Das Schädel-Hirn-Trauma gehört in Deutschland zu den häufigsten verletzungsbedingten Todesursachen. Posttraumatisch sind neurologische Defektzustände häufig. Je nach dem welches Kompartiment im Bereich des Unterschenkels von der Lähmung betroffenen ist, wird die Fehlstellung durch die beteiligte Muskulatur und das dadurch ausgelöste Muskelungleichgewicht bestimmt. Dadurch kommt es durch die deformierende Kraft unbehandelt zu einer zunehmenden Fehlstellung. Je stärker der Kraftunterschied zwischen Agonisten und Antagonisten ist, desto schneller und ausgeprägter entwickelt sich die Deformität. Eine sorgfältige Evaluation des klinischen Bildes sowie der Muskelfunktion ist präoperativ essentiell. Das therapeutische Prinzip besteht aus der Korrektur der Deformität, der Stabilisierung instabiler Gelenke sowie der Balancierung der verbliebenen Muskelkraft. Es gibt zahlreiche operative Verfahren, die zum Einsatz kommen können. Als Operateur sollte man aus diesem Grund das gesamte Spektrum der knöchernen und weichteiligen Verfahren der Vor- und Rückfußkorrektur beherrschen. Traumatic brain injuries are the most common cause of death after accidents. Neurologic deficits are frequent. Muscular imbalance is the main cause for progredient deformities in the foot and ankle. The permanent strength imbalance between agonists and antagonists is leading to structural soft tissue and bony changes. Therefore, a careful evaluation of the clinical findings is mandatory for planning the surgical procedures. The main goal is to correct the deformities, to stabilize instable joints and to rebalance the remaining muscle power. Schmerzen am Ansatz der Achillessehne stellen mit einer Lebenszeitinzidenz von 6% ein häufiges Krankheitsbild dar. Mittels klinischer Untersuchung, Röntgen und MRT können die Pathologien entsprechend ihrer Lokalisation in drei verschiedene Kompartimente (retro-, intra-, prä-achillär) eingeteilt werden. Führt die konservative Therapie nicht zu einer ausreichenden Besserung der Beschwerden, sollte die Operation erwogen werden. Am häufigsten wird das offene, vollständige Debridement sämtlicher im Röntgen und MRT identifizierter Pathologien über einen transachillären Zugang mit türflügelartigen Ablösen des Achillessehnenansatzes durchgeführt. Auch wenn die meisten Autoren bei einer Ablösung der Achillessehne von mehr als 50% die Refixation der Achillessehne mittels Anker empfehlen, fehlen hierfür valide Daten. Ebenso limitiert ist die Studienlage für einen zusätzlichen Transfer der Flexor Hallucis Longus (FHL) Sehne bei Sehnendebridement von mehr als 50% des Sehnendurchmessers oder höherem Patientenalter. In dem einzigen vorhandenen RCT wurden ausschließlich Patienten > 50 Jahre eingeschlossen unabhängig vom Ausmaß des Debridements. Mit und ohne FHL Transfer zeigten sich keine signifikanten Unterschiede für die subjektive Patientenzufriedenheit. Grundsätzlich führt die offene Operation zu guten bis sehr guten subjektiven Behandlungsergebnissen. Teilweise werden aber Komplikationsraten von bis 38% beschrieben. Eine Möglichkeit diese zu senken, stellt die endoskopische Operation dar. Allerdings kann mittels Endoskopie nur das retro-achilläre Kompartiment behandelt werden. Sieben Studien berichteten bei kumulativ 377 Patienten über gute bis exzellente subjektive Behandlungsergebnisse mit geringer Komplikationsrate. Auf Basis der vorhandenen Literatur führen die Autoren bei isolierter retro-achillärer Pathologie ein endoskopisches Debridement und Kalkaneoplastie durch. Bei klinisch retro-achillären Beschwerden, aber in der Bildgebung zusätzlich intra-achillären Pathologien, wird eine retro-achilläre Testinfiltration durchgeführt. Kommt es dadurch zu einer deutlichen Beschwerdebesserung wird dem Patienten eine endoskopische Operation angeboten. Führt dies nicht zu einer Beschwerdebesserung, oder ist die Beschwerdelokalisation / Pathologie inträ- bzw. prä-achillär, wird eine offene Operation empfohlen. Wird dabei aufgrund intratendinöser Veränderungen eine Ablösung der Achillessehne über 50% notwendig, wird eine Refixation mittels Fadenankern in Betracht gezogen. Ein FHL-Transfer wird bei Verlust von über 50% des Sehnendurchmessers erwogen. Wann und ob diese beiden Strategien durchgeführt werden sollten ist aber aufgrund der eingeschränkten Studienlage nicht eindeutig zu beantworten. With a lifetime incidence of 6%, insertional tendinopathy of the Achilles tendon is a common pathology in orthopaedic practice. By clinical examination, radiographs and MRI, pathologies can be divided into three different compartments (retro-, intra-, pre-achillary) according to their localization. If the non-operative therapy does not lead to a sufficient improvement of the symptoms, operative treatment should be considered. Most frequently, open, complete debridement of all pathologies identified in X-rays and MRI is performed via a trans-achillary approach and central tendon splitting of the Achilles tendon. Even if most authors recommend refixation of the Achilles tendon with suture anchors in case of an Achilles tendon detachment of more than 50%, valid data are missing. Similarly, no valid studies are available clearly supporting the transfer of the flexor hallucis longus tendon in case of tendon debridement of more than 50% of its diameter. The only RCT available included only patients > 50 years independent of the amount of tendon debridement. With and without FHL transfer no significant differences were observed for patient rated outcome parameters. Taken together, open surgery leads to good to excellent subjective results. However, some studies reported complication rates of up to 38%. Endoscopic surgery is an option to reduce these complication rates. Nevertheless, endoscopy can only address the retro-achillary compartment. Seven studies reported good to excellent subjective treatment results with a low complication rate for retro-achillary pathologies in a cumulative total of 377 patients. Based on the studies currently available, the authors perform endoscopic debridement and calcaneoplasty for isolated retro-achillary pathologies. In the case of contradictory results, meaning retro-achillary symptoms on clinical examination, but additional intra-achillary pathologies in imaging, retro-achillary test infiltration is performed. If this leads to a significant improvement of pain, the patient is offered endoscopic surgery. If this does not lead to a sufficient improvement of the complaints, or if the localization of the complaints/pathology is intra- or pre-achillary, open surgery is recommended. If due to intratendinous changes, a detachment of more than 50% of the Achilles tendon becomes necessary, a refixation using suture anchors can be performed. A flexor hallucis longus transfer can be considered if 50% of the tendonś diameter was debrided, even though these two strategies cannot be clearly recommended due to the limited amount and quality of studies available. Material & Methode Der Tibialis posterior-Sehnentransfer auf den Fußrücken zielt auf eine partielle Wiederherstellung der aktiven Fußhebung bei gleichzeitiger Eliminierung einer deformierenden Kraft. Voraussetzungen sind eine erhaltene Beweglichkeit im oberen Sprunggelenk und Kraftentfaltung des M. tibialis posterior. Der Transfer der Sehne des M. flexor digitorum longus auf den Stumpf der Tibialis posterior-Sehne lässt eine gewisse aktive Pro- und Supination zu. Je nach Ausmaß und Rigidität der Fehlstellung kommen zusätzliche Maßnahmen wie die perkutane Z-Plastik der Achillessehne, serielle Sehnenverlängerungen der tiefen Beuger, Arthrolysen und Arthrodesen in Frage. Ergebnisse und Schlussfolgerungen Die häufigsten Komplikationen sind der postoperative Sehnenabriss, die Über- oder Unterkorretur sowie Bewegungseinschränkungen durch sekundäre Vernarbungen. Ein sekundärer Pes planovalgus wurde bislang nur fallweise berichtet. Insgesamt ist die Patientenzufriedenheit nach Tibialis posterior-Sehnentransfer aufgrund des deutlichen Funktionsgewinnes hoch. Summary Background Drop foot secondary to peripheral nerve injury leads to severe functional limitation for the affected patients. With failed nonoperative treatment, surgical reconstruction has to be considered. Materials and Methods Transfer of the tibialis posterior tendon to the lateral cuneiform aims at partial restoration of active dorsiflexion while eliminating a deforming force. A functional ankle joint and preserved power of the tibialis posterior muscle are prerequisites for the procedure. Transfer of the flexor digitorum longus tendon on the tibialis posterior stump helps restoring some active eversion and inversion of the foot after the tibialis posterior transfer. Depending on the amount and rigidity of the deformity, additional procedures such as percutaneous Achilles tendon lengthening, serial lengthening of the deep flexor tendons, arthrolysis, or arthrodesis have to be considered. Results and Conclusions The most frequently observed complications are graft loosening, over- and undercorrection, and restricted motion due to adhesions. Rotator cuff tears are among the most common causes of pain and disability in the upper extremity. Despite significant advances in repair techniques and instrumentation, retear rates after surgery remain high. Numerous avenues of structural and biological augmentation have been explored to increase healing potential and achieve successful outcomes particularly in patients with massive cuff tears and those undergoing revision surgery. The purpose of this article is to present and discuss various techniques currently published that are designed to augment this process through variable methodologies. A common methodology is the use of graft tissue to "load share" with the repair or facilitate placement of pluripotential stem cell or growth factors at the site of needed healing or both. Although this field remains in development, this article attempts to explain the concepts currently employed and summarize the current Food and Drug Administration-approved options. Although rare, scapular winging can be a painful and disabling condition. Scapular winging has been categorized as medial or lateral winging, with medial being the more common type. Medial winging is the result of serratus anterior paralysis from injury to the long thoracic nerve. The most common etiology is neuropraxia after blunt or stretch injury. Lateral winging, from trapezius paralysis with injury to the spinal accessory nerve, is most likely iatrogenic from procedures involving the posterior cervical triangle. Recovery of nerve function can take up to 2 years. For those cases that do not have return of nerve function, operative treatment is an option. Tendon transfers were introduced in the early 1900s and have been met with satisfactory results in small case series. The purpose of this article is to describe the indications, operative technique, postoperative rehabilitation, and results of tendon transfer for medial and lateral scapular winging.
