Introduction Rosai Dorfman disease (RDD) is a rare benign histiocytic proliferative disorder of lymph node sinuses. Langerhans cell histiocytosis (LCH) is a solitary or multisystem clonal proliferation of abnormal dendritic cells (Langerhans cells) with varied presentations. The co-occurrence of these two entities is quite rare.Case description A six-year-old boy presented with multiple mass lesions in the neck since two years and a nodular lesion in right upper eyelid for the past 4 months. He was diagnosed with tubercular lymphadenitis 2 years back, and was given a course of anti-tubercular therapy (ATT) elsewhere. No improvement was seen. Fine needle aspiration cytology (FNAC) of the cervical lymph nodes revealed reactive lymphadenitis while lymph node biopsy showed features of RDD. Excision biopsy of the orbital mass showed features of both RDD and LCH. The patient was started on tablet prednisolone. Six months later, complete resolution of lymph node enlargement and remaining orbital mass was noted. Post operative contrast enhanced Magnetic Resonance Imaging of head and neck was normal.Conclusion The coexistence of RDD and LCH may be a result of divergent differentiation from a common lineage or a de novo phenotypic evolution.

Prostatitis may present with lower urinary tract symptoms (LUTS) attributable to acute and chronic bacterial infections (NIH Category I/II) or as asymptomatic inflammatory prostatitis (NIH Category IV). Patients with chronic prostatitis/chronic pelvic pain syndrome, (CP/CPPS, NIH Category III) may present with a wide range of symptoms resulting from varied etiology, however, seldom caused by fungal infections. Occasional case reports have been published on prostatitis due to Candida sp. We report a case of an elderly diabetic patient who underwent perurethral prostatic resection (TURP) for benign prostatic hyperplasia (BPH) and returned with complaints of LUTS and perineal discomfort one month later. After repeat surgery, the TURP chips on histopathology showed features of prostate hyperplasia and prostatitis with numerous hyphae and yeast forms of Candida admixed with acute and chronic inflammatory exudate. After confirmation by special stains and positive urine culture, a final diagnosis of prostatic candidiasis was made.

In today's world, the IT industry is emerging day by day; therefore, the need for storage and computing is increasing multifold. Cloud computing has transformed the IT sector to much greater heights by virtualizing the systems, thereby reducing cost of the hardware to greater extent. Cloud computing is based on the pay as per use policy. Due to the exponential growth in cloud computing, users demand supplementary services and improved results which makes load balancing a major challenge. Load balancing distributes the workload across multiple nodes to optimize the performance of the system. Various load balancing algorithms exist to provide better resource utilization. This paper gives a brief analysis of load balancing algorithms and also compared these algorithms on the basis of certain metrics like average response time, processing cost, and data servicing time.

Information retrieval from a large amount of information available in a database is a major issue these days. The relevant information extraction from the voluminous information available on the web is being done using various techniques like natural language processing, lexical analysis, clustering, categorisation, etc. In this paper, we have discussed the clustering methods used for clustering of large amount of data using different features to classify the data. In today's era, various problem solving techniques makes the use of a heuristic approach for designing and developing various efficient algorithms. In this paper, we have proposed a clustering technique using a heuristic function to select the centroid so that the clusters formed are as per the need of the user. The heuristic function designed in this paper is based on the conceptually similar data points so that they are grouped into accurate clusters. k-means clustering algorithm is majorly used to cluster the data which is also focussed in this paper. It has been empirically found that the clusters formed and the data points which belong to a cluster are close to human analysis as compared to existing clustering algorithms.

This article describes how cloud computing utilizes the benefits of web engineering and its applications by improving the performance and reducing the load on cloud providers. As the cloud is one of the emerging technology in the field of computing, it is used to provide various services to the user through the internet. One of the major concerns in cloud computing is accessibility of cloud. For estimating the availability of cloud, various load balancing algorithms are deployed in data centers of the cloud environment. Load balancing is a technique that distributes a signal load across various computers for optimizing resource usage, reducing response time, etc. There are different load balancing algorithms, for performing the load distribution across various centers. This article analyses different load balancing algorithms and develop a new algorithm for efficient load balancing. The proposed load balancing algorithm utilizes the concepts of web engineering to prioritize the request of end user using parsing technique, which will assign the resources to the end users based on the priority set by the data centers.

Mobile ad hoc network (MANET) is a group of self-organised nodes which behaves as a transceiver. The data will pass through intermediate nodes from a source node to the destination node using communication links. The main problem arises if the communication link is broken or going to be broken soon. A new algorithm is proposed that uses a link failure avoidance mechanism (LFAM) and a route availability check mechanism (RACM), for the development of a new secure and efficient AODV routing protocol. The goal of a LFAM mechanism is to locate a new path to the preferred destination when the connection to the next hop is going to be broken and the RACM mechanism is to find an alternate route to the destination.

Sleep is recognized as an essential component to optimize health and human performance, and as such the topic is of increasing interest to athletes. Yet, the sleep of athletes is affected by multiple sports-specific, and societal factors. Hence the recent emergence of the important role of sleep science in athlete health and performance. This review focuses on the fundamentals of sleep physiology and elaborates on the affects sleep has on athlete health and performance. Despite sports practitioners recognizing the importance of sleep on health and performance, findings report that insufficient sleep, and poor sleep quality are prevalent among athletic populations. This paper will describe sleep in athletes, define sleep characteristics, discuss the impact of sleep restriction following travel and competition, and strategies to mitigate sleep difficulties. Background. Cesarean section (CS) is one of the most performed surgeries in obstetrics. Surgical site infection is the major cause of morbidity and mortality causing an increase in the duration of hospitalization as well as the cost of admission for the patient. Objective. To determine incidence of surgical site infection following cesarean section, classify them according to CDC criteria, and identify the different risk factors. Methodology. This is a case-control study conducted at the Department of Obstetrics and Gynecology at Tribhuvan University Teaching Hospital (TUTH), main campus of Institute of Medicine (IOM), Kathmandu, Nepal. Surgical site infections (SSI) in patients who underwent cesarean sections from February 2019 to August 2019 were taken as cases, while the patients who underwent cesarean section before or after the procedure and did not develop SSI comprised the controls. Visual inspection during ward rounds, reports from laboratory, and postprocedure follow-ups for up to 30 days formed the basis of identifying infections on the patients. Risk factors were identified by bivariate and multivariate logistic regression. Results. Out of 1135 cases of cesarean sections, 97 of them developed SSI with incidence rate of 8.54%. Among them, 94.85% were superficial incisional and 5.15% were deep incisional type of SSI with no organ space type. Cases had higher mean age 26.88±4.38 years compared to 24.81±5.08 years in controls. Host-related risk factors which led to higher odds of developing surgical site infection (SSI) were obesity with adjusted odds ratio (AOR) 15.72 (confidence interval (CI): 4.60-53.67), diabetes/hypertension in pregnancy with AOR 4.75(CI 1.69-13.32), and other medical diseases with AOR 9.38 (CI 2.89-30.46). Duration of the rupture of membrane for more than 18 hours with AOR 8.38 (CI 1.48-47.35), more than five per vaginal (PV) examination with AOR 1.93 (95% CI 1.03-3.64), and in labor status with AOR 6.52 (CI 1.17-36.38) were some procedure-related factors resulting into higher odds of infection. Conclusion. Multiple risk factors like age, obesity, medical complications during pregnancy, occurrence of labor status during cesarean section, prolonged duration of rupture of membrane for more than 18 hours, and more than five vaginal examinations before the procedure increases the chance of surgical site infection (SSI) following cesarean section.

The presence of 2 or more sleep-onset rapid eye movement periods (SOREMPs) on a Multiple Sleep Latency Test (MSLT) has been used as 1 of the criteria for the diagnosis of narcolepsy and is thought to be specific to this disorder. However, previous studies have shown the prevalence of SOREMPS in healthy volunteers and apneic patients to be higher than expected. The present study determined the prevalence of 2 or more SOREMPs in a representative sample of the population from southeast Michigan and investigated potential associations with other sleep-related variables.
