Distinctive [...], The False Claims Act (FCA) authorizes private parties known as relators to bring qui tam actions on behalf of the United States against entities alleged to be presenting false or fraudulent claims to the government for payment. The article addresses a scenario in which a new party is injected into a qui tam suit: a third-party litigation funder, who provides funding to the relator in exchange for part of the recovery. Third-party litigation funding (TPLF) arrangements are controversial in any context, as they create the risk that a financier will wrest control over the litigation away from the parties and gain access to privileged information. These concerns are heightened in the qui tam setting, where the relator brings suit in the name of the United States and where attorneys from the Department of Justice (DoJ) serve a key function. This article argues that if TPLF is to play a role in qui tam litigation, then the DoJ should not operate in the dark. For the government to carry out its statutorily required function in qui tam actions, and for the relator's role in qui tam actions to be constitutional, the government's attorneys at the DoJ must have visibility into TPLF of a relator's claim. Thus, DoJ has the authority to mandate disclosure of TPLF arrangements to the DoJ. Such a mandate is the minimum required to maintain the proper statutory function of qui tam suits in light of the risks that TPLF poses.

7. “We’re Not Gonna Talk about This, It Didn’t Happen. You’re Confused”: Adverse Communication in Family Responses to Mental Health, Childhood Sexual Assault, and LGBTQ Identities

- Abstract
Lesbian, gay, bisexual, transgender, and queer (LGBTQ) people disproportionately report high exposure to adverse childhood experiences (ACEs). In this study, we examine the ways that LGBTQ people with high ACEs also describe experiencing adverse communicationwith family systems about mental health, childhood sexual abuse (CSA), and their gender identity. From interviews with a racially diverse sample of 82 LGBTQ people in South Texas, we analyze how this adverse communication—including gaslighting, silence, denial and ignoring—is attentive to courtesy stigma dynamics. This adverse communication impacts transgender people, cisgender LGBTQ people, and Black or Latinx LGBTQ people differently; for example, Black and Latinx LGBTQ people discussed adverse communication about mental health and therapy within their families that prioritized the respectability of the family. These findings provide insight into family dynamics and communication practices in the lives of LGBTQ people, particularly at the intersections of multiple marginalized identities.

8. Gaps in the Diagnosis and Treatment of Drug-resistant Tuberculosis in Mexico

- Abstract
Objective: The study aimed to determine the critical gaps in the care of drug-resistant tuberculosis to implement strategies for its elimination.Material and Methods: Cases with various resistance profiles that were presented to the National Advisory Group for Drug Resistance during the years 2019-2022 have been described.Results and Discussion: 104 cases were included in this study. 41% of the patients had diabetes (41%) as a comorbidity. There have been observed delays in the reporting of rapid molecular tests; with the Xpert MTB/RIF, delays of 6.33 days have been reported. There have also been observed delays in the request for phenotypic drug susceptibility tests; from the time of initial diagnosis, patients have been found to receive their first drug susceptibility test, on average, after 20 months. The bacteriological follow-up during the treatment with monthly culture has been found to only be carried out in 8% of the cases. In the contact investigation, only 16% of the listed contacts had been studied for infection or active disease.Conclusion: The elimination of these gaps requires the decentralization of susceptibility tests for fluoroquinolones, shortening the times between the diagnosis of drug resistance and the start of treatment, a close clinical and bacteriological follow-up, and an exhaustive investigation of contacts.

Backgreund Cancer stem cells are presumed to have virtually unlimited proliferative and self-renewal abilities and to be highly resistant to chemotherapy, a feature that is associated with overexpression of ATP-binding cassette transporters. We investigated whether prolonged continuous selection of cells for drug resistance enriches cultures for cancer stem-like cells. Methods Cancer stem cells were defined as CD44+/CD24- cells that could self-renew (ie, generate cells with the tumorigenic CD44+/CD24- phenotype), differentiate, invade, and form tumors in vivo. We used doxorubicin-selected MCF-7/ADR cells, weakly tumorigenic parental MCF-7 cells, and MCF-7/MDR, an MCF-7 subline with forced expression of ABCB1 protein. Cells were examined for cell surface markers and side-population fractions by microarray and flow cytometry, with in vitro invasion assays, and for ability to form mammospheres. Xenograft tumors were generated in mice to examine tumorigenicity (n = 52). The mRNA expression of multidrug resistance genes was examined in putative cancer stem cells and pathway analysis of statistically significantly differentially expressed genes was performed. All statistical tests were two-sided. Results Pathway analysis showed that MCF-7/ADR cells express mRNAs from ABCB1 and other genes also found in breast cancer stem cells (eg, CD44, TGFB1, and SNAI1). MCF-7/ADR cells were highly invasive, formed mammospheres, and were tumorigenic in mice. In contrast to parental MCF-7 cells, more than 30% of MCF-7/ADR cells had a CD44+/CD24- phenotype, could self-renew, and differentiate (ie, produce CD44+/CD24- and CD44+/ CD24+ cells) and overexpressed various multidrug resistance-linked genes (including ABCB1, CCNE1, and MMP9). MCF-7/ADR cells were statistically significantly more invasive in Matrigel than parental MCF-7 cells (MCF-7 cells = 0.82 cell per field and MCF-7/ADR = 7.51 cells per field, difference = 6.69 cells per field, 95% confidence interval = 4.82 to 8.55 cells per field, P < .001). No enrichment in the CD44+/CD24- or CD133+ population was detected in MCF-7/MDR. Conclusion The cell population with cancer stem cell characteristics increased after prolonged continuous selection for doxorubicin resistance. DOI: 10.1093/jnci/djq361

22. Acute cerebrovascular events in severe and nonsevere COVID-19 patients: a systematic review and meta-analysis

- Abstract
The degree to which COVID-19 severity influences the development of acute cerebrovascular events (ACVE) is unknown. Therefore, we aimed to describe the prevalence and risk of ACVE in patients with severe and nonsevere COVID-19. We systematically reviewed MEDLINE, EMBASE, Web of Science, and Scopus and identified observational and interventional studies of patients with COVID-19 allocated by respiratory severity that reported ACVE development. Case reports/series were excluded. The main outcome assessed was the pooled rate of ACVE in patients with severe and nonsevere COVID-19. To determine the risk of ACVE development by COVID-19 severity, a meta-analysis was performed. PROSPERO registration number: CRD42020178905. About 19 of 5758 identified studies were analyzed. From 11,886 COVID-19 patients analyzed, 421 had at least one ACVE [3.6%, 95% confidence interval (CI) 2.904–4.179]. Severe COVID-19 increased the risk of ACVE (odds ratio 1.96, 95% CI 1.22–3.15; P = 0.005; I2= 64%), specifically hemorrhagic stroke (4.12, 2.0–8.53; P = 0.001; I2= 0%). There was no difference in the risk of developing ischemic stroke between patients with severe and nonsevere COVID-19 (1.53, 0.87–2.7; P = 0.14; I2= 52%). From the patients who developed any ACVE, those with severe COVID-19 had a greater mortality risk than those with nonsevere COVID-19 (3.85, 1.08–13.70; P = 0.04; I2= 0%). The main limitations of our study were the heterogeneity found in the main meta-analysis studies and in their reported definition for COVID-19 severity. In conclusion, our findings provide evidence that COVID-19 respiratory severity could lead to ACVE development that increases mortality. The effect of COVID-19 management in ACVE needs to be evaluated.

- Abstract
The draft Post-2020 Global Biodiversity Framework commits to achievement of equity and justice outcomes and represents a “relational turn” in how we understand inclusive conservation. Although “inclusivity” is drawn on as a means to engage diverse stakeholders, widening the framing of inclusivity can create new tensions with regard to how to manage protected areas. We first offer a set of tensions that emerge in the light of the relational turn in biodiversity conservation. Drawing on global case examples applying multiple methods of inclusive conservation, we then demonstrate that, by actively engaging in the interdependent phases of recognizing hybridity, enabling conditions for reflexivity and partnership building, tensions can not only be acknowledged but softened and, in some cases, reframed when managing for biodiversity, equity, and justice goals. The results can improve stakeholder engagement in protected area management, ultimately supporting better implementation of global biodiversity targets.

- Abstract
With recent advances and success in several drugs designed to treat acute and chronic diseases, targeted covalent inhibitors show a resurgence in drug discovery. As covalent inhibition is time-dependent, the preferred quantitative potency metric of irreversible inhibitors is the second-order rate constant kinact/Ki, rather than IC50. Here, we present the development of a mass spectrometry-based platform for rapid kinetic analysis of irreversible covalent inhibitors. Using a simple liquid handling robot for automated sample preparation and a solid-phase extraction-based RapidFire–MS system for rapid MS analysis, kinetic characterization of covalent inhibitors was performed in high throughput both by intact protein analysis and targeted multiple reaction monitoring (MRM). In addition, a bimolecular reaction model was applied to extract kinact/Kiin data fitting, providing tremendous flexibility in the experimental design to characterize covalent inhibitors with various properties. Using KRASG12Cinhibitors as a test case, the platform was demonstrated to be effective for studying covalent inhibitors with a wide range of kinact/Kivalues from single digit to 3 × 105M–1s–1.

The COVID-19 pandemic and associated need for social isolation left in-person youth mentoring programs scrambling to keep mentees and mentors connected, and many programs turned to e-mentoring. To better understand the transition period and to inform e-mentoring practice in a post-COVID world, this study explored the experience of mentoring programs shifting to e-mentoring during the first year of the pandemic. Seven remote focus group discussions were conducted with twenty-three staff members from twenty U.S. youth mentoring organizations that used the iCouldBe e-mentoring platform during Spring/summer 2020 or Fall/Winter 2020–2021. Thematic content analysis was used to uncover insights from the data. E-mentoring was successful overall for keeping mentees and mentors in touch, especially for matches with a strong connection before the pandemic. Zoom and text messaging were the most used virtual communication methods. Programs faced many challenges but also experienced unexpected positives, including a strong interest in future e-mentoring implementation. Participants recommended that programs interested in e-mentoring start small and with intention; they also requested a central website with e-mentoring support and ways to connect with other programs and mentors. Although the literature on e-mentoring remains limited, this study contributes a picture of e-mentoring success even during a global crisis.

- Abstract
AbstractThe families of people of Color and indigenous people (POCI) are often analyzed as hostile institutions for sexual and gender diverse (SGD) adults. Using 58 interviews with SGD POCI from the Strengthening Colors of Pride 2018 Interview Study, we argue that Black and Latinx SGD adults gain resilience from family role models, mostly resilient mothers and othermothers. These resilient family members model three things: 1) how to overcome adversity and trauma, 2) being providers, and 3) emotional strength. This resilient modeling facilitates the adult resilience of SGD POCI who are navigating the complexity of intersections of race, sexuality, gender, poverty, and childhood trauma.

- Abstract
Purpose: To examine the proportion of diabetes-focused clinical encounters in primary care and endocrinology practices where the evaluation for hypoglycemia is documented; and when it is, identify clinicians’ stated actions in response to patient-reported events. Methods: A total of 470 diabetes-focused encounters among 283 patients nonpregnant adults (≥18 years) with type 1 or type 2 diabetes mellitus in this retrospective cohort study. Participants were randomly identified in blocks of treatment strategy and care location (95 and 52 primary care encounters among hypoglycemia-prone medications (i.e. insulin, sulfonylurea) and others patients, respectively; 94 and 42 endocrinology encounters among hypo-treated and others, respectively). Documentation of hypoglycemia and subsequent management plan in the electronic health record were evaluated. Results: Overall, 132 (46.6%) patients had documentation of hypoglycemia assessment, significantly more prevalent among hypo-treated patients seen in endocrinology than in primary care (72.3% vs. 47.4%; P= 0.001). Hypoglycemia was identified by patient in 38.2% of encounters. Odds of hypoglycemia assessment documentation was highest among the hypo-treated (OR 13.6; 95% CI 5.5−33.74, vs. others) and patients seen in endocrine clinic (OR 4.48; 95% CI 2.3−8.6, vs. primary care). After documentation of hypoglycemia, treatment was modified in 30% primary care and 46% endocrine clinic encounters; P= 0.31. Few patients were referred to diabetes self-management education and support (DSMES). Conclusions: Continued efforts to improve hypoglycemia evaluation, documentation, and management are needed, particularly in primary care. This includes not only screening at-risk patients for hypoglycemia, but also modifying their treatment regimens and/or leveraging DSMES.

- Abstract
Cancer cells often use alternative nutrient sources to support their metabolism and proliferation. One important alternative nutrient source for many cancers is acetate. Acetate is metabolized into acetyl-CoA by acetyl-CoA synthetases 1 and 2 (ACSS1 and ACSS2), which are found in the mitochondria and cytosol, respectively. We show that ACSS1 and ACSS2 are differentially expressed in cancer. Melanoma, breast cancer, and acute myeloid leukemia cells expressing ACSS1 readily use acetate for acetyl-CoA biosynthesis and to fuel mitochondrial metabolism. ACSS1-dependent acetate metabolism decreases the relative contributions of glucose and glutamine to the TCA cycle and alters the pentose phosphate pathway and redox state of cancer cells. ACSS1 knockdown decreases acute myeloid leukemia burden in vivo and inhibits melanoma tumor and metastatic growth. Our study highlights a key role for ACSS1-dependent acetate metabolism for cancer growth, raising the potential for ACSS1-targeting therapies in cancer.

50. Transformative potential of nature-based values that influence the relationships between reported and intended pro-environmental behavior

- Abstract
Protected area landscapes embody multiple values of nature that can create meaning in everyday life. Though the values ascribed to these environments theoretically inspire changes in human behavior, surprisingly few studies have empirically evaluated how ‘specific values’ affect actions that benefit the environment. We used Public Participation in Geographic Information Systems (PPGIS) methods to evaluate the relationships among four nature-based values and the patterns of both reported and intended behavior among visitors to Denali National Park and Preserve, Alaska, USA (n = 667). We found that wilderness, recreation, ecological integrity, and scientific qualities of places were particularly important for characterizing the Denali landscape and accounted for more variation in intended than reported pro-environmental behaviors. We provide new insights on how nature-based values underpin the decisions of visitors and lead to transformative changes after experiencing a high profile, charismatic protected area. Understanding the reasons why people forge connections with natural areas and modeling how these associations relate to different types of behavior advances knowledge of how to effectively build environmental stewardship and guide public land management decisions.
