Research in electrophysiologic monitoring in anesthesia and intensive care has focussed mostly on questions pertinent for patient care: First how to quantitate drug effects on brain electrical activity and the degree of anesthetic-induced suppression of the central nervous system. Second, how to monitor functional impairment following cerebral ischemia and hypoxia. And third, how to differ­ entiate between drug-induced effects on the central nervous system and deleteri­ ous events related to reductions in cerebral blood flow and/or oxygen delivery. Even though progress has been achieved over the last 10 years in this field and fascinating new techniques have been developed, it is still not clear which monitor parameter will provide adequate information on the depth of anes­ thesia and the analgesic level. Because the central nervous system has been one of the main research areas in our department over the last 10 years, we organized a workshop to summarize the latest developments in central nervous system monitoring. This book comprises the topics of this workshop and is intended to provide insight into the current status of central nervous system monitoring, elucidating possible indications and delineating its limitations.

El paciente neurológico grave requiere para su manejo integral de un monitoreo estrecho y a profundidad que tiene como objetivo poder detectar de manera temprana y oportuna cualquier alteración fisiopatológica que comprometa aún más la función, la integridad y la estabilidad de un sistema nervioso central ya de por sí lesionado. Por este motivo, en los últimos años se han desarrollado varios dispositivos que tienen como objetivo evaluar diferentes marcadores de función neurológica, como son el flujo sanguíneo cerebral, la relación aporte–consumo de oxígeno, la función metabólica, la temperatura y las estructuras cerebrales, desde un punto de vista anatomofuncional. Lo anterior ha dado origen a un sinnúmero de publicaciones que evalúan en diferentes escenarios las múltiples variables de monitoreo que, como marcadores, nos ayudan a dirigir el diagnóstico y de esta manera un manejo adecuado. La medicina intensiva y la anestesiología tienen como área de interés común el abordaje del paciente neurológico en un continuo que debe interrelacionarse, ya que comparten un buen número de procedimientos diagnósticos y terapéuticos que al combinarse dan como resultado la mejor evolución de los enfermos. Ejemplo de esto es el neuromonitoreo, cuyos principios y fundamentos deben ser conocidos a profundidad por los profesionales de ambas áreas de la medicina para poder interactuar de una manera coordinada en beneficio del enfermo neurológico grave. El Colegio Mexicano de Anestesiología, refrendando su compromiso en la difusión del conocimiento científico para elevar la calidad y la seguridad de atención en medicina, desarrolló desde hace algunos años una serie de documentos científicos dirigidos a todos los interesados en el manejo del enfermo neurológico, tanto en los quirófanos como en las salas de terapia intensiva. El primer libro editado estuvo relacionado con los fundamentos de la neuroanestesiología, para continuar con dos grandes obras, en primer lugar un texto de neuroanestesiología y terapia intensiva neurológica y en segundo un libro que aborda todos los conceptos de actualidad sobre el manejo integral del traumatismo craneoncefálico.
