Abstract

AbstractBackgroundTo chart the evolution of the CTO-PCI landscape in Belgium and Luxembourg, the Belgian Working Group on Chronic Total Occlusions (BWGCTO) was established in 2016.MethodsBetween May 2016 and December 2019, patients undergoing a CTO-PCI treatment were prospectively and consecutively enrolled. Twenty-one centres in Belgium and one in Luxembourg participated. Individual operators had mixed levels of expertise in treating CTO lesions. Demographic, angiographic, procedural parameters and incidence of major adverse cardiac and cerebrovascular events (MACCE) were systematically registered.ResultsOver a four-year enrolment period, 1832 procedures were performed in 1733 patients achieving technical success in 1474 cases (80%), with an in-hospital MACCE rate of 2.3%. Fifty-nine (3%) cases were re-attempt procedures of which 41 (69%) were successful. High-volume centres treated more complex lesions (mean J-CTO score: 2.15 ± 1.21) as compared to intermediate (mean J-CTO score: 1.72 ± 1.23; p < 0.001) and low-volume centres (mean J-CTO score: 0.99 ± 1.21; p = 0.002). Despite this, success rates did not differ between centres (p = 0.461). Overall success rates did not differ over time (p = 0.810). High-volume centres progressively tackled more complex CTOs while keeping success rates stable. In all centres, the most applied strategy was antegrade wire escalation (83%). High-volume centres more often successfully applied antegrade dissection and re-entry and retrograde techniques in lesions with higher complexity.ConclusionWith variable experience levels, operators treated CTOs with high success and relatively few complications. Although AWE remains the most used technique, it is paramount for operators to be skilled in all contemporary techniques in order to be successful in more complex CTOs.

AbstractAimsThe current study assessed the impact of COVID-19-related public containment measures (i.e. lockdown) on the ST elevation myocardial infarction (STEMI) epidemic in Belgium.Methods and resultsClinical characteristics, reperfusion therapy modalities, COVID-19 status and in-hospital mortality of consecutive STEMI patients who were admitted to Belgian hospitals for percutaneous coronary intervention (PCI) were recorded during a three-week period starting at the beginning of the lockdown period on 13 March 2020. Similar data were collected for the same time period for 2017–2019. An evaluation of air quality revealed a 32% decrease in ambient NO2concentrations during lockdown (19.5 µg/m³ versus 13.2 µg/m³, p < .001). During the three-week period, there were 188 STEMI patients admitted for PCI during the lockdown versus an average 254 STEMI patients before the lockdown period (incidence rate ratio = 0.74, p = .001). Reperfusion strategy was predominantly primary PCI in both time periods (96% versus 95%). However, there was a significant delay in treatment during the lockdown period, with more late presentations (>12 h after onset of pain) (14% versus 7.6%, p = .04) and with longer door-to-balloon times (median of 45 versus 39 min, p = .02). Although the in-hospital mortality between the two periods was comparable (5.9% versus 6.7%), 5 of the 7 (71%) COVID-19-positive STEMI patients died.ConclusionThe present study revealed a 26% reduction in STEMI admissions and a delay in treatment of STEMI patients. Less exposure to external STEMI triggers (such as ambient air pollution) and/or reluctance to seek medical care are possible explanations of this observation.

Abstract

To assess the results of a conservative coronary angioplasty strategy in unstable angina pectoris, the records of 1,421 consecutive patients without previous myocardial infarction undergoing a first percutaneous transluminal coronary angioplasty (PTCA) between 1986 and 1990 were reviewed. Of these patients, 631 had unstable and 790 had stable angina pectoris. Only after an intense effort to medically control symptoms, the unstable patients underwent PTCA at an average of 15.4 days (range 1 to 76) after hospital admission. Primary clinical success was achieved in 91.7% of patients with unstable and in 94.4% of those with stable angina pectoris (p = not significant). In-hospital mortality rates were 0.3 and 0.1%, respectively (p = not significant). Nonfatal in-hospital event rates for acute myocardial infarction, cerebrovascular accident and coronary bypass surgery were only slightly higher in patients with unstable angina pectoris; however, the difference from the stable group was significant when all events were combined (9 vs S.9%; p

Abstract

The safety of the combination of heparin and ridogrel therapy and its antiplatelet efficacy was examined in the setting of percutaneous transluminal coronary angioplasty (PTCA). In 32 patients without known aspirin intake for 10 days before PTCA, therapy with ridogrel (300-mg intravenous bolus) was begun just before PTCA and continued orally at a dose of 300 mg twice daily until discharge. Heparin was administered as a 10,000 IU bolus dose before PTCA and followed by an intravenous infusion at a rate of 1,000 IU/hour for 24 hours. Bleeding problems at the arterialentry site occurred in 13 patients, which required a blood transfusion in only 2 patients. One patient underwent emergency bypass surgery without specific problems of hemostasis. Ridogrel virtually eliminated thromboxane [B.sub.2] from the serum (29,2900 [+ or -] 6,555 pg/0.1 ml before vs 63 [+ or -] pg/0.1 ml at 2 hours after ridogrel), with a concomitant increase in serum 6-keto-prostaglandin [F.sub.1[alpha]] (511 [+ or -] 34 pg/0.1 ml before vs 1,190 [+ or -] 146 pg/0.1 ml at 24 hours after ridogrel). There were no acute reocclusions in the ridogrel-treated patients, whereas acute reocclusions occurred in 5.6% of the patients taking the standard aspirin + heparin regimen during the same period. Furthermore, at 6-month clinical follow-up patients treated with ridogrel compared favorably with those receiving standard treatment. (Am J Cardiol 1991;68:463-466), Percutaneous transluminal coronary angioplasty (PTCA) is a technique used to dilate (widen) the coronary arteries, which supply blood to the heart. PTCA is used in the treatment of conditions such as atherosclerosis, in which cholesterol-rich deposits narrow the coronary arteries, diminishing the cardiac blood supply. While this therapy is quite effective, 2 to 11 percent of patients undergoing PTCA suffer reocclusion shortly after the procedure. One strategy for preventing reocclusion after PTCA has been the administration of drugs that prevent adhesion of blood platelets to the arterial wall (the precipitating factor in initiating reocclusion); however, the drugs currently available are not completely effective. A newly available drug, ridogrel, has been proposed as a more effective antiplatelet drug for use in this setting. This agent acts by inhibiting key enzymes involved in the platelet-initiated clotting process. To evaluate the utility and safety of ridogrel, a study involving 32 eligible patients (of 352 potential enrollees) referred for PTCA were administered either ridogrel and heparin or aspirin and heparin immediately after PTCA. (Aspirin and heparin are anticlotting medications.) Biochemical measurements indicated that ridogrel virtually eliminated blood levels of the enzymes on which it works. None of the patients treated with ridogrel suffered reocclusion after PTCA; 5.6 percent of the aspirin and heparin-treated patients did. Abstract

To determine the relevance of recent refinements in angioplasty technology to our particular practice, the records of 507 consecutive patients undergoing a first percutaneous transluminal coronary angioplasty (PTCA) at our center between October 1988 and May 1989 were reviewed. At the time of PTCA, 41% of these patients had class IV angina and 44% were identified as having multivessel disease. Dilatation was attempted in 734 lesions (mean 1.5 per patient), of which 95 (13%) were chronic total occlusions. Overall, 69% of the 734 lesions were judged anatomically complex, and, in dilating these lesions, a rail-type device was used almost exclusively. Successful dilatation was achieved in 659 of the 734 (90%) attempted lesions. There were low incidences of the major complications of death (0.4%), myocardial infarction (1.8%) and emergency bypass surgery (1.8%). Acute rethrombosis occurred in 54 patients (11). in these patients, initial strategy of repeat dilatation was successful in 38 of 47 patients (81%). Overall, primary clinical success at PTCA was achieved in 480 patients (95%). At a mean follow-up of 7.5 [plus or minus] 1.5 months in 497 of the study patients, the event-free rate (freedom from cardiac death, myocardial infarction, repeat PTCA or coronary bypass surgery or recurrence of severe [class ill to IV] angina) was 71%. in conclusion, despite the often complex coronary disease in patients currently presenting to our center, a high initial success rate and acceptable short-term outcome of PTCA was achieved. (Am J Cardiol 1991;67:253-258), Coronary artery disease (CAD) involves stenosis (narrowing) of the arteries supplying the heart with blood, to the extent that cardiac blood flow is compromised. This is usually caused by the formation of fatty atherosclerotic plaques on the inner surface of the arteries. A common treatment for CAD is percutaneous transluminal coronary angioplasty (PTCA), in which a catheter with an inflatable balloon tip is passed through the blood vessels to the occluded region of the coronary arteries, inflated (distending the narrowed region to restore blood flow), and removed. In recent years, improved maneuverability of the catheter system has increased the success rate for this procedure. To obtain an overview of how successful the use of one particular type of angioplasty device (the rail-type dilatation system) has been, the records of 507 consecutive patients undergoing their first PTCA at a Belgian cardiology practice between October 1988 and May 1989 were reviewed. At the time PTCA was performed, 41 percent of the patients had very severe angina pectoris (chest pains resulting from the CAD), and 44 percent had CAD afflicting two or more coronary blood vessels (multivessel CAD). Angioplasty was attempted on 734 narrowed arterial segments (an average of 1.5 per patient); 95 percent of these were total chronic occlusions. Successful restoration of blood flow was obtained in 90 percent of the cases. Low incidence of death (0.4 percent), heart attack (1.8 percent) and the need for emergency bypass surgery (1.8 percent) was noted. Recurrence of the blockage was seen in 11 percent of the patients; 81 percent of these were successfully treated with a repeat PTCA procedure. At 7.5-month (average) follow-up, 71 percent of the procedures were judged to have been successful (i.e. no incidence of cardiac death, heart attack, repeat PTCA, bypass surgery, or recurrence of severe angina pectoris). Abstract

Patients with stable coronary artery disease and diabetes with previous percutaneous coronary intervention (PCI), particularly those with previous stenting, are at high risk of ischaemic events. These patients are generally treated with aspirin. In this trial, we aimed to investigate if these patients would benefit from treatment with aspirin plus ticagrelor.

7. A quality project for radiation reduction in the cath lab

Abstract

AbstractThe radiation exposure in the cath lab of patients, cardiologists, and nurses was measured during three consecutive periods of 8 weeks. The first 8 weeks the baseline radiation exposure was obtained. In the second period standard incidences for coronarography and frame rate were changed, without compromising the image quality of the examination. In the third period, a pelvic shield covered the lower part of the patient. This pilot quality project demonstrates that further significant reduction in radiation exposure of 37% is possible for patients. A significant reduction in radiation exposure of 53 and 62% was obtained among cardiologists and nurses working in the cath lab, even with the already diminished radiation exposure over the last years by better equipment and general radioprotection measures.

Abstract

AbstractBackground:Important developments in materials, devices, and techniques have improved outcomes of chronic total occlusion (CTO) percutaneous coronary intervention (PCI), and resulted in a growing interest in CTO-PCI. The Belgian Working Group on Chronic Total Occlusions (BWGCTO) working group aims to assess the evolution within the CTO-PCI landscape over the next years.Methods:From May 2016 onwards, patients undergoing CTO-PCI were included in the BWGCTO registry by 15 centres in Belgium and Luxemburg. Baseline, angiographic, and procedural data were collected. Here, we report on the one-year in-hospital outcomes.Results:Over the course of one year, 411 procedures in 388 patients were included with a mean age of 64 ± 11 years. The majority were male (81%). Relatively complex CTOs were treated (Japanese CTO score =2.2 ± 1.2) with a high procedure success rate (82%). Patient- and lesion-wise success rates were 83 and 85%, respectively. Major adverse in-hospital events were acceptably low (3.4%). Antegrade wire escalation technique was applied most frequently (82%). On the other hand, antegrade dissection and re-entry and retrograde strategies were more frequently applied in higher volume centres and successful for lesions with higher complexity.Conclusion:Satisfactory procedural outcomes and a low rate of adverse events were obtained in a complex CTO population, treated by operators with variable experience levels. Antegrade wire escalation was the preferred strategy, regardless of operator volume.

Abstract

To investigate the antiischemic efficacy of intravenously administered diltiazem, 42 patients were randomlY allocated to receive placebo or active treatment before 1-vessel percutaneous transluminal coronary angioplasty (PTCA). The development of myocardial ischemia was studied using subjective (pain) and objective electrocardiography) parameters. Pretreatment with intravenous diltiazem resulted in a significantly delayed onset of ischemic pain and ST-segment elevation; these variables also returned to baseline earlier after balloon deflation. Thus, intravenous diltiazem prevents or delays the onset of myocardial ischemia during repetitive transient coronary occlusions; improvement of the myocardial blood flow distal to the coronary occlusion or impedance of calcium entry into the ischemic cell are considered as possible mechanisms. Because PTCA is increasingly used in patients with poor left ventricular function and more extensive disease, and because recent evidence suggests that better PTCA results could be obtained by the use of longer inflation times, the addition of diltiazem to the classic armamentarium could be beneficial. (Am J Cardiol 1989;64:1103-1107), Percutaneous transluminal coronary angioplasty (PTCA) is a procedure for treating narrowed coronary arteries, which are the major blood vessels that bring blood to the heart. A catheter or tube-like structure with a small inflatable balloon is inserted into the narrowed portion of the artery, and the balloon is inflated, thereby mechanically dilating the narrowed coronary artery. In PTCA, inflation of the balloon causes transient occlusion, or blockage, of the coronary artery. This leads to regional ischemia, or insufficient blood flow to specific portions of the heart. Medications can be given before PTCA to reduce or prevent regional ischemia. The anti-ischemic effect of the drug diltiazem was assessed in 42 patients who were scheduled to undergo PTCA; half the patients received pretreatment with diltiazem while the other half received an inert placebo. During the PTCA procedure, chest pain and changes in the electrocardiogram, a recording of the electrical activity of the heart, were used as indicators of ischemia. Pretreatment with diltiazem delayed the onset of pain and ECG changes associated with ischemia, and both of these indicators returned to normal after deflation of the balloon more quickly with diltiazem than without. The results show that intravenous diltiazem delays the onset of myocardial ischemia during repetitive and brief blockage of the coronary arteries. Diltiazem may exert its effects by improving blood flow beyond the blocked portion of the coronary artery, or by blocking the entry of calcium into the ischemic cell; diltiazem is a calcium blocking agent. Thus, diltiazem may be a useful medication for delaying the onset of ischemia in patients undergoing PTCA; it may be particularly helpful if longer balloon inflation periods (which may prove to be more effective than briefer periods) are utilized in the future. Abstract

AimsLiterature suggests a beneficial effect of percutaneous left atrial appendage occlusion (LAAO) to prevent thrombo-embolic events in patients with non-rheumatic atrial fibrillation (AF). We compared outcome of LAAO versus ‘suboptimal standard’ treatment in AF patients with high bleeding risk.Methods and resultsPatients with sufficient follow-up data (n = 125) who underwent LAAO with the Amplatzer Cardiac Plug (ACP) were selected from the Belgian ACP database. AF patients who survived intracranial haemorrhage were recruited from the Leuven Neurosurgical Registry (LNR, n = 113). After propensity score adjustment, the outcome of both groups was compared for the combined end point (death, stroke, transient ischaemic attack, systemic emboli, and major bleeding event). The LAAO group did not differ from the LNR group for mean age and gender (74 ± 7 versus 75 ± 10 years, P= 0.29; female 39% versus 48%, P= 0.18). However, the CHA2DS2-VASc and HAS-BLED scores were both higher in the LAAO group (4.8 ± 1.7 versus 3.9 ± 1.7, P= 0.0001; 3.5 ± 1.4 versus 3.2 ± 1.4, P= 0.036). After propensity score adjustment, the risk for the primary end point was significantly higher for the LNR group (HR 2.012, 95% CI 1.113-3.638).ConclusionLAAO with ACP seems to improve the combination of survival and the prevention of thrombo-embolic and major bleeding events in patients with atrial fibrillation and increased bleeding risk.

12. Transcatheter left atrial appendage closure for stroke prevention in atrial fi brillation with Amplatzer cardiac plug: the Belgian Registry

Abstract

AimsThe aim of the present study was to evaluate the procedural feasibility, the safety and the 1-year outcome following left atrial appendage (LAA) closure using the Amplatzer cardiac plug (ACP) in Belgium.Methods and resultsData were prospectively collected among 90 consecutive patients, undergoing LAA closure with an ACP in 7 Belgian centres between June 2009 and September 2012. The patients (56 males, 74 ± 8 years) were at high risk for stroke (CHA2DS2-VASc = 4.4 ± 1.8) and bleeding (HAS-BLED = 3.3 ± 1.3). Technical success was obtained in all but one patient and procedural success was 95%. Procedural major adverse events (MAE) were 3 tamponades resulting in death in one case. Minor complications were 3 insignifi cant pericardial eff usions, 2 transient myocardial ischaemia due to air embolism and 1 femoral pseudoaneurysm. At 1-y follow-up, there were 4 deaths, 2 minor strokes, 1 tamponade and 1 myocardial infarction. Overall survival was 94% and freedom from MAE was 88%. In our population, the expected annual stroke risk according to the CHA2DS2-VASc score was 5.08%, while the observed stroke rate was 2.14%/year.ConclusionsThe Belgian registry shows that LAA closure using the ACP device is feasible and safe. At 1-y follow-up, the observed stroke rate was 2.14%/year, less than predicted by the CHA2DS2-VASc score. Longer follow-up is needed to evaluate the long-term safety and its effi cacy in reducing stroke.

Abstract

ObjectivesThe long-term clinical outcome of covered stents in the percutaneous treatment of diseased saphenous vein graft (SVG) has been disappointing. The single self-expanding polytetrafl uoroethylene (PTFE)-covered Symbiot stent with a unique profi le may present advantages that translate into superior long-term clinical outcomes. This study evaluated the safety, eff ectiveness and clinical outcome of the SymbiotTM covered stent system (Boston Scientifi c, Natick, Mass.) and FilterWireTM EX (Boston Scientifi c, Natick, Mass.) versus bare metal stents (BMS) in SVG intervention.Methods and resultsBetween January 2003 and August 2005, 90 patients with degenerative SVG lesions were prospectively randomized at 6 study sites to Symbiot implantation (n = 30), BMS with FilterWire as embolic protection device (EPD, n = 30), or BMS without EPD (control group, n = 30). The primary end point was reduction in peri-procedural cardiac enzyme rise. The major secondary end points were in-hospital, 6-month and long-term target vessel failure rates, defi ned as the cumulative of death, myocardial infarction and clinically driven target lesion revascularization. There was no signifi cant reduction in median [IQR] post-procedural troponin-I rise in the Symbiot group compared to the FilterWire or the control group (0.08 [0-1.40] μg/l; 0.06 [0-0.28] μg/l; and 0.04 [0-0.31] μg/l, P= 0.58). At 7.4 ± 1.3 (mean ± SD) years, there were numerically less deaths in the Symbiot group, although this did not reach statistical signifi cance (P= 0.20). There was no signifi cant diff erence in TVF-free survival between the treatment groups (P= 0.98).ConclusionThis study failed to show a procedural or long-term clinical advantage for the Symbiot PTFE-covered stent in the treatment of degenerated SVG.
