A description of a detailed clinical approach provides clinical practitioners with the most up-to-date recommendations for the prevention and treatment of chemotherapy-induced nausea and vomiting (CINV) in various clinical settings.CINV is one of the most feared treatment related toxicities. Patient surveys for the past thirty years consistently demonstrate patients'perception of deterioration in quality of life due to chemotherapy treatments. The introduction of the antiemetics, serotonin 5-HT3 receptor antagonists and the neurokinin-1 receptor antagonists, have improved the control of chemotherapy-induced emesis, but the treatment of chemotherapy-induced nausea remains a significant clinical problem. Patients continue to have quality of life issues which prevent normal functioning during active treatment. New agents such as the second generation 5-HT3 receptor antagonist palonosetron and the new neuroknin-1 receptor antagonists rolapitant and netupitant are being introduced into clinical practice, and it is anticipated that these new agents will improve the control of CINV. Agents such as olanzapine (a FDA approved anti-psychotic), gabapentin (a FDA approved neuroleptic), and ginger (a food additive), which have been used primarily for other indications, are now being tested as potential, effective antiemetics. This work represents the first available comprehensive summary that details all new antiemetic agents and, particularly, their clinical role in treating patients; an important reference for practitioners seeking to improve the quality of life of patients undergoing chemotherapy.

Epilepsy is a devastating group of neurological disorders characterized by periodic and unpredictable seizure activity in the brain. There is a critical need for new drugs and approaches given than at least one-third of all epilepsy patients are not made free of seizures by existing medications and become'medically refractory'. Much of epilepsy research has focused on neuronal therapeutic targets, but current antiepileptic drugs often cause severe cognitive, developmental, and behavioral side effects. Recent findings indicate a critical contribution of astrocytes, star-shaped glial cells in the brain, to neuronal and network excitability and seizure activity. This book will describe the nuclear encoded genes and their expressed proteins of mitochondrial oxidative phosphorylation. Most of these genes occur in eukaryotic cells, but not in bacteria or archaea. The main function of mitochondria, the synthesis of ATP, is performed at subunits of proton pumps (complexes I, III, IV and V), which are encoded on mitochondrial DNA. The nuclear encoded subunits have mostly a regulatory function. However, the specific physiological functions of the nuclear encoded subunits of complexes I, III, IV, and V are mostly unknown. New data indicates that they are essential for life of higher organisms, which is characterized by an adult life without cell division (postmeiotic stage) in most tissues, after the juvenile growth. For complex IV (cytochrome c oxidase) some of these subunits occur in tissue-specific (subunits IV, VIa, VIb, VIIa, VIII), developmental-specific (subunits IV, VIa, and VIIa) as well as species-specific isoforms. Defective genes of some subunits were shown to induce mitochondrial diseases. Mitochondrial genes and human diseases will also be covered.

In the first 20 years that followed the purinergic signalling hypothesis in 1972, most scientists were sceptical about its validity, largely because ATP was so well established as an intracellular molecule involved in cell biochemistry and it seemed unlikely that such a ubiquitous molecule would act as an extracellular signalling molecule. However, after the receptors for ATP and adenosine were cloned and characterized in the early 1990s and ATP was established as a synaptic transmitter in the brain and sympathetic ganglia, the tide turned. More recently it has become clear that ATP is involved in long-term (trophic) signalling in cell proliferation, differentiation and death, in development and regeneration, as well as in short-term signalling in neurotransmission and secretion. Also, important papers have been published showing the molecular structure of P2X receptors in primitive animals like Amoeba and Schistosoma, as well as green algae. This has led to the recognition of the widespread nature of the purinergic signalling system in most cell types and to a rapid expansion of the field, including studies of the pathophysiology as well as physiology and exploration of the therapeutic potential of purinergic agents.In two books, Geoffrey Burnstock and Alexej Verkhratsky have aimed at drawing together the massive and diverse body of literature on purinergic signalling. Galanin is a neuropeptide found both in the central and peripheral nervous system. The 29-amino acid peptide (named after its N-terminal glycine and C-terminal alanine) was identified in 1983 by its C-terminal amidation. This'reverse'approach, that is to discover a substance through a distinct chemical feature, and only subsequently to characterize its biological activity, was novel and has been successful in the identification of several other peptides. After the structure of galanin was determined in 1983, functional studies were performed with material purified from natural sources until the synthetic form of the peptide became available. Galanin can act as transmitter, modulator and trophic factor, and is involved in a number of physiological processes such as hormone secretion, cardiovascular mechanisms, feeding and cognition. This peptide may also be of significance for a number of pathological processes/disorders including pain, depression, Alzheimer's disease, epilepsy, addiction and cancer. This wide diversity of actions is mediated by three galanin receptor subtypes. The studies reviewed in this volume give a fairly complete overview of the spectrum of the biological actions and functions of galanin and its receptors and on possible therapeutic applications in a number of pathological conditions.

This has resulted in a dramatic increase in obesity-related metabolic and cardiovascular complications, making it an important public health issue. Type 2 diabetes associated with obesity or'diabesity'is today the most common form of type 2 diabetes. What does it mean to talk about a `national'cinema? To what extent can British cinema, dominated for so many years by Hollywood, be considered a national cinema?
