A physician mother shares her true story of learning about opioids in a rural community as she begins medical practice. This class of medications, highly desired for treatment of pain but resulting in addiction for many due to liberal prescribing, fuels an opioid epidemic. Following a reduction in legal opioid availability in her town, heroin arrives to fill the gap. The market for this illegal drug had already been created, leading to many more developing opioid use disorder. Her own son makes a single bad decision and is exposed to heroin. As a result of her experiences doing everything a mother can to keep her son alive, she is shocked by the stigma and lack of effective and affordable care for people with this disease. She discovers that the treatment with the best track record in helping people achieve recovery is a medication called buprenorphine. When emergency departments provide the first dose when a person with opioid use disorder is in withdrawal, then refer to out-patient clinics plus community programs to provide jobs and housing, people with this disease can recover. She initiated inductions with buprenorphine in her ER and demonstrates how easily this can be done in every hospital.

How withdrawal distress and cravings can haunt current and former addicts, and what they can teach us about addiction and its treatments.“The dead drug leaves a ghost behind. At certain hours it haunts the house,” Jean Cocteau once wrote. In The Ghost in the Addict, Shepard Siegel offers a Pavlovian analysis of drug use. Chronic drug use, he explains, conditions users to have an anticipatory homeostatic correction, which protects the addict from overdose. This drug-preparatory response, elicited by drug-paired cues, is often mislabeled a “withdrawal response.” The withdrawal response, however, is not due to the baneful effects of previous drug administrations; rather, it is due to the body's preparation for the next drug administration—a preparatory response that can haunt addicts like a ghost long after they have conquered their usage.Examining the failure of legislation, the circumstances of overdose, and the cues that promote drug use, Siegel seeks to counter the widespread belief that addiction is evidence of a pathology. Instead, he proposes that the addict has an adaptive, learned response to the physiological changes wrought by drug use. It is only through understanding so-called withdrawal symptoms as a Pavlovian response, he explains, that we can begin to understand why addicts experience cravings long after their last drug use.

In our current digital era, imagination and the cultural and material conditions by which it is developed are more crucially than ever implicated in the experienced adversities and contradictions of drug use. The technological changes of society underscore the need for rethinking dominant understandings which portray addiction as an immediate and even mindless relation between a person and a substance or behavior, only minimally affected by subjective significance and historical alterations of everyday life. Indeed, from ancient mythology to our modern times drugs have been part of our cultural history. Understandings and practices of their uses have developed through cultural ideas and cultural-material conditions like traditions, rituals and routines. Today, the omnipresence of digital media in everyday life is massively changing and expanding such cultural and material conditions. Digital media equip people with associations between drugs and an incredible abundance of images, ideas, facts, fiction, narratives, plots, soundtracks, characters, and much more, and thereby expanding their imaginable potentials for providing answers to biographical questions. People and potential drug use become connected in novel and labyrinthine ways through digital communities and arrangements of everyday life. And digital media are part of and transform the cultural-material practices in which activities and experiences of intoxication actually take place. In the book, all these details are extensively analyzed empirically based on qualitative data on the lives of a number of young, Danish people who were undergoing treatment for drug-related problems at the time of the research. An underlying premise of the entire work is that addiction may be seen as a more extreme expression of how the technological developments in our contemporary world more generally speaking magnify the contradictory implications of imagination for modern living. Over the recent years, psychological research into the significance of the human capacity to imagine for how people deal with and live their lives has received growing attention. Yet, the complex involvement of imagination in actual living and consequently the theoretical cruxes this engenders continue to amaze and surprise research and researchers. This book also contributes to these theoretical ambitions with a substantial work on the concept of imagination. It primarily suggests that a critical discussion of how imagining is essentially a contradictory process in everyday life and how it is always grounded in the agency of material aspects, ranging anywhere from mundane artifacts over mediated content to advanced technologies, is ultimately what makes the scientific study of imagination relevant to understanding and intervening in the dilemmas and crises of modern life and society. The book will primarily interest scholars of social psychology of everyday life, scholars working conceptually and empirically on imagination, scholars of social studies of media, materiality and technology, and researchers or practitioners working with addictions.

- Abstract
Behavioral health and substance use disorders affect approximately 20 percent of the U.S. population. Of those with a substance use disorder, approximately 60 percent also have a mental health disorder. Together, these disorders account for a substantial burden of disability, have been associated with an increased risk of morbidity and mortality from other chronic illnesses, and can be risk factors for incarceration, homelessness, and death by suicide. In addition, they can compromise a person's ability to seek out and afford health care and adhere to treatment recommendations. To explore data, policies, practices, and systems that affect the diagnosis and provision of care for mental health and substance use disorders, the Health and Medicine Division of the National Academies of Sciences, Engineering, and Medicine created the Forum on Mental Health and Substance Use Disorders. The forum activities are expected to advance the discussion and generate potential ideas on ways to address many of the most persistent problems in delivering mental health and substance use services. The inaugural workshop, held October 15-16, 2019, in Washington, DC, explored the key policy challenges that impede efforts to improve care for those individuals with mental health and substance use disorders. This publication summarizes the presentations and discussion of the workshop.

41. Prescription Drugs: Development, Pricing and Abuse

The Food and Drug Administration (FDA) awards priority review vouchers (PRV) to drug sponsors that develop drugs for tropical diseases or rare pediatric diseases or to use as medical countermeasures. The PRV—which can be sold to another drug sponsor—may be redeemed later to receive priority review from FDA with a targeted review time of 6 months, rather than the 10-month standard review, for a drug application of the PRV holder's choice. Chapter 1 examined the number of PRVs awarded and redeemed and the drugs for which they were awarded or redeemed, and what is known about the extent to which the PRVs provide incentives for developing drugs to meet unmet needs. Chapter 2 examines the prevalence of obesity in the United States, and what is known about the use and health insurance coverage of obesity drugs. Chapter 3 discusses how to get medical care and prescription drugs during a disaster or when an emergency has been declared. The 340B Drug Pricing Program was created in 1992 to enable eligible covered entities—including certain categories of hospitals—to obtain discounts on covered outpatient drugs. Chapters 4-6 examine various aspects of this Medicare program. Chapter 7 provides an overview of how drug prices are established under Part D and describes elements of various proposals for Secretarial negotiation. To mitigate the effects of the opioid crisis in Philadelphia, a nonprofit called Safehouse announced plans to open a supervised injection site where illicit drug users could consume opioids under the supervision of trained staff, receive medical intervention in case of an overdose, and access services including addiction treatment. Chapter 8 discusses the Safehouse litigation's importance for Congress by first providing relevant factual and legal background, before identifying select considerations for Congress related to the treatment of supervised injection sites under the CSA. Parents are often concerned that their children will start (or are already) using drugs such as tobacco, alcohol, marijuana, and others—including the misuse of prescription drugs. Research supported by the National Institute on Drug Abuse (NIDA) shows the important role that parents play in preventing drug use in their children. Chapter 9 highlights parenting skills that are important in preventing the initiation and progression of drug use among youth.

Bachelor Thesis from the year 2019 in the subject Didactics - Social Studies/ Civics, grade: 5,, language: English, abstract: Problem of drug abuse among secondary schools is one of the major social problems. It affects significant number of students and therefore intervention is required. It is within this context that the researchers embarked purposefully on the study of present study'effect of drug abuse and addiction among secondary school students in Nassarawa local government area, Kano state'. Methods used in collecting data were distribution of questionnaires to respondents. Additionally documentary materials, such as books, newspaper, internet, leaflet, journals related to drug abuse among students were reviewed. Data obtained were analysed by using simple percentage. This package was used because the study was explorative and it draws conclusions, opinions and confirmations from nature. According to empirical findings, the causes of drug abuse among students are evident among: Boredom with academic work or domestic activities, students that are not doing very well in school are not friends of lecturers easily get disinterested and tired with academic work. So to get occupied with pleasant thoughts and activities, they indulge in the use of hard drugs, peer group influence is also one of the strongest motives, which drive students into drug abuse.

This book considers how largely accepted ‘legal truths'about drugs and addiction are made and sustained through practices of lawyering.Lawyers play a vital and largely underappreciated role in constituting legal certainties about substances and ‘addiction', including links between alcohol and other drugs, and phenomena such as family violence. Such practices exacerbate, sustain and stabilise ‘addicted'realities, with a range of implications – many of them seemingly unjust – for people who use alcohol and other drugs. This book explores these issues, drawing upon data collected for a major international study on alcohol and other drugs in the law, including interviews with lawyers, magistrates and judges; analyses of case law; and legislation. Focussing on an array of legal practices, including processes of law-making, human rights deliberations, advocacy and negotiation strategies, and the sentencing of offenders, and buttressed by overarching analyses of the ethics and politics of such practices, the book looks at how alcohol and other drug ‘addiction'emerges and is concretised through the everyday work lawyers and decision makers do. Foregrounding ‘practices', the book also shows that law is more fragile than we might assume.
