- Abstract
Background and objective:: Accurate IVD segmentation is crucial for diagnosing and treating spinal conditions. Traditional deep learning methods depend on extensive, annotated datasets, which are hard to acquire. This research proposes an intensity-based self-supervised domain adaptation, using unlabeled multi-domain data to reduce reliance on large annotated datasets. Methods:: The study introduces an innovative method using intensity-based self-supervised learning for IVD segmentation in MRI scans. This approach is particularly suited for IVD segmentations due to its ability to effectively capture the subtle intensity variations that are characteristic of spinal structures. The model, a dual-task system, simultaneously segments IVDs and predicts intensity transformations. This intensity-focused method has the advantages of being easy to train and computationally light, making it highly practical in diverse clinical settings. Trained on unlabeled data from multiple domains, the model learns domain-invariant features, adeptly handling intensity variations across different MRI devices and protocols. Results:: Testing on three public datasets showed that this model outperforms baseline models trained on single-domain data. It handles domain shifts and achieves higher accuracy in IVD segmentation. Conclusions:: This study demonstrates the potential of intensity-based self-supervised domain adaptation for IVD segmentation. It suggests new directions for research in enhancing generalizability across datasets with domain shifts, which can be applied to other medical imaging fields.

3. Efficacy and Adverse Effects of IV Morphine for Burn Pain Management in the Emergency Department: An Observational Study

- Abstract
The management of pain following a burn is extremely complex because of the multifactorial nature of burn pain. The main goal of acute pain management is to reduce the patient’s pain, allowing them to maintain function and to prevent the chronification of pain. Opioids are used as first-line treatment in management of burn pain. In this single-center observational study, patients presenting with severe burns were enrolled in our hospital. Pain intensity and anxiety level were evaluated at admission and after 1 h. We evaluated pain treatment using intravenous (IV) morphine. Thirty patients were included in this study. At the time of arrival in hospital, almost all patients reported severe pain and received IV morphine to achieve pain relief. After 1 h, > 65% of patients had no pain. No adverse events were recorded related to morphine administration. The anxiety level improved after pain treatment. Finally, IV morphine used for burn pain management in the emergency setting significantly improves patient outcomes in terms of pain and reduced agitation.

4. LISA and LISA-like mission test-mass charging for gamma-ray burst detection

- Abstract
Cubic gold-platinum test-masses (TMs) play the role of free-falling geodesic reference and interferometer end mirrors of the future Laser Interferometer Space Antenna (LISA) observatory for low-frequency gravitational wave detection in space. A similar arrangement has been proposed for the Chinese missions Taiji and TianQin and for the LISA follow-on missions such as ALIA and BBO. The TMs are charged by high-energy particles and photons. The deposited charge couples with stray electric fields surrounding the TMs thus inducing spurious forces that limit the sensitivity of the mission mainly at low frequencies. The TM charging was measured in space in 2016-2017 with LISA Pathfinder (LPF), meant for the testing of the LISA instrumentation. Unfortunately, during the time LPF remained in orbit, no solar energetic particle events or major astrophysical phenomena were observed. We aim at estimating the LISA TM charging attributable to long, short gamma-ray bursts (GRBs) and magnetar flares in comparison to that of charged particles of galactic and solar origin. The contribution of these major astrophysical phenomena to the LISA TM charging is discussed here for the first time. The results found here can be extended to LISA-like missions. The response of the radiation monitors hosted on the three LISA S/C is also evaluated. We show that long, intense extragalactic GRBs and galactic magnetar flares at kpc distances can be detected and monitored through a sudden change from positive to negative charging of the TMs and an increase of the TM charging noise. This is a unique signature since both galactic and solar particles charge positively the LISA TMs. This peculiar behavior of the TM charging would allow monitoring the whole dynamics of GRBs. The suggestion reported in the literature, about the detection of long GRBs and gravitational waves from the same sources, in principle, may apply to LISA and other LISA-like missions since the increase of the TM charging noise during these extreme transient phenomena is estimated to remain below the mission sensitivity while particle detectors are expected to saturate.

- Abstract
Understanding the health consequences associated with exposure to risk factors is necessary to inform public health policy and practice. To systematically quantify the contributions of risk factor exposures to specific health outcomes, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 aims to provide comprehensive estimates of exposure levels, relative health risks, and attributable burden of disease for 88 risk factors in 204 countries and territories and 811 subnational locations, from 1990 to 2021.

- Abstract
Estimates of demographic metrics are crucial to assess levels and trends of population health outcomes. The profound impact of the COVID-19 pandemic on populations worldwide has underscored the need for timely estimates to understand this unprecedented event within the context of long-term population health trends. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 provides new demographic estimates for 204 countries and territories and 811 additional subnational locations from 1950 to 2021, with a particular emphasis on changes in mortality and life expectancy that occurred during the 2020–21 COVID-19 pandemic period.

- Abstract
Rheology is a highly interdisciplinary approach, transversal to all natural sciences and technologies. Chemistry plays a fundamental role in it: the models at the molecular level are the interpretative key of the observed phenomena. The mathematics used can be very sophisticated, however using only the elements of mathematical analysis and the fundamental concepts of structure and molecular interactions it is possible to develop a coherent and rigorous framework, certainly useful for the teacher and to the student of chemistry and of natural sciences. In this paper the principles of the viscometry of polymeric materials are introduced and the non-Newtonian behaviors of the pseudoplastic, dilatant, thixotropic, rheopectic and plastic types are discussed. Then, some cases of non-Newtonian behavior of commonly used polymers are considered. A very vast subject which often, due to its complexity, is not explored in school programs, but whose knowledge is increasingly required in current technological applications. [ABSTRACT FROM AUTHOR]

10. Treatment of Severe Palmoplantar Pustular Psoriasis With Bimekizumab

- Abstract
IMPORTANCE: Palmoplantar pustulosis (PPP) and palmoplantar plaque psoriasis with pustules remain challenging to treat. Studies suggest that an interleukin 17 or interleukin 36 loop acts synergistically in these diseases to induce palmoplantar pustules. OBJECTIVE: To assess the effectiveness of bimekizumab in treating PPP and palmoplantar plaque psoriasis with pustules. DESIGN, SETTING, AND PARTICIPANTS: This case series involved 21 adults with PPP (11 patients) or palmoplantar plaque psoriasis with pustules (10 patients) treated at 1 of 7 tertiary dermatological centers in France from September 2022 through June 2023. All patients treated with bimekizumab for at least 3 months were included in the analyses. MAIN OUTCOMES AND MEASURES: The main outcome was the posttreatment Investigator Global Assessment (IGA), scored as 0 (complete clearance), 1 (almost clear), 2 (mild), 3 (moderate), or 4 (severe). When relevant, evolution of joint pain and nail involvement was reported. Tolerance and potential adverse events were noted. RESULTS: A total of 21 patients (mean [range] age, 46 [24-68] years; 19 females) were included. Eleven patients had isolated PPP, and 10 had palmoplantar plaque psoriasis with pustules. All of them, except 2 who received bimekizumab as first systemic therapy, had not responded to at least 1 systemic treatment (median [range], 3 [1-7] treatments), and/or had adverse events leading to the discontinuation of the treatment. Complete clearance (IGA score, 0) was achieved by 17 patients in 1 to 4 months. Three patients achieved an IGA score of 1, and 1 achieved an IGA score of 2. Three patients with PPP also presented with acrodermatitis continua of Hallopeau. Nail involvement showed 50% to 70% improvement after 4 to 6 months of bimekizumab treatment for these 3 patients. Two patients had SAPHO (synovitis, acne, pustulosis, hyperostosis, osteitis) syndrome; both had complete clearance of skin lesions associated with joint pain improvement. Four patients (19%) with candidiasis were successfully treated with oral antifungal agents. None of the patients had to stop bimekizumab treatment due to adverse events. CONCLUSIONS AND RELEVANCE: The findings of this case series suggest that bimekizumab could be an appealing approach for treating PPP, palmoplantar plaque psoriasis with pustules, and SAPHO syndrome. Prospective randomized placebo-controlled clinical trials are needed to confirm these encouraging initial results.

11. The Utility of Echocardiogram in the Workup of Ischemic Stroke Patients

ABSTRACT:Background:Cardiac sources of emboli can be identified by transthoracic echocardiogram (TTE). The Canadian Best Practice Guidelines recommend routine use of TTE in the initial workup of ischemic stroke when an embolic source is suspected. However, TTEs are commonly ordered for all patients despite insufficient evidence to justify cost-effectiveness. We aim to evaluate the TTE ordering pattern in the initial workup of ischemic stroke at a regional Stroke Center in Central South Ontario and determine the proportion of studies which led to a change in management and affected length of stay (LOS).Methods:Hospital records of 520 patients with a discharge diagnosis of TIA or ischemic stroke between October 2016 and June 2017 were reviewed to gather informationResults:477 patients admitted for TIA or ischemic stroke met inclusion criteria. 67.9% received TTE, out of which 6.0% had findings of cardiac sources of emboli including left ventricular thrombus, atrial septal aneurysm, PFO, atrial myxoma, and valvular vegetation. 2.5% of all TTE findings led to change in medical management. The median LOS of patients who underwent TTE was 2 days longer (p< 0.00001).Conclusion:TTE in the initial workup of TIA or ischemic stroke remains common practice. The yield of TTEs is low, and the proportion of studies that lead to changes in medical management is minimal. TTE completion was associated with increased LOS and may result in increased healthcare spending; however, additional factors prolonging the LOS could not be excluded.

- Abstract
In naive individuals, sensory neurons directly detect and respond to allergens, leading to both the sensation of itch and the activation of local innate immune cells, which initiate the allergic immune response1,2. In the setting of chronic allergic inflammation, immune factors prime sensory neurons, causing pathologic itch3–7. Although these bidirectional neuroimmune circuits drive responses to allergens, whether immune cells regulate the set-point for neuronal activation by allergens in the naive state is unknown. Here we describe a γδ T cell–IL-3 signalling axis that controls the allergen responsiveness of cutaneous sensory neurons. We define a poorly characterized epidermal γδ T cell subset8, termed GD3 cells, that produces its hallmark cytokine IL-3 to promote allergic itch and the initiation of the allergic immune response. Mechanistically, IL-3 acts on Il3ra-expressing sensory neurons in a JAK2-dependent manner to lower their threshold for allergen activation without independently eliciting itch. This γδ T cell–IL-3 signalling axis further acts by means of STAT5 to promote neuropeptide production and the initiation of allergic immunity. These results reveal an endogenous immune rheostat that sits upstream of and governs sensory neuronal responses to allergens on first exposure. This pathway may explain individual differences in allergic susceptibility and opens new therapeutic avenues for treating allergic diseases.

Rhabdoid tumor predisposition syndrome type-1 (RTPS1) is characterized by germline pathogenic variants in SMARCB1and development of INI1-deficient rhabdoid tumors in early childhood. Due to its poor prognosis, the risk of subsequent tumor development and the impact of surveillance at later ages are poorly understood. We retrospectively reviewed individuals referred to the Cancer Genetics Program at The Hospital for Sick Children for SMARCB1genetic testing and/or surveillance for RTPS1. In addition, to explore characteristics of late-onset tumors in RTPS1, a literature review was conducted. Of eighty-three individuals (55 probands and 28 family members), 12 probands and 4 family members were genetically confirmed with RTPS1. Four pediatric probands with RTPS1 underwent surveillance. An additional three individuals, including one patient with 22q11.2 distal deletion without history of tumor, one patient with negative genetic testing results but clinically diagnosed with RTPS1, and one sibling identified through cascade testing, underwent surveillance. Three patients with RTPS1 developed tumors between the ages of 9 and 17, including malignant rhabdoid tumors (N= 3), schwannomas (N= 4), and epithelioid malignant peripheral nerve sheath tumor (N= 1). Three of these lesions were asymptomatically detected by surveillance. A literature review revealed 17 individuals with RTPS1 who developed INI1-deficient tumors after age five. Individuals with RTPS1 remain at elevated risk for developing INI1-deficient tumors after the peak age of rhabdoid tumor in early childhood. Extension of surveillance beyond 5 years of age could lead to improved survival and reduced morbidity for these patients, and prospective evaluation of revised approaches will be important.

This article presents machinery fault diagnosis in electric motors through mechanical vibration monitoring. Using a commercial machinery fault simulator, two fiber Bragg grating (FBG) accelerometers are evaluated under one healthy condition and nine different fault conditions, including rotors with broken bars, faulted bearings, misalignment, and imbalance. The mechanical vibration spectrum for each fault condition is assessed by analyzing the Bragg wavelength shift modulated by the acceleration. Offline analysis is carried out to identify the mechanical vibration pattern characteristics. In this preliminary analysis, the high-order frequency characteristics of faulted-bearing conditions and the slip frequency characteristics of broken rotor bar conditions were well-identified by both accelerometers. Following this analysis, this article uses two concurrent approaches to indicate the feasibility of classifying and/or clustering the data from supervised or unsupervised approaches. Hydrogenated amorphous silicon (a-Si:H) is a material having an intrinsically high radiation hardness that can be deposited on flexible substrates such as polyimide (PI). For these properties, a-Si:H can be used for the production of flexible sensors. a-Si:H sensors can be successfully utilized in dosimetry, beam monitoring for particle physics (X-ray, electron, gamma ray, and proton detection) and radiotherapy, radiation flux measurement for space applications (study of solar energetic particles and stellar events), and neutron flux measurements. In this article, we have studied the dosimetric X-ray response of n-i-p diodes deposited on PI. We measured the linearity of the photocurrent response to X-rays versus dose rate from which we have extracted the dosimetric X-ray sensitivity at various bias voltages. In particular, low bias voltage operation has been studied to assess the high energy efficiency of these kinds of sensors. A measurement of stability of X-ray response versus time has been shown. The effect of detectors annealing has been studied. Operation under bending at various bending radii is also shown.

Robust manipulation of mechanical parts in different grasping configurations is a challenging problem in autonomous robotic assembly that can be overcome by adopting suitable mechatronic solutions. This article proposes a tactile-sensor-based approach that exploits in-hand pose estimation and contact perception to compensate for unavoidable picking, placing, and insertion errors that may occur during task assembly execution under uncertain/perturbed conditions. The main objective of this work is to demonstrate how the use of tactile data, together with both machine learning and model-based methods, allows us to obtain an advanced system able to successfully complete a task that requires the manipulation of boltlike fasteners with different shapes and grasped in different poses. Experiments carried out using the proposed robotic system are reported for a specific assembly task in order to evaluate the effectiveness of the proposed solution. By means of suitable calibration procedures exploiting the same methods proposed here, the system can be easily adapted to different objects and shapes.

17. Neonatal osteomyelitis: an Italian multicentre report of 22 cases and comparison with the inherent literature

Objective: The primary objective of this study is to report and compare our data with the most relevant literature of the past decade about neonatal osteomyelitis. Study design: We retrospectively review the data of 22 subjects aged 35 days or less who were admitted to three different sites in Italy with a radiological diagnosis of osteomyelitis. The inherent literature was searched and reviewed: five studies were considered for comparison with our data. Results: All the neonates, except three (two pre-term and one post-term), were born at term. The male to female ratio was 1.75 (14 males and 8 females). The mean age at presentation was 19.5 days. The most common presenting signs of the infection were local swelling and reduced mobility of the affected segment. The most common sites of infection were the femur, humerus, and tibia. The mean duration of intravenous antibiotic therapy was 29.5 days. In most neonates the diagnosis was prompt and the antibiotic treatment immediate. A low rate of sequelae was reported. All infants survived through follow up. The data from the inherent literature showed a wide variability, probably owing to the setting and the historical period of the different studies. Conclusion: Neonatal osteomyelitis is an alarming yet poorly understood disease. Nonetheless, our report suggests that a quick diagnosis and treatment can be easily achieved, with good outcome on the remarkably plastic structure of neonatal bones.

A low-gain avalanche diode (LGAD) achieves intrinsic gain and is thus of interest for low-linear-energy-transfer (LET) microdosimetry. A novel LGAD device with two geometric configurations was characterized using an ion microprobe with proton energies of 1, 1.8, 3, and 8 MeV as well as carbon ions with an energy of 2 MeV/u using the SIRIUS accelerator at the Australian Nuclear Science and Technology Organisation (ANSTO). A 2-D assessment of charge collection in the device showed distinct regions in the detector with gain and without gain. The gain of the device was characterized for all ion configurations and at different reverse biases. The gain did not monotonically increase with a larger reverse bias and was also observed to reduce with lower proton energies and carbon ions. This is due to an increase of the induced charge density in the device for higher LET ions resulting in charges being screened from the strong localized electric field in the gain layer. Thus, due to the nonlinear behavior of these devices with the deposited energy, a thorough investigation is required for the assessment of these devices for microdosimetry in proton therapy, heavy ion therapy, and space radiation.

ObjectivePrevious studies indicate that eosinophils are recruited into the allograft following orthotopic liver transplantation and protect from ischaemia reperfusion (IR) injury. In the current studies, we aim to explore whether their protective function could outlast during liver repair.DesignEosinophil-deficient mice and adoptive transfer of bone marrow-derived eosinophils (bmEos) were employed to investigate the effects of eosinophils on tissue repair and regeneration after hepatic IR injury. Aside from exogenous cytokine or neutralising antibody treatments, mechanistic studies made use of a panel of mouse models of eosinophil-specific IL-4/IL-13-deletion, cell-specific IL-4rα-deletion in liver macrophages and hepatocytes and macrophage-specific deletion of heparin-binding epidermal growth factor-like growth factor (hb-egf).ResultWe observed that eosinophils persisted over a week following hepatic IR injury. Their peak accumulation coincided with that of hepatocyte proliferation. Functional studies showed that eosinophil deficiency was associated with a dramatic delay in liver repair, which was normalised by the adoptive transfer of bmEos. Mechanistic studies demonstrated that eosinophil-derived IL-4, but not IL-13, was critically involved in the reparative function of these cells. The data further revealed a selective role of macrophage-dependent IL-4 signalling in liver regeneration. Eosinophil-derived IL-4 stimulated macrophages to produce HB-EGF. Moreover, macrophage-specific hb-egf deletion impaired hepatocyte regeneration after IR injury.ConclusionTogether, these studies uncovered an indispensable role of eosinophils in liver repair after acute injury and identified a novel crosstalk between eosinophils and macrophages through the IL-4/HB-EGF axis.

Immune checkpoint inhibitor (ICI) therapy has revolutionized oncology, but treatments are limited by immune-related adverse events, including checkpoint inhibitor colitis (irColitis). Little is understood about the pathogenic mechanisms driving irColitis, which does not readily occur in model organisms, such as mice. To define molecular drivers of irColitis, we used single-cell multi-omics to profile approximately 300,000 cells from the colon mucosa and blood of 13 patients with cancer who developed irColitis (nine on anti-PD-1 or anti-CTLA-4 monotherapy and four on dual ICI therapy; most patients had skin or lung cancer), eight controls on ICI therapy and eight healthy controls. Patients with irColitis showed expanded mucosal Tregs, ITGAEHiCD8 tissue-resident memory T cells expressing CXCL13and Th17 gene programs and recirculating ITGB2HiCD8 T cells. Cytotoxic GNLYHiCD4 T cells, recirculating ITGB2HiCD8 T cells and endothelial cells expressing hypoxia gene programs were further expanded in colitis associated with anti-PD-1/CTLA-4 therapy compared to anti-PD-1 therapy. Luminal epithelial cells in patients with irColitis expressed PCSK9, PD-L1 and interferon-induced signatures associated with apoptosis, increased cell turnover and malabsorption. Together, these data suggest roles for circulating T cells and epithelial–immune crosstalk critical to PD-1/CTLA-4-dependent tolerance and barrier function and identify potential therapeutic targets for irColitis.

21. Outcomes of Stapled Transanal Rectal Resection vs . Biofeedback for the Treatment of Outlet Obstruction Associated with Rectal Intussusception and Rectocele: A Multicenter, Randomized, Controlled Trial

Abstract: Purpose: This study was designed to assess the safety and outcomes achieved with stapled transanal rectal resection vs. biofeedback training in obstructed defecation patients. Methods: A total of 119 women patients who suffered from obstructed defecation with associated rectocele and rectal intussusception were randomized to stapled transanal rectal resection or biofeedback training. Stapled transanal rectal resection was performed by using two circular staplers to produce transanal full-thickness rectal resection. Primary outcome was symptoms of obstructed defecation resolution at 12 months; secondary outcomes included safety, change in quality of life score, and anatomic correction of rectocele and rectal intussusception. Results: Fourteen percent (8/59) stapled transanal rectal resection and 50 percent (30/60) biofeedback training patients withdrew early. Eight (15 percent) patients treated with stapled transanal rectal resection and 1 (2 percent) biofeedback patient experienced adverse events. One serious adverse event (bleeding) occurred after stapled transanal rectal resection. Scores of obstructed defecation improved significantly in both groups as did quality of life (both P?0.0001). Successful treatment was observed in 44 (81.5 percent) stapled transanal rectal resection vs. 13 (33.3 percent) evaluable biofeedback training patients (P?0.0001). Functional benefit was observed early and remained stable during the study. Conclusions: In this controlled trial, stapled transanal rectal resection was well tolerated, was more effective than biofeedback training for the resolution of obstructed defecation symptoms, and improved quality of life, with minimal risk of impaired continence. Thus, stapled transanal rectal resection offers a new treatment alternative for obstructed defecation after failure of conservative measures including biofeedback training, a noninvasive approach.

22. Outcomes of adult patients discharged at scene by emergency medical services

- Abstract
BackgroundThe outcomes of patients who call an ambulance but are discharged at scene reflect the safety and quality of emergency medical service (EMS) care. While previous studies have examined the outcomes of patients discharged at scene, none have specifically focused on paramedic-initiated discharge. This study aims to describe the outcomes of adult patients discharged at scene by paramedics and identify factors associated with 72-hour outcomes.MethodsThis was a retrospective data linkage study on consecutive adult EMS patients discharged at scene by paramedics in Victoria, Australia, between 1 January 2015 and 30 June 2019. Multivariable logistic regression was used to investigate factors associated with EMS recontact, ED presentation, hospital admission and serious adverse events (death, cardiac arrest, category 1 triage or intensive care unit admission) within 72 hours of the initial emergency call.ResultsThere were 375 758 cases of adults discharged at scene following EMS attendance, of which 222 571 (59.2%) were paramedic-initiated decisions. Of these, 6.8% recontacted EMS, 5.0% presented to ED, 2.4% were admitted to hospital and 0.3% had a serious adverse event in the following 72 hours. The odds of EMS recontact were increased in cases related to mental health (adjusted OR (AOR) 1.41 (95% CI 1.33 to 1.49)), among low-income government concession holders (AOR 1.61 (95% CI 1.55 to 1.67)) and in areas of low socioeconomic advantage (AOR 1.19 (95% CI 1.13 to 1.25)). The odds of hospital admission were increased in cases related to infection (AOR 3.14 (95% CI 2.80 to 3.52)) and pain (AOR 1.93 (95% CI 1.75 to 2.14)). The strongest driver of serious adverse events was an abnormal vital sign (AOR 4.81 (95% CI 3.87 to 5.98)).ConclusionThe occurrence of hospital admission and adverse events is rare in those discharged at scene, suggesting generally safe decision-making. However, increased attention to elderly, multimorbid patients or patients with infection and pain is recommended, as is further research examining the use of tools to aid paramedic recognition of potential for deterioration.

This article presents a unified procedure for commissioning synchronous reluctance motor drives based on a fast sequence of targeted feedings using the drive inverter. The method requires measuring the values of the phase currents and their derivatives by time-based samplings with still rotor and subsequent model-based computations. It allows the self-evaluation of the phase resistance and the d-q axes inductances for the pre-set of the field-oriented current regulators. Moreover, it detects the rotor position without movement, a functionality needed at start-up for motors equipped with incremental encoders in the standard service. The study refers to a 3 kW synchronous reluctance motor prototype with a flux barrier rotor. Both realistic simulations accounting for the non-linear machine behavior and experimental tests are presented, showing the error between the estimated position, the phase resistance, and the d-q axes inductances and the effective ones. The results prove the reliability of the method.

- Abstract
During development, inflammation or tissue injury, macrophages may successively engulf and process multiple apoptotic corpses via efferocytosis to achieve tissue homeostasis1. How macrophages may rapidly adapt their transcription to achieve continuous corpse uptake is incompletely understood. Transcriptional pause/release is an evolutionarily conserved mechanism, in which RNA polymerase (Pol) II initiates transcription for 20–60 nucleotides, is paused for minutes to hours and is then released to make full-length mRNA2. Here we show that macrophages, within minutes of corpse encounter, use transcriptional pause/release to unleash a rapid transcriptional response. For human and mouse macrophages, the Pol II pause/release was required for continuous efferocytosis in vitro and in vivo. Interestingly, blocking Pol II pause/release did not impede Fc receptor-mediated phagocytosis, yeast uptake or bacterial phagocytosis. Integration of data from three genomic approaches—precision nuclear run-on sequencing, RNA sequencing, and assay for transposase-accessible chromatin using sequencing (ATAC-seq)—on efferocytic macrophages at different time points revealed that Pol II pause/release controls expression of select transcription factors and downstream target genes. Mechanistic studies on transcription factor EGR3, prominently regulated by pause/release, uncovered EGR3-related reprogramming of other macrophage genes involved in cytoskeleton and corpse processing. Using lysosomal probes and a new genetic fluorescent reporter, we identify a role for pause/release in phagosome acidification during efferocytosis. Furthermore, microglia from egr3-deficient zebrafish embryos displayed reduced phagocytosis of apoptotic neurons and fewer maturing phagosomes, supporting defective corpse processing. Collectively, these data indicate that macrophages use Pol II pause/release as a mechanism to rapidly alter their transcriptional programs for efficient processing of the ingested apoptotic corpses and for successive efferocytosis.

25. The Outsourcing Wage Gap: Exploring the Interplay of Gender and Tasks Along the Job Distribution

This paper examines the wage penalty of outsourced workers in France, providing novel insights to the existing literature. First, it investigates the extent to which the wage penalty differs between outsourced male and female workers. Our results reveal that outsourced workers experience pronounced wage penalty, with this effect being stronger for women. Second, in contrast to most studies, we analyse outsourcing across the entire job distribution. We find that the wage penalty is significantly higher for outsourced workers employed in jobs at the lower end of the wage distribution compared to those employed at the top. Third, we assess whether the wage penalty experienced by outsourced workers can be attributed to the tasks they perform, such as repetitiveness, forms of control, and managerial duties. The results show that the tasks performed, while can alter the wage penalty, do not significantly reshape the main results. Thus, differences in tasks do not appear to be a determining factor in the wage penalty of outsourced workers. Longitudinal analysis strengthens the validity of the cross-sectional findings, highlighting that the wage penalty associated with outsourcing does not result from shifts in individual employee characteristics but rather from disparities in their job status and other time-invariant attributes. In summary, being an outsourced worker implies a degradation in wage treatment, which is exacerbated by gender. Given the increasing importance of this employment practice, it underscores the urgent need for policy interventions to address these critical issues.

ABSTRACTIntroductionDespite surgical approach is still the mainstay for basal cell carcinoma (BCC) management, several issues may limit the use of this technique, leading to the need for new treatments to offer patients a personalized approach.Areas coveredA comprehensive review of the available and emerging pharmacologic strategies for BCC management, including mechanisms of action, and potential adverse effects, has been performed to provide with an up-to-date manuscript on the current treatment scenario of BCC. Globally, targeting the Sonic-Hedgehog pathway is one of the main mechanisms of action of currently investigated drugs. Other alternatives are based on the concept of an enhancement of the immune response such as immune checkpoint inhibitors, or intra-tumor treatments.Expert opinionAlthough low-risk BCCs are often treated with destructive methods or topical treatments, surgery is the mainstay of treatment for the majority of BCCs. However, several factors may limit the use of surgery in BCC management. Recently, major knowledge on BCCs pathogenesis has led to the development of effective and selective drugs. In our opinion, soon many drugs will be licensed, allowing clinicians to offer patients with BCC the right treatment at the right moment. Certainly, further studies are needed.

31. Serum HMGB1 levels are independently associated with glucose clamp-derived measures of insulin resistance in women with PCOS

Purpose: PCOS is associated with low grade inflammation which could play a role in insulin resistance and ovarian dysfunction. Preliminary findings suggested that serum levels of HMGB1, a cytokine involved in inflammation, might be altered in women with PCOS. Primary aim of this study was to assess whether HMGB1 serum concentrations are associated with PCOS and with the state of insulin resistance of these women. Methods: Sixty women with PCOS, selected to have a similar proportion of subjects with altered or normal insulin sensitivity, and 29 healthy controls were studied. Serum HMGB1 levels were compared in subgroups of PCOS women and controls. In PCOS women, insulin sensitivity was assessed by the glucose clamp technique and HMGB1 was measured at baseline and after acute hyperinsulinemia. Results: HMGB1 levels were similar in women with PCOS and controls and no elements used for diagnosing PCOS were associated with serum HMGB1. However, HMGB1 concentrations were higher in insulin-resistant vs insulin-sensitive PCOS women (p= 0.017), and inversely associated with insulin-induced total and non-oxidative glucose metabolism. In both subgroups of PCOS women, serum HMBG1 levels significantly increased after acute hyperinsulinemia. Conclusions: These data suggest that HMGB1 levels are not associated with PCOS per se, but with insulin resistance. Further research should establish the underlying nature of this relationship, and whether this protein might play a role in the metabolic complications of PCOS.

Dibenzocyclooctynes have emerged as promising scaffolds for bioorthogonal ligation. An important structural aspect that has not been addressed so far relates to their chirality. Herein, we explore, by theoretical and experimental methods, this structural aspect that has been neglected so far. First, computational analysis is conducted, and the results are used as a guide for the experimental investigation. Next, an array of different experiments (high-performance liquid chromatography (HPLC) on chiral columns, chiroptical spectroscopy, and X-ray diffraction) for structure elucidation is scrutinized in concert. Finally, this work demonstrates the chirality and the stereodynamic behavior of dibenzocyclooctynes and their triazole derivatives with simple azides and also uncovers their conformational behavior.

A significant electrocardiography false-positive rate makes pre-participation screening (PPS) for sudden cardiac death (SCD) in athletes controversial. Focused cardiac ultrasound (FoCUS) may increase sensitivity and specificity however, its cost-effectiveness is unknown. This study evaluates the diagnostic performance and cost-effectiveness of FoCUS in PPS. Incorporating a simplified echocardiographic exam called FoCUS into PPS resulted in higher diagnostic reliability, with a lower rate of false negatives and a higher number of athletes at risk for SCD identified.The integration of FoCUS into the screening process resulted to be cost-effective in our athletes’ cohort.Graphical abstract

IMPORTANCE: The pathogenesis of eosinophilic cellulitis (EC) is poorly understood, limiting available treatment options. The current treatment paradigm focuses on delayed type 2 hypersensitivity reaction to various triggers. OBJECTIVE: To gain further insight into the nature of EC inflammation and into the cellular signal transduction pathways that are activated in the context of EC. DESIGN, SETTING, AND PARTICIPANTS: This case series was conducted in Lyon, France, from January 2018 to December 2021. Analysis of archival skin biopsy samples from patients with EC and from healthy control participants was performed using histology, Janus kinase (JAK)–signal transducer and activator of transcription (STAT) immunohistochemistry, and gene profiling. Data analysis was conducted between January 2020 and January 2022. MAIN OUTCOMES AND MEASURES: Pruritus (visual analog score), percentage of body surface area with lesional skin, and RNA transcripts of inflammatory biomarkers from the skin (threshold cycle) were assessed in 1 index patient with refractory EC who received oral JAK1/JAK2 inhibitor baricitinib (4 mg/d). RESULTS: This study included samples from 14 patients with EC (7 men and 7 women) and 8 healthy control participants (4 men and 4 women). The mean (SD) age of patients was 52 (20) years. Marked type 2 inflammation (chemokines CCL17, CCL18, and CCL26 and interleukin 13) with preferential activation of the JAK1/JAK2–STAT5 pathways in EC lesions was observed. In the 1 index patient with refractory EC, complete clinical remission of skin lesions was observed after 1 month of treatment with baricitinib. CONCLUSIONS AND RELEVANCE: These findings suggest that EC is a type 2 inflammatory disease with preferential activation of the JAK1/JAK2-STAT5 pathways. In addition, these results suggest the potential of treatment approaches targeting JAK1/JAK2 for patients with EC.

36. A relocated earthquake catalog and ground motion database for the southern East African rift system

- Abstract
The southern East African rift system (EARS) is geologically rare considering its early-stage continental rift setting combined with a deep seismogenic zone. Several seismically vulnerable communities are located within this tectonically active region, resulting in a significant seismic risk. However, the ground motion and seismic hazard analyzes necessary to increase the earthquake preparedness in the region have been limited due to the relatively short instrumentation history and scarce ground motion data available. Here, we present a newly compiled ground motion database for the southern EARS which is critically lacking and preventing local ground motion studies. This database includes a regional catalog of 882 earthquakes spanning 1994–2022 (magnitudes 3–6.5) with available waveform records within epicentral distances of 300 km. Three different velocity models were used to relocate 256, 255, and 252 events, respectively, to quantify depth sensitivity, relocating events down to depths of 35–40 km. The final database contains 10,725 time-series records from 353 stations along with P- and S-wave phase arrivals for each record. The ground motion database contains peak ground acceleration and velocity and 5% damped pseudo-spectral acceleration for 291 frequencies from 1.0 to 30 Hz for the horizontal components. In addition, a Fourier amplitude spectrum table for 212 frequencies from 0.1 to 30 Hz is included. The database is accessible through the ISC repository (https://doi.org/10.31905/4GGVBFBE).

37. System Dynamics for E-Health: An Experimental Analysis of Digital Transformation Scenarios in Health Care

In the last decade, the process of digital transformation has affected all economic activities and has offered new opportunities in health care requiring the adoption of new approaches and ICT tools to redesign processes and policies for the sector, allowing the diffusion of e-health, that is, health practices supported by technological tools. Digital technologies, such as electronic health records, telemedicine, and artificial intelligence, are the technological pillars on which a new health model can be structured over the next few years. The objective of this article is to analyze the impact of digital technologies in e-health through a system dynamic (SD) approach applied to intensive care units to determine how these technologies interact and which outputs they return within complex systems. The findings demonstrate how SD can be used to simulate the effects of e-health technologies’ adoption as a policy-making tool, overcoming the main barriers to the development of sustainable health care. The implications and limitations of the study are also provided.

ABSTRACTIntroductionSurgery is the standard management for most of basal cell carcinomas (BBCs). In some cases, also radiotherapy may be a valuable weapon as well as ablative and topical treatments. However, all these approaches may be limited by some tumor features. In this scenario, locally advanced BCCs (laBCC) and metastatic BCC, also defined as ‘difficult-to-treat’ BCC, remain the real treatment challenge. New knowledge on BCC pathogenesis, particularly the Hedgehog (HH) pathway, led to the development of new selective therapies such as vismodegib and sonidegib. In particular, sonidegib is an orally administered small molecules, which inhibits the HH signaling pathway through the binding to SMO receptor, recently approved for the management of adult patients with laBCC who are not amenable to curative surgery or radiation therapy.Areas coveredThe purpose of this review is to analyze and discuss the efficacy and safety of sonidegib for the management of BCC, to provide a broad perspective on the currently available data.Expert opinionSonidegib is a valuable weapon for the management of difficult-to-treat BCC. Current data showed promising results in terms of effectiveness and safety. However, more studies are needed to underline its role in BCC management, also considering the presence of vismodegib, and to investigate its use in a long-term period.

- Abstract
Here the Human Pangenome Reference Consortium presents a first draft of the human pangenome reference. The pangenome contains 47 phased, diploid assemblies from a cohort of genetically diverse individuals1. These assemblies cover more than 99% of the expected sequence in each genome and are more than 99% accurate at the structural and base pair levels. Based on alignments of the assemblies, we generate a draft pangenome that captures known variants and haplotypes and reveals new alleles at structurally complex loci. We also add 119 million base pairs of euchromatic polymorphic sequences and 1,115 gene duplications relative to the existing reference GRCh38. Roughly 90 million of the additional base pairs are derived from structural variation. Using our draft pangenome to analyse short-read data reduced small variant discovery errors by 34% and increased the number of structural variants detected per haplotype by 104% compared with GRCh38-based workflows, which enabled the typing of the vast majority of structural variant alleles per sample.

ABSTRACTIntroductionBiological treatments deeply changed the management of moderate-to-severe forms of psoriasis. Among the available biological therapies, interleukin (IL)-17 inhibitors, secukinumab, ixekizumab, brodalumab, and bimekizumab represent one of the most rapid and effective biologic classes available for psoriasis. Bimekizumab, the latest available IL-17 inhibitor, is a humanized monoclonal immunoglobulin (Ig)G1 antibody that acts by neutralizing both IL-17A and IL-17F, showing a unique mechanism of action differing from ixekizumab and secukinumab (selective IL17A inhibitor), as well as brodalumab (antagonist of IL17 receptor).Areas coveredThis review aims to evaluate the safety profile of bimekizumab in the treatment of moderate-to-severe plaque psoriasis.Expert opinionThe efficacy and safety of bimekizumab have been reported by several phase II and III clinical trials, even in a longer-term period. Moreover, clinical trials also showed bimekizumab to have significantly higher efficacy compared to other biological classes, including anti-TNF, anti-IL-12/23, and even to another IL-17 inhibitor, secukinumab. Although numerous biologics are currently available for psoriasis, some patients may result resistant to other treatments and/or experience psoriatic flares during or after treatment withdrawal. In this scenario, bimekizumab may represent an additional valuable alternative for patients with moderate-to-severe forms of psoriasis.

50. Clitoral reconstruction: challenges and new directions

- Abstract
Clitoral reconstruction (CR) has been the subject of several studies in recent years, mainly in the medical field. Women with female genital mutilation or cutting (FGM/C) seek clitoral reconstructive surgery to improve their sexual well-being, but also because they are affected by poor self- and body image. CR is supposed to help women with FGM/C reconstruct their sense of self, but the benefits and risks of this surgery have not been sufficiently explored. There are currently no recommendations supporting CR from mainstream medical bodies, and there have been very few ethical studies of the procedure. This article critically discusses the principal studies produced in the medical field and available reflections produced in the social sciences. Through the theoretical frameworks of postcolonial and feminist studies, the article discusses sexuality and pleasure, gender and identity, and race and positionality, with the aim of promoting collaborative work on CR between researchers and social and health professionals.
