4. Environmentally Induced Damage to Children: A Call for Broadening the Critical Agenda.

Abstract

The incidence of environmentally related illnesses, such as tuberculosis, asthma, allergies, respiratory disease, depression, and violent anger is increasing, particularly in the inner cities. The effects of these illnesses is often overlooked in discussions of educational and social inequity. This article discusses the significance of this increase in disease with regard to the welfare of children and the impact on their academic achievement, due to physical and mental impairments. Information is provided about the causes, consequences, and rates of incidence of tuberculosis, lead poisoning and asthma. The article comments on the ways in which society and the schools have responded to these illnesses, and then considers the context of discussion about these illnesses and the social response to them. The article notes that, often, these illnesses are considered an affliction of the poor. The article issues a challenge for school reform that addresses environmentally induced damage to children as an educational issue, as well as a social one. Contains 44 references. (JPB)

5. Childhood Epilepsy and Asthma: A Test of an Extension of the Double ABCX Model.

Abstract

The Double ABCX Model of Family Adjustment and Adaptation, a model that predicts adaptation to chronic stressors on the family, was extended by dividing it into attitudes, coping, and adaptation of parents and child separately, and by including variables relevant to child adaptation to epilepsy or asthma. The extended model was tested on 246 children (126 with epilepsy and 120 with asthma, ages 8-12), using data gathered in interviews and questionnaires involving the children, their mothers, and their school teachers. Structural equation modeling was carried out to test the model's relationships. Coping and adaptation measures were treated as endogenous variables and family adaptive resources, demographics, family demands, attitudes, health condition, and school status measures were treated as exogenous variables. Moderate support was found for the variables proposed in the extended Double ABCX Model child adaptation to epilepsy or asthma. Child coping patterns were the strongest predictors of child adaptation at home and at school, and child's attitude was the strongest predictor of child self-concept. However, parental coping, demographic, and health condition variables accounted for essentially no variance in child adaptation. The model to predict child adaptation to epilepsy was more complex than the model for asthma in that more exogenous variables were retained. (14 references.) (JDD)

6. Childhood Epilepsy and Asthma: Family Strengths Associated with Child Adaptation.

Abstract

This study aimed to describe demographic, seizure, and family characteristics associated with good and poor adaptation to childhood epilepsy and contrast them with demographic, asthma, and family characteristics associated with good and poor adaptation to childhood asthma. Children with asthma were selected as a comparison sample because epilepsy and asthma are both chronic medical conditions that are characterized by unpredictable episodes that generally require regular intake of medication and regular visits to a physician. Data were gathered from the children (ages 8-12), their mothers, and their school teachers. The theoretical framework for the selection of family characteristics was the Double ABCX Model of Family Adaptation and Adjustment in which three factors affecting coping are family demands or stressors on the family, family adaptive resources, and family definition or attitudes toward the condition and its impact. Results showed that family strength variables were strong predictors of child adaptation to both epilepsy and asthma. The mother's attitude was the strongest predictor of child adaptation for asthma but was not a strong predictor for epilepsy. (14 references.) (JDD)

7. Chronically Ill Children: A Psychologically and Emotionally Deviant Population?

Abstract

To investigate vulnerability to psychological and emotional stress among chronically ill children, a battery of personality tests was selectively administered to 144 children (5- to 19-years-old) afflicted with one of the following conditions: diabetes, asthma, cystic fibrosis, or hearing impairment. Analyses centered on comparisons of norms developed for each scale with overall sample scores and scores broken down by age, sex, and type of illness. While some differences from normal children were noted (such as the hearing impaired Ss deviated from established norms in terms of lowered self-concept and that adolescents tended toward introversion), results consistently demonstrated Ss' functional strengths and coping abilities rather than their deviance from the norm. 9. My School Makes Me Sick: Cheap Solutions to Environmental Problems in Schools.

Abstract

This paper presents 19 solutions to problems within the school environment: (1) ventilation (e.g., keep the thermostat fan on whenever the room is occupied); (2) filters (e.g., get rid of 20 percent cheap filters); (3) clean the ductwork; (4) avoid car and bus fumes by keeping vehicles 50 feet from the building; (5) sewer vents (vents must terminate at least 10 feet from a powered fresh air intake); (6) furnace exhaust pipes (if the furnace vent or other vent is closer than 10 feet from a powered fresh air intake, it must extend at least 3 feet above the intake); (7) floor and roof traps; (8) unvented science labs (which can send fumes into classrooms); (9) cosmetology odors (when there is no lab hood for ventilation); (10) CO2 testing (e.g., CO2 builds up when there are several persons in a room over several hours with inadequate ventilation); (11) asthma and respiratory ailments (e.g., carpeting should not be in schools, and ductwork needs regular cleaning); (12) UV lights (which kill most organisms in the air that pass through the ductwork); (13) cold temperatures (an adequate thermometer is important); (14) increased complaints about cold when proper ventilation blows air into the room; (15) asbestos testing; (16) radon testing; (17) lead paint testing; (18) water quality; and (19) mold. 12. An Approach to Management of Critical Indoor Air Problems in School Buildings.

Abstract

This study was conducted in a school center in Finland that had been the focus of intense public concern over 2 years because of suspected mold and health problems. Because several attempts to find solutions to the problem within the community were not satisfactory, outside specialists were needed for support in solving the problem. The study group consisted of experts in civil engineering, indoor mycology, and epidemiology. The studies were conducted in close cooperation with the city administration. Structures at risk were opened, moisture and temperature were measured, and the causes of damage were analyzed. Microbial samples were taken from the air, surfaces, and materials. Health questionnaires were sent to the school children and personnel. Information on the measurements and their results was released regularly to school employees, students and their parents, and to the media. Repairs were designed on the basis of this information. Moisture damage was caused mainly by difficult moisture conditions at the building site, poor ventilation, and water leaks. Fungal genera typical to buildings with mold problems were collected from the indoor air and surfaces of the school buildings. Where moisture-prone structures were identified and visible signs of damage or elevated moisture content were recorded, the numbers of microbes also were high; thus microbial results from material samples supported the conclusions made in the structural studies. Several irritative and recurrent symptoms were common among the upper secondary and high school students. The prevalence of asthma was high (13 percent) among the upper secondary school students. During the last 4 years, the incidence of asthma was three-fold that of the previous 4-year period. (Contains 18 references.) (Author/EV)

13. Imaginal Coping and Childhood Illness: How Children Relate to Treatments for Chronic Illness.

Abstract

This ethnographic study used multiple approaches to try to determine the emotional experience of young children (ages 5 to 8) with chronic illnesses. Forty-six children with severe asthma and diabetes were interviewed on two separate occasions using child-centered in-depth interviews that included play-based interviewing. The study also employed indirect observation with mothers keeping a notebook of observations and also photographing relevant moments. Findings focused on children's utilization of imaginal coping, in which the child makes use of a non-literal reality, towards which the child suspends disbelief and gains a sense of serenity and trust--such as the use of a special blanket to provide comfort during treatment. Children tended to define their illness not in bodily or organic terms but in terms of the concrete suffering derived from treating the illness. Children repeatedly ascribed special powers to their comfort object or ritual acts associated with treatment procedures. Pretend play and giving symbolic importance to medication procedures were other imaginal coping strategies used by the children. Health care providers are urged to consider children's coping strategies in planning or adjusting treatments. (Contains 12 references.) (DB)
