As we began to plan this book about 10 years ago, we wanted to write a practical book for medical students, doctors in training, internists, gas and surgeons faced with everyday clinical problems in troenterologists, the care of patients with acute and chronic pancreatitis. Now, 10 years lat er, our book has become much more than what we had originally intend ed, with so many new findings that have expanded our knowledge of pancreatic diseases, not to mention new imaging procedures and con trolled studies on the effectiveness of various therapies. Consequently, we decided to include: • The state-of-the-art on diagnosis and therapy in inflammatory pan creatic diseases • Recommendations for diagnosis and therapy based on the published results from experienced clinicians. In the many areas still untested we have relied on the decades of experience of the two authors on both sides of the Atlantic • A discussion of unanswered questions for future research by physi cians interested in pancreatitis who can draw upon our bibliography of almost 1900 references complete through 1996. We also wanted to demonstrate with this work that two authors from two continents can come to common conclusions that can help physicians and students in their everyday clinical work and their clinical research. The IFIP International Working Conference on Intelligent Networks'95 was held at the Technical University of Denmark (DTU) in Copenhagen. The conference was organised jointly by the Center for Tete-Information at DTU and Tele Danmark Research! during August 30-31 1995 and was hosted by the Center for Tete-Information. The conference was sponsored by IFIP TC6. The papers contained in this book are edited versions of the papers from the conference and some of the tutorial presentations that were arranged prior to the conference. The mixture of tutorial papers and conference papers gives a broad and yet thorough coverage of activities and central areas of development in the IN world at this point in time. Future developments, especially in the area of extended mobility support and multimedia capabilities, are also pre sented. BACKGROUND The theme of the conference was the two major trends in Intelligent Networks (IN) develop ment: • ITU and ETSI based IN standards, such as CS-1 and the upcoming CS-2 and • long term development as undertaken by, for example, the TINA consortium and the Euro pean RACE and ACTS programmes. The present IN technology based on capability sets is on its way into the networks, and opera tors are pushing for open platforms to be deployed in the networks in the near future in order to obtain greater freedom in selecting platforms for service provisioning.

The purpose of this book is to provide a special viewpoint on the development of the Finnish school system and teacher education. Understanding the success of today requires information about the past. The book covers the history, ideological background, and development of Finnish teacher education from the 19th century to today. The historical review uses a northern Finnish teacher training college of Tornio as the example. This book provides interesting information about the ideological foundation of the first teacher training colleges, description of how the ideology was applied in the practice of teacher training, and how students were selected for teacher training. The development of teacher training faced certain hardships too. What happened in Finnish society since the establishment of teacher training colleges in the second half of the 19th century affected Finnish teacher training significantly. The book brings out the special circumstances in Finland at the beginning of the 20th century focusing on the war years of 1939-1945. This book, in a very tangible manner, presents the implementation of the educational ideologies of Finnish colleges and elementary schools shedding light on the success story of the Finnish educational system as it has been continuously ranked among the best in the world. The Finnish ethos of seeing education as an act of social justice seems still functional. The belief in the value of learning and education as the means to achieve success has been strong in Finland already for over a hundred years. This book provides a multidimensional description of the historical phases of the Finnish teacher education up to today. Carbonic anhydrase (CA) is a seemingly ubiquitous enzyme of profound physiological importance, which plays essential roles in respiration, acid-base homeostasis, bone resorption, calcification, photosynthesis, several biosynthetic pathways and a variety of processes involving ion, gas and fluid transfer. There have been many advances in the field of gastrointestinal pa thology which are of considerable clinical significance during the 13 years since the last publication of a volume of Current Topics in Pathology devoted to this subject. Many have arisen from the app lication of new techniques of histochemistry, immunocytochemi stry, quantitative morphometry and molecular and cell biology to gastrointestinal diseases, but some, notably the recognition of the association of Campylobaeter pylori with the commonest type of chronic gastritis, have been achieved using such long established'routine'histological procedures that one wonders how their signifi cance had escaped recognition for so long. The topics covered in this volume have been selected because they present advances of relevance to the diagnostic clinical pathologist. However, they re present the personal selection of the editor, and are in no way exhaustive. Many other examples of progress in our understanding of the pathophysiology of gastrointestinal diseases have been omit ted, either because of the confines of space or because they have been well reviewed recently in other publications. Most of the workload of the practising gastrointestinal patholo gist involves the diagnosis and assessment either of inflammation or of neoplasia in the alimentary tract, and this is reflected in the topics presented in this book.

At the beginning of the century the average life span in developed societies was less than 50 years. It is now more than 70 years and will continue to increase in the near future. Aging is a challenge to science and society. There are several theories of aging. The programmed aging hypothesis suggests that aging cannot be altered by any external influence, that aging is programmed from the beginning of life until the end of senescence. Another theory is based on genetic instability, with repair processes correcting errors in the cells that cause aging. Aging processes manifest themselves at the molecular and cellular level, in different organs, and in the whole organism. Most older persons acquire multiple diseases, primarily chronic ones. The multimorbidity of the elderly raises many questions as to nutrition, metabolism, immunity, drug therapy, and rehabilitation. Nutrition may be able to intervene in the aging process. Aging is associated with dec lining immunity and reduced resistance to many diseases, and it is pos sible that nutritional changes in old age contribute to the declining immunity in the elderly. The elderly take 40% of the prescription drugs in Germany and adverse drug reactions are more frequent in geriatric patients. There are not only problems in drug therapy for elderly pa tients, but also in their rehabilitation. The aim of rehabilitation is the restoration of geriatric patients to their former functional and environ mental status.

The impact of Helicobacter pylori on basic science and the clinical management of patients with the complications of this infection is bewildering. The explosion of new information both in the laboratory and at the bedside has progressed at an unprecedented rate. Our main objective in furthering this progress has been to integrate this new information and organize a series of top-quality presentations and discussions between investigators and clinicians on all aspects of H. pylori research and to review the current position and future research directions. To that end, the second meeting'Helicobacter pylori: Basic Mechanisms to Clinical Cure'was organized in June 1996 in Ottawa, Canada, following the successful format of the first such meeting held in Amelia Island, Florida, in 1993. The meeting again focused on all timely aspects of H. pylori research. This Trends in Inflammatory Bowel Disease Therapy Symposium was held in Vancouver, British Columbia, Canada, 27~29 August 1999. This is the seventh international symposium, sponsored by Axcan Pharma Canada Inc., since the first symposium in 1986. The Canadian Association of Gastroenterology has co-sponsored these symposia since 1990. Each symposium has been published, and reflects and updates the extensive research and education, and the understanding of the mechanisms and treatment of inflammatory bowel disease. From the beginning, Canadian and international experts have been involved, maintaining a consistently high quality, both in their presentations, during discussions, and the subsequent publication of their work. There have been major advances since the symposium held two years ago, and these are presented in this book. In keeping with modern educational practice, objectives are provided, and all presentations were subject to written independent evalu ation. The fourth meeting in the very successful series Helicobacter pylori: Basic Mechanisms to Clinical Cure was held on the island of Bermuda in late March 2000. This was only some two years after the third meeting in San Diego and it seemed hardly possible that there would be so much new information. However, as the contributions in this volume testify, there was plenty of exciting new information with important implications for both understanding this infection and for clinical management. Some of this information was of a fundamental nature, such as the role of the acid sensitive urel channel in regulating the influx of urea and the formation of ammonia transported back in the microbial periplasmic space to neutralize acid; the observation of genetic polymorphism of the IL-1~ gene as an explanation of achlorhydria and gastric cancer risk in the first-degree relatives of gastric cancer patients; and the peculiar biochemical and physio logical consequences of the genome of the microorganisms. The format of the meeting, with short fifteen-minute state-of-the-art pre sentations by world experts closely involved in Helicobacter research fol lowed by ample time for panel discussions, was again followed this year. Traditional aspects included detailed study of the microbial characteristics, the novel Helicobacters, the interaction with the human host, the peculiarities of the inflammatory immune response, the short and long-term mucosal consequences, the effects on acid secretion, the problem of gastric malignancy and the therapeutic possibilities.

Heat shock proteins (HSP) have received ample interest by immunologists over recent years. Initially they were found to be dominantly immunogenic microbial antigens. In 1868, Ernst Neumann recognized that blood cells re quire continuous replenishment during postnata1life. Before him, the assumption was that cells of the blood, like nerves once formed in the embryo, remain in the body throughout life. Neumann also recognized that this process occurred within the bone marrow, because this tissue provided a fa vorable environment for proliferation and differentiation of blood cell precursors. Vera Danchakoff, the Russian embryologist working in the US, in 1916 made an analogy to the soil and the seed. Bone marrow forms the soil, providing a favorable environment for the growth of seed, the hemopoietic stem cell, and other progenitor cells. Imagine in the remote past a heap of similar tree seeds. These seeds develop in our moderate climate into a tall and many branched tree. Suppose the wind bears a part of the seeds away and brings them to a land possessing different environmental conditions, we will say the arc tic lands. There the seeds may develop but they may pro duce trees no higher than our moss.

The fact that tumors are composed of both tumor cells and host cells has long been known. These tumor-associated cells include vascular endothelial cells and pe- cytes, as well as inflammatory cells such as neutrophils, monocytes, macrophages, mast cells and eosinophils, and lymphocytes. The tumor cells also interact with stromal cells and with elements of the tissue extracellular matrix. What has been less appreciated is the role that these cells could have in modulating the growth, invasion, and metastasis of the tumor. Early on, the elements of what we now call the tumor microenvironment were considered to be more or less innocent bysta- ers to the role of the tumor cells as they grew and invaded local sites. Today, there is an increased understanding of the critical role of the tumor microenvironment as dramatically influencing the course of tumor development and dissemination. This volume represents a superb compilation of the latest thoughts and data regarding the role of each essential component of the tumor microenvironment in cancer development and progression. Perhaps, the earliest recognition of the role of nonmalignant cells as cancer re- lators was the recognition that lymphocytes can participate in what was termed “immune surveillance” in the 1960s.
