A review of the book “A Really Good Day: How Microdosing Made a Mega Difference in My Mood, My Marriage, and My Life,” by Ayelet Waldman, is presented.

Based on the idea that mood helps to tune cognitive processes to current demands, we hypothesised that mood should influence the abstraction level in which people think about behaviour, and that such mood‐induced differences in cognitive attention should be manifested in behaviour descriptions. In this study, participants re‐described a number of daily behaviours in their own words after a mood manipulation. As predicted, people in a positive mood were more likely to re‐describe behaviours in general why terms, whereas people in a negative mood were relatively more likely to re‐describe behaviours in specific how terms. Objective: To assess the clinical characteristics and the optimal treatment of obsessive compulsive symptoms in patients with familial background, current or past symptoms consistent with a bipolar spectrum diagnosis. Methods: Longitudinal clinical observation of seven cases with obsessive-compulsive-bipolar comorbidity. Results: In the cases presented that met the DSM-IV-TR criteria for obsessive compulsive disorder (OCD), the concurrent symptoms and the clinical course pointed to a different principal diagnosis and suggested therapeutic strategies different from those currently recommended in OCD. Clear-cut onset of symptoms, high prevalence of mood disorders in relatives, revaluation of their history, switch into mania induced by antidepressants, and clinical follow-up suggested a hierarchical priority of bipolar diagnosis. Conclusions: The presence of the aforementioned atypical symptoms should alert the physician about the possibility of bipolar comorbidity in OCD patients. If the suspicion is confirmed, bipolar diagnosis should have priority, at least from a therapeutic point of view. The first choice in treatment should be with mood stabilizers or second generation antipsychotics. Mood stabilization should be achieved as a first objective. [ABSTRACT FROM AUTHOR]

Objective: To report a case of a patient with Behcet's syndrome who developed treatment resistant bipolar disorder (BD) several years after the onset of Behcet's syndrome. Methods: A 62-year-old woman suffering from Behcet's syndrome since the age of 38, who developed a typical BD 6 years after the first manifestations of the syndrome was described. Results: Once BD occurred, Behcet's syndrome became milder, while BD deteriorated and evolved into a rapid cycling illness. Lithium and carbarnazepine were ineffective in controlling the affective symptoms, while sodium valproate combined with low doses of carbamazepine and olanzapine resulted in sufficient stabilization of her mood state. Conclusions: Behcet's syndrome may have been the organic substrate for BD in this case. The appearance of BD in the setting of an organic- immune disorder, like Behcet's syndrome, suggests that such disorders may be the neurobiologic substrate or contributor for BD, at least in certain cases. Bipolar Disord 2004: 6: 162–165. © Blackwell Munksgaard, 2004 The syndrome of folie a deux is uncommon and often described in the context of schizophrenia. We report a case of induced delusional disorder associated with bipolar affective disorder (BAD). We present a case of monozygotic twins in their late 60s with an unusually close relation with one another and relative isolation from other people. Both twins have been diagnosed as suffering from BAD and relapsed into mania with psychotic symptoms. During their hospital stay they exhibited features consistent with folie a deux. Separation caused disappearance of the phenomenon whilst the affective disorder persisted. This case highlights the unusual and rare phenomenon of folie a deux occurring in the context of BAD. It also suggests current difficulty in defining folie a deux as an entity according to current diagnostic criteria. [ABSTRACT FROM AUTHOR]
