- Abstract
Post‐pulmonary embolism (PE) syndrome is an important clinical condition that can affect the long‐term prognosis after acute PE. We aimed to evaluate the prevalence of residual pulmonary thrombi and the thrombotic burden 1 year after acute PE, by using our refined computed tomography (CT) imaging method. In this prospective study, patients diagnosed with acute PE were recruited and examinations were conducted at 1 month, 6 months, and 1 year. Especially at 1 year, patients were evaluated multifacetedly, including by laboratory tests, quality‐of‐life, 6‐min walking test, and enhanced CT. Fifty‐two patients were enrolled. Two patients (3.8%) developed chronic thromboembolic pulmonary hypertension. A total of 43 patients completed evaluation at 1 year, among whom (74%) had residual thrombi, with a median modified CT obstruction index (mCTOI) of 10.7%. In multivariate analysis, residual thrombi at 1 month was the only factor significantly related to residual thrombi at 1 year (odds ratio, 103.4; 95% confidence interval, 4.2–2542.1). The tricuspid regurgitation pressure gradient ≥60 mmHg and left ventricular end‐diastolic dimension at diagnosis were significantly related to mCTOI at 1 year (β= 0.367, P= .003; and β= –0.435, P= .001, respectively). Using our improved CT imaging protocol, we found a high prevalence of residual thrombi 1 year after acute PE. Furthermore, right ventricular overload was related to the thrombotic burden. The long‐term treatment strategy of acute PE could be modified to include precise CT imaging.

- Abstract
It is important to evaluate whether a new treatment for heart failure with reduced ejection fraction (HFrEF) provides additive benefit to background foundational treatments. As such, we aimed to evaluate the efficacy and safety of empagliflozin in patients with HFrEF in addition to baseline treatment with specific doses and combinations of disease-modifying therapies.

- Abstract
A prospective randomized open-labeled study was performed to compare the renoprotective effects of enarapril and losartan on the development of microalbuminuria in type 2 diabetic patients with hypertension. Diabetic patients who have hypertension and microalbuminuria at base line (n=19) were recruited. Enarapril (n=11) or losartan (n=8) was randomly chosen by envelope methods and was prescribed for one year. At the end of this study, the blood pressure of patients in these two groups decreased significantly. Urine albumin-creatinine ratio (U-ACR) also decreased in these two groups, however, U-ACR of enarapril group was not significantly different from that of losartan group at any time of this study. Body mass index, HbA1c, blood pressure, and serum lipid profiles were not significantly different between the two groups. From these results, we conclude that the effects of enarapril and losartan on the development of microalbuminuria in type 2 diabetic patients along with hypertension seem to be equivalent in terms of clinical renoprotection.

- Abstract
Aims.We investigate how ellipticity, asymmetries and substructures separately affect the ability of galaxy clusters to produce strong lensing events, i.e. gravitational arcs, and how they influence the arc morphologies and fluxes. This is important for studies aiming, for example, at constraining cosmological parameters from statistical lensing, or at determining the inner structure of galaxy clusters through gravitational arcs.

- Abstract
We have studied the generation of magnetic fields by the Biermann mechanism in the pair-instability supernovae explosions of the first stars. The Biermann mechanism produces magnetic fields in the shocked region between the bubble and interstellar medium (ISM), even if magnetic fields are absent initially. We have performed a series of two-dimensional magnetohydrodynamic simulations with the Biermann term and estimate the amplitude and total energy of the produced magnetic fields. We .nd that magnetic fields with amplitude 10–14–10–17 G are generated inside the bubble, though the amount of magnetic fields generated depend on specific values of initial conditions. This corresponds to magnetic fields with total energy of 1028–1031 erg per each supernova remnant, which is strong enough to be the seed magnetic field for a galactic and/or interstellar dynamo. - Abstract
A single i.v. injection of daunomycin (10 mg/kg) into rats produced severe proteinuria and hypercholesterolemia without atherosclerosis on the 20th and 40th days after the treatment. However, these changes were not observed on the 5th day. No change in systolic blood pressure was seen through the 40-day experimental period. Relaxation to acetylcholine, A23187 and nitroprusside was examined in aortic rings precontracted with phenylephrine (3 x 10(-6) M). Acetylcholine-induced relaxation was significantly attenuated in the nephrotic rats on the 20th and 40th days, in comparison to the control animals. In aortic rings taken from control and nephrotic rats on the 40th day, removal of the endothelium or treatment with methylene blue (10(-5) M) completely abolished the relaxation induced by acetylcholine (10(-5) M). In addition, acetylcholine (10(-5) M) induced a transient increase in the aortic cyclic GMP and this increase was completely abolished by removal of the endothelium. In the preparations of nephrotic rats on the 20th and 40th days, the cyclic GMP levels stimulated by acetylcholine (10(-5) M) were decreased to about 50% in comparison to their respective control. A23187 also evoked diminished relaxation in nephrotic rats on the 20th and 40th days. However, on the 40th day after the treatment, the effects of nitroprusside in relaxing the aorta and in elevating the cyclic GMP level in the aorta were not altered by nephrosis. In addition, the nitroprusside-induced relaxation and cyclic GMP accumulation were not affected by removal of the endothelium. These results indicate that endothelium-dependent relaxation is attenuated with the development of nephrosis.

- Abstract
The plasma concentrations of cortisol, sodium, potassium and calcium and plasma osmolarity were determined in freshwater silver eels, after intravascular injections of eel renin preparations, mammalian ACTH, mammalian angiotensin II and eel muscle extracts. Arterial blood specimens were taken before and after injection of test substances. Partially purified eel and rat renal renins gave prolonged pressor responses in intact and hypophysectomized eels and in the nephrectomized rat anaesthetized with sodium pentobarbitone.Angiotensin, but not ACTH, produced obvious pressor responses in intact and hypophysectomized eels and in eels without their corpuscles of Stannius. Hypophysectomized eels 4–8 days after operation had reduced plasma cortisol concentrations. No change in cortisol occurred in eels after removal of the corpuscles of Stannius. Eel renin preparations and ACTH gave increased concentrations of plasma cortisol 30 min after injection into hypophysectomized and intact eels. In general, the length of the renin-generated pressor response and the increased cortisol concentration were concomitant occurrences.Angiotensin injected into eels with corpuscles of Stannius removed and into hypophysectomized eels also increased cortisol levels. Control muscle extracts produced no significant changes. There were no acute changes in plasma electrolyte concentrations after the injections.Plasma renin activity measured indirectly by bioassay of angiotensin generated in vitrowas more than twice as great in eels adapted to seawater than in eels in fresh water. Plasma renin activity gradually fell when eels were transferred from seawater to fresh water, and increased when the reverse transfer was carried out.

14. Nephrosis augments contractile response to adrenoceptor agonists by the decrease in release of endothelium-derived relaxing factor from the endothelial cells.

- Abstract
The thoracic aorta was taken from nephrotic rats on the 40th day after a single i.v. injection of daunomycin (10 mg/kg). In the endothelium-intact aorta, the contractions induced by norepinephrine or B-HT 933, an alpha-2 adrenoceptor agonist, were significantly greater in nephrotic rats than in normal animals. However, such a difference was not observed in the KCl- or U46619-induced contractions. The difference in norepinephrine-induced contraction between nephrotic and normal preparations was enhanced by zaprinast, a cyclic GMP phosphodiesterase inhibitor. The contractions elicited by norepinephrine and B-HT 933 were potentiated by either removal of the endothelium or pretreatment with methylene blue, a guanylate cyclase inhibitor. The difference in the contractile response to these agonists between nephrotic and normal preparations was eliminated completely by either treatment. The cyclic GMP level in the endothelium-intact aorta in the presence of zaprinast was significantly lower in nephrotic rats than in normal animals. In the presence of zaprinast, norepinephrine, but not B-HT 933, caused an increase in the cyclic GMP level, which was abolished completely by pretreatment with prazosin, but not by yohimbine. These results suggest that the augmented contractile response to norepinephrine observed in nephrotic aorta may be due to the decrease in the stimulated release of endothelium-derived relaxing factor from the endothelial cells via the stimulation of endothelial alpha-1 adrenoceptors, whereas the augmented response to B-HT 933 may be due, at least in part, to the decrease in spontaneously released endothelium-derived relaxing factor.
