Strokes are divided into two groups: ischemic strokes (85%) and hemorrhagic strokes (15%). Brain tissue is very sensitive to ischemia. When the blood flow is interrupted in the vessels feeding the brain, permanent damage occurs within minutes in the areas fed by that vessel. Especially in large vessel occlusions, an irreversible area called ischemic nucleus occurs in areas where the minute blood flow falls below the critical level. There are different blood flow belts that increase from the regions surrounding the ischemic nucleus towards the periphery and are fed by collateral vascular systems. Infarcts have not yet occurred in these areas under ischemic stress. This region is called the penumbra. This area can be saved by opening the vessel in suitable patients presented in the acute period. This is valid for vascular occlusion, but not for brain hemorrhages. Rarely, if an aneurysm is detected in subarachnoid hemorrhages, angiographic intervention may be required to prevent re-bleeding. Early and appropriate treatment of a stroke, which is important in terms of mortality and morbidity, is of vital importance. In our book, the identification and prevention of risk factors, as well as stroke treatment, will be covered. In this context, our book is intended to be a resource for medical doctors and students.

The brain is the body's most vulnerable organ due to the defined roles of neurons within circuits. Neurons are vastly outnumbered by microglia, astrocytes, oligodendrocytes, macrophages, cells of the blood brain barrier and invading immune cells. These cells display different grades of reactivities and interactions. They integrate their responses and not only change phenotypes but can also completely reprogram after damage to protect the neuronal complexity. The interactions of these satellite cells in the healthy brain are described as well as their roles in all major brain diseases. Special emphasis is put on immune system – brain interactions and regenerative and repair processes. The gliotic response is compared with the reactions to injuries of the skin and other organs. A final chapter addresses the definition of a cell type. It concludes that cell types can no longer be regarded as defined entities over the body's lifetime but are prone to phenotype plasticity and even completereprograming.

Stroke is one of the leading causes of disability and death worldwide. It has become the first disease leading to neurologic referrals to hospital and, while recognized in medicine since antiquity, stroke did not acquire a proper place within neurology until the second part of the 20th century. The main reason for this is that, because it deals with blood vessels and circulation, neurologists themselves did not want to include stroke as a primary disorder of the nervous system. However, this is one of the many reasons the historical development of stroke and cerebrovascular disease is of unique interest; it touches not only on neurology but also on internal medicine, angiology, cardiology, rehabilitation, and psychiatry within the frame of secondary behavioral and personality changes after stroke. For the same reasons, the study of the developments in the field of stroke over time is an excellent mirror of historical developments in medicine in general, encompassing many different domains. It is, thus, rather paradoxical that the last book on the history of stroke was published over thirty years ago (Fields and Lemak's History of Stroke in 1989). Along with the many new developments in stroke diagnosis, acute management and prevention, a new look at the evolution of stroke concepts over time has become necessary, and this is what the present volume is attempting to accomplish. Since stroke has developed into several different smaller fields, we have organized the book with four editors from different fields who have chosen authors with specific expertise in the corresponding overviewed issues. Our principle goal is to underline how and to follow the paths of these developments over the years. History is not a minor part of medical knowledge. Indeed, history defines what we are and think today and how we will envision and plan the future. This fresh look at history of stroke may also have a significant impact on future advances and research in cerebrovascular disease and stroke.

Medical and surgical care have made considerable progress in the last two decades in the care of the mother, fetus, and neonate. Most of the improvement in outcomes has been related to advances in diagnostic and therapeutic modalities. These advances have only been possible due to dramatic progress in our knowledge of the physiological mechanisms that determine the normal development of organ systems, as well as the alterations that can lead to disease states and their multifarious consequences. Because improved antenatal care has resulted in the survival of greater numbers of immature infants, they have become a major challenge for clinicians due to the immaturity of multiple organs and their susceptibility to long-term sequelae. The aim of this book is not to address all aspects related to the newborn lung, but to discuss those areas that are more novel or controversial or have been of greater relevance in the progress of maternal, fetal, and neonatal care.
