Objective: To determine the distinct influences of rural background and rural residency training on rural practice choice among family physicians. Data Sources and Study Setting: We used a subset of The RTT Collaborative rural residency list and longitudinal data on family physicians from the American Board of Family Medicine National Graduate Survey (NGS; three cohorts, 2016-2018) and American Medical College Application Service (AMCAS). Study Design: We conducted a logistic regression, computing predictive marginals to assess associations of background and residency location with physician practice location 3 years post-residency. Data Collection/Extraction Methods: We merged NGS data with residency type-rural or urban-and practice location with AMCAS data on rural background. Principal Findings: Family physicians from a rural background were more likely to choose rural practice (39.2%, 95% CI = 35.8, 42.5) than those from an urban background (13.8%, 95% CI = 12.5,15.0); 50.9% (95% CI = 43.0, 58.8) of trainees in rural residencies chose rural practice, compared with 18.0% (95% CI = 16.8,19.2) of urban trainees. Conclusions: Increasing rural programs for training residents from both rural and urban backgrounds, as well as recruiting more rural students to medical education, could increase the number of rural family physicians. Objective. The purpose of this study was to examine the association between frailty and the quantity and quality of freeliving walking and the mediating effect of frailty on the relationship between disability and walking performance in people with multiple sclerosis (MS). Methods. Ninety-nine people with relapsing-remitting MS (mean age = 49.3 [SD = 9.8] years; 73.7% women; Expanded Disability Status Scale [EDSS] score range = 2.0-6.0) wore a triaxial accelerometer for 7 days. Recorded measures reflected the quantity (daily step counts, number of 30-second walking bouts, and signal vector magnitude [SVM]) and quality (gait speed, step cadence, step and stride regularity, and sample entropy) of walking. For each walking quality measure, the typical (median), best (90th percentile), and worst (10th percentile) values were calculated. Frailty was evaluated through a 38-item frailty index. Results. Participants were classified as not frail (n = 31), moderately frail (n = 34), and severely frail (n = 34) on the basis of established procedures. Patients who were moderately and severely frail exhibited poorer performance in all measures of walking quantity and quality, except for sample entropy, than individuals who were not frail. No differences in freeliving walking performance were observed between the moderately and severely frail groups. Frailty did not mediate the relationship between disability (EDSS) and measures of walking quality. Conversely, frailty had a significant mediating effect on the relationship between disability and measures of walking quantity, such as daily step counts (indirect effect: b =-220.42, 95% CI = -452.03 to -19.65) and SVM (indirect effect: b =-1.00, 95% CI = -1.86 to -0.30). Conclusion. Frailty is associated with poorer free-living walking performance in people with MS. The study findings suggest that frailty, rather than disability, may be primarily responsible for the lower amount of physical activity performed by people with MS in the real world. Impact. The observation that frailty and disability are differently related to measures of walking quality and quantity underscores the importance of a targeted approach to rehabilitation in people with MS. Lipidomic studies are often conducted using shotgun mass spectrometry (MS) or reversed-phase liquid chromatography coupled with MS (LC-MS). However, chromatographic separation offers several advantages such as an additional identification parameter (retention time), lower ion suppression, and separation of isobaric species. In contrast, quantification is more difficult because ion suppression is not the same over the whole analysis, and as a consequence more standards are needed to compensate for this. Supercritical fluid chromatography (SFC) offers orthogonal separation compared to reversed-phase LC. While the separation of lipids in reversed-phase LC is mainly based on the length of the carbon chain and the number of double bonds, lipids in SFC are mainly separated according to their lipid classes, which simplifies quantification with standards. This article introduces a new high step-up nonisolated dc–dc converter with continuous input current and minimal ripples. It is based on the conventional Landsman converter while incorporating coupled inductor and voltage multiplier techniques. By differentially connecting the load between the input and output capacitors, the converter's voltage gain is further enhanced. The converter also operates in a quasi-resonant mode (QRM), ensuring zero-current switching (ZCS) turn-on and turn-off for diodes, while the current ripple of the magnetic devices aids zero-voltage switching (ZVS) turn-on of active switches. This design yields reduced voltage stresses on the active switches and enables the usage of compact capacitors and magnetic devices, improving power density. To validate its performance, a prototype circuit was constructed with input voltage range of 35–45 V, output voltage of 400 V, and output power of 150 W. The efficiency tests indicate that the highest efficiency obtained is 97.6%, with weighted efficiencies exceeding 96.5% across the entire input voltage range. The analysis of the converter and a comparison with other similar topologies are also presented.

•Analysis of 201 patients with R/R THRLBCL vs 5453 with DLBCL undergoing auto-HCT reveals superior PFS, OS, and relapse incidence for THRLBCL.•Auto-HCT for R/R THRLBCL resulted in a 2-year PFS of 78% and 59% in DLBCL, making it attractive for consolidating chemosensitive disease.

Weak nonspecific interactions between biomacromolecules determine the cytoplasmic organization. Despite their importance, it is challenging to determine these interactions in the intracellular dense and heterogeneous mixture of biomacromolecules. Here, we develop a method to indicate electrostatic and hydrophobic associative interactions and map these interactions. The method relies on a genetically encoded probe containing a sensing peptide and a circularly permuted green fluorescent protein that provides a ratiometric readout. Inside bacterial and mammalian cells, we see that the cytoplasmic components interact strongly with cationic and hydrophobic probes but not with neutral hydrophilic probes, which remain inert. The Escherichia colicytoplasm interacts strongly with highly negatively charged hydrophilic probes, but the HEK293T cytoplasm does not. These associative interactions are modulated by ATP depletion. Hence, the nonspecific associative interaction profile in cells is condition- and species-dependent.

The gut microbiome plays a vital role in the overall health of dogs and cats, particularly their gastrointestinal (GI) health. Imbalances in the gut microbiome, termed dysbiosis, are associated with a range of GI disorders, including diarrhoea and chronic enteropathy. Emerging evidence tentatively supports the therapeutic use of pre‐, pro‐ and postbiotics (collectively called biotics) to restore gut homeostasis and manage these conditions, especially due to their potential antibiotic‐sparing effects. This article explores their evidence‐based use in the treatment of GI disorders in dogs and cats.

Purpose: Children with neuromuscular scoliosis (NMS) resultant to cerebral palsy (CP) are at a heightened risk for complications following surgical treatment. These children have a reported 22–64% rate of post-operative fever development, and additional fever workup has been shown to have limited clinical utility. However, this has yet to be investigated in the setting of an accelerated discharge (AD) pathway. Methods: A retrospective review of children with non-ambulatory CP treated at 2 centers with posterior spinal fusion (PSF) for NMS was performed. One institution uses a standardized AD post-operative pathway for NMS patients, whereas the second institution had no standard pathway. A post-operative fever was defined as temperature > 38.5 °C. Target outcome variables included the development of a fever as well as re-admission within 90 days of surgery. Results: A total of 122 non-ambulatory children were identified (82% GMFCS V, mean 14.3 ± 3.4 years at surgery). A post-operative fever was documented in 75.4% of patients (N= 92) and all additional culture studies reported negative results. Children admitted to the PICU were more likely to undergo a fever workup (P< 0.001) and more likely to receive additional or extended antibiotic therapy (P< 0.001). Children treated at the AD pathway had a significantly lower rate of PICU admission (P< 0.001). Post-operative PICU admission was associated with a post-operative fever (49.5% vs 25%, P= 0.03). Conclusion: Non-ambulatory CP children with NMS undergoing PSF have a 75.4% rate of developing early post-operative fevers. Reflexive fever work-ups provided limited clinical utility while increasing the hospital length of stay and potentially exposing patients to antibiotic-related complications.

The Companion Animal Fecal Microbiota Transplantation (FMT) Consortium is an international group of veterinary experts that are currently performing FMT in dogs and cats. Based on available evidence and expert opinions, the Companion Animal FMT Consortium developed the first clinical guidelines for FMT in companion animals aimed at increasing the accessibility of this microbial-directed therapeutic in veterinary medicine. These clinical guidelines include recommendations and protocols for fecal donor screening, FMT product processing and preparation, and current FMT clinical indications and administration. These clinical guidelines are intended to be utilized by veterinarians in all practice types.

The capacity of roundabout entries and exits can be significantly influenced by crossing pedestrians. To analyze this effect, an extensive microscopic simulation calibrated with empirical findings from field measurements at five single-lane roundabouts in Germany was conducted. The results revealed that the entry capacity adjustment factors used in the U.S. Highway Capacity Manualand the German Highway Capacity Manualdo not sufficiently account for the capacity reduction resulting from crossing pedestrians. The results confirmed neither the convex shape of the adjustment factor functions nor the total extent of the capacity reduction. Thus, a new model was derived using gap acceptance theory. The new model accurately estimated the capacity reduction from conflicting pedestrians. The model used the entry capacity function as an input parameter and is therefore applicable for any type of roundabout. Furthermore, local boundary conditions like the blockage time of a single pedestrian, the grouping of pedestrians, and yielding behavior were considered as individual input parameters and could be modified by field measurements if needed. For all parameters, reasonable defaults were given. The new model has been derived in a way that it can be directly incorporated into existing quality-of-service assessment procedures to determine appropriate entry capacity adjustment factors. Moreover, the model could be extended to consider crossing bicycles simultaneously and is also valid for calculating the influence on the exit capacity.
