Luminescent metal–organic frameworks exhibit great potential as materials for nanophotonic applications because of their programmable properties and tunable structures. In particular, luminescent guests (LG) can be hosted by metal–organic frameworks due to their porosity and guest confinement capacity, forming LG@MOF composite systems. However, such guest–host systems are mainly produced as loose powders, preventing their widespread use in practical devices and technological applications that require implementation of a stable continuum solid. In this regard, using monolithic MOF hosts might be a workable option to solve this challenge. Herein, we reported the facile synthesis and fabrication of novel prototypical sol–gel monolithic systems, designated as LG@monoMOF. Red (rhodamine B), blue (7-methoxycoumarin), and yellow (fluorescein) emitting dyes were encapsulated in a robust UiO-66 monolithic host, resulting in the red, blue, and yellow light-emitting luminescent monoliths. The mechanical and photophysical characterization of the three LG@monoMOF systems was systematically carried out in order to unravel the role of guest–host interactions in the mechanical and optical response of the bespoke LG@monoMOF composites.

Background: Trans-radial access is considered the best approach for cardiac catheterization. The choice of an alternative access route may be complex and trans-femoral access (TFA) is generally preferred. However, trans-brachial approach (TBA) may represent another feasible alternative. We therefore aimed to compare TBA and TFA in terms of access site bleeding and complications in a meta-analysis study.Methods: We systematically searched principal databases for studies comparing femoral and brachial approach in terms of in-hospital vascular complications in patients undergoing cardiac catheterization (coronary angiography or percutaneous coronary intervention).Results: Five retrospective studies and one randomized study were identified for the meta-analysis; 2756 patients undergoing a TBA and 331.208 patients undergoing a TFA for cardiac catheterization were included in the final study. No significant differences between access routes were found in terms of risk of any vascular complications (relative risk 1.18; 95% CI: 0.91–1.53; p n.s.). Brachial access was associated with a significantly lower risk of access site bleeding (relative risk 0.46; 95% CI 0.24–0.88, p= 0.02).Conclusions: TBA for cardiac catheterization was associated with a lower risk of access site bleeding and a comparable risk of any vascular complications compared with TFA. TBA may be considered a reasonable alternative access route for cardiac catheterization, at least as femoral approach.

Parts with an auxetic structure, which exhibit negative Poisson’s ratio, can be associated with a novel class of smart materials. Such interesting property has been widely explored, over the time, for different applications, i.e., medical, automotive, robotic and aeronautic field. However, the research about the design and analysis of auxetic behavior is still on the way. In this paper, a 2D re-entrant honeycomb structure was realized using material extrusion additive manufacturing technology. Two different materials, acrylonitrile butadiene styrene and carbon fiber-reinforced polyamide were adopted. The technique of digital image correlation was implemented during the tensile test to evaluate over the time and in different areas of specimens the strain behavior of the auxetic structure for both investigated materials. The measured negative Poisson’s ratio confirmed the auxetic behavior of the designed structure. The comparison between the two investigated materials showed a different trend of negative Poisson’s ratio.

Background: PICC-ports may be defined as totally implantable central venous devices inserted in the upper limb using the current state-of-the-art techniques of PICC insertion (ultrasound-guided venipuncture of deep veins of the arm, micro-puncture kits, proper location of the tip preferably by intracavitary ECG), with placement of the reservoir at the middle third of the arm. A previous report on breast cancer patients demonstrated the safety and efficacy of these devices, with a very low failure rate.Methods: This retrospective multicenter cohort study—developed by GAVeCeLT (the Italian Group of Long-Term Venous Access Devices)—investigated the outcomes of PICC-ports in a large cohort of unselected patients. The study included 4480 adult patients who underwent PICC-port insertion in five Italian centers, during a period of 60 months. The primary outcome was device failure, defined as any serious adverse event (SAE) requiring removal. The secondary outcome was the incidence of temporary adverse events (TAE) not requiring removal.Results: The median follow-up was 15.5 months. Device failure occurred in 52 cases (1.2%), the main causes being local infection (n= 7; 0.16%) and CRBSI (n= 19; 0.42%). Symptomatic catheter-related thrombosis occurred in 93 cases (2.1%), but removal was required only in one case (0.02%). Early/immediate and late TAE occurred in 904 cases (20.2%) and in 176 cases (3.9%), respectively.Conclusions: PICC-ports are safe venous access devices that should be considered as an alternative option to traditional arm-ports and chest-ports when planning chemotherapy or other long-term intermittent intravenous treatments.

We report here the synthesis of polyureas from the dehydrogenative coupling of diamines and diformamides. The reaction is catalysed by a manganese pincer complex and releases H2gas as the only by-product making the process atom-economic and sustainable. The reported method is greener in comparison to the current state-of-the-art production routes that involve diisocyanate and phosgene feedstock. We also report here the physical, morphological, and mechanical properties of synthesized polyureas. Based on our mechanistic studies, we suggest that the reaction proceeds viaisocyanate intermediates formed by the manganese catalysed dehydrogenation of formamides.

Grid-forming (GFM) converter aims to enforce the voltage and frequency at its terminals in grid-connected and islanded microgrid scenarios. The generalized closed control (GCC) scheme is a well-known solution to achieve this objective, composed of a single-loop voltage control with embedded virtual impedance. Although originally proposed to operate in converters with LC filters at their output terminals, GFM converters that operate with LCL filters have been reported in the literature. The objective of this paper is to analyze the GCC scheme in GFM converters with LCL filters. To this end, the step-by-step design procedure for the damping virtual impedance is presented. In addition, the virtual impedance also allows the design of the voltage controller gains by approximating the plant to be controlled by a first-order system. Three well-known design methodologies for resonant controllers are also compared in the proposed scheme. The results obtained validate the virtual impedance design method and summarize the differences between the design methods for resonant controllers.

Press hardening is a technology increasingly used in the automotive industries to produce high-strength structural parts in boron steel. The part strengthening is due to a martensitic microstructure, which is obtained during the quenching phase of the process by imposing cooling rates typically higher than 27 K/s. To improve cooling efficiency, conformal cooling channels (CCC) are increasingly adopting thanks to opportunities offered by additive manufacturing (AM) technologies in combination with the development of powders with high thermal conductivity.In this work a methodology for the design of CCC inside hot stamping tools is presented and the press hardening of an automotive B-Pillar in 22MnB5 has been used as case study. The proposed methodology provides for an optimization of the distance between the cooling channels (p) and distance between the tool surface and the cooling channel center (d).

Tailored Tool Tempering (TTT) is an innovative method able to calibrate the strength and ductility characteristics of the components manufacture by means of Press-Hardening process. The process parameters that most influence the final mechanical properties of the soft zone are quenching time and temperature of the heated tools.In this work, with the aim of defining a process window to estimate the soft zone properties of an automotive B-pillar in Usibor®2000 steel using TTT Press-Hardening approach, the strength and ductility of the soft zone are studied varying the quenching time and the temperature of the heated tools. Using a numerical-experimental approach, a Finite Element (FE) model is firstly developed in AutoForm to simulate the TTT Press-Hardening process and to define thermo-mechanical cycles that are characteristics of the soft zone as a function of quenching parameters (quenching time and temperature of the heated tools). FE thermo-mechanical cycles are then physically simulated on Usibor®2000 specimens using Gleeble 3180 system. The treated specimens are subsequently subjected to micro-hardness and tensile tests. Experimental results are adopted to train an artificial neural network used to construct the process window.

AbstractBackgroundLeft ventricular (LV) remodelling is a major mechanism underlying disease progression in patients with heart failure (HF) with reduced ejection fraction (EF). Previous studies that LVEF improvement and reverse remodelling can be achieved after therapy with Sacubitril/Valsartan in real-world settings. Therefore, we sought to investigate possible predictors of LV remodelling, in particular echocardiographic parameters derived by Tissue Doppler Imaging.MethodsPatients with chronic HF, LV dysfunction (EF < 35%), NYHA class II–III were followed up between September 2016 and January 2019. All patients underwent clinical and echocardiography follow up at baseline and after 12 months of therapy with sacubitril/valsartan.ResultsFifty-four consecutive outpatients were enrolled in the study. At follow-up visit LVEF (38 ± 9 vs. 30 ± 5%, p < 0.0001), LVEDD (61 ± 8 vs. 62 ± 8 mm, p = 0.0085), LVESV (114 ± 57 vs. 130 ± 56 mm3, p = 0.0001), mitral regurgitation severity (1 ± 1 vs. 2 ± 1, p < 0.0001), and left atrial area (23 ± 6 vs. 24 ± 6 mm2, p = 0.0121) changed compared to the baseline value. Changes in LVEF (follow up vs baseline) correlated with baseline levels of heart rate (r = 0.24, p = 0.048), LVEDD (r= −0.33, p = 0.004), LVEDV (r= −0.39, p = 0.001), LVESV (r = 0.37, p = 0.002), and changes in LVESV (r=−0.34, p = 0.006). Correlations remained significant even after correction at multivariate analysis including age and gender.ConclusionsTreatment with sacubitril/valsartan in patients with systolic dysfunction is associated with an improvement in LVEF in a real world scenario. Smaller LV volumes are associated with better reverse LV remodelling.

This article proposes a shunt active filter (SAF) operating as a Synchronverter to enhance its performance, including the virtual inertia functionality. The SAF behaves as a voltage source with a low-impedance path for harmonic contents. Synchronverter behavior is used together with selective filtering through proportional-resonant controllers to provide selective harmonic filtering of the load current, selective harmonic blocking of the source current, and low-impedance path for nonselected harmonic contents from the load and the source currents. The virtual inertia functionality aims to contribute to frequency stability during power system transients. Besides filtering unbalanced and harmonic contents and compensating for reactive power, the SAF contributes to inertia response during frequency transients. A tuning design method is proposed to ensure stability and proper conditioning of the power-quality issues. The reduced quantity of parameters to be tuned is another advantage of the proposed controller. Simulation and experimental results validate the compensation performance of the SAF.

Manual segmentation of lesions for Total Metabolic Tumor Volume (TMTV) measurement in 18F-FDG-PET is a time-consuming and poorly reproducible process, despite the well-established prognostic value of this metric in several tumors. Artificial intelligence (AI) offers a solution for automating TMTV measurement, thereby enhancing its clinical utility. This study aimed to develop and clinically evaluate a deep-learning-based image segmentation algorithm for automatic TMTV measurement in metastatic non-small cell lung cancer (NSCLC) patients.

This paper aims to describe the impact of the COVID-19 pandemic on the socio-economic structure of cities, and discuss the possible responses of a place-based agenda of urban policies built around the concept of "proximity economy". With this objective, the present work provides an interpretative framework for understanding the impact on urban economy with respect to Milan and Italy, by observing the emergence of the perspective of proximity in the debate to respond to the healthcare crisis due to the current pandemic. On this conceptual basis, the paper suggests measures and gives examples for implementing a proximity-based urban development agenda, concluding with a final framework of policy recommendations relating to the implementation of these policies.

Allogeneic hematopoietic stem cell transplantation (HSCT) is the most effective treatment in eradicating high-risk acute myeloid leukemia (AML). Here, we present data from a novel HLA-haploidentical HSCT protocol that addressed the 2 remaining major unmet medical needs: leukemia relapse and chronic graft-versus-host disease (cGVHD). Fifty AML patients were enrolled in the study. The conditioning regimen included total body irradiation for patients up to age 50 years and total marrow/lymphoid irradiation for patients age 51 to 65 years. Irradiation was followed by thiotepa, fludarabine, and cyclophosphamide. Patients received an infusion of 2 × 106/kg donor regulatory T cells on day −4 followed by 1 × 106/kg donor conventional T cells on day −1 and a mean of 10.7 × 106 ± 3.4 × 106/kgpurified CD34+ hematopoietic progenitor cells on day 0. No pharmacological GVHD prophylaxis was administered posttransplantation. Patients achieved full donor–type engraftment. Fifteen patients developed grade ≥2 acute GVHD (aGVHD). Twelve of the 15 patients with aGVHD were alive and no longer receiving immunosuppressive therapy. Moderate/severe cGVHD occurred in only 1 patient. Nonrelapse mortality occurred in 10 patients. Only 2 patients relapsed. Consequently, at a median follow-up of 29 months, the probability of moderate/severe cGVHD/relapse-free survival was 75% (95% confidence interval, 71%-78%). A novel HLA-haploidentical HSCT strategy that combines an age-adapted myeloablative conditioning regimen with regulatory and conventional T-cell adoptive immunotherapy resulted in an unprecedented cGVHD/relapse-free survival rate in 50 AML patients with a median age of 53 years. This trial was registered with the Umbria Region Institutional Review Board Public Registry as identification code 02/14 and public registry #2384/14 and at www.clinicaltrials.gov as #NCT03977103.

The recent Sars-Cov-2 pandemic (COVID-19) has led to growing research on the relationship between thromboembolism and Sars-Cov-2 infection. Nowadays, endothelial dysfunction, platelet activation, coagulation, and inflammatory host immune response are the subject of extensive researches in patients with COVID-19 disease. However, studies on the link between microorganisms or infections and thrombotic or thromboembolic events met fluctuating interest in the past. We, therefore, aimed to briefly summarize previous evidence on this topic, highlighting common points between previous data and what experienced today with SARS-COV2 infections.

ABSTRACTBackgroundIn Italy vaccine hesitancy worsened after a failure to vaccinate episode that took place in Friuli Venezia-Giulia Region until early 2017 which undermined herd immunity by leaving unprotected more than 5,444 children.MethodsBetween May and June 2017, 2,557 parents were surveyed at the local vaccination clinic where they were invited within the subsequent extraordinary vaccination campaign. The aim of the survey was to evaluate whether the multi-channel extraordinary vaccination campaign had reached the target population and to know parental beliefs and trusted sources of information after the failure to vaccinate event.ResultsWhile 279 parents were non-hesitant (10.9%) and 1,491 hesitant acceptors (58.3%), just 38 (1.5%) refused to have their children revaccinated. Overall, the most consulted sources of information were print media (18.8%), physicians (16.0%), relatives and friends (12.1%). The majority of parents considered vaccination as a fundamental practice (73.9%), but many were worried about potential side effects (38.8%) or doubtful about the effectiveness of some vaccines (11.0%). According to parents, 19.7% of them (57) changed their opinion about vaccines after the Codroipo case.ConclusionsAfter the Codroipo case, most parents chose to have their children re-vaccinated and just a little proportion refused the re-administration of vaccines. More studies are needed to confirm the importance of a coherent multi-channel communication strategy using both traditional and new media in order to counteract vaccine hesitancy.

Cystic fibrosis (CF) is a life-threatening recessive genetic disease caused by mutations in the gene encoding for the cystic fibrosis transmembrane conductance regulator (CFTR). With the discovery of Ivacaftor and Lumacaftor, it has been shown that administration of one or more small molecules can partially restore the CFTR function. Correctors are small molecules that enhance the amount of CFTR on the cell surface, while potentiators improve the gating function of the CFTR channel. Herein, we describe the discovery and optimization of a novel potentiator series. Scaffold hopping, focusing on retaining the different intramolecular contacts, was crucial in the whole discovery process to identify a novel series devoid of genotoxic liabilities. From this series, the clinical candidate GLPG2451 was selected based on its pharmacokinetic properties, allowing QD dosing and based on its low CYP induction potential.

Background in the era of new biological agents it is important to identify patients who may benefit from conventional therapies such as endoscopic sinus surgery (ESS) plus long-term local corticosteroids from those with patterns of inflammation that are more difficult to control post-operatively and who may benefit from other therapies.Objective determine if preoperative assessment of type and grade of inflammation and clinical factors can predict disease control with ESS plus long-term local corticosteroids in chronic rhinosinusitis with nasal polyps (CRSwNP).Methods Eighty patients treated with ESS plus mometasone-furoate 200 μg BID for CRSwNP and followed for at least 1 year were enrolled (November 2017–December 2018) in this prospective observational study. Type and grade of inflammation were evaluated preoperatively by nasal cytology. Based on cellular pattern, patients were grouped as neutrophilic (n = 20), eosinophilic (n = 38), or mixed eosinophil-neutrophilic (n = 22). SNOT-22 and Lund-Kennedy Endoscopic Score were evaluated at baseline and at 3, 6, 9, and 12 months after surgery and used to define disease control.Results The cumulative probability of remaining free of significant modification of endoscopic score (Lund-Kennedy Endoscopic Score >2) at 3, 6, 9, and 12 months was 0.84, 0.76, 0.71, and 0.68, respectively. At 12-month postoperative evaluation good disease control was observed in 54 of 80 patients (67.5%). Compared to those with good post-operative disease control, those with poor control had a significantly higher pre-operative mean count of eosinophils and neutrophils (p < 0.05). The preoperative inflammatory pattern was associated with relative risk of poor control: neutrophilia (RR: 3.10; CI:1.24–7.71), eosinophilia (RR:8.42; CI:2.72–15.12), and mixed eosinophilic and neutrophilic (RR:25.11; CI:19.41–30.01). We also confirmed that asthma, allergy, blood eosinophilia, and ASA triad could predict poor control.Conclusions The type and load of inflammation evaluated preoperatively and selected clinical factors can predict poor control of CRSwNP treated with ESS and local corticosteroids.

AbstractHuman-induced biological introductions pose a major threat to global biodiversity, and this is especially frequent in the eastern Mediterranean region, which is a globally important biodiversity hotspot area of high conservation value. To predict at which level introduced species in this region might become invasive under current and projected climate conditions, 232 non-native aquatic organisms were screened using the Aquatic Species Invasiveness Screening Kit. Based on receiver operative characteristic curve analysis, thresholds were identified to distinguish between low, medium and high risk species. The “top invasive” (very high risk) species identified were: brown bullhead Ameiurus nebulosus, blue crab Callinectes sapidus, gibel carp Carassius gibelio, Philippine catfish Clarias batrachus, Chinese mitten crab Eriocheir sinensis, bluespotted cornetfish Fistularia commersonii, silver carp Hypophthalmichthys molitrix, silver-cheeked toadfish Lagocephalus sceleratus, half-smooth golden pufferfish Lagocephalus spadiceus, Suez pufferfish Lagocephalus suezensis, signal crayfish Pacifastacus leniusculus, fathead minnow Pimephales promelas, channeled applesnail Pomacea canaliculata, red swamp crayfish Procambarus clarkii, devil firefish Pterois milesand European catfish Silurus glanis. The risk of being invasive of more than half of the screened species increased after taking global warming predictions into account, and several species considered to be globally invasive (cf. high risk) were classified as posing only a medium risk for the eastern Mediterranean region. Region-specific risk screenings, as implemented in this study, are therefore essential for setting priorities in preventative management for the conservation of key biodiversity hotspots and the optimal allocation of resources in view of full risk assessment for the species identified as (very) high risk.

Heart failure is a complex syndrome affecting several organs including kidney, lungs, liver, brain muscles and sympathetic system. Each of these organs might contribute to its severity and prognosis. The prognosis assessment is critical for a correct heart failure management. It has already been demonstrated that a single parameter is weaker for prognosis than different parameters combined. The Metabolic Exercise test data combined with Cardiac and Kidney Indexes (MECKI) score has been built and validated for heart failure with reduced ejection fraction (HFrEF) patients by considering cardiopulmonary exercise test data combined with clinical, laboratory and echocardiographic measurements. The betablockers treatment is a milestone in the HFrEF management. In the MECKI score database, the association of betablockers treatment with outcome has been investigated in different settings.

A 15-year-old girl with a history of complex congenital heart disease and prior pulmonary valve replacement presented with suspected endocarditis. PET/CT imaging with 18F-FDG was performed to evaluate the potential presence of intracardiac vegetations after previously inconclusive findings from CT angiography, transthoracic echocardiography, and transesophageal echocardiography. PET/CT detected heterogeneous, asymmetric, increased 18F-FDG uptake in the region of the pulmonary valve prosthesis, typical for infection, and confirmed diagnosis of bacterial infective endocarditis. This report highlights the utility of 18F-FDG PET/CT imaging to complement the Duke criteria for determining the diagnosis and therapeutic management of pediatric patients with infective endocarditis.

Melanoma is an aggressive neoplasm with increasing incidence that is classified by the NCI as a recalcitrant cancer, i.e., a cancer with poor prognosis, lacking progress in diagnosis and treatment. In addition to conventional therapy, melanoma treatment is currently based on targeting the BRAF/MEK/ERK signaling pathway and immune checkpoints. As drug resistance remains a major obstacle to treatment success, advanced therapeutic approaches based on novel targets are still urgently needed. We reasoned that the base excision repair enzyme thymine DNA glycosylase (TDG) could be such a target for its dual role in safeguarding the genome and the epigenome, by performing the last of the multiple steps in DNA demethylation. Here we show that TDGknockdown in melanoma cell lines causes cell cycle arrest, senescence, and death by mitotic alterations; alters the transcriptome and methylome; and impairs xenograft tumor formation. Importantly, untransformed melanocytes are minimally affected by TDGknockdown, and adult mice with conditional knockout of Tdgare viable. Candidate TDG inhibitors, identified through a high-throughput fluorescence-based screen, reduced viability and clonogenic capacity of melanoma cell lines and increased cellular levels of 5-carboxylcytosine, the last intermediate in DNA demethylation, indicating successful on-target activity. These findings suggest that TDG may provide critical functions specific to cancer cells that make it a highly suitable anti-melanoma drug target. By potentially disrupting both DNA repair and the epigenetic state, targeting TDG may represent a completely new approach to melanoma therapy.

We demonstrate the directional scattering cancellation for a dielectric sphere of radius up to 10 times the incident wavelength, by coating it with a surface of finite conductivity. Specifically, the problem of determining the values of surface conductivity that guarantee destructive interference among hundreds of multipolar scattering orders at the prescribed angular direction is reduced to the determination of the zeros of a polynomial, whose coefficients are analytically known.

Sporadic Hidradenitis Suppurativa (spHS) is a multifactorial disease in which genetic predisposition is intertwined with environmental factors. Due to the still to date limited knowledge of spHS genetics, we calculated Polygenic scores (PGS) to study the genetic underpinnings that contribute to spHS within European demographic.

Ceres is the largest body in the main asteroid belt. It was recently explored by the Dawn mission to uncover strong similarities with other icy bodies. The morphological features observed on the surface of Ceres indicate a relatively wide range of water ice concentrations, leading us to investigate the magnitude and distribution of crustal density heterogeneities, and to consider whether they could have caused a reorientation of Ceres. Here, we present three independent and corroborating lines of evidence for the true polar wander of Ceres. Thanks to the global gravity inversion approach applied to the shape and gravity data of Ceres, we find crustal density heterogeneities up to approximately ±0.3 g cm−3, with a prominent positive density anomaly aligned with the equator, in the region of Ahuna Mons. The topography shows the remnants of an equatorial ridge compatible with the position of the palaeo-equator, and indicates that Ceres reoriented by approximately 36°, with the palaeo-pole following an indirect path to the current pole of Ceres. The tectonic patterns generated by the true polar wander are in close agreement with the location and orientation of the Samhain Catenae and Uhola Catenae crustal fractures. These results highlight the complex interior structure and richness of processes taking place in Ceres-scale icy bodies.

Different methods used to store specimens, even for short periods, can notably affect morphometric characteristics such as colour, length and shape. As a consequence, in the case of samples that have to be analysed morphometrically, the right preservation method is crucial. In this study, two comparably easy applicable methods, the effects of freezing and moderate cool storage on morphological traits, were studied on specimens of the scaleless North American channel catfish Ictalurus punctatus, an alien species to European freshwaters of increasing concern. Results revealed that freezing, like preservation with alcohol or formalin, affected the generally considered characteristics (colour, standard length, fork length, total length, shape), while cool storage was shown to preserve most traits without notable alteration, an important difference when studying potentially adapting alien species. Implications are discussed.

Background: Lopinavir and Ritonavir (LPV/r) treatment is widely used to prevent HIV mother-to-child transmission. Nevertheless, studies related to the impact of these compounds on patients, in particular in the foetus and newborns, are strictly required due to the controversial findings reported in the literature concerning possible neurologic side effects following the administration of these drugs. Objectives: In our study, we evaluated the impact of LPV/r treatment on the human glioblastoma U- 87 MG cell line. Methods: In order to evaluate the influence of Lopinavir and Ritonavir in terms of oxidative stress (ROS production), mitochondrial morphology and apoptotic cell death, the latter either in the presence or in the absence of caspase-3 and -9 inhibitors, we treated U-87 MG with increasing doses (0.1-1-10-25-50 μM) of Lopinavir and Ritonavir for 24h, either in single formulation or in combination. ROS production was measured by flow cytometry using H2DCFDA dye, mitochondrial morphology was evaluated using MitoRed dye and apoptotic cell death was monitored by flow cytometry using Annexin V-FITC and Propidium Iodide. Results: We observed that co-treatment with Lopinavir and Ritonavir (25 and 50 μM) promoted a significant increase in ROS production, caused mitochondrial network damage and induced apoptosis in a caspase-independent manner. Conclusion: Based on our findings, concordant with others reported in the literature, we hypothesize that LPV/r treatment could not be entirely free from side effects, being aware of the need of validation in in vivo models, necessary to confirm our results.

Purpose:To identify anatomic and hemodynamic changes associated with impending visceral chimney stent-graft occlusion after endovascular aneurysm repair (EVAR) with the chimney technique (chEVAR). Methods:A retrospective evaluation was performed of computed tomography scans from 41 patients who underwent juxtarenal chEVAR from 2008 to 2012 to identify stent-grafts demonstrating conformational changes following initial placement. Six subjects (mean age 74 years; 3 men) were selected for detailed reconstruction and computational hemodynamic analysis; 4 had at least 1 occluded chimney stent-graft. This subset of repairs was systematically analyzed to define the anatomic and hemodynamic impact of these changes and identify signature patterns associated with impending renovisceral stent-graft occlusion. Spatial and temporal analyses of cross-sectional area, centerline angle, intraluminal pressure, and wall shear stress (WSS) were performed within the superior mesenteric and renal artery chimney grafts used for repair. Results:Conformational changes in the chimney stent-grafts and associated perturbations, in both local WSS and pressure, were responsible for the 5 occlusions in the 13 stented branches. Anatomic and hemodynamic signatures leading to occlusion were identified within 1 month postoperatively, with a lumen area <14 mm2(p=0.04), systolic pressure gradient >25 Pa/mm (p=0.03), and systolic WSS >45 Pa (p=0.03) associated with future chimney stent-graft occlusion. Conclusion:Chimney stent-grafts at increased risk for occlusion demonstrated anatomic and hemodynamic signatures within 1 month of juxtarenal chEVAR. Analysis of these parameters in the early postoperative period may be useful for identifying and remediating these high-risk stent-grafts.

ABSTRACTThe ability of cells to transmit bioactive molecules to recipient cells and the extracellular environment is a fundamental requirement for both normal physiology and disease pathogenesis. It has traditionally been thought that soluble factors released from cells were responsible for this cellular signaling but recent research has revealed a fundamental role for microvesicles in this process. Microvesicles are heterogeneous membrane-bound sacs that are shed from the surface of cells into the extracellular environment in a highly regulated process. They are shed following the selective incorporation of a host of molecular cargo including multiple types of proteins and nucleic acids. In addition to providing new insight into the etiology of complex human diseases, microvesicles also show great promise as a tool for advanced diagnosis and therapy as we move forward into a new age of personalized medicine. Here we review current status of the rapidly evolving field of microvesicle biology, highlighting critical regulatory roles for several small GTPases in the biology and biogenesis of shed microvesicles.

Economists, nutritionists, and geneticists have attempted to describe dairy cattle efficiency in simple, quantifiable terms. On-farm measures of dairy efficiency include physical feed efficiency, efficiency of nutrient usage, economic feed efficiency, total dairy enterprise efficiency, and lifetime efficiency. Each calculated measure of dairy efficiency has its own advantages and limitations. Each measure has merit for describing a segment of dairy efficiency, yet no one measure can sufficiently describe dairy efficiency or be applicable across all farms. Use of multiple dairy efficiency metrics, each with a moving target specific to the individual dairy enterprise, should be considered. Nutrition and nutrient management interventions can improve the use of dairy resources, increasing both economic and environmental sustainability. With greater DMI and milk yield, a smaller proportion of dietary nutrients are used for maintenance functions, improving productive efficiency and reducing the environmental impact of the dairy cow. Nutritional factors independent of cow genetic merit affect energetic losses in the form of feces, heat of digestion and metabolism, or methane. Improvements in nutrient retention can occur with increases in rate of digestion and decreases in rate of passage of feed ingredients. Forage and grain losses, feed ingredient options, and forage and feed ingredient targeting according to cow potential need to be considered. Consistency of delivery and consumption of the formulated ration without high feed refusal rates typically improves dairy efficiency. Cow grouping affects social behavior, cow well-being, nutrient wastage, milk yield, and expenses, with optimum strategies being farm specific.

AbstractThe community concept has maintained a constant and growing interest in urban studies and many related fields. The origin of this continuing interest seems to derive from the importance of the concept of community within diverse forms of political language and interpretations within different planning practices. In this contribution, through the analysis of different ethical and planning theories, we want to provide an update framework on community action. According to this objective, the argumentation will proceed through a literature review on four ethics theories and three key aspects related to spatial planning, as well as matching this theoretical analysis with exemplifying practices. The final objective is to provide an original analysis on drivers and outcomes of different forms of community, raising general issues that refer to spatial planning, social organization and regulation.
