The high prevalence and burden of cervical cancer in low- and middle-income countries have spurred much research into preventing and screening for the disease. However, little research has focussed on the experience of living with the disease and undergoing treatment in South Africa. Our aim was to explore knowledge, misinformation, stigma, and disclosure hesitancy among women receiving curative treatment for cervical cancer at a tertiary hospital in South Africa. We conducted semistructured interviews with 15 women who completed curative treatment for cervical cancer at an academic hospital in South Africa. Interviews were audio-recorded, transcribed, and analysed using thematic analysis. In this article, we describe participants’ knowledge and understanding of cervical cancer, their experience of misinformation and stigma and hesitancy to disclose their illness to others. Participants reported little knowledge about cervical cancer. They reported many sources of misinformation, such as family members and even medical professionals, which at times contributed to delays in seeking diagnosis and treatment. Furthermore, participants reported feeling stigma associated with their diagnosis. We highlight the central role that communication can play in increasing knowledge, reducing stigma and misinformation, and facilitating disclosure among women with cervical cancer. We include recommendations for health care practitioners and researchers.

Background HPV-related vulvar cancer is increasing in prevalence, especially in women living with HIV. Treatment of vulva cancer is based on evidence from HPV-independent cancers, which affect older women. The impact of HIV on vulvar cancer characteristics and treatment outcomes needs to be elucidated.Patients and Methods A retrospective observational study compared the clinical characteristics, treatment, and outcomes of 92 HIV-positive and 131 HIV-negative women with vulvar cancer at our institution. Using descriptive statistics, HIV-positive and negative patients were compared and Cox regression models were tested for differences in mortality and recurrence.Results HIV-positive patients were 20 years younger than HIV-negative patients (p< 0.001). More than 50% of patients presented with advanced stage cancer, however this was independent of HIV-status. Although HIV infection was associated with poorer survival (p= 0.022); rates of cure (p= 0.933) and recurrence rates (p= 0.8) were similar in HIV-positive and negative women.Conclusions Vulvar cancer occurs at a much younger age in women living with HIV. Awareness among HIV-positive women and health care providers would lead to diagnosis of vulvar cancer at an earlier stage. Treatment protocols for HPV-related vulvar cancer should not be altered due to HIV status and should take into consideration the young age of the patients.

Introduction:Locally advanced stages of invasive cervical cancer (ICC) are associated with poor outcomes; factors influencing survival include increased tumour volume. In resource-constrained settings access to diagnostic imaging with CT and MRI is limited. Alternative methods of establishing tumour volume can be defined with use of the planning target volume (PTV) delineated prior to radiotherapy. The aim of this study is to determine whether increased PTV size impacted on overall survival in a cohort of cervical cancer patients with Stage IIB/IIIB disease who completed radical radiotherapy.Materials and methods: A retrospective analysis was undertaken of patients with histologically confirmed Stage IIB/IIIB ICC treated with radical radiotherapy. Exclusion criteria included patients who did not complete prescribed radiotherapy and brachytherapy. Demographic and treatment details were collected. Planning target volumes were retrieved. Kaplan–Meier analysis was used to calculate the overall survival rate. A multivariate Cox proportional hazard model was derived to assess associations with all-cause mortality.Results:A total of 71 patients met the inclusion/exclusion criteria. The median PTV was 653 cc. On univariate analysis factors significantly associated with a lower overall survival included HIV positivity and the presence of hydronephrosis. Increased PTV size paradoxically showed a trend to improved overall survival. On multivariant analysis HIV status, advanced stage, hydronephrosis and a smaller PTV were significantly related to higher all-cause mortality.Conclusion:It is concluded that, when using planning target volumes, the hypothesis that larger volumes impact on overall survival was disproved. A larger cohort and more accurate methods of determining tumour volume, including PET/CT, will be considered in future prospective studies.

The use of chemotherapy and endocrine therapies as adjuncts to the treatment of early-stage breast cancer has yielded small but significant improvements in disease-free and overall survival. Increased understanding of the role of growth factor receptors enabled the rational development of agents that are capable of modulating their function. A humanised monoclonal antibody to the HER2 receptor, trastuzumab, has demonstrable single-agent activity in metastatic breast cancer and enhances the antitumour effects of chemotherapy. As a consequence, trastuzumab has been tested in the adjuvant setting the results of which have been presented recently. This review briefly summarises the use of trastuzumab in advanced breast cancer and describes recent studies of its use in the adjuvant setting.
