A Closer Look at Heart Rate opens with an examination of the latent structures underlying short-term heart rate variability indices in a sample of 96 young adults under 4-min eyes-closed followed by 4-min eyes-open resting-state conditions. Electrocardiograms recorded during the two resting-state conditions were then analyzed using a variety of heart rate variability indices in which latent structures were identified using principal component analysis. Additionally, the authors test the hypothesis that the statistical artefact regression to the mean explains part of the baseline effect. To do this, the regression to the mean effect is illustrated through heart rate recording carried out on 1233 volunteers, from which 137 were randomly selected to obtain an estimate of the stress response. Next, 14,000 antepartum fetal heart rate tracings obtained during 2,000 pregnancies are analyzed. After 30 weeks, fetal behavior progressively changes and differentiates into three main states: active sleep, quiet sleep, and active wakefulness. The fetal heart rate pattern during quiet sleep is usually homogeneous. It appears that a heterogeneous pattern reflecting a specific quiet sleep variant has been unrecognized. In the closing chapter, the role of meditation in cardiovascular risk reduction through beneficial modulation in heart rate, and heart-rate variability is reviewed and discussed. Diploma Thesis from the year 2008 in the subject Psychology - Clinical Psychology, Psychopathology, Prevention, grade: 1,3, Dresden Technical University, language: English, abstract: Reduced heart rate variability (HRV) has proved to be an independent risk factor for cardiac emergencies. The present study aimed to evaluate differences in HRV in healthy volunteers high or low in anxiety sensitivity (AS) and potential changes in HRV following an interoceptive exposure (IE) training. We aimed to identify subject features that are associated with potential changes in HRV. Methods: Data were obtained in five subjects high and five subjects low in AS (aged 19 to 23). ECG recordings were conducted in supine position in a mock scanner environment during three experimental conditions. Recordings were repeated after three to seven days of IE training. The square root of successive R-R- interval differences (RMSSD) was calculated for HRV assessment. Potential correlations between subject features and change in HRV were tested with Spearman's rank correlation. Results: No significant HRV differences between subjects high and low in AS in any of the experimental conditions were observed, neither before nor after the IE training. On a descriptive level, subjects high in AS showed lowered HRV compared to subjects low in AS before IE. After IE, subjects low in AS demonstrated increased HRV, while subjects high in AS showed decreased HRV measures. Correlation analyses revealed no significant associations. Conclusion: Descriptive results indicate that there are AS-related differences in HRV, with subjects high in AS showing lowered HRV as hypothesized. Following IE, subjects low in AS showed an increase in HRV, while a decrease occurred in subjects high in AS. The mechanisms of IE require further investigation. If replicated in a larger sample and with adequate study design, IE could prove to lower individual risk for cardiac complications by increasing HRV. Future design implications are discussed.

Book Description: Early detection of subclinical autonomic dysfunction is of vital importance in patients with diabetes mellitus (DM) for the prevention of subsequent serious adverse consequences. Reduction in heart rate variability (HRV) is now regarded as the earliest indicator of cardiovascular dysregulation in DM. HRV has traditionally been quantified using linear measures, which describe the magnitude of RR interval oscillations, but are insufficient to characterize complex heart rate dynamics. While HRV is mostly mediated by parasympathetic nervous system, beat-to-beat blood pressure recordings may provide information regarding sympathetic activity. A variety of novel measures has been developed to quantify nonlinear features of cardiovascular signals, providing information on the complexity of the dynamical system involved in the genesis of these short-term fluctuations. In this book, it is demonstrated that novel nonlinear methods are often more sensitive to autonomic dysregulation than linear methods and therefore may improve the diagnostic power of cardiovascular variability analysis for cardiovascular autonomic neuropathy in DM. Our data indicate that cardiovascular dysregulation progresses in relatively short time frames, depending on the history of DM. Further, its progression appears to be associated with glycemic control. Different methods of cardiovascular variability analysis can provide mutually independent information and therefore should be used simultaneously for a comprehensive analysis of autonomic dysfunction to identify patients at risk for autonomic neuropathy.
