The modulation of protein function via designed small molecules is providing new opportunities in chemical biology and medicinal chemistry. While drugs have traditionally been developed to block enzymatic activities through active site occupation, a growing number of strategies now aim to control protein functions in an allosteric fashion, allowing for the tuning of a target’s activation or deactivation via the modulation of the populations of conformational ensembles that underlie its function. In the context of the discovery of new active leads, it would be very useful to generate hypotheses for the functional impact of new ligands. Since the discovery and design of allosteric modulators (inhibitors/activators) is still a challenging and often serendipitous target, the development of a rapid and robust approach to predict the functional profile of a new ligand would significantly speed up candidate selection. Herein, we present different machine learning (ML) classifiers to distinguish between potential orthosteric and allosteric binders. Our approach integrates information on the chemical fingerprints of the ligands with descriptors that recapitulate ligand effects on protein functional motions. The latter are derived from molecular dynamics (MD) simulations of the target protein in complex with orthosteric or allosteric ligands. In this framework, we train and test different ML architectures, which are initially probed on the classification of orthosteric versus allosteric ligands for cyclin-dependent kinases (CDKs). The results demonstrate that different ML methods can successfully partition allosteric versus orthosteric effectors (although to different degrees). Next, we further test the models with FDA-approved CDK drugs, not included in the original dataset, as well as ligands that target other kinases, to test the range of applicability of these models outside of the domain on which they were developed. Overall, the results show that enriching the training dataset with chemical physics-based information on the protein–ligand dynamic cross-talk can significantly expand the reach and applicability of approaches for the prediction and classification of the mode of action of small molecules.

Semantically tagging a corpus is useful for many intermediate NLP tasks such as: acquisition of word argument structures in sublanguages; acquisition of syntactic disambiguation cues; terminology learning; etc. The general idea is that semantic tags allow the generalization of observed word patterns, and facilitate the discovery of recurrent sublanguage phenomena and selectional rules of various types. Yet, as opposed to POS tags in morphology, there is no consensus in the literature about the type and granularity of the semantic tags to be used. In this paper, we argue that an appropriate selection of semantic tags should be domain-dependent. We propose a method by which we select from WordNet an inventory of semantic tags that are ‘optimal’ for a given corpus, according to a scoring function defined as a linear combination of general and corpus-dependent performance factors. We believe that an optimal selection of a category inventory is a necessary premise for obtaining better results in all lexically learning algorithms that are based on, or concerned with, semantic categorization of words. Furthermore, an adequate inventory (one which intuitively ‘fits’ with the semantics of a domain, e.g. phenomenon for Natural Science, or part, piece for a technical handbook) may facilitate the manual annotation of large corpora.

ABO-group major incompatibility hematopoietic stem cell transplantation (HSCT) increases the risk of delayed red cell engraftment and other immunological complications. In this study, we evaluated the efficacy of pre-transplant infusion of rituximab in patients with ABO-incompatibility in improving red blood cell engraftment after HSCT, measured by time to reach transfusion independence. We performed a retrospective, single-center study including 131 consecutive patients transplanted with major or bidirectional ABO-incompatible grafts between 1st January 2013 and 31st December 2019. Fifty-one patients received an infusion of rituximab during the conditioning regimen, while 80 patients did not receive any additional preventive treatment. Time to transfusion independence was significantly reduced for patients treated with rituximab (1 month, 95% CI, 0.5–2) compared with the control group (3.2 months, 95% CI 1.5–3.2, p= 0.02). By multivariable analysis, rituximab use was associated with a faster red blood cell (RBC) engraftment (RR 1.88, 95% CI 1.17–3.03, p= 0.009), while a pre-transplant anti-donor isohemagglutinins titer >1:128 was associated with delayed transfusion independence (RR 0.61, 95% CI 0.37-0.99, p= 0.05). Although limited by the retrospective nature of the study, the results of this analysis suggest that rituximab added to conditioning regimens is feasible, safe, and able to improve post-transplant red blood cell engraftment.

Condition Monitoring (CM) aims at damage identification through the measurement of various data collected from the systems under investigation (civil structures, mechanical parts, or industrial machineries) such as acceleration, temperature, displacement, and dynamic properties variation. Synchronization between the measured signals is mandatory for the processing of diagnostic metrics and modal analysis, which is why most of the monitoring systems today consist of a centralized acquisition unit to which analog transducers are connected. However, this solution entails a high cost of equipment and the use of long analog cables for each transducer. A full-digital network solution has been developed to solve these problems. The acquisition network is composed of Interface Boards for interfacing with the data storage system (e.g., a personal computer) and Acquisition Nodes integrating Micro Electro-Mechanical Systems (MEMS) digital triaxial accelerometers. The Interface Boards can be connected via Universal Serial Bus (USB) or Local Area Network (LAN), while the Acquisition Nodes are connected in daisy-chain via a digital bus implemented on an Unshielded Twisted Pair (UTP) cable. The proposed solution simplifies wiring, reduces system cost, and provides accurately synchronized signals. Experimental measurements on a truss model and a motor test bench for ball bearing failures diagnostic are presented, to demonstrate the effectiveness of the solution in civil and industrial applications, compared to a traditional analog laboratory-grade system.

We study the delegation problem between a principal and an agent, who not only has better information about the performance of the available actions but also superior awareness of the set of actions that are actually feasible. We provide conditions under which the agent finds it optimal to leave the principal unaware of relevant options. By doing so, the agent increases the principal's cost of distorting the agent's choices and increases the principal's willingness to grant him higher information rents. We further show that the principal may use the option of renegotiation as a tool to implement actions that are not describable to her at the contracting stage. If the agent renegotiates, his proposal signals information about the payoff state. Due to her limited awareness, the principal makes a coarse inference from the agent's recommendations and, as a result, accepts a large number of the agent's proposals, which ultimately benefits both.

Transplant-associated thrombotic microangiopathy (TA-TMA) is a severe complication following hematopoietic stem cell transplantation (HSCT). No approved treatments are currently available. This study presents real-world data obtained with narsoplimab, a human immunoglobulin G4 monoclonal antibody that inhibits MASP-2, the effector enzyme of the lectin pathway of the complement system. Between January 2018 and August 2023, 20 (13 adult and 7 pediatric) patients diagnosed with TA-TMA received narsoplimab under an ongoing compassionate use program. The diagnosis was based on internationally defined criteria for pediatric and adult patients. Fifteen patients fulfilled the criteria recently established by an international consensus on TA-TMA. Nineteen patients exhibited high-risk characteristics. Thirteen patients (65%) responded to narsoplimab, achieving transfusion independence and significant clinical improvement. The one-hundred-day Overall Survival (OS) post-TA-TMA diagnosis was 70%, and 100% for responders. Narsoplimab proved to be effective and safe in the treatment of high-risk TA-TMA, with no increased infectious complications or other safety signals of concern across all age groups. The high response rates and the encouraging survival outcomes underscore the potential of narsoplimab as a valuable therapeutic option, particularly for high-risk cases.

This article describes a method for compensating thermal effects on tiltmeter (TM) readings using operating temperature measurements. This practice is necessary when the contribution of thermal effects is comparable with the measurand inclination, which is typical of monitoring applications. In such contexts, several temperature-related phenomena take place concurrently and a customized compensation model is necessary. A tailored moving least square (MLS) approach is presented. This formulation is able to separate the contribution of the unknown measurand signal from the contribution of influence quantities, considered as exogenous inputs. Moreover, this method returns the model coefficients over time. However, the uncertainty in the temperature measurements, used as regressors, may induce bias in coefficient estimates. Therefore, a strategy for a posteriori evaluation and correction of the coefficients is proposed and validated with the Monte Carlo method (MCM). The effectiveness of this method is illustrated on a real case, consisting of the inclination monitoring of a wind-turbine tower.

The optimal management of naïve and not naïve Helicobacter pyloripatients remains unclear. Therefore, it is essential to evaluate whether the actual clinical practice mirrors the indications suggested by the guidelines. This study aimed to assess the effectiveness and the safety of the empirical first‐ and second‐line treatments prescribed to patients enroled at Italian centres participating in the European Registry on H. pyloriManagement (Hp‐EuReg). The Hp‐EuReg is an international multicentre prospective non‐interventional registry starting in 2013 aiming to evaluate the management of H. pyloriinfection by European gastroenterologists. Patients were registered in an e‐CRF by AEG‐REDCap. Variables assessed included demographics, previous eradication attempts, treatment regimen, effectiveness, and tolerance. Overall, 3723 patients from 2013 to February 2021 were included: 2996 and 727 received an empirical first‐ and second‐line treatment, respectively. According to the modified ITT analysis, among the first‐line regimens, only the bismuth quadruple therapy with three‐in‐one‐single capsule (BQT‐TSC), the concomitant, and the sequential treatment ‐ all lasting 10 days ‐ achieved an eradication rate >90%. Among the second‐line regimens, only the 10‐day BQT‐TSC reported an effectiveness >90%. High‐dose PPI twice daily also significantly increased the effectiveness of some therapies. The BQT‐TSC was the regimen with the highest incidence of adverse events. Only quadruple therapies lasting at least 10 days achieved over 90% eradication rates among the empirical first‐ and second‐line regimens. It remains unclear whether high‐dose PPI twice daily can improve the efficacy of quadruple treatment.
