability measures. This paper presents new findings on the experiences of Indian mothers in the roles they play in their daughters' higher education ( HE) subject choices. Whilst there are existing studies on White British mothers and parents more generally in terms of parental involvement in education, and a growing number of papers presenting results on Bangladeshi and Pakistani parents, evidence on Indian parents (and students) is relatively sparse. In particular, there is a lack of research where Indian parents are given space to voice their own narratives in their own words. In the main, existing studies focus on parents from the viewpoint of their children as students. This qualitative study involved semi-structured interviews with nine Indian mothers from different areas in England. Commonalities and disparities between the experiences of Indian mothers presented here and accounts of White mothers of similar class and educational backgrounds presented in existing literature are analysed using an intersectional lens. Results show that working class Indian mothers could be making up some of their disadvantage in terms of social and cultural capital through family and community networks. Furthermore, mothers regard younger relatives as potential sources of important knowledge and resources that could help them and their daughters navigate the HE landscape. This has not been found in studies on White mothers where the emphasis instead has been on the exchange of knowledge from adults to the younger generation. The spatial experiences of transgender and gender non‐conforming ("trans") people continue to occupy the margins of geography, especially compared to the amount of work centred on lesbian and gay lives. Though research on trans geographies in educational and health settings is expanding, most literatures stem from the USA. This paper shares findings from a study about the experiences of trans people who study and/or work on a specific university campus in northern England. The contribution of nearly three decades of research, much of it published in Aquatic Conservation: Marine and Freshwater Ecosystems (AQC), to the conservation of the crucian carp Carassius carassius in the East of England, including work coordinated by the Norfolk Crucian Project, is summarized.Although recent genetic studies indicate that this species was probably introduced to England about the same time as common carp Cyprinus carpio, the crucian carp is considered a cultural heritage species worthy of conservation in eastern England.This research covers the environmental biology of the species in the East of England, documentation of the species' decline in the county of Norfolk, and the pond management practices implemented in Norfolk to rehabilitate existing and fully terrestrialized pond habitat specifically for crucian carp conservation.The AQC papers that contributed to this line of research showed that England offers a particularly favourable environment for crucian carp growth and reproduction. These AQC articles provided the evidence base to complement crucian carp conservation initiatives in the London area (mainly the counties of Essex and Hertfordshire), as well as forming the basis for the designation of crucian carp as a Biodiversity Action Priority species in the county of Norfolk.The broader impact of the work in the East of England was to inspire the recently‐formed English National Crucian Conservation Project, which aims to promote the conservation of crucian carp and its habitat, and to encourage the development of well‐managed crucian fisheries.These evidence‐based conservation initiatives, perhaps the first throughout Europe, have witnessed a reversal of the species' fortunes in England, which is effectively an ideal geographical region in which to promote the conservation of this species within a wider, European context. This paper combines novel data on the time use, home‐learning practices and economic circumstances of families with children during the COVID‐19 lockdown with pre‐lockdown data from the UK Time Use Survey to characterise the time use of children and how it changed during lockdown, and to gauge the extent to which changes in time use and learning practices during this period are likely to reinforce the already large gaps in educational attainment between children from poorer and better‐off families. We find considerable heterogeneity in children's learning experiences – amount of time spent learning, activities undertaken during this time and availability of resources to support learning. Concerningly, but perhaps unsurprisingly, this heterogeneity is strongly associated with family income and in some instances more so than before lockdown. Furthermore, our analysis suggests that any impacts of inequalities in time spent learning between poorer and richer children are likely to be compounded by inequalities not only in learning resources available at home, but also in those provided by schools. This paper reports on an empirical study of child protection services in a local authority where rates of investigations and interventions rose to unprecedented levels during the course of a single year. The aim of the research was to explore explanations for this rise in demand among the providers of children's social care in the area. Using an interpretative qualitative design, a series of focus groups and interviews were carried out with practitioners and managers (n = 25) from statutory services and Early Help. The findings identified a combination of long‐term and short‐term drivers of demand. Long‐term factors emphasized the impact of rising levels of deprivation combined with cuts to community‐based services for children and young people. Short‐term factors ranged from a more proactive approach to child neglect to more effective multi‐agency partnerships and joint decision making. The interaction between these factors was found to be accentuating an underlying shift to "late intervention" across the sector. The findings are contextualized in relation to contemporary debates about the crisis of demand for children's social care and the complex relationship between prevention and protection. The issue of microplastic pollution has seen an increasing focus over the last decade, both as a scientific research topic and due to intensive media interest. Despite this, there has been, until recently, a relative paucity of data relating to the volume and type of microplastics in the marine environment. The Cell 1 Regional Coastal Monitoring Programme, which covers approximately 300 km of the northeast England coastline, has recently collected subtidal sediment samples from the bed of the North Sea and analysed these for particle size distribution and microplastic content. The extreme conditions of the summer season in 2018 in England gave rise to comparisons with earlier drought events. Using the information provided by documentary records, such as chronicles, accounts and diaries, this paper investigates extreme summer droughts in the period 1200–1700, and their impacts under the conditions of natural climate variability and before the provision of modern infrastructure. In this paper I seek to explore the role of biodiversity offsetting in the evolution of public debates over controversial urban development projects and the way it may influence the outcome of social‐environmental conflicts. By drawing on empirical data obtained through 25 interviews with key stakeholders involved in a conflict around executive housing in North East England, 48 interviews with several stakeholders involved in offsetting across England, and an extensive document analysis, I explore the depoliticising effects of offsetting as an indicative example of actually existing neoliberal conservation. I also pay attention to the way offsetting relates to the fast policy complex to trace the links between neoliberalisation and depoliticisation particularly in the era following the 2008 financial crash. Understanding social‐environmental conflicts through the lens of the post‐political hypothesis can shed light on the way both participatory planning and neoliberal conservation, by favouring technocratic management and consensual policy‐making, attempt to remove contestation and agonistic engagement from the public terrain. On the other hand, seeing conflicts only through the post‐political hypothesis has important limits, primarily because it implies a catholic hegemony of post‐politics that can be both disorienting and paralysing. I conclude by showing through five key propositions the necessity for contextualised readings of historically and geographically specific political practices and for analysing depoliticisation by means of a theory of hegemony. Background and context: Breastfeeding peer support is valued by women, but UK trials have not demonstrated efficacy. The ABA feasibility trial offered proactive peer support underpinned by behaviour change theory and an assets‐based approach to women having their first baby, regardless of feeding intention. This paper explores women's and infant feeding helpers' (IFHs) views of the different components of the ABA intervention. Setting and participants: Trained IFHs offered 50 women an antenatal meeting to discuss infant feeding and identify community assets in two English sites—one with a paid peer support service and the other volunteer‐led. Postnatally, daily contact was offered for the first 2 weeks, followed by less frequent contact until 5 months. Methods: Interviews with 21 women and focus groups/interviews with 13 IFHs were analysed using thematic and framework methods. Results: Five themes are reported highlighting that women talked positively about the antenatal meeting, mapping their network of support, receiving proactive contact from their IFH, keeping in touch using text messaging and access to local groups. The face‐to‐face antenatal visit facilitated regular text‐based communication both in pregnancy and in the early weeks after birth. Volunteer IFHs were supportive of and enthusiastic about the intervention, whereas some of the paid IFHs disliked some intervention components and struggled with the distances to travel to participants. In boundary line analysis a biological response (e.g., crop yield) is assumed to be a function of a variable (e.g., soil nutrient concentration), which limits the response in only some subset of observations because other limiting factors also apply. The response function is therefore expressed by an upper boundary of the plot of the response against the variable. This model has been used in various branches of soil science. In this paper we apply it to the analysis of some large datasets, originating from commercial farms in England and Wales, on the recorded yield of wheat and measured concentrations of soil nutrients in within‐field soil management zones. We considered boundary line models for the effects of potassium (K), phosphorus (P) and magnesium (Mg) on yield, comparing the model with a simple bivariate normal distribution or a bivariate normal censored at a constant maximum yield. We were able to show, using likelihood‐based methods, that the boundary line model was preferable in most cases. The boundary line model suggested that the standard RB209 soil nutrient index values (Agriculture and Horticulture Development Board, nutrient management guide (RB209), 2017) are robust and apply at the within‐field scale. However, there was evidence that wheat yield could respond to additional Mg at concentrations above index 0, contrary to RB209 guidelines. Furthermore, there was evidence that the boundary line model for yield and P differs between soils at different pH and depth intervals, suggesting that shallow soils with larger pH require a larger target P index than others. Highlights: Boundary line analysis is one way to examine how soil variables influence crop yield in large datasets.We showed that boundary line models could be applied to large datasets on soil nutrients and crop yield.The resulting models are consistent with current practice for P and K, but not for Mg.Models suggest that more refined recommendations for P requirement could be based on soil pH and depth. Background: The existing evidence is limited in terms of perspectives of preschool children with speech and language needs and their views on activities used to support their needs. This paper discusses a stream of work from the interdisciplinary research programme known as "Child Talk," based in England, UK. The overall purpose of this work stream was to gain the perspectives of preschool children aged 2 to 5 years and 11 months, with speech and language needs, to use in the development of an evidence‐based framework of activities. Methods: Twenty‐four preschool children with a variety of needs from diverse backgrounds took part. An observational methodology was used to capture children's experiences. Children were filmed during a series of sessions, with innovative head‐mounted cameras worn by the children and supported by researcher field notes. Framework analysis was used to analyse the data based on the body movement, vocalization, and visual attention of the children during these sessions. Results and Conclusions: Results included that children expressed enjoyment and engagement in the activities. The children expressed themselves and demonstrated their focus "multimodally" through combinations of body language, vocalization, and visual attention. These modalities were present across all contexts and children. It highlights the importance of encouraging participation in preschool children and consequently this innovative piece of work has national and international importance. [ABSTRACT FROM AUTHOR]

Background: Hospital admissions for preventable reasons [ambulatory care sensitive (ACS) conditions] can indicate gaps in access to or quality of primary care. This paper seeks to document the numbers and causes of these admissions in England for people with intellectual disabilities (ID) compared with those without. Methods: Observational cohort study of number and duration of emergency admitted patient episodes for ACS conditions, overall and by cause, using the Clinical Practice Research Datalink GOLD primary care database and the linked Hospital Episode Statistics Admitted Patient Care dataset. Results: The study covered 5.2% of the population of England from April 2010 to March 2014 giving a total population base of 59 280 person‐years for people with ID and 11 103 910 for people without identified ID. The rate of emergency admissions for ACS conditions for people with ID was 77.5 per 1000 person‐years. As a crude comparison, this was 3.0 times the rate for those without ID, but standardising for the distinct demography of this group, the number of episodes was 4.8 times that expected if they had the same age‐specific and sex‐specific rates. Stay durations for these episodes were longer for both young‐age and working‐age people with ID. Overall people with ID used 399.8 bed‐days per 1000 person‐years. As a crude comparison, this is 2.8 times the figure for people without ID. Standardising for their age and sex profile, it is 5.4 times the number expected if they had the same age‐specific and sex‐specific rates. For patients with ID, 16.6% (one in six) of all admitted patient episodes and 24.3% (one in four) of in‐patient care days for people with ID were for ACS conditions. Corresponding figures for those without ID were 8.3% (one in 12) and 14.4% (one in seven). The difference in rates between those with and without ID was most marked in people of working age. The three most common causes of emergency episodes for ACS conditions in people with ID were convulsions and epilepsy, influenza pneumonia and aspiration pneumonitis. Influenza pneumonia was also a common cause for people without ID. Episodes for convulsions and epilepsy and aspiration pneumonitis were specifically associated with people with ID. Conclusions: Rates of hospital admissions for ACS conditions provide an important indicator of health literacy, basic self‐care (or support by carers) and the accessibility of primary care. High rates are seen for some conditions specifically associated with premature death in people with ID. Local monitoring of these figures could be used to indicate the effectiveness of local primary health services in providing support to people with ID. [ABSTRACT FROM AUTHOR]

Despite the introduction of guidelines and procedures aimed at encouraging and supporting children and young people to complain about the services they receive, children in care still face barriers to doing so in practice. This paper explores what happens when children in care are dissatisfied with the services they receive. Specifically, this study examines the complaints procedure for children in care. The findings are based on semistructured interviews with children in care, social workers, senior managers, and independent reviewing officers from one English local authority. Thematic analysis of these data identified five emergent themes: (a) complaints by children in care are managed at the lowest possible level, (b) senior managers have an overly optimistic view about children in care being informed of complaint procedures and being encouraged to do so, (c) children in care are worried about complaining, which is recognized by professionals, (d) children's voices are often not heard, and (e) when issues are clearly defined, independent reviewing officers have some degree of success in resolving complaints from children in care. Smoking in high‐income countries is now concentrated in poor communities whose relatively high smoking prevalence is explained by greater uptake but above all by lower quit rates. Whilst a number of barriers to smoking cessation have been identified, this is the first paper to situate cessation itself as a classed and cultural practice. Drawing on ethnographic research carried out in a working‐class community in the North of England between 2012 and 2015, I theorise smoking cessation as a symbolic practice in relation to the affective experience of class and social mobility. I show that ambivalence about upward mobility as separation and loss translated into ambivalence about smoking cessation. The reason for this was that the social gradient in smoking operated dynamically at the level of the individual life course, i.e. smoking cessation followed upward mobility. A serious health problem was an appropriate reason to quit but older women continued to smoke despite serious health problems. This was linked to historical gender roles leading to women placing a low priority on their own health as well as the intergenerational reproduction of smoking through close affective links with smoking parents. Health and employment are strongly correlated. This paper reviews the existing evidence and brings in new evidence on the following issues: (a) the measurement of health; (b) the impact of health on employment rather than just the association between health and employment; (c) the mechanisms by which health impacts employment; and (d) the likely effect of recent retirement and disability policy changes in the UK. Although the magnitude of the estimated effect of health on employment varies greatly from study to study, some of this variation is driven by the health measure used. Given our preferred measure, the evidence suggests that 5–10 per cent of the employment decline between ages 50 and 70 is due to declining health in England, with the largest effects among low‐educated men. Most of the effect comes through declining preferences for work and lower productivity when in bad health, although some of the effect is from government‐provided incentives to not work when in bad health, such as from disability benefits. Objectives: To understand the potential impact of exposure misclassification on water fluoridation studies in England, this paper aims to describe the long‐term variation in water fluoride concentrations in both artificially and naturally fluoridated water supplies. Methods: Water fluoridation dose monitoring data were requested from all five English public water suppliers who artificially fluoridate their water, as well as from one water company that supplies naturally fluoridated water. Descriptive statistics were calculated, including annual means, standard deviations, minimum‐maximum and absolute and relative frequencies. Results: Data were made available by two of the five English water companies who supply artificially fluoridated water and one water company that supplies naturally fluoridated water (40 398 individual samples). The data for fluoridated water spanned 18‐35 years, whilst the data on naturally fluoridated water spanned 14 years. The artificially fluoridated samples showed wide variation in fluoride dose control, both between different water treatment works and over time. Mean fluoride concentrations in the artificially fluoridated supplies ranged from 0.53 (SD 0.47) to 0.93 (SD 0.22) mg F/L and were within the optimal range of 0.7‐1.0 mg F/L in 27.7%‐77.8% of samples. The naturally fluoridated supplies had a higher mean fluoride concentration of 1.06 (SD 0.18) and 1.15 (SD 0.16) mg F/L than the artificially fluoridated supplies, with lower variation over time. The naturally fluoridated supplies were above the optimal range in 75.5% and 53% of samples. Conclusions: Assumptions that populations living in areas with a water fluoridation scheme have received optimally fluoridated water (0.7‐1.0 mg F/L) are invalid. To support future research endeavours, as well as to provide 'external control' and facilitation of optimal dosing, it is recommended that a quarterly record of water fluoride concentrations (mean, standard deviation and minimum and maximum) are made available for every water supply in England, in a format that can be mapped against residential postcodes. Rationale, aims, and objectives: Decisions about whether to refer or admit a patient to an intensive care unit (ICU) are clinically, organizationally, and ethically challenging. Many explicit and implicit factors influence these decisions, and there is substantial variability in how they are made, leading to concerns about access to appropriate treatment for critically ill patients. There is currently no guidance to support doctors making these decisions. We developed an intervention with the aim of supporting doctors to make more transparent, consistent, patient‐centred, and ethically justified decisions. This paper reports on the implementation of the intervention at three NHS hospitals in England and evaluates its feasibility in terms of usage, acceptability, and perceived impact on decision making. Methods: A mixed method study including quantitative assessment of usage and qualitative interviews. Results: There was moderate uptake of the framework (28.2% of referrals to ICU across all sites during the 3‐month study period). Organizational structure and culture affected implementation. Concerns about increased workload in the context of limited resources were obstacles to its use. Doctors who used it reported a positive impact on decision making, with better articulation and communication of reasons for decisions, and greater attention to patient wishes. The intervention made explicit the uncertainty inherent in these decisions, and this was sometimes challenging. The patient and family information leaflets were not used. Conclusions: While it is feasible to implement an intervention to improve decision making around referral and admission to ICU, embedding the intervention into existing organizational culture and practice would likely increase adoption. The doctor‐facing elements of the intervention were generally acceptable and were perceived as making ICU decision making more transparent and patient‐centred. While there remained difficulties in articulating the clinical reasoning behind some decisions, the intervention offers an important step towards establishing a more clinically and ethically sound approach to ICU admission. Background and Aims: Youth alcohol consumption has declined significantly during the past 15 years in many high‐income countries, which may have significant public health benefits. However, if the reductions in drinking occur mainly among lighter drinkers who are at lower risk, then rates of alcohol‐related harm among young people today and adults in future may not fall in line with consumption. There is conflicting evidence from Swedish school studies, with some suggesting that all young people are drinking less, while others suggest that alcohol consumption among heavier drinkers may be stable or rising while average consumption declines. This paper extends the geographical focus of previous research and examines whether the decline in youth drinking is consistent across the consumption distribution in England. Design Quantile regression of 15 waves of repeat cross‐sectional survey data. Setting: England, 2001–16. Participants: A total of 31 882 schoolchildren (50.7% male) aged 11–15 who responded to the Smoking Drinking and Drug Use among Young People surveys. Measurements Past‐week alcohol consumption in UK units at each fifth percentile of the consumption distribution. Findings Reductions in alcohol consumption occurred at all percentiles of the consumption distribution analysed between 2001 and 2016, but the magnitude of the decline differed across percentiles. The decline in consumption at the 90th percentile [β = −0.21, confidence interval (CI) = −0.24, −0.18] was significantly larger than among either lighter drinkers at the 50th percentile (β = −0.02, CI = −0.02, −0.01) or heavier drinkers at the 95th percentile (β = −0.16, CI = −0.18, −0.13). Conclusions: Alcohol consumption among young people in England appears to be declining across the consumption distribution, and peaks among heavy drinkers. The magnitude of this decline differs significantly between percentiles of the consumption distribution, with consumption falling proportionally less among the lightest, moderate and very heaviest youth drinkers. Summary: This paper is concerned with the deposition of Middle and Late Bronze Age metalwork in three study areas extending from the coast of south‐east England to the edge of the Fenland. It considers where hoards and single finds have been discovered and is based on inspection of over 300 findspots on the ground. It investigates regions in which tools and ornaments are common, but also extends to those with a stronger representation of weapons. While rapiers, swords and spears are found in rivers, terrestrial finds often come from sites close to springs, aquifers and confluences and favour areas with fresh water rather than salt water. Others are from more prominent locations on high ground or in low‐lying positions like dry valleys. We suggest that such places played a role in a tiered cosmology. Metalwork was deposited where the underworld or the sky came into contact with the landscapes inhabited in daily life. [ABSTRACT FROM AUTHOR]

Meat production and consumption are major contributors to global greenhouse gas emissions and other aspects of environmental degradation. It is the aim of this paper to explore red meat in the configuration of main meals eaten out in England across types and styles of cuisine, and to consider the implications for transition towards less resource intensive ways of eating in the future. We show that the odds ratio of eating a dish without red meat is significantly lower in North American/European and Near/Middle Eastern cuisines compared with East Asian (with no difference between South and East Asian), that women are more likely than men to eat fish and poultry (with no gender differences in vegetarian dishes), that Prestonians are the least likely to select a vegetarian dish, compared with people in London and Bristol, and that the odds of a vegetarian dish compared with red meat is higher amongst managerial workers compared with the routine manual workers (with no other statistically significant class differences). We suggest the term "translated cuisine" to refer cuisines that travel and become incorporated into the palate of the new food culture, and discuss how this could play a role in transitions towards less meat‐centred patterns of food consumption in the future. In addition to evaluating the long‐term and short‐term relationships between housing markets on the regional and nationwide scales, this study also explores risk diffusion associated with market integration. Data from January 1995 to July 2017 in nine regions in England and in the overall England housing market are adopted. Through simultaneous estimates of volatility in various regions, the systemic risk and overall risk of each region are evaluated. The results reveal that housing markets in the central regions diffuse risks easily, and their volatility is easily affected by stock market return and money supply changes. By contrast, the volatility of housing markets in the southern regions is easily affected by changes in the interest rate, and these markets demonstrate high transmission with the overall housing market in the short term. Notably, except for times when financial crises occurred, housing market risks in the southern regions have been lower than those of other regions. In particular, London exhibits significantly low risk and high return in its housing market and high informativeness in its house prices. Reduced tillage systems have been argued to provide several potential benefits to soil, environment and to farm incomes. In England, while many farms have partially adopted such practices, a large proportion of arable farmers do not undertake reduced tillage in any form. This paper analyses the rationale for and uptake of different cultivation techniques, including analysis of the barriers to adoption of reduced tillage, aiming to benefit policymakers and researchers and increase the spread of smart agricultural practices. Based on a postal questionnaire, we estimated that 47.6% of English arable land is cultivated using minimum‐tillage and 7% under no‐tillage. As farm size increased, so did the probability of reduced tillage uptake. Furthermore, farms growing combinable crops were more likely to utilize reduced tillage approaches than other farm types. Soil type, weed control and weather conditions were noted as the main drivers for 'strategic' and 'rotational' ploughing, constraining continuous reduced tillage use. To effect greater reduced tillage uptake, greater communication between researchers and farmers is needed to facilitate the implementation of sustainable soil management solutions, supported by current legislation permitting responsible herbicide use in arable production. Financial support to access reduced tillage machinery may also be required for farmers operating smaller holdings. Adopting reduced tillage is a continuous learning process requiring ongoing training and information gathering; supporting a network of reduced tillage 'farmer champions' would facilitate practical knowledge exchange, allow farmers to observe soil improvements, understand transition phase barriers and ultimately encourage increased reduced tillage uptake. The Common Assessment Framework provides a model of early intervention, which is familiar in local authorities throughout England, and asserts a participatory framework of child and family engagement. This paper is based on a qualitative study of parents and children who were subject to a multi‐agency process of early intervention in children's social care in a local authority in the Midlands of England. I advance the concept of assemblage to consider the basis of an active service user participation as rather more a struggle to achieve than something that has been granted by practitioner agencies. Interview extracts are used to show the enrolment and assembly of participation as a process of service users developing their active human agency in a multi‐agency setting. The article explores the assembling of skills in administration and management of meetings and plans, accessing knowledge and expertise through service user networks, and challenging professional expertise and institutional space while developing personal qualities of confidence and voice as a means of marshalling an effective participation in a multi‐agency setting. Rationale, aims and objectives: Although Bangladeshis are three times more likely to be carers than White British, Bangladeshi family carers are the most deprived, neglected, and effectively a hidden group in the United Kingdom.1 There is a paucity of research within the Bangladeshi community that is capable of explaining and predicting the experiences and concerns of Bangladeshi family carers providing care for their relatives with dementia. The purpose of this study is to explore the perspectives of Bangladeshi family carers' knowledge and day‐to‐day experiences living in England. Methods: This is a qualitative study involving semistructured face‐to‐face interviews with six Bangladeshi family carers living in London and Portsmouth. Interviews were recorded with the consent and transcribed verbatim. Data were managed by using NVivo software, and thematic analysis was performed. Results: This paper explores that most carers have a lack of knowledge and awareness of the symptoms of dementia. The results of this study are in contrast to previous studies, where South Asian carers perceived dementia as being possessed by evil spirits or God's punishment for previous life's sins; this study reveals that Bangladeshi family carers believed dementia was a medical condition. Unlike earlier South Asian studies, however, all family carers in this study also believed that there was no stigma attached to dementia. Conclusions: Further research is warranted to investigate the religious beliefs, familism, and interpersonal motives as theoretical perspectives to explain how Bangladeshi family carers negotiate and construct their caregiving roles for their relatives with dementia. This paper problematises the concept of social mobility through an exploration of it in relation to Higher Education policy in England. Based upon a content analysis of a number of key policy documents from distinct eras, it identifies definitions and understandings of social mobility within them, exploring how such references have changed over time, and critiquing the differences between the imagined ideals of what policy rhetoric seeks to do and the reality of policy implementation. In particular, it considers the characterisation of social mobility as an individualised concern; it positions aspirations of improving social mobility within the market of Higher Education; and it ultimately asks whether Higher Education can solve the government's social mobility problem. This study aims to compare the drivers of clustering of rural and urban creative industries in England. We use pre‐pandemic web‐scraped data from 154,618 creative industry organizations in England, and use a novel technique to identify 71 distinct rural creative "microclusters" of geographically proximate creative firms. We then consider the role of place‐based assets and agglomeration in the presence of microclusters at a micro‐level geography and find that the determinants of microclustering are generally consistent between rural and urban areas. This paper addresses the potential difficulties associated with researching controversial and/or sensitive issues. Drawing on the findings from in-depth interviews with farmers, the paper provides a reflexive commentary on the use of visual vignettes to explore farmers' attitudes towards the control of bovine tuberculosis in England - currently a highly controversial subject. Five short video clips were shown to interviewees to prompt discussions about different aspects of disease control, including methods to vaccinate and cull badgers. Visual vignettes have not been used widely outside the fields of health and social care. The method is used here to encourage interviewees to discuss sensitive issues related to badger control, including their trust in various stakeholders involved in the management of bovine tuberculosis, their own knowledge and understanding about disease control, and their attitudes towards control methods. The results suggest that visual vignettes have a number of advantages that could benefit research in human geography and the wider social sciences. [ABSTRACT FROM AUTHOR]

Many new graduates are finding it difficult to obtain graduate-level work and impossible to break into the sectors they were aiming at. In order to address this, higher education institutions are examining the methods they use to enhance student employability and deploying various measures to grow and strengthen this activity, with an increasing focus on providing work experience. One popular way of doing this is via internships (placements). This paper examines the context in which work experience has come to the fore as an approach to enhancing employability by interrogating recent research and policy related to this agenda. Employability is presented as an on-going debate that cannot be viewed as a finite entity but must move and develop with the market, society and the global situation. In addition the paper closely examines a graduate internship programme in a university in North East England. Comparing and contrasting findings from this scheme, with other examples of experiential learning through work experience, provides some evidence to suggest the value of internships and furthermore supports some formative ideas about how internships might be developed to best serve those taking part in them: interns; employers and universities. The question of where to locate teaching about the relationships between science and religion has produced a long-running debate. Currently, science and religious education (RE) are statutory subjects in England and are taught in secondary schools by different teachers. This paper reports on an interview study in which 16 teachers gave their perceptions of their roles and responsibilities when teaching topics that bridge science and religion and the extent to which they collaborated with teachers in the other subject areas. We found that in this sample, teachers reported very little collaboration between the curriculum areas. Although the science curriculum makes no mention of religion, all the science teachers said that their approaches to such topics were affected by their recognition that some pupils held religious beliefs. All the RE teachers reported struggling to ensure students know of a range of views about how science and religion relate. In the context of the international problem of 'early school leaving', this paper explores the issue of sustained participation in upper secondary education in England. It focuses in particular on the position of middle attainers, who constitute a large proportion of the cohort and whose progress will be vital in realising the government's goal of 'Raising the Participation Age' to 18 by 2015. The paper draws on evidence from national research undertaken as part of the Nuffield Review of 14-19 Education and Training in England and Wales and analysis of New Labour and Coalition policy between 2000-2012. It uses a three-year local study of 2400 14- and 16-year-olds in an established school/college consortium to illustrate the effects of policy and practice on middle attainers. We argue that this important group of young people was 'half-served' by New Labour, because of its incomplete and contradictory 14-19 reforms, and is now being 'overlooked' by Coalition policy because of its emphasis on high attainers. We conclude by suggesting a range of measures to support the 14+ participation, progression and transition of middle attainers in the English education and training system. [ABSTRACT FROM AUTHOR]

This paper argues that direct control of the early years literacy curriculum recently exercised by politicians in England has made the boundaries between research, policy and practice increasingly fragile. It describes how policy came to focus most effort on the use of synthetic phonics programmes in the early years. It examines why the Clackmannanshire phonics intervention became the study most frequently cited to justify government policy and suggests a phonics research agenda that could more usefully inform teaching. It argues that, whilst academics cannot control how their research is eventually used by policy-makers, learned societies can strengthen their ethics policies to set out clearer ground rules for academic researchers working across knowledge domains and with policy-makers. A stronger framework to guide the ethical interpretation of research evidence in complex education investigations would allow more meaningful conversations to take place within and across research communities, and with research users. This paper generates new evidence for England and Wales on the importance of labour pooling as a source of agglomeration economies. Estimates of worker and firm productivity are obtained from longitudinal worker and firm micro-data and used to test the hypothesis that denser labour markets increase the quality of the matching between employees and employers across labour markets. Our findings provide evidence supportive of a positive relationship between the quality of the employee-employer matching and the economic size of labour markets. [ABSTRACT FROM AUTHOR]

Background This paper tells the story of Central England People First's ( CEPF) History Project. Method This was an inclusive research project, owned and controlled by members of CEPF which sought to chart its 21-year history, 1990-2012. Results It illustrates both the strengths of such a project and some of the challenges. Conclusion It concludes that using inclusive research methods enabled the story to be told, but that it was less successful in addressing questions about why the organization grew and prospered in the 1990s, only to struggle in its later years, and what this tells us about the conditions which enable self-advocacy to flourish. The paper was collaboratively written by the CEPF History Project team and an academic ally. Different fonts differentiate the contributions, although it is acknowledged that lots of the ideas were shared. It explains how and why CEPF recorded its history, what we found out, and some of the questions we have had to think about: whose voices we hear, what to include, what to leave out, what parts of the research people with learning difficulties can do, what self advocacy means to different people, how to make use of research other people have done., It raises some new questions about directions for inclusive research.
