Root canal re-treatment (RCR) cases are considered some of the most challenging cases in the field of endodontics, as they are mostly associated with various iatrogenic errors such as ledge formation, incomplete biomechanical preparation, file separation, and incomplete obturation. These iatrogenic errors lead to defective niches within root canals that may act as reservoirs for various viable microorganisms. Such residual microbial niches may cause postoperative pain even after thorough debridement and reshaping the canals, ultimately leading to a poor prognosis for the tooth. Nowadays, prevention of postoperative pain in re-treatment cases and prognosis are effectively managed by photobiomodulation therapy (PBMT).

This report discusses the case of a failed root canal treatment in the right mandibular first molar, which exhibited a chronic periapical pathology in its distal and mesial roots. The tooth was managed surgically by periapical curettage and apicoectomy. This approach is indicated in teeth having periapical pathosis which does not resolve by routine endodontic treatment. The aim of periapical curettage is to make the apical periodontium and root canals of the treated tooth free of bacteria and their products. With the advancements in magnification, armamentaria, and materials, the ease of operation and success rate of apicoectomy have underwent a significant improvement. The treated case showed a complete resolution of clinical symptoms and the periapical pathology, thus substantiating the effectiveness of this treatment approach.

Endodontic retreatment with direct crown access is the main choice in cases of endodontic treatment failure, even if the practitioner`s effort and duration is significantly greater compared with other procedures. The retreatment with crown access repeats root canal treatment through direct access at the level of tooth crown. The objective is to clean up the canal from irritant agents, such as microorganisms, surviving from the previous therapy or passed through afterwards. Thus, a treatment restart is in accordance with the logics of root canal therapy and is preferred at any time if possible. The aim of this study was to observe, over a 4-year span, the efficiency of endodontic retreatment with direct coronary approach. From 2012 to 2016, 148 patients, with ages ranging from 18 to 65 years, were retreated endodontically. All patients were included in a regular monitoring plan of 3 months and 1-year post treatment. X-rays were performed during these observations, assessed with Orstavik criteria (1991), each radiography having assigned a periapical index (PeriApical Index – PAI). In conclusion, this study confirms that first choice of retreatment in case of endodontic treatment failure is restarting of the retreatment with direct crown access (91% success rate), failure of which leads to a surgical approach using endodontic microsurgery techniques.

The outlined systematic approach is accessible in general practice, where the vast majority of endodontic treatment is conducted, requires basic equipment and with the correct experience can be applied to both straight and curved canals. By overcoming this initial stage of retreatment, subsequent chemical disinfection, which is critical to success, can be carried out to a higher standard reducing risks of re-infection.

A recent report from the University of California San Francisco (UCSF) highlights the findings of a systematic review on contemporary nonsurgical endodontic retreatment (NS-ReTx). The review aimed to identify rigorous studies on NS-ReTx and investigate periapical healing and success rates. Background: The failure of endodontic treatment caused by failure of the restoration and bacterial invation. Alongside proper chemical and mechanical preparation and hermetic obturation of the root canal system, another essential factor is ensuring a permanent and hermetically-sealed restoration following endodontic treatment. Endodontic retreatment encompasses the efforts to salvage a failing previously endodontically treated tooth. As a result of advances in endodontic materials and adhesive systems, restorations in the form of fiber post and adhesively cemented crown are becoming a choice in cases of teeth that have extensive damage. Case: A case is presented a 45-year-old female was referred to Prof. Soedomo Dental Hospital with a complaint of pain in mandibular second premolar which was earlier endodontically treated. Clinical and radiographic examinations showed damaged restorations, gutta percha filling that are not hermetic and radiolucent on periapical. A successful endodontic retreatment premolar using rotary retreatment file and preparation with crown down technique using rotary progressive multiple tapering file, with comprehensive endo-prosthetic treatment using prefabricated fiber post and porcelain fused to metal crown. Conclusion: Clinical and radiographic evaluation after 6 months showed complete healing and good adaptation of the restoration. The outcomes of this case showed that failure of endodontic retreatment respond favorably to endodontic retreatment, with proper indication, prefabricated fiber post and porcelain fused to metal crown can provide an effective conservative and esthetic option for reinforcing endodontically treated teeth.

Nonsurgical endodontic retreatment can be a successful treatment option, especially if the existing root canal therapy is technically deficient. It is the preferred treatment for most endodontic failures unless iatrogenic, restorative, or morphological factors dictate otherwise. A restorative post in the involved canal is usually considered an indication for apical surgery; however, in cases where nonsurgical retreatment is preferable to surgical management, many posts can be removed.

Endodontic failure is a difficult diagnosis to confirm. Various other pathologic entities that must be ruled out by radiographs and tests have been discussed. Because of significant and, as yet, unresolved problems with surgical endodontic treatment, conservative retreatment should be given priority in treatment planning. Specialized techniques that can be used successfully to remove endodontic posts, gutta-percha silver cones, Thermafil, and paste filling materials from root canals have been discussed as well.

Anatomical and morphological variations in teeth may affect the success and prognosis of endodontic therapy. The reported data regarding such variations in the maxillary central incisors are limited. This article presents a case of endodontic retreatment in unusually long maxillary central incisors.

The present communication describes the endodontic retreatment of a rare four-rooted maxillary second molar. A 25-year-old patient was referred to our dental practice requesting an apicoectomy because of continuous and permanent pain reaction six months after the first endodontic treatment. The sent radiograph demonstrated three filled root canals (one mesial, two distal) and four radiographically superimposing roots (two mesial, two distal). Due to the diagnosed chronic apical periodontitis and based on the visible untreated root canal, we decided to endodontically re-treat the quadrangular tooth against the referred apicoectomy. In the first session, only the previously untreated mesiopalatal root canal was mechanically prepared and filled with a corticosteroid- and tetracycline-containing paste. After two symptom-free weeks, the gutta-percha was removed from the other canals and calcium hydroxide was applied. Another two weeks later, the four root canals, whose orifices were “irregular quadrilateral” shaped on the pulp chamber floor representing Versiani Type A, were obturated. After the verification of this rare anatomy by cone beam computed tomography, the tooth was classified as Christie’s radicular type II. One characteristic of this type is parallel running buccal and palatal roots, which caused a radiographic superimposition and probably led to the endodontic mistreatment in the first case.
