Estimates of demographic metrics are crucial to assess levels and trends of population health outcomes. The profound impact of the COVID-19 pandemic on populations worldwide has underscored the need for timely estimates to understand this unprecedented event within the context of long-term population health trends. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 provides new demographic estimates for 204 countries and territories and 811 additional subnational locations from 1950 to 2021, with a particular emphasis on changes in mortality and life expectancy that occurred during the 2020–21 COVID-19 pandemic period.

In Notaufnahmen werden regelmäßig PatientInnen am Lebensende versorgt. Neben PatientInnen, die im Rahmen der notfallmedizinischen Akutversorgung einer plötzlichen Erkrankung oder eines Traumas trotz Rettungsversuche des Behandlungsteams versterben, betrifft dies auch PatientInnen in der Finalphase chronischer Erkrankungen oder im hohen Alter. Um diesen PatientInnen gerecht zu werden, sollten NotfallmedizinerInnen die Grundlagen der Therapiezielfindung und die Eruierung des Patientenwillens kennen und ein palliativmedizinisches Basiswissen besitzen, um auftretende Symptomkrisen adäquat zu beherrschen. Ebenfalls sollten sie Grundkenntnisse in der Kommunikation bei der Überbringung schlechter Nachrichten besitzen.

9. Economical aspects of coronary angiography for diagnostic purposes: a Belgian perspective

- Abstract
AbstractCoronary angiography (CA) is an increasing diagnostic procedure in Belgium. The aim of this analysis was to look at the financial aspects of CA in a large tertiary Belgium hospital to establish if current reimbursement is appropriate. For the analysis of costs we considered the use of the catheterisation laboratory, personnel costs and material costs during multiple weekly periods in the spring of 2023. We calculated that one cathlab needs to perform 8.21 CA's to equal incomes with costs. To allow for a small positive income (200€) for the hospital/cardiologist 9 procedures per cathlab day are required. Our hospital performs a 7 (mean) ± 0.75 (standard deviation) of CA's per cathlab day and therefore does not reach this financial break-even point. Our calculations are on the safe side, since coronary physiological interrogation with fractional flow reserve (FFR) was excluded from this analysis. Nevertheless, this is a cost-effective technique for which no extra reimbursement is foreseen in the current Belgium system.

10. “It’s a bit like saying: I don’t see colour”: Unpacking Coloniality in Kalaallit Nunaat (Greenland) through Epistolary Collaborative Practice

Abstract:In this article we argue that the epistolary form can be used as collaborative practice—further expanding researcher-informant relations—where both parties enter an epistemic partnership and become co-researchers, co-theorizing and co-creating the research output together. We have been using the epistolary form as a means to exchange observations, ideas, and positionalities about coloniality in Kalaallit (Greenlandic Inuit) society today, and to investigate the stance young people take in this discussion. The article has developed from correspondence via letter-writing between the two authors and renders visible the dynamics of our epistemic relationship, an essential element of the collaborative process that often stays hidden. We hereby experiment with collaborative research practices and alternative ways of creating knowledge, as advocated by practitioners working with collaborative and experimental research practices in anthropology, clearly positioning ourselves as authors, an Indigenous scholar and a non-Indigenous researcher, in relation to each other as well as to the world around us.

11. Ist die posttraumatische Radiuskopfresektion noch zeitgemäß?

Die Resektion des Radiuskopfes war in der Vergangenheit bei nicht rekonstruierbaren Radiuskopffrakturen häufig die einzige Therapieoption. Mit den modernen Prothesen steht nun eine weitere Behandlungsmethode zur Auswahl. Die vorliegende Arbeit befasst sich mit der Frage in welchen Fällen die ersatzlose Resektion noch vertretbar ist. In einem Studienkollektiv von 44 Patienten wurden 28 Patienten mit einer Radiuskopfprothese, 9 Patienten mit einer Radiuskopfresektion und 7 Patienten mit einer Radiuskopfteilresektion versorgt. Die mittlere Nachuntersuchungszeit betrug 29 Monate. Insgesamt wurden 13 Mason III- und 31 Mason IV-Frakturen versorgt. Der Score nach Morrey ergab bei den Radiuskopfprothesen im Mittel 79 (83; 47–98) Punkte; bei den Radiuskopfresektionen einen Durchschnittswert von 79 Punkten (74; 56–95) und bei den Teilresektionen des Kopfes einen Mittelwert von 74 Punkten (72; 48–93). Die Resektion des Radiuskopfes bei ansonsten stabiler ligamentärer Führung des Ellenbogens kann zu guten Ergebnissen führen. Bei ligamentären Begleitverletzungen ist jedoch die Radiuskopfprothese vorzuziehen, da durch sie der radiale Pfeiler und somit die Gelenkgeometrie wieder hergestellt werden kann, wodurch eine frühfunktionelle Beübung möglich ist. Unter Berücksichtigung dieser Tatsachen zeigen die RK-Resektion und die RK- Prothese bessere Ergebnisse als die Teilresektion des Radiuskopfes. Diese ist aufgrund verstärkter Schmerzen und Bewegungseinschränkungen nicht zu empfehlen. The resection of the radial head was previously the only therapy option for radial head fractures. Today, modern prosthetics present further alternatives for treatment. The following paper deals with answering the question of whether there are still instances in which resection is justifiable. The patient collective consisted of 44 patients: 28 received a radial head prosthetic implant, 9 a radial head resection, and 7 a partial radial head resection. The average follow up time was 29 months. Altogether, 13 Mason III and 31 Mason IV fractures were treated. The mean Morrey Score after implantation of a radial head prosthetic device was 79 points (83; 47–98); for radial head resections 79 points (74; 56–95), and for the partial resection of the head 74 points (72; 48–93). The resection of the radial head after fracture can yield good outcome in the short-term in a stable joint. If there is a ligamentous instability, the implantation of a radial head prosthetic device is recommended. It stabilizes the radial axis and reconstructs joint geometry, allowing for early functional use. Because of intensified pain and movement restriction partial resection of the radial head is not recommended.

Zusammenfassung: Hintergrund: Radiusköpfchentrümmerfrakturen sind häufig mit Begleitverletzungen vergesellschaftet, die zu einer Beeinträchtigung der Ellenbogenfunktion und -stabilität führen können. Prothesen neuerer Generation sollen eine Verbesserung der Ergebnisse liefern. Ziel dieser Studie war es, die Ergebnisse der modularen EVOLVE®-Prothese zu evaluieren. Patienten und Methoden: Von Juni 2001 bis Januar 2005 wurde bei 30 Patienten mit komplexen Radiusköpfchenfrakturen eine Prothese implantiert; 28 Patienten wurden nachuntersucht. Die mittlere Nachuntersuchungszeit betrug 29 Monate. Insgesamt lag 7-mal (25%) eine Mason-III- und 21-mal (75%) eine Mason-IV-Fraktur vor. Ergebnisse: Nach den Kriterien des Morrey-Scores wurden 19 Patienten mit sehr gut und gut beurteilt, 8 Patienten wurden mit ausreichend eingestuft und 1 Ergebnis wurde als mangelhaft bewertet. Es wurde keine Instabilität des Ellenbogengelenks festgestellt. Schlussfolgerung: Radiusköpfchentrümmerfrakturen mit ligamentären Begleitverletzungen können durch die primäre Radiusköpfchenprothese und ligamentäre Rekonstruktion mit einem guten funktionellen Ergebnis behandelt werden. Langzeitergebnisse sind zwar noch nicht verfügbar; aus mittelfristiger Sicht ist dies eine effektive Methode zur Wiederherstellung der Gelenkgeometrie bei einer übungsstabilen Versorgung des Ellenbogengelenks.

- Abstract
T-cell–mediated help to B cells is required for the development of humoral responses, in which the cytokine interleukin (IL)-21 is key. Here, we studied the mRNA-1273 vaccine–induced SARS-CoV-2–specific memory T-cell IL-21 response, memory B cell response, and immunoglobulin (Ig)G antibody levels in peripheral blood at 28 days after the second vaccination by ELISpot and the fluorescent bead–based multiplex immunoassay, respectively. We included 40 patients with chronic kidney disease (CKD), 34 patients on dialysis, 63 kidney transplant recipients (KTR), and 47 controls. We found that KTR, but not patients with CKD and those receiving dialysis, showed a significantly lower number of SARS-CoV-2–specific IL-21 producing T cells than controls (P< .001). KTR and patients with CKD showed lower numbers of SARS-CoV-2–specific IgG–producing memory B cells when compared with controls (P< .001 and P= .01, respectively). The T-cell IL-21 response was positively associated with the SARS-CoV-2–specific B cell response and the SARS-CoV-2 spike S1–specific IgG antibody levels (both Pearson r= 0.5; P< .001). In addition, SARS-CoV-2–specific B cell responses were shown to be IL-21 dependent. Taken together, we show that IL-21 signaling is important in eliciting robust B cell–mediated immune responses in patients with kidney disease and KTR.

Hy-Fi , a system that aggregates light fidelity (LiFi) and radio frequency (RF)-based communication on the 802.11 (WiFi) physical layer by utilizing the MIMO capabilities in IEEE 802.11-compliant commodity WiFi chips.Hy-Fi is based on two key ideas. First, we use inexpensive commodity hardware to facilitate direct transmission of WiFi waveforms over the optical wireless channel, as this is proposed in the IEEE P802.11bb task group. Second, we use the MIMO signal processing from WiFi to aggregate LiFi and radio signals directly at the physical layer.Hy-Fi was implemented as a prototype and evaluated in a small testbed. Experimental results reveal that our approach offers excellent robustness against signal fading, blockage and external interference in both, the optical and radio channels making it suitable for applications with very strict requirements to the packet delay and loss ratio. Moreover, the two channels, LiFi and RF, can be aggregated to double the link capacity in the best case. Finally, we demonstrate howHy-Fi could be used as wireless access technology in next-generation indoor enterprise networks providing both high capacity and seamless mobility.- Published

23. Strategy analysis of badminton players using deep learning from IMU and UWB wearables

Badminton is a fast-paced sport that requires a high level of skill and coordination. To improve their skills, players can use activity trackers to monitor different shots and activities. These trackers utilize inertial measurement units (IMUs), which measures acceleration and angular velocity on the rackets, and the ultra-wideband (UWB) sensors, which measure the location of the player. This study first analyzes the use of UWB localization for tracking badminton players on the court and analyzes the location where shots are played. Furthermore, this study focuses on using both IMU at the racket and wrist and UWB sensors to recognize strategies utilized in badminton matches, employing convolutional neural network (CNN) and Long-Short-Term Memory (LSTM) models. The goal is to classify thirteen badminton shots, as well as an extra class that contains non-shot activities. The output of this shot classifier is provided to the strategy recognition model, which can identify four main strategies, with eleven variations in total, alongside an additional class designated for non-strategy instances such as movement or rest intervals. We trained and tested the models on data from six skilled badminton players. The best results were achieved by using both IMU and UWB data. The proposed 2D-CNN achieved a shot classification accuracy of 90.9%, while the proposed LSTM achieved a strategy recognition accuracy of 80%. The results of this study suggest that neural networks can be used to effectively classify badminton shots and strategies to improve the training of badminton players, as well as to analyze match data.

30. A Thousand Miles of Streams and Mountains: Daoist Self-Cultivation in a Song Landscape Painting

Abstract:A Thousand Miles of Streams and Mountains (Qianli jiangshan tu 千里江山圖) is the name of a famous 12-meter-long landscape painting of the Northern Song dynasty. It was painted in 1113 by a 17-year-old prodigy, Wang Ximeng 王希孟, under Emperor Huizong’s 徽宗 (r. 1101–1125) supervision, an accomplished painter and a Daoist initiate himself. In this paper I argue that the handscroll depicts a story of Daoist self-cultivation, from lay-life, initiation, receiving training, to eventually attaining Dao (becoming immortal) and the afterlife as an immortal. This story is depicted in both the iconographic details of the figures, buildings, paths, bridges and other elements, guiding the viewer through the landscape, and the composition of the handscroll, mirroring the development of the story by means of the particular placement and the form of the landscape. Because of his close involvement, the landscape painting offers a unique and amazing insight into Huizong’s personal views on what constitutes a Daoist and Daoist self-cultivation, if not his ideas on Daoist landscape painting in the Song dynasty.

Diagnostik und Therapie von kinderurologischen Patienten und Patientinnen können im klinischen Alltag äußerst herausfordernd sein. Im Vergleich zu erwachsenen Patienten stehen bei der Diagnostik kongenitaler Erkrankungen neben einer Erwartungshaltung der Eltern des Kindes anspruchsvollere Untersuchungen, erhöhter Zeitaufwand und limitierte Optionen im Vordergrund. Die genaue Kenntnis der diagnostischen Möglichkeiten in Verbindung mit der richtigen Indikationsstellung ist hier essenziell. Einfache Prozesse können aufgrund der fehlenden Compliance besonders kleiner Kinder wesentlich zeitaufwendiger sein. Die Sonographie gilt als der Standard in der bildgebenden Kinderurologie. Ein fundiertes Wissen über die embryonale Entwicklung wie auch über physiologische Prozesse im Laufe der Kindheit trägt zur korrekten Diagnosestellung bei. Diese Arbeit befasst sich mit den Möglichkeiten der nuklearmedizinischen Diagnostik, der weiterführenden Diagnostik bei Blasenentleerungsstörungen sowie zuletzt der bildgebenden Diagnostik im kinderurologischen Operationssaal.

Die genaue Kenntnis der diagnostischen Möglichkeiten in Verbindung mit der richtigen Indikationsstellung ist hier essenziell. Dieser erste von zwei Beitragsteilen widmet sich zum Einen der Sonographie als primärer Standarduntersuchungsmethode in der Kinderurologie und geht im speziellen auf das contrast enhanced ultrasound (CEUS) ein. Das konventionelle Röntgenunterstützte Miktionszystourethrographie ist weiterhin fester Bestandteil des Diagnosealgorithmus des vesikoureterorenalen Refluxes und wird in dieser Arbeit eingängig behandelt. Der Einsatz der CT beschränkt sich aufgrund der Strahlenexposition in der Kinderurologie auf die Abklärung des schweren Polytraumas, und in Ausnahmefällen auf die Tumor- bzw. Steindiagnostik. Die Magnetresonanztomographie ermöglicht eine hervorragende anatomische Beurteilung des gesamten Urogenitaltraktes, ohne das Kind einer unnötigen Strahlenexposition auszusetzen. Diese Fortbildungseinheit gibt Ihnen eine aktuelle Übersicht und bietet einige Tipps und Tricks in der kinderurologischen Diagnostik.

34. Advancing intraoperative magnetic tracing using 3D freehand magnetic particle imaging

Purpose: Sentinel lymph node biopsy is a routine procedure for nodal staging in penile cancer. Most commonly, this procedure is guided by radioactive tracers, providing various forms of preoperative and intraoperative guidance. This is further extended with fluorescence imaging using hybrid radioactive–fluorescence tracers. Alternatively, a magnetic-based approach has become available using superparamagnetic iron-oxide nanoparticles (SPIONs). This study investigates a novel freehand magnetic particle imaging and navigation modality (fhMPI) for intraoperative localization, along with a hybrid approach, combining magnetic and fluorescence guidance. Materials and methods: The fhMPI set-up was built with a surgical navigation device, optical tracking system and magnetometer probe. A dedicated reconstruction software based on a look-up-table method was used to reconstruct a superficial 3D volume of the SPION distribution in tissue. For fluorescence guidance, indocyanine green (ICG) was added to the SPIONs. The fhMPI modality was characterized in phantoms, ex vivo human skin and in vivo porcine surgery. Results: Phantom and human skin explants illustrated that the current fhMPI modality had a sensitivity of 2.2 × 10
–2 mg/mL SPIONs, a resolving power of at least 7 mm and a depth penetration up to 1.5 cm. Evaluation during porcine surgery showed that fhMPI allowed for an augmented reality image overlay of the tracer distribution in tissue, as well as 3D virtual navigation. Besides, using the hybrid approach, fluorescence imaging provided a visual confirmation of localized nodes. Conclusion: fhMPI is feasible in vivo, providing 3D imaging and navigation for magnetic nanoparticles in the operating room, expanding the guidance possibilities during magnetic sentinel lymph node procedures. Furthermore, the integration of ICG provides the ability to visually refine and confirm correct localization. Further clinical evaluation should verify these findings in human patients as well.- Published

- Abstract
Deglacial transitions of the middle to late Pleistocene (terminations) are linked to gradual changes in insolation accompanied by abrupt shifts in ocean circulation. However, the reason these deglacial abrupt events are so special compared with their sub-glacial-maximum analogues, in particular with respect to the exaggerated warming observed across Antarctica, remains unclear. Here we show that an increase in the relative importance of salt versus temperature stratification in the glacial deep South Atlantic decreases the potential cooling effect of waters that may be upwelled in response to abrupt perturbations in ocean circulation, as compared with sub-glacial-maximum conditions. Using a comprehensive coupled atmosphere–ocean general circulation model, we then demonstrate that an increase in deep-ocean salinity stratification stabilizes relatively warm waters in the glacial deep ocean, which amplifies the high southern latitude surface ocean temperature response to an abrupt weakening of the Atlantic meridional overturning circulation during deglaciation. The mechanism can produce a doubling in the net rate of warming across Antarctica on a multicentennial timescale when starting from full glacial conditions (as compared with interglacial or subglacial conditions) and therefore helps to explain the large magnitude and rapidity of glacial terminations during the late Quaternary.

38. Family technoference: Exploring parent mobile device distraction from children’s perspectives

- Abstract
The goal of the present study is to understand how preadolescents’ perceptions of their parents’ mobile device distraction (“technoference”) are related to their global assessment of family satisfaction. Building on existing research, a model is established that investigates this association through three mediators: parent–child conflict; companionship; and perceived emotional support. Data from a cross-sectional survey (n= 742, 9–13 years, 50.9% girls, mean age =10.85, standard deviation = 0.73) supported most of the associations specified in our model. First, our findings demonstrate that preadolescents who perceived higher amounts of parent technoference also rated the relationship with their parents significantly higher in conflict, and lower in terms of perceived companionship. Moreover, maternal mobile device distraction was significantly associated with less emotional support. All but one of the indirect associations were found to be statistically significant, thus revealing a number of pathways through which technoference is related to preadolescents’ assessments of family satisfaction.

- Abstract
The general aerodynamic problem of arbitrarily oriented in-plane vortex-rotor interaction was investigated in the past only by numerical simulation. Just one special case of in-plane vortex-rotor interaction with the vortex axis in flight direction was recently solved analytically. In this article, the analytical solution for arbitrary in-plane vortex orientation and position relative to the rotor is given for the first time. The solution of the integrals involved as derived here encompasses and simplifies the previous derivation of the special case significantly. Results provide the vortex impact on rotor trim (thrust, aerodynamic rolling and pitching moments about the hub) and the rotor controls required to mitigate these disturbances. For the special case with the vortex axis in flight direction, the results are identical to the former solution and results for the other in-plane vortex orientations and positions agree with the numerical results obtained so far.

ObjectiveTo investigate the potential impacts of several tobacco control interventions on adult daily smoking prevalence in the Australian state of Queensland, using a system dynamics model codeveloped with local and national stakeholders.MethodsEight intervention scenarios were simulated and compared with a reference scenario (business as usual), in which all tobacco control measures currently in place are maintained unchanged until the end of the simulation period (31 December 2037).FindingsUnder the business as usual scenario, adult daily smoking prevalence is projected to decline from 11.8% in 2017 to 5.58% in 2037. A sustained 50% increase in antismoking advertising exposure from 2018 reduces projected prevalence in 2037 by 0.80 percentage points. Similar reductions are projected with the introduction of tobacco wholesaler and retailer licensing schemes that either permit or prohibit tobacco sales by alcohol-licensed venues (0.65 and 1.73 percentage points, respectively). Increasing the minimum age of legal supply of tobacco products substantially reduces adolescent initiation, but has minimal impact on smoking prevalence in the adult population over the simulation period. Sustained reductions in antismoking advertising exposure of 50% and 100% from 2018 increase projected adult daily smoking prevalence in 2037 by 0.88 and 1.98 percentage points, respectively.ConclusionsThese results suggest that any prudent approach to endgame planning should seek to build on rather than replace existing tobacco control measures that have proved effective to date. Additional interventions that can promote cessation are expected to be more successful in reducing smoking prevalence than interventions focussing exclusively on preventing initiation.

The novel coronavirus disease COVID-19 that emerged in 2019 is caused by the virus SARS CoV-2 and named for its close genetic similarity to SARS CoV-1 that caused severe acute respiratory syndrome (SARS) in 2002. Both SARS coronavirus genomes encode two overlapping large polyproteins, which are cleaved at specific sites by a 3C-like cysteine protease (3CLpro) in a post-translational processing step that is critical for coronavirus replication. The 3CLprosequences for CoV-1 and CoV-2 viruses are 100% identical in the catalytic domain that carries out protein cleavage. A research effort that focused on the discovery of reversible and irreversible ketone-based inhibitors of SARS CoV-1 3CLproemploying ligand-protease structures solved by X-ray crystallography led to the identification of 3and 4. 48. The importance of dose optimisation in the treatment of iron deficiency in heart failure

- Abstract
AbstractBackground:Treatment with ferric carboxymaltose (FCM) is limited to 1 g during one administration, which is insufficient in patients with a higher body weight or low haemoglobin (Hb). As a consequence, under-dosing might be common in clinical practice, yet the consequences remain unstudied.Methods:We retrospectively assessed all HFrEF-patients with iron-deficiency (ferritin <100 µg/l or between 100 and 300 µg/l if TSAT < 20%) receiving treatment with FCM between 2015 and 2017. This time-frame was chosen as during this we used a 1-g FCM-regimen for all patients (unless Hb = 14–15 mg/dl, than 500 mg). We compared the actual given dose versus the calculated target dose (according to the SmPC, with the difference between both being the dose deficit). We assessed the impact of dose deficits on clinical and biochemical status after 12 weeks.Results:A total of 211 HFrEF patients were analysed. The actual given dose FCM was 918 ± 188 mg, while the calculated target dose was 1308 ± 470 mg. In 121(61%) patients, a standard dose of 1-g FCM resulted in a dose deficit, of whom 93 had a dose deficit of 500 mg and 35 had a dose deficit of 1000 mg. Follow-up was available in 81% of patients (median = 12 weeks). A dose deficit of 500 mg was associated with a 4.93 higher odds, while a dose deficit of 1000 mg was associated with a 7.78 higher odds of residual iron deficiency. After adjusting for baseline NYHA-class, a dose deficit was associated with less symptomatic improvement. During 442 ± 292 days of follow-up, 68 patients were readmitted with heart failure and 15 patients died. In an univariate model (but not in a multivariate model), a dose deficit was associated with adverse clinical outcome.Conclusion:A majority of HFrEF patients with iron deficiency require doses exceeding 1 g of FCM, and thus require follow-up appointments to correct a residual dose deficit. A residual dose deficit is associated with less functional and biochemical improvement.

50. How progressive vegetation die‐off in a tidal marsh would affect flow and sedimentation patterns: A field demonstration

- Abstract
Coastal marshes provide valuable ecosystem functions, but some are facing increasing risks of vegetation loss due to sea level rise and other stressors. A key question is how tidal flow and sedimentation patterns are affected by the spatiotemporal patterns of vegetation loss, as sediment accretion with sea level rise largely affects the potential for marsh recovery. Here, we performed a field study in a macrotidal reed marsh and simulated typical spatiotemporal patterns of vegetation loss by consecutive mowing. For each mowing pattern, the spatial patterns of flow velocities and sedimentation rates were recorded. Our results indicate that initial vegetation loss in inner marsh portions, with an intact vegetation belt alongside channel edges, has limited effect on tidal flows over the marsh. However, subsequent creation of unvegetated corridors connecting the bare inner marsh and the channels increases flow velocities in these corridors but not in remaining vegetation patches. Finally, when all vegetation is removed, sheet flow occurs over the whole marsh instead of concentrated channel flow. Effects on spatial sedimentation patterns are complex and not significant on all measuring locations. Nevertheless, our study indicates that complete vegetation removal results in redistributed sedimentation patterns, with a general tendency of locally reduced sedimentation rates close (< 15 m) to channels and increased sediment supply to inner marshes 15–50 m from channels. Our results highlight that feedbacks between spatial patterns of vegetation loss, tidal sediment transport, and deposition are key to understanding and mitigating risks of marsh loss in face of sea level rise.
