Patients with aortic root pathology may benefit from ‘valve-conservation’ surgery although application of this philosophy is limited by a lack of ‘standardized’ surgical techniques. A functional classification of aortic root and valvular abnormalities has been developed in 260 patients and correlated with the etiology of the pathologic process and the surgical procedure performed.

Objectives:to evaluate the feasibility and preliminary results at 1 year of subintimal angioplasty of tibial occlusions in critical limb ischaemia (CLI). Material:from December 1997 to December 1999, we intended to treat 36 patients and 40 limbs by subintimal angioplasty of occlusions of tibial vessels. Thirty-one had gangrene or ulceration and nine had rest pain. Twenty-seven occlusions were more than 10 cm, 10 were 5 to 10 cm and three were less than 5 cm in length. Three patients had an occluded previous ipsilateral bypass graft. All patients were followed 3 monthly for a median of 10 months by means of clinical and duplex examination. Results:the technical success rate was 78% (31/40). Nine technical failures were treated by conventional surgery or angioplasty of another diseased tibial vessel. The clinical success rate was 68% (27/40). Four below-the-knee amputations were performed despite a patent recanalisation. Primary and secondary patency rates at 12 months were 56% (72% without technical failures). The 12-month limb salvage rate was 81% and survival rate was 78%. Three of five complications were treated by endovascular procedures. The length of occlusion (>10 cm) but not the location of distal re-entry, the type of vessel re-entry and the presence of diabetes are predictors of technical success and patency. Conclusions:subintimal angioplasty can be used to treat tibial occlusions in patients with CLI. Technical failure does not preclude conventional surgery and complications may often be treated by endovascular procedures. However, the durability of angioplasty is as yet uncertain.

Sequential internal thoracic artery (ITA) grafting allows a more complete arterial revascularization of the myocardium. We wanted to verify whether the excellent clinical and angiographic short term results reported by us before where maintained over 10 years and more.

Objectifs. —Le neuromonitorage peropératoire par potentiels évoqués somesthésiques (PES) des nerfs médians dans la chirurgie vasculaire pratiquée sous arrêt circulatoire en hypothermie profonde présente un double intérêt: déterminer le degré optimal d’hypothermie auquel peut être réalisé l’arrêt circulatoire et détecter la survenue précoce d’épisodes de souffrance cérébrale. L’objectif de cette étude rétrospective portant sur 58 patients est de déterminer la sensibilité, la spécificité et l’impact de cette technique sur la stratégie opératoire.

The usefulness of somatosensory evoked potential (SEP) monitoring as a means of preventing paraplegia in descending aorta surgery was evaluated in 47 consecutive cases operated on for isthmic (14 cases), thoracic (22 cases), or thoraco-abdominal (11 cases) repair. An aortic dissection was found in 11 cases (acute in 6). Somatosensory evoked potentials were obtained by unilateral left and right posterior tibial nerve (PTN) stimulation at the ankle and recordings were performed on four channels: peripheral nerve, lumbar spinal, brain-stem, and cortical recordings. Our experience led to the following current strategy: the establishment of atrio(aorto)-femoral(aortic) bypass (29 cases), proximal and distal aortic cross-clamping, aortic repair with reimplantation of the culprit artery(ies) as indicated by SEP alterations. Five types of SEP alterations were defined on the basis of the neural level involved: type I (27.7% of cases) = distal spinal ischemia due to proximal aortic cross-clamping in the absence of bypass; type II (21.3%) = PTN ischemia due to left common femoral artery cross-clamping; type III (12.8%) = segmental spinal ischemia due to the exclusion of critical feeding arteries; type IV (4.3%) = ischemia in the left carotid artery territory; type V (4.3%) = global brain hypoperfusion due to systemic hypotension. Forty-five patients survived the operation and could be tested for neurological dysfunction. Three patients presented a postoperative spinal cord deficit, but this deficit was already present preoperatively in one case, so that the actual incidence of a new paraplegia in our series was 2/45 cases (4.4%). One of the two cases was clearly a delayed paraplegia with SEP alterations appearing several hours after the operation. Somatosensory evoked potentials were evaluated on the basis of their sensitivity, specificity, and impact on the surgical strategy. Regarding SEP sensitivity, we did not encounter any unexpected immediate paraplegia, but the critical factor appeared to be the duration of SEP absence due to spinal cord ischemia, which, according to the literature, should never exceed 30 min; after a longer absence, SEP return does not guarantee neurological recovery. Somatosensory evoked potential specificity was also 100%, but only 58% of the abnormalities found were actually consequent to spinal cord ischemia, the rest of the abnormalities being consequent to peripheral nerve or brain ischemia. Finally, SEP monitoring had a significant impact on surgical strategy in 19% of the cases. It is concluded that distal aortic perfusion and multilevel SEP monitoring play a significant role in preventing paraplegia in descending aorta surgery.

The conventional surgical treatment of isolated critical stenosis of the left main coronary artery (LMCA) leads to the definitive occlusion of LMCA, restores only a retrograde perfusion to a rather extensive myocardial area and consumes bypass material. Direct surgical angioplasty avoids these inconveniences.

AbstractSubacute ischaemic proctitis is a rare condition. We describe the case of a 60-year-old male patient who developed, after aortic aneurysm repair, a transient ischaemic colitis that totally healed without sequelae. He eventually developed symptoms of severe proctitis. Investigations identified a stenosis of the mid rectum, while the upper rectum was inflammatory. On angiogram, there was a poor blood flow through the Riolan’s arcade and a stenosis of the proximal aorto-graft anastomosis. Diagnosis of subacute ischaemic proctitis due to poor blood supply through the internal iliac arteries was made. Anti-inflammatory drugs and dilations were inefficient. A subtotal proctectomy with low colorectal anastomosis was required. On pathological specimen, the lesions were strongly suggestive of an ischaemic process. The patient had an excellent recovery and was asymptomatic 8 months after the operation.
