CMV-encoded US28 is detected in VSMCs, arterial neointima and TECs of renal allografts from CMV-positive donors. US28 facilitates CMV spreading in VSMCs in vitro. US28 may provide a potential target for therapeutic intervention. Supplemental digital content is available in the text.

Plasma and urine measurements of cotinine are probably better than self-reported smoking histories and are associated with dose dependent mortality rates. Two central pathways of innate immunity, complement and Toll‐like receptors (TLRs), play an important role in the pathogenesis of renal injury inherent to kidney transplantation. Recent findings indicate close crosstalk between complement and TLR signaling pathways. It is suggested that mitogen activated protein kinases (MAPKs) might be the key molecules linking both the complement and TLR pathways together. Complement and TLRs are important mediators of renal ischemia‐reperfusion injury (IRI). Besides IRI, complement C3 can also be upregulated and activated in the kidney before transplantation as a direct result of brain death (BD) in the donor. This local upregulation and activation of complement in the donor kidney has been proven to be detrimental for renal allograft outcome. Also TLR4 and several of its major ligands are upregulated by donor BD compared to living donors. Important and in line with the observations above, kidney transplant recipients have a benefit when receiving a kidney from a TLR4 Asp299Gly/Thr399Ile genotypic donor. The role of complement and TLRs and crosstalk between these two innate immune systems in relation to renal injury during donor BD and ischemia‐reperfusion are focus of this review. Future strategies to target complement and TLR activation in kidney transplantation are considered.

Hypoalbuminemia is an established predictor of poor outcome in renal transplant recipients (RTR). It is considered to reflect inflammation, poor nutritional status, or proteinuria. We explored the roles of high-sensitivity C-reactive protein (hsCRP) and urinary protein excretion in prediction of graft failure and mortality by serum albumin in RTR.

Context Transplantation improves health-related quality of life in patients with end-stage renal disease. However, primarily because of adverse effects of medication, among other gastrointestinal symptoms, health-related quality of life is not completely restored to normal. Although many patients have various gastrointestinal symptoms only a small proportion may be reported spontaneously.Objective To evaluate the prevalence of gastrointestinal symptoms in kidney transplant recipients, also the difference between spontaneously reported symptoms and symptoms elicited by specific questioning was assessed. The burden of these symptoms in daily life also was analyzed.Design A single-center, sequential, mixed method study to assess the difference between spontaneous patient reports of gastrointestinal symptoms and active screening by a questionnaire in kidney transplant patients.Patients In February 2008, patients received a questionnaire on gastrointestinal symptoms; notes in medical records were consulted for patients scoring less than 100. In June 2008, those patients received a second, extended questionnaire aimed to assess the burden of gastrointestinal symptoms in daily life.Results Ninety-two of 513 patients eventually proved to have gastrointestinal symptoms. Completed questionnaires were compared with notes in the patients' files of the past year. A total of 51 of these 92 patients appeared to have not mentioned their gastrointestinal symptoms during the outpatient clinic visits. Of these 51 patients, 37 reported a significant impact of gastrointestinal symptoms on daily life.Conclusions The silent sufferer exists. Specific questioning helps to improve communication concerning bothersome gastrointestinal symptoms. To assess the burden of these symptoms, a validated questionnaire should be developed.

Accumulation of advanced glycation end-products (AGEs) has been implicated in the pathogenesis of chronic transplant dysfunction and cardiovascular disease in renal transplant recipients. We aimed to investigate which factors are associated with tissue AGE accumulation in renal transplant recipients.

C-reactive protein (CRP) is a predictor of coronary heart disease, total mortality and chronic allograft nephropathy in renal transplant recipients. The determinants of CRP have been investigated in the general population, but not in renal transplant recipients. CRP might reflect metabolic aberrations in association with central obesity and systemic atherosclerosis. However, it may also reflect a low-grade immune-mediated response to the graft. In this study we investigated the factors associated with CRP in a renal transplant population.

Chronic renal transplant dysfunction (CRTD) remains a leading cause of renal allograft loss. Evidence suggests that immunological and ischemic insults are mainly associated with CRTD occurring within the first year after transplantation, whereas nonimmunological insults are predominantly associated with CRTD beyond the first year. Several cardiovascular risk factors, such as obesity, dyslipidemia, hypertension, and diabetes mellitus have been identified as important nonimmunological risk factors for CRTD. These risk factors constitute the metabolic syndrome (MS). As renal allograft function is a surrogate marker of renal allograft loss, we investigated the association of MS with impairment of renal allograft function beyond the first year after transplantation in a cross-sectional study of 606 renal transplant outpatients. Metabolic syndrome was defined using the definition of the National Cholesterol Education Program. Renal allograft function was assessed as the 24-h urinary creatinine clearance. A total of 383 out of 606 patients (63%) suffered from MS at a median time of 6 years (2.6–11.4) post-transplant. Presence of MS was associated with impaired renal allograft function beyond 1 year post-transplant [−4.1 mL/min, 95%CI (−7.1, −1.1)]. The impact of MS did not change appreciably after adjustment for established risk factors for CRTD [−3.1 mL/min, 95%CI (−6.0, −0.2)]. However, not all component criteria of MS contributed equally. Only systolic blood pressure and hypertriglyceridemia were independently associated with impaired renal allograft function beyond 1 year post-transplant in multivariate analyses.

Chronic renal transplant dysfunction is one of the leading causes of graft failure in kidney transplantation. A complex interplay of both alloantigen-related and alloantigen-unrelated risk factors is believed to underlie its development. We propose that advanced glycation end products (AGEs) are involved in the development of chronic renal transplant dysfunction. AGE formation is associated with different alloantigen-unrelated risk factors for chronic renal transplant dysfunction, such as recipient age, diabetes, proteinuria, hypertension, and hyperlipidemia. In vitro studies have shown that AGEs induce the expression of various mediators associated with chronic renal transplant dysfunction. Furthermore, AGE-induced renal damage has been found in multiple experimental studies. This renal damage shows similarity to the damage found in chronic renal transplant dysfunction. Together, several lines of evidence support a role of AGEs in the development of chronic renal transplant dysfunction and suggest that preventive therapy with AGE inhibitors may be helpful in preserving renal function in transplant recipients.

Transplantation is the preferred organ replacement therapy for most patients with end-stage renal disease. Despite impressive improvements over recent years in the treatment of acute rejection, approximately half of all grafts will loose function within 10 years after transplantation. Chronic renal transplant dysfunction, also known as transplant atherosclerosis, is a leading cause of late allograft loss. To date, no specific treatment for chronic renal transplant dysfunction is available. Although its precise pathophysiology remains unknown, it is believed that it involves a multifactorial process of alloantigen-dependent and alloantigen-independent risk factors. Obesity, posttransplant diabetes mellitus, dyslipidemia, hypertension, and proteinuria have all been identified as alloantigen-independent risk factors. Notably, these recipient-related risk factors are well-known risk factors for cardiovascular disease, which cluster within the insulin resistance syndrome in the general population. Insulin resistance is considered the central pathophysiologic feature of this syndrome. It is therefore tempting to speculate that it is insulin resistance that underlies the recipient-related risk factors for chronic renal transplant dysfunction. Recognition of insulin resistance as a central feature underlying many, if not all, recipient-related risk factors would not only improve our understanding of the pathophysiology of chronic renal transplant dysfunction, but also stimulate development of new treatment and prevention strategies.

Chronic allograft nephropathy (CAN) is a major cause of deterioration of kidney function in transplanted patients. It is thought that glomerular ischaemia may contribute to glomerular dysfunction and proteinuria in these subjects. As reduced expression of glomerular ecto-ATPase concurrent with upregulation of glomerular ecto-AMPase activity is associated with local ischaemia, we compared the expression of these glomerular ecto-enzymes in kidney biopsies from subjects with CAN or with acute rejection vs normal human kidney tissue.

Abstract: The pathophysiology of HCMV infection may involve many different organs including the lungs. In this study we investigated HCMV antigenemia levels and cytomegalic endothelial cells (CEC) in blood in relation to the pulmonary diffusion capacity. Patients with high HCMV antigenemia ( ≥ 100 pp65+ PMNs/50.000) (n = 8) showed a more extensive decrease in the membrane factor (Dm) than patients with lower levels of HCMV antigenemia (n = 7). The decline of the diffusion capacity of the alveolar capillary membrane (KCOc) and of the pulmonary capillary volume (Vcap) was the same in both groups. Four out of nine patients had CEC in the range of 0.22 CEC/ml to 30.26 CEC/ml. All the HCMV patients showed a decreased KCOc together with a decrease of Dm and Vcap but no difference was observed between patients with and without CEC. We conclude that a higher viral load is associated with a more extensive decrease in the membrane factor and therefore with more subclinical pneumonitis. No relation was observed between CEC and pulmonary dysfunction. Therefore, we postulate that CEC levels are related indirectly to subclinical pneumonitis mediated via the viral load.

Although diagnosis of CMV infections and treatment of CMV disease with effetive antiviral drugs have become much easier, the persistent problem of CMV infection after solid-organ transplantation still requires solid knowledge of the pathophysiology of its clinical manifestations in order to minimize the impact of CMV infections in the future. The complex symptomatology of CMV infection after solid-organ transplantation is reviewed as well as some of the new theories attempting to explain the myriad of symptoms seen after transplantation.

The Human Cytomegalovirus antigenemia (HCMV-Agemia) test has been accepted worldwide as a clinical tool in the diagnosis and management of HCMV-associated syndromes in immunocompromised patients. The many modifications proposed since the first description by our laboratory make standardisation of the HCMV-Agemia assay necessary to enable multicenter clinical trials. We report the initial work for standardization of the HCMV-Agemia assay. A standard protocol is proposed, the optimal distribution conditions are investigated and the results of the shipment of positive and negative test slides as well as of two sets of coded internal standard slides are discussed. The main conclusions are that standard slides can be distributed at room temperature and that the results of participating laboratories with the coded internal standard slides were strikingly similar in spite of differences in HCMV-Agemia protocols used by participating laboratories.

Redox-sensitive cytokine cargoes backpacked on regulatory T cells promote local immune regulation in areas of rejection.

We report a case of a neurogenic bladder in which hydronephrosis recurred early after renal transplantation. After 2 unsuccessful attempts at ureteroneovesicostomy of the donor ureter a vesicocalicostomy was performed, since reconstruction with the donor pelvis or ureter was impossible owing to the extended fibrosis and scar formation around the renal hilus and ureter. During the 23 months of followup renal function has been good without serious infectious sequela with a regimen of self-catheterization and no antibiotic maintenance therapy.

Posttransplant lymphoproliferative disease (PTLD) is a serious complication that may arise after solid organ transplantation. PTLD may involve nodal as well as extranodal sites, with frequent involvement of the allograft and digestive tract. Extranodal localizations of PTLD may not always be visualized with routine conventional diagnostic methods such as computer tomography (CT) or magnetic resonance imaging (MRI). Our aim was to evaluate the value of 2-[18F] fluoro-2-deoxy-D-glucose (FDG)-PET in the staging and treatment evaluation of histologically confirmed PTLD (patient #3 confirmed by ascites cytology) in 10 kidney transplant recipients transplanted between January 2000 and June 2004. At time of PTLD diagnosis patients were fully staged by standard conventional diagnostic methods according to lymphoma protocols, including at least whole body CT scanning and bone marrow biopsy. In addition, whole body-mode PET studies were performed. All patients had their scans prior to therapy. In addition, four had repeated PET-scans during and after therapy. To compare CT with FDG-PET, all scans were re-evaluated (blinded) by a radiologist and an expert in nuclear medicine, respectively. All PET-scans at initial diagnosis could readily be interpreted and showed high FDG uptake in the primary sites of histological confirmed PTLD. This was true for monomorphic as well as polymorphic PTLD. The large majority of other sites of involvement as detected by conventional diagnostic methods at diagnosis were also PET positive. However, additional extranodal sites were detected by FDG-PET not readily visualized by CT scanning (table 1). After treatment with Rituximab, a decline or disappearance of FDG-uptake coincided with clinical response as detected by conventional diagnostic methods. These findings suggest that FDG-PET is suitable in visualization of (different histological subtypes of) PTLD and useful for both staging and response to treatment evaluation. The ability of FDG-PET to visualize extranodal localizations of PTLD, often present but not readily detectable by routine conventional diagnostic methods, is of additional value.

The number of CD8bright and CD56+ lymphocytes in the peripheral blood and their activation status were monitored by flow cytometry in 23 renal transplant recipients with cytomegalovirus (CMV) infection and were correlated with the virus load (as determined by CMV antigenemia) and clinical symptoms. Recovery from CMV infection coincided with expansion of the CD8bright and CD56+ subsets and with increased expression of the activation marker HLA-DR. Primary infection was associated with activation of both subsets, whereas during secondary infection, mainly CD8bright cells responded. Progressive CMV disease (requiring antiviral treatment) and relapse occurred in association with low numbers of activated CDSbright and CD56+ cells. Lymphocyte activation and antibody responses against CMV often occurred simultaneously, but different kinetics of these responses in some patients indicated that cellular responses are necessary to control viral replication, whereas humoral responses alone may be insufficient. Monitoring of lymphocyte activation may provide clinically useful information during CMV infection.
