Initial doubts about the existence of this new disease were quickly eliminated by its dramatic increase. Nevertheless, it is worthwhile to draw up an interim balance. GERD is the most common gastrointestinal disease in the world affecting over 10-20% of the adult population. GERD is often perceived as a Western disease and very little literature available about this disease in the Asian population. With growing obesity and westernization in Asia, the prevalence of GERD is rapidly increasing. With growing morbidity and health care cost there is a significant need for understanding the disease from the Asian perspective. This book would fill the gap in the knowledge on GERD among Asians and will highlight the difference in terms of epidemiology, diagnosis and management of GERD from the Western perspective. Granulocytes are a category of white blood cells characterized by the presence of granules in their cytoplasm. More than 180 participants and experts from 31 countries met for the fifth time in 10 years in St. Gallen, Switzerland for a 3-day conference to discuss important current issues of clinical cancer prevention. The meeting was again organized and co-sponsored by St. Gallen Oncology Conferences (SONK). While SONK has been extremely successful in organizing large international c- gresses on “Primary Therapy of Early Breast Cancer” as well as “Supportive Care in Cancer” for more than 20 years, the idea of promoting interdisciplinary, clinically oriented meetings on cancer prevention is a more recent and not yet generally accepted and w- comed concept in modern oncology. Since today's medical expenses are soaring and me- cal research budgets are stagnating or even being cut, neither politicians nor industry is willing to risk an additional unpredictable channel of expenses, such as that demanded by clinical cancer prevention efforts! In Switzerland—and we fear in many other parts of the globe—some 97%–98% or even a greater percentage of health budgets is spent for curative and palliative/rehabilitative m- icine. Since a meager 2%–3% of national health budgets is for preventive medicine, even less than that proportion is specifically allocated for cancer prevention. When the money for “curing and caring” for the diseased populace runs short, there is likely not much left for partly controversial disease prevention in the (still) healthy part of the population.

The discovery of molecular and cellular mechanisms of pathology, together with the present data on drug treatment result in enhanced drug design and testing. The known mechanisms of biological activity as displayed by ascorbic acid and other natural reductones are the reason that they are considered to be physiologically compatible antioxidants, many of which are trace elements in food. The pharmacological potential of natural antioxidants may be enhanced by chemical modification. Biotic antioxidants possess an important potential to be a reliable basis of the development of new multipurpose drugs, within the framework of existing technological structures. This book shows approaches for possible use of these natural antioxidants. In particular, such an approach is described for the case of the design of superoxide dismutase models with anti-tumour activity.

I am delighted to be able to write the foreword for this new book on Helicobacter pylori by three pioneers in the field, Vlf West­ blom, Steven Czinn and John Nedrud. Because of their long experience in both basic and clinical aspects of H. pylori, the editors have been able to produce a volume which is authoritative and up to date in the science of H. pylori, while still being concise and interesting for the practicing physician or H. pylori novice. To achieve this, they have collected a very distinguished group of authors from within the Vnited States and around the world. The chapters are sequenced in approximately the same order as developments in H. pylori science over the past 15 years. The first chapter on the discovery of H. pylori is by Cliodna McNulty, who was the first person to culture the organism in Europe. The epidemiology is then described followed by the main clinical as­ sociations, which are gastritis and dyspepsia, peptic ulcers, can­ cers and lymphoma. This naturally leads to discussion of the laboratory aspects of H. pylori, especially the microbiology, in­ cluding essential information on antibiotic resistance patterns. Next, virulence and pathogenicity of H. pylori are explained as defined in studies using animal models, then by discussion of the metabolism of the organism. Finally, the interaction of the bac­ terium with the host immune systems is dealt with, including the implications of these findings as they relate to the development of future vaccines.
