Zusammenfassung: Die toxisch epidermale Nekrolyse (TEN) ist eine Erkrankung mit geringer Inzidenz, aber vergleichsweise hoher Letalität. Die vitale Gefährdung geht in erster Linie von septischen Komplikationen aus. Ausgeprägte Schädigungen der Schleimhäute stellen eine zusätzliche Bedrohung dar und können die Rekonvaleszenz verzögern. Wir berichten über eine TEN bei einem 51-jährigen Mann, welche die gesamte Körperoberfläche erfasste und 3 Wochen nach Carbamazepineinnahme auftrat. Die langwierige und aufwendige Intensivbehandlung konzentrierte sich auf die Unterstützung sepsisassoziierter wie auch primärer Organinsuffizienzen sowie die Wundbehandlung. Ungewöhnlich waren die ausgedehnte Beteiligung des Intestinaltraktes, das Auftreten einer subglottischen Trachealstenose und die Ausbildung einer Sattelnase. Die Versorgung derartig schwerer Krankheitsverläufe sollte in Einrichtungen erfolgen, die in der Lage sind, eine adäquate intensivmedizinische Betreuung zu gewährleisten und gleichzeitig ausgedehnte Schädigungen der Haut fachgerecht zu behandeln.

Zusammenfassung.: Einleitung: Rekombinantes Wachstumshormon (rGH) wurde bei verbrannten Kindern bereits erfolgreich eingesetzt. Bei Kindern wurde eine beschleunigte Spenderheilung, sowie ein verkrzter Krankenhausaufenthalt beschrieben, ebenso ein proteinsparender Effekt. Bei erwachsenen Patienten existiert keine kontrollierte Studie, die dies besttigt. Material und Methoden: Diese Studie wurde an 49 Erwachsenen zwischen 18 und 60 Jahren als eine randomisierte, placebo-kontrollierte, Doppelblindstudie durchgefhrt, mit einem durchschnittlichen Abbreviated Burn Severity Index (ABSI) von 7–11. Der Behandlungszeitraum war 28 Tage, der Nachuntersuchungszeitraum 1 Jahr. rGH wurde subcutan verabreicht in einer Dosierung von 0,5 IU/kgKG bei 26 Patienten. 23 Patienten erhielten ein Placebo. Die Wundheilungsbeurteilung wurde am Tag der Aufnahme, sowie bei jedem Verbandswechsel durchgefhrt. Ein „Wound Closure Index (WCI)“ wurde errechnet. Ergebnisse: 37 Patienten, 19 in der rGH Gruppe und 18 in der Placebogruppe, berlebten und konnten bezglich der Wundheilung ausgewertet werden. Die verbrannte Krperoberflche berug 41,5 % in der rGH- und 36,7 % in der Placebogruppe. Der durchschnittliche ABSI-Score betrug 8,27 (rGH) und 7,9 (Placebo). Der WCI zeigte keinen signifikanten Unterschied zwischen den beiden Gruppen (rGH: 1,92; Placebo 1,72). Der WCI fr zweitgradige Verbrennungen zeigte ebenfalls keinen signifikanten Unterschied (rGH: 0,9; Placebo: 0,69). Die Spenderheilung war in der mit rGH therapierten Gruppe mit durchschnittlich 12 Tagen im Vergleich zur Placebogruppe (10,4 Tage) nicht signifikant verndert. Schlufolgerung: Bei schwerverbrannten Erwachsenen hat rGH keinen positiven Effekt auf die Wundheilung und die Heilung der Spenderstellen.

Abstract: Functional results after deep excision and split thickness skin grafts are often limited as a consequence of unstable grafted areas and contractures. In two patients with a full-thickness thermal injury to the lower extremities and one patient with a chronic unstable skin area over the knee, the IntegraTM bilayered membrane dermal substitute was applied. After uneventful dressing changes graft take was complete without infection or other complications. Complete wound closure was achieved in all three patients within 3–4 weeks after the initial operation. The functional range of motion of the involved joints and the skin quality and contour was superior to conventional grafted skin after excision down to fascia and rather more comparable to skin grafts over a tangentially excised eschar. After wound healing, the neodermis was histologically similar to normal dermis. Considering the high incidence of unstable skin and contractures after regular grafting of deeply excised burns, this concept may present a significant improvement not only for the primary but also for secondary reconstructive procedures, with respect to the long-term quality of life for burn patients.

The first clinical studies with the application of growth hormone (GH) under postoperative and post-traumatic conditions date back to the early 1960 s. The introduction of recombinant human growth hormone made the drug generally available for clinical studies. Administration of GH after elective general surgery improves nitrogen balance. Interruption of the post-traumatic hypermetabolism has been occasionally described, but most studies only demonstrate a slight reduction of the catabolic response. An increasing number of papers report positive influence of GH on wound healing, especially skin graft donor sites. The impact on the immune system has yet to be clarified. This paper gives an comprehensive overview of the current state of knowledge of the clinical effect of GH in various surgical specialities with special respect to septic conditions, burns, trauma, and wound healing. Die ersten klinischen Studien zur Wirkung des Wachstumshormons (GH) in der postoperativen und der Postaggressionsphase reichen in die frühen 60 er Jahre zurück. Ziel der Gabe von GH war und ist es, die hypermetabole Reaktion des Körpers abzumildern und gleichzeitig die anabolen Prozesse zu stimulieren. Mit der Einführung des humanen, rekombinanten GH steht die Substanz in breitem Umfang zur klinischen Prüfung zur Verfügung. Nach elektiven chirurgischen Eingriffen gelingt es die Stickstoffbalance positiv zu gestalten. Nach schwerem Trauma ist bisher keine Durchbrechung des Hypermetabolismus beschrieben. Allerdings mehren sich Berichte über eine günstige Auswirkung der Wachstumshormongabe auf die Wundheilung. Eine endgültige Wertung der beschriebenen, positiven Auswirkungen auf die Immunantwort läßt sich derzeit nicht geben. Die vorliegende Übersicht stellt in einer komprimierten Form den derzeitigen Erkenntnisstand zum Einsatz des GH in den unterschiedlichen chirurgischen Disziplinen unter besonderer Berücksichtigung von Immunsystem, Wundheilung und Metabolismus dar.

The second case involves a microvascular arterial revascularisation of a 1st dorsal metacarpal artery island flap. Neither procedure has previously been described.

Quantitative reverse transcription polymerase chain reaction (RT-PCR) is being used increasingly as an alternative to Northern blots analysis or RNase protection assays for quantitation of gene expression. To quantify different samples, measurements are often normalized using the expression of so-called “housekeeping” genes, such as cytoplasmic β-actin or glyceraldehyde-3-phosphate dehydrogenase. This approach can produce false results because the presence of processed pseudogenes in the genome, which are related to some of the commonly used transcripts of housekeeping genes, leads to coamplification of contaminating genomic DNA. By yielding amplification products of the same or similar size as the reverse-transcribed target, mRNA quantitation of expression is prone to error. In this paper, we report the results of using three sets of β-actin primers for RT-PCR in the presence and absence of genomic DNA. In addition, we propose two new pairs of oligonucleotide primers that specifically amplify the human and rat β-actin reverse-transcribed mRNA but not pseudogene sequences. These primers are especially suitable for quantitation of mRNA in small tissue samples (e.g., biopsies), where DNase digestion is not feasible, and therefore DNA contamination cannot be avoided.

The use of the reverse pedicle island flap as a heterodigital cross-finger flap is reported in five patients in whom conventional cross-finger flaps or homodigital flaps could not be used. All flaps survived and patient satisfaction was high. However, the indication for the flap must be considered carefully as the dissection is technically demanding and there is some donor site morbidity.

After recurrent failure of surgical debridement and wound closure under antituberculous therapy, wound closure was established by means of a radial forearm fascial flap with an excellent functional and cosmetic result. Extra-pulmonary tuberculosis must be kept in mind in the diagnosis of slowly growing tumours and chronic wounds in the upper extremity.
