- Abstract
Investigations into household structure in low- and middle-income countries (LMICs) provide important insight into how families manage domestic life in response to resource allocation and caregiving needs during periods of rapid sociopolitical and health-related challenges. Recent evidence on household structure in many LMICs contrasts with long-standing viewpoints of worldwide convergence to a Western nuclearized household model. Here, we adopt a household-centered theoretical and methodological framework to investigate longitudinal patterns and dynamics of household structure in a rural South African setting during a period of high AIDS-related mortality and socioeconomic change. Data come from the Agincourt Health and Socio-Demographic Surveillance System (2003–2015). Using latent transition models, we derived six distinct household types by examining conditional interdependency between household heads’ characteristics, members’ age composition, and migration status. More than half of households were characterized by their complex and multigenerational profiles, with considerable within-typology variation in household size and dependency structure. Transition analyses showed stability of household types under female headship, while higher proportions of nuclearized household types dissolved over time. Household dissolution was closely linked to prior mortality experiences—particularly, following death of a male head. Our findings highlight the need to better conceptualize and contextualize household changes across populations and over time.

- Abstract
Telemedicine (TM) contributes to bridge the gap between healthcare facilities and patients’ homes with neuromuscular disease (NMD) because of mobility issues. However, its deployment is limited due to difficulties evaluating subtle neurological signs such as mild weakness or sensory deficits. The COVID-19 pandemic has disrupted healthcare delivery worldwide, necessitating rapid measures implementation by health care providers (HCPs) to protect patients from acquiring SARS-CoV-2 while maintaining the best care and treatment. Given the challenges faced by remote healthcare assistance of NMD patients, we aim to evaluate the use of TM in NMD during the COVID-19 pandemic. Based on the Model for Assessment-of-Telemedicine-Applications (MAST), we conducted a survey amongst clinicians of the ERN EURO NMD (European-Reference-Network-for-Rare-Neuromuscular-Diseases). Based on 42 responses over 76 expected ones, our results show that the COVID-19 pandemic significantly increased the number of HCPs using TM (from 60% to 100%). The TM types most used during the COVID-19 period are teleconsultation and consultation by phone, particularly in the context of symptoms worsening in NMD patients with COVID-19 infection. Most European HCPs were satisfied when using TM but as a complementary option to physical consultations. Many responses addressed the issue of technical aspects needing improvement, particularly for elderly patients who need caregivers’ assistance for accessing the TM platform. TM has been essential during COVID-19, but its use still presents some limitations for NMD patients with cognitive deficits or for first-time diagnosis. Thus, TM should be used as complement to, rather than substitute, for face-to-face consultations.

Latest studies in patients with first episode psychosis (FEP) have shown alterations in cardiovascular, immune and endocrinological systems. These findings could indicate a systemic onset alteration in the metabolic disease as opposed to justifying these findings exclusively by antipsychotics’ side effects and long-term lifestyle consequences. In any case, this population is considered at higher risk for developing cardiometabolic disorders than their age-matched peers.

- Abstract
Identification of convulsive epilepsy in sub-Saharan Africa relies on access to resources that are often unavailable. Infrastructure and resource requirements can further complicate case verification. Using machine-learning techniques, we have developed and tested a region-specific questionnaire panel and predictive model to identify people who have had a convulsive seizure. These findings have been implemented into a free app for health-care workers in Kenya, Uganda, Ghana, Tanzania, and South Africa.

- Abstract
IMPORTANCE: The role of endovascular thrombectomy is uncertain for patients presenting beyond 24 hours of the time they were last known well. OBJECTIVE: To evaluate functional and safety outcomes for endovascular thrombectomy (EVT) vs medical management in patients with large-vessel occlusion beyond 24 hours of last known well. DESIGN, SETTING, AND PARTICIPANTS: This retrospective observational cohort study enrolled patients between July 2012 and December 2021 at 17 centers across the United States, Spain, Australia, and New Zealand. Eligible patients had occlusions in the internal carotid artery or middle cerebral artery (M1 or M2 segment) and were treated with EVT or medical management beyond 24 hours of last known well. INTERVENTIONS: Endovascular thrombectomy or medical management (control). MAIN OUTCOMES AND MEASURES: Primary outcome was functional independence (modified Rankin Scale score 0-2). Mortality and symptomatic intracranial hemorrhage (sICH) were safety outcomes. Propensity score (PS)–weighted multivariable logistic regression analyses were adjusted for prespecified clinical characteristics, perfusion parameters, and/or Alberta Stroke Program Early CT Score (ASPECTS) and were repeated in subsequent 1:1 PS-matched cohorts. RESULTS: Of 301 patients (median [IQR] age, 69 years [59-81]; 149 female), 185 patients (61%) received EVT and 116 (39%) received medical management. In adjusted analyses, EVT was associated with better functional independence (38% vs control, 10%; inverse probability treatment weighting adjusted odds ratio [IPTW aOR], 4.56; 95% CI, 2.28-9.09; P < .001) despite increased odds of sICH (10.1% for EVT vs 1.7% for control; IPTW aOR, 10.65; 95% CI, 2.19-51.69; P = .003). This association persisted after PS-based matching on (1) clinical characteristics and ASPECTS (EVT, 35%, vs control, 19%; aOR, 3.14; 95% CI, 1.02-9.72; P = .047); (2) clinical characteristics and perfusion parameters (EVT, 35%, vs control, 17%; aOR, 4.17; 95% CI, 1.15-15.17; P = .03); and (3) clinical characteristics, ASPECTS, and perfusion parameters (EVT, 45%, vs control, 21%; aOR, 4.39; 95% CI, 1.04-18.53; P = .04). Patients receiving EVT had lower odds of mortality (26%) compared with those in the control group (41%; IPTW aOR, 0.49; 95% CI, 0.27-0.89; P = .02). CONCLUSIONS AND RELEVANCE: In this study of treatment beyond 24 hours of last known well, EVT was associated with higher odds of functional independence compared with medical management, with consistent results obtained in PS-matched subpopulations and patients with presence of mismatch, despite increased odds of sICH. Our findings support EVT feasibility in selected patients beyond 24 hours. Prospective studies are warranted for confirmation.

The development of acute neurologic dysfunction associated with tumefactive demyelinating lesions and mild diffuse involvement of the corpus callosum has been described in children as a sentinel event that may allow diagnosis of juvenile metachromatic leukodystrophy (MLD) at an early and potentially treatable stage. We report a child who developed this clinico-radiologic pattern associated with myelin oligodendrocyte glycoprotein antibodies several months before the onset of progressive symptoms of late infantile MLD.

•La capilaroscopia permite evaluar la morfología de la microcirculación a través de la observación de los capilares del lecho ungueal de los dedos. Se utiliza para el estudio de los fenómenos vasculares acrales, siendo su fenómeno de Raynaud el principal síntoma clínico a estudio.•La capilaroscopia es una técnica no invasiva, barata, rápida y sencilla que aporta mucha información al clínico para la correcta caracterización del fenómeno de Raynaud.•La importancia de la correcta caracterización del fenómeno de Raynaud radica en que puede tratarse del primer síntoma de una enfermedad autoinmune. Además, el fenómeno de Raynaud secundario puede complicarse con aparición de úlceras e isquemia digitales.•El paciente no debe realizarse ningún tipo de manicura, manipulación de las cutículas ni aplicación de pintaúñas los 15 días previos a la realización de la prueba. También debe evitar la onicofagia, ya que todo ello puede conducir a un resultado falsamente positivo de la misma.•Cuando el resultado de la capilaroscopia no es normal, especialmente si presenta un patrón de esclerodermia, hay que sospechar que el paciente puede tener una enfermedad autoinmune y remitirlo al reumatólogo.

- Abstract
Desmin (DES) is the main intermediate muscle filament that connects myofibrils individually and with the nucleus, sarcolemma, and organelles. Pathogenic variants of DEScause desminopathy, a disorder affecting the heart and skeletal muscles. We aimed to analyze the clinical features, morphology, and distribution of desmin aggregates in skeletal muscle biopsies of patients with desminopathy and to correlate these findings with the type and location of disease-causing DESvariants. This retrospective study included 30 patients from 20 families with molecularly confirmed desminopathy from 2 neuromuscular referral centers. We identified 2 distinct patterns of desmin aggregates: well-demarcated subsarcolemmal aggregates and diffuse aggregates with poorly delimited borders. Pathogenic variants located in the 1B segment and the tail domain of the desmin molecule are more likely to present with early-onset cardiomyopathy compared to patients with variants in other segments. All patients with mutations in the 1B segment had well-demarcated subsarcolemmal aggregates, but none of the patients with variants in other desmin segments showed such histological features. We suggest that variants located in the 1B segment lead to well-shaped subsarcolemmal desmin aggregation and cause disease with more frequent cardiac manifestations. These findings will facilitate early identification of patients with potentially severe cardiac syndromes.

Here we present the dental pathologies found in the population of Cortijo Coracho, which dates from Late Antiquity (4th–8th centuries AD) and is located in the region of Lucena (Córdoba). Using a sample of 160 individuals over the age of twenty years at death, with a total of 1681 dental crowns, the frequency of various oral diseases (dental calculus, alveolar reabsorptions, periodontal diseases, and dental abscesses) were analysed. The frequency of oral disease was compared between the mandible and the maxilla, the anterior and posterior teeth as well as the right and left sides. Consideration was given to ante-mortem tooth loss, which may be caused by dental disease. Statistical analysis showed a significantly greater frequency of oral disease in the posterior teeth of the mandible.

- Abstract
Many patients experiencing a rare disease remain undiagnosed even after genomic testing. Reanalysis of existing genomic data has shown to increase diagnostic yield, although there are few systematic and comprehensive reanalysis efforts that enable collaborative interpretation and future reinterpretation. The Undiagnosed Rare Disease Program of Catalonia project collated previously inconclusive good quality genomic data (panels, exomes, and genomes) and standardized phenotypic profiles from 323 families (543 individuals) with a neurologic rare disease. The data were reanalyzed systematically to identify relatedness, runs of homozygosity, consanguinity, single-nucleotide variants, insertions and deletions, and copy number variants. Data were shared and collaboratively interpreted within the consortium through a customized Genome-Phenome Analysis Platform, which also enables future data reinterpretation. Reanalysis of existing genomic data provided a diagnosis for 20.7% of the patients, including 1.8% diagnosed after the generation of additional genomic data to identify a second pathogenic heterozygous variant. Diagnostic rate was significantly higher for family-based exome/genome reanalysis compared with singleton panels. Most new diagnoses were attributable to recent gene-disease associations (50.8%), additional or improved bioinformatic analysis (19.7%), and standardized phenotyping data integrated within the Undiagnosed Rare Disease Program of Catalonia Genome-Phenome Analysis Platform functionalities (18%).

The development of nanotechnology-based siRNA-delivery systems offers an avenue to improve gene therapy and combined therapies. The radiopharmaceutical 225Ac-DOTA-Bombesin is internalized in cells with high expression of gastrin release peptide receptor (GRPR), such as breast cancer cells (T47D). α-radiation emitted by 225Ac is lethal to the cell due to a high rate of induced double-strand breaks (DSBs). siRNA-RAD51 inhibits RAD51 protein synthesis, preventing the repair of DSBs in DNA. If gene silencing of RAD51 and 225Ac converge in T47D cells, a combined gene/radiation therapy is achieved, since RAD51 silencing by siRNA-RAD51 will prevent cellular repair of 225Ac-induced DSBs. However, siRNA requires an efficient delivery system. In this work, a hybrid technology based on mimetic high-density lipoprotein (mHDL) was developed for siRNA-RAD51 transport. Chitosan/siRNA (CS/siRNA) complexes were prepared and were coated with lipids and Apo-A1 mimetic peptide (D-4F), to assemble mHDL(CS/siRNA). The presence of D-4F peptide targeted the nanosystem to scavenger receptor B1 (SR-B1), which is overexpressed in cancer cells (e.g., T47D cancer cell line). A CS/siRNA-Cy3 complex, with similar chemical properties to CS/siRNA-RAD51, was used for chemical characterization and evaluation of in vitrotransfection. The results showed that mHDL(CS/siRNA-Cy3) is stable in serum, protects siRNA from RNases, and transfects by specific recognition of SR-B1. Pretreatment with mHDL(CS/siRNA-RAD51) resulted in gene silencing and, in combination with 225Ac-DOTA-Bombesin, significantly reduced the viability of T47D cells. The mHDL(CS/siRNA-RAD51) nanosystem is suitable for siRNA-specific transfection, being an efficient and specific/hybrid technology to sensitize cancer cells overexpressing SR-B1.

29. Heat-flow parameters affecting microstructure and mechanical properties of Al–Cu and Al–Ni alloys in directional solidification: an experimental comparative study

A comparative analysis was carried out of the alloys examined, using the results found for thermal variables, macrostructure, dendrite arm spacing, modulus of elasticity and microhardness. Comparing all the results for variables, a predominance was found of higher values for the Al-5.0 wt.% Cu alloy in relation to those for the Al-5.0 wt.% Ni alloy. Although structural transition has occurred in both alloys, the columnar–equiaxed transition position in castings was not altered during the solidification experiments. Addition of Ni solute into pure aluminum favors a significant decrease in tertiary dendrite spacing, while Cu addition leads to a coarser microstructure. Primary dendrite arm spacing, also, was measured along the castings of both alloys. Theoretical approaches such as the well-known Hunt–Lu, Bouchard–Kirkaldy, and Kurz–Fisher models were used to determine quantitatively this particular microstructural parameter. Good agreement was observed between experimental data and predictions by Hunt–Lu and Bouchard–Kirkaldy models, which assume solidification under a transient heat-flow condition. Finally, empirical equations relating tertiary dendritic arm spacing and mechanical properties to the scale of the dendritic microstructure have been proposed. While no relationship was observed between modulus of elasticity and tertiary dendritic arm spacing, addition of Cu into pure aluminum as an alloying element favored a reduction in the average value of modulus of elasticity when compared with those observed for the Al–Ni alloy. It was found that the microhardness decreases with increasing tertiary dendritic arm spacings. On the other hand, solute addition of Cu into commercially pure aluminum favors a microhardness higher than that verified during upward unidirectional solidification of the Al-5.0 wt.% Ni alloy.

BackgroundValosin-containing protein (VCP) disease, caused by mutations in the VCPgene, results in myopathy, Paget’s disease of bone (PBD) and frontotemporal dementia (FTD). Natural history and genotype–phenotype correlation data are limited. This study characterises patients with mutations in VCPgene and investigates genotype–phenotype correlations.MethodsDescriptive retrospective international study collecting clinical and genetic data of patients with mutations in the VCPgene.ResultsTwo hundred and fifty-five patients (70.0% males) were included in the study. Mean age was 56.8±9.6 years and mean age of onset 45.6±9.3 years. Mean diagnostic delay was 7.7±6 years. Symmetric lower limb weakness was reported in 50% at onset progressing to generalised muscle weakness. Other common symptoms were ventilatory insufficiency 40.3%, PDB 28.2%, dysautonomia 21.4% and FTD 14.3%. Fifty-seven genetic variants were identified, 18 of these no previously reported. c.464G>A (p.Arg155His) was the most frequent variant, identified in the 28%. Full time wheelchair users accounted for 19.1% with a median time from disease onset to been wheelchair user of 8.5 years. Variant c.463C>T (p.Arg155Cys) showed an earlier onset (37.8±7.6 year) and a higher frequency of axial and upper limb weakness, scapular winging and cognitive impairment. Forced vital capacity (FVC) below 50% was as risk factor for being full-time wheelchair user, while FVC <70% and being a full-time wheelchair user were associated with death.ConclusionThis study expands the knowledge on the phenotypic presentation, natural history, genotype–phenotype correlations and risk factors for disease progression of VCP disease and is useful to improve the care provided to patient with this complex disease.

IMPORTANCE: Direct transfer to angiography suite (DTAS) for patients with suspected large vessel occlusion (LVO) stroke has been described as an effective and safe measure to reduce workflow time in endovascular treatment (EVT). However, it is unknown whether DTAS improves long-term functional outcomes. OBJECTIVE: To explore the effect of DTAS on clinical outcomes among patients with LVO stroke in a randomized clinical trial. DESIGN, SETTING, AND PARTICIPANTS: The study was an investigator-initiated, single-center, evaluator-blinded randomized clinical trial. Of 466 consecutive patients with acute stroke screened, 174 with suspected LVO acute stroke within 6 hours of symptom onset were included. Enrollment took place from September 2018 to November 2020 and was stopped after a preplanned interim analysis. Final follow-up was in February 2021. INTERVENTIONS: Patients were randomly assigned (1:1) to follow either DTAS (89 patients) or conventional workflow (85 patients received direct transfer to computed tomographic imaging, with usual imaging performed and EVT indication decided) to assess the indication of EVT. Patients were stratified according to their having been transferred from a primary center vs having a direct admission. MAIN OUTCOMES AND MEASURES: The primary outcome was a shift analysis assessing the distribution of the 90-day 7-category (from 0 [no symptoms] to 6 [death]) modified Rankin Scale (mRS) score among patients with LVO whether or not they received EVT (modified intention-to-treat population) assessed by blinded external evaluators. Secondary outcomes included rate of EVT and door-to-arterial puncture time. Safety outcomes included 90-day mortality and rates of symptomatic intracranial hemorrhage. RESULTS: In total, 174 patients were included, with a mean (SD) age of 73.4 (12.6) years (range, 19-95 years), and 78 patients (44.8%) were women. Their mean (SD) onset-to-door time was 228.0 (117.9) minutes, and their median admission National Institutes of Health Stroke Scale score was 18 (interquartile range [IQR], 14-21). In the modified intention-to-treat population, EVT was performed for all 74 patients in the DTAS group and for 64 patients (87.7%) in the conventional workflow group (P = .002). The DTAS protocol decreased the median door–to–arterial puncture time (18 minutes [IQR, 15-24 minutes] vs 42 minutes [IQR, 35-51 minutes]; P < .001) and door-to-reperfusion time (57 minutes [IQR, 43-77 minutes] vs 84 minutes [IQR, 63-117 minutes]; P < .001). The DTAS protocol decreased the severity of disability across the range of the mRS (adjusted common odds ratio, 2.2; 95% CI, 1.2-4.1; P = .009). Safety variables were comparable between groups. CONCLUSIONS AND RELEVANCE: For patients with LVO admitted within 6 hours after symptom onset, this randomized clinical trial found that, compared with conventional workflow, the use of DTAS increased the odds of patients undergoing EVT, decreased hospital workflow time, and improved clinical outcome. 34. Rasche Wirkung von Benralizumab bei einer Exazerbation von schwerem eosinophilen Asthma im Zusammenhang mit eosinophiler Granulomatose mit Polyangiitis

Hintergrund:Hintergrund: Bei der eosinophilen Granulomatose mit Polyangiitis (EGPA) handelt es sich um eine Erkrankung, die mit schwerem unkontrolliertem eosinophilem Asthma einhergeht. Eosinophile Granulozyten spielen bei der Pathogenese beider Erkrankungen eine wichtige Rolle. Benralizumab ist ein anti-eosinophiler monoklonaler Antikörper. Er bindet an die Alpha-Untereinheit des menschlichen Interleukin-5-Rezeptors (IL-5Rα), der auf der Oberfläche von Eosinophilen und Basophilen exprimiert wird. Wir stellen den ersten Fall einer raschen Besserung der Symptomatik und Lungenfunktion bei Aufnahme eines Patienten wegen der Exazerbation eines schweren eosinophilen Asthmas im Zusammenhang mit EGPA vor. Fallvorstellung:Ein 57-jähriger Mann, bei dem schweres eosinophiles Asthma im Zusammenhang mit EGPA diagnostiziert worden war, wurde aufgrund eines schweren Bronchospasmus in der Abteilung für Lungenheilkunde aufgenommen. Bei der Aufnahme wies der Patient 2300 Eosinophile/μl auf. Trotz Intensivbehandlung mit Bronchodilatatoren, intravenösem Methylprednisolon in einer Dosis von 80 mg/Tag, Sauerstofftherapie und Verneblung mit Budesonid traten bei dem Patienten weiterhin täglich Bronchospasmus-Episoden auf. Zehn Tage nach der Aufnahme wurde dem Patienten bei einer Eosinophilenzahl im Blut von 1700 Zellen/μl Benralizumab 30 mg s.c. verabreicht. An diesem Tag lag das forcierte exspiratorische Volumen in 1 Sekunde (FEV
1 ) bei 28% des theoretischen Wertes (1150 ml). Am dritten Tag hatte sich das FEV1 auf 110 ml (31%) verbessert. Am neunten Tag betrug das FEV1 5% (2100 ml). Die Eosinophilenzahl im Blut betrug am neunten Tag 0 Zellen/μl. Schlussfolgerungen:Die rasche Verbesserung des FEV1 steht im Einklang mit Untersuchungen auf der Grundlage klinischer Studien, in denen nach zwei Tagen eine Verbesserung des Spitzenflusses (peak flow) festgestellt wurde. Zudem zeigte sich in einer Phase-II-Studie ein schnelles Ansprechen bei exazerbiertem Asthma in der Notaufnahme. Die rasche Besserung und der potenzielle Vorteil von Benralizumab bei der Behandlung von EGPA werden durch die anti-eosinophile Wirkung nach 24 Stunden sowie die Wirkung in unterschiedlichen Geweben bestimmt. Der vorliegende Fall lässt auf den Nutzen von Benralizumab bei Patienten mit EGPA und eosinophilem schwerem Asthma schließen, deren Bronchospasmus während eines stationären Aufenthalts aufgrund einer Exazerbation des Asthmas auf eine herkömmliche Behandlung nicht anspricht.- Published

43. Habitat and Harvesting Practices Influence Horn Growth of Male Ibex

Size‐selective harvesting of wild ungulates can trigger a range of ecological and evolutionary consequences. It remains unclear how environmental conditions, including changes in habitat, climate, and local weather conditions, dilute or strengthen the effects of trophy hunting. We analyzed horn length measurements of 2,815 male ibex (Capra pyrenaica) that were harvested from 1995 to 2017 in Els Ports de Tortosa i Beseit National Hunting Reserve in northeastern Spain. We used linear mixed models to determine the magnitude of inter‐individual horn growth variability and partial least square path models to evaluate long‐term effects of environmental change, population size, and hunting strategy on horn growth. Age‐specific horn length significantly decreased over the study period, and nearly a quarter (23%) of its annual variation was attributed to individual heterogeneity among males. The encroachment of pine (Pinusspp.) forests had a negative effect on annual horn growth, possibly through nutritional impoverishment. The harvesting of trophy and selective individuals (e.g., small‐horned males) from the entire population increased horn growth, probably because it reduced the competition for resources and prevented breeding of these smaller males. Local weather conditions and population size did not influence horn growth. Our study demonstrates how habitat changes are altering the horn growth of male ibex. We suggest that habitat interventions, such the thinning of pine forests, can contribute to securing the sustainability of trophy hunting. Even in situations where size‐selective harvesting is not causing a detectable phenotypic response, management actions leading to the expansion of preferred land cover types, such as grass‐rich open areas, can have a positive effect on ungulate fitness. Forest encroachment on open meadows and heterogeneous grasslands is pervasive throughout Mediterranean ecosystems. Therefore, our management recommendations can be extended to the landscape level, which will have the potential to mitigate the side effects of habitat deterioration on the phenotypic traits of wild ibex. © 2020 The Wildlife Society. The encroachment of coniferous forests and the consequent loss of natural pastures had a negative effect on the annual horn growth of male ibex inhabiting a Mediterranean ecosystem. The selective harvesting of small‐horned males had a positive effect on annual horn growth. The expansion of grass‐rich open areas should be prioritized to reduce the negative effects of habitat deterioration on the phenotypic traits of male ibex.

La enfermedad de Pompe es una enfermedad rara con herencia autosómica recesiva por un déficit de maltasa ácida. Este déficit produce un acúmulo de glucógeno en los tejidos. Desde una perspectiva clínica, se caracteriza principalmente por una debilidad de cinturas y una afectación de la musculatura respiratoria. En 2013 se creó el registro español de enfermedad de Pompe. El objetivo de este artículo es analizar las características de los primeros 49 pacientes y divulgar la existencia de este registro dentro de la comunidad médica.

- Abstract
To estimate the incidence and analyze any cancer‐associated factors in patients with systemic lupus erythematosus (SLE), differentiating between hormone‐sensitive (HS) and non‐HScancers. This was a retrospective multicenter study of a patient cohort from the Systemic Lupus Erythematosus Registry of the Spanish Society of Rheumatology. Included were the first cancer post‐SLEdiagnosis, clinical and sociodemographic information, cumulative damage, severity, comorbidities, treatments, and refractoriness. Cancers were classified as HS(prostate, breast, endometrium, and ovarian) and non‐HS(the remainder). The standardized incidence ratio (SIR) was calculated and logistic regression models were built. A total of 3,539 patients (90.4% women) were included, 154 of whom had cancer (91% female), and 44 had HScancer (100% female). The cancer SIR was 1.37 (95% confidence interval [95% CI] 1.15–1.59), with higher values in women age <65 years (SIR 2.38 [95% CI1.84–2.91]). The SIR in women with HSversus non‐HScancer was 1.02 (95% CI0.13–1.91) and 1.93 (95% CI0.98–2.89). In HSversus non‐HScancers, SLEdiagnostic age (odds ratio [OR] 1.04 [P= 0.002] versus 1.04 [P= 0.019]), and period of disease evolution (OR1.01 [P< 0.001] versus 1.00 [P= 0.029]) were associated with cancer. The Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index (OR1.27 [P= 0.022]) and angiotensin‐converting enzyme (ACE) inhibitor prescriptions (OR2.87 [P= 0.048]) were associated with non‐HScancers. Cancer incidence in patients with SLEwas higher than in the Spanish population, particularly among young women. This increase might be due to non‐HScancers, which would be associated with SLEinvolving greater cumulative damage where more ACEinhibitors are prescribed.

49. HIV treatment cascade for older adults in rural South Africa

ObjectivesThe HIV treatment cascade is a powerful framework for understanding progress from initial diagnosis to successful treatment. Data sources for cascades vary and often are based on clinical cohorts, population cohorts linked to clinics, or self-reported information. We use both biomarkers and self-reported data from a large population-based cohort of older South Africans to establish the first HIV cascade for this growing segment of the HIV-positive population and compare results using the different data sources.MethodsData came from the Health and Aging in Africa: A Longitudinal Study of an INDEPTH Community in South Africa (HAALSI) 2015 baseline survey of 5059 adults aged 40+ years. Dried blood spots (DBS) were screened for HIV, antiretroviral drugs and viral load. In-home surveys asked about HIV testing, diagnosis and antiretroviral therapy (ART) use. We calculated proportions and CIs for each stage of the cascade, conditional on attainment of the previous stage, using (1) biomarkers, (2) self-report and (3) both biomarkers and self-report, and compared with UNAIDS 90-90-90 targets.Results4560 participants had DBS results, among whom 1048 (23%) screened HIV-positive and comprised the denominator for each cascade. The biomarker cascade showed 63% (95% CI 60 to 66) on ART and 72% (95% CI 69 to 76) of those on ART with viral suppression. Self-reports underestimated testing, diagnosis and ART, with only 47% (95% CI 44 to 50) of HIV-positive individuals reporting ART use. The combined cascade indicated high HIV testing (89% (95% CI 87 to 91)), but lower knowledge of HIV-positive status (71% (95% CI 68 to 74)).ConclusionsOlder South Africans need repeated HIV testing and sustained ART to reach 90-90-90 targets. HIV cascades relying on self-reports are likely to underestimate true cascade attainment, and biomarkers provide substantial improvements to cascade estimates.

- Abstract
Objective.To describe the incidence of bacteremia in a large multicentric cohort of patients with systemic lupus erythematosus (SLE) and their clinical characteristics and to identify risk factors.Methods.All bacteremic episodes from the Spanish RELESSER registry were included. Clinical and laboratory characteristics concerning bacteremia and SLE status, as well as comorbidities at the time of infection, were retrospectively collected. A comparison with sex- and age-matched SLE controls without bacteremia was made. A logistic regression was conducted.Results.The study included 114 episodes of bacteremia in 83 patients. The incidence rate was 2.7/1000 patient-years. At the time of bacteremia, the median age was 40.5 (range: 8–90) years, and 88.6% of patients were female. The Safety of Estrogens in Lupus Erythematosus National Assessment–Systemic Lupus Erythematosus Disease Activity Index was 4 [interquartile range (IQR) 8]; 41% had an SLE flare (66% severe); Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index was 3 (IQR 4). A comorbidity was recorded in 64% of cases. At the time of bacteremia, 88.6% received corticosteroids (68.6% > 10 mg/day) and 57% immunosuppressors. Gram-negative bacilli, most frequently Escherichia coli(29.8%), caused 52.6% of the episodes. The bacteremia-related mortality was 14% and bacteremia was recurrent in 27.2% of cases. A dose-response relationship was found between corticosteroids and bacteremia risk. In the multivariate analysis, these factors were associated with bacteremia: elevated creatinine (OR 1.31, 95% CI 1.01–1.70; p = 0.045), diabetes (OR 6.01, 95% CI 2.26–15.95; p < 0.001), cancer (OR 5.32, 95% CI 2.23–12.70; p < 0.001), immunosuppressors (OR 6.35, 95% CI 3.42–11.77; p < 0.001), and damage (OR 1.65, 95% CI 1.31–2.09; p < 0.001).Conclusion.Bacteremia occurred mostly in patients with active SLE and was frequently associated with severe flares and corticosteroid use. Recurrence and mortality were high. Immunosuppressors, comorbidities, and disease-related damage were associated with bacteremia.
