Metastatic melanoma lymph nodes (MMLns) might be challenging to detect on MR-WBI, as both MMLns and normal lymph nodes (NLns) can show restricted water diffusion. Our purpose is to assess the potential contribution of the DIXON sequence in differentiating MMLns from NLns.

BackgroundPaediatric procedural sedation using propofol has been shown to be safe and effective and is widely used. Pain at the injection site is a frequent complaint and can be particularly distressing for children, especially for those undergoing repeated procedures. Ketamine has analgesic properties and can diminish the incidence of propofol infusion pain in adults. The aim of the study was to investigate whether pretreatment with ketamine would reduce infusion line pain in propofol sedation in children.

Abstract: Background: We report our preliminary results with endovascular treatment of abdominal aortic aneurysms (AAA). Methods: Between October 1998 and June 2000, 64 patients (62 male, two female; mean age = 70 years) underwent endovascular repair of AAA. Different types of prostheses were used, both bifurcated (n= 58) and straight (n= 6). We performed duplex sonography and spiral computed tomographic angiography (CTA) at discharge and at 3, 6, 12, and 18 months. Follow-up ranged from 1 to 20 months. Results: All procedures were successful, except for three immediate and one late surgical conversions (6.2%). One patient died 14 days after immediate surgical conversion. At discharge, CTA showed 13 endoleaks: three resolved spontaneously, six persisted during follow-up, and four (one angioplasty and three embolizations) were treated successfully. Stenosis of an iliac branch occurred in one patient after 3 months and was successfully treated by angioplasty. Late endoleaks were detected by imaging follow-up in four cases, three at 1 year and one at 6 months, requiring deployment of distal extender cuffs (n= 2), a proximal cuff (n= 1), and lumbar embolization (n= 2). Conclusion: Our preliminary experience supports the efficacy of endovascular repair in selected patients, but strict and accurate follow-up is required.

A case of ectopic pancreas located on the anterior gastric wall treated with a combined gastroscopic and laparoscopic approach, which permitted isolation and complete resection of the lesion, is reported. The patient was discharged without complication.

We describe a case of graft-related pseudoaneurysm of the abdominal aorta evaluated with spiral CT and DSA. Spiral CT data sets were processed to obtain surface-rendered internal views (virtual angioscopy, VA) of the graft and the pseudoaneurysm, and to demonstrate from inside the lumen the site of dehiscence. A jet flow phenomenon inside the pseudoaneurysm was observed at DSA. Spiral-CT axial images, multiplanar volume reconstructions with maximum intensity projections, and shaded surface display showed the site of rupture. The VA findings were: (a) from inside the graft lumen, the evidence of a communication canal between the graft and the pseudoaneurysm; and (b) from inside the pseudoaneurysm, the presence of a jet flow. The VA findings showed good correlation with those obtained with the other imaging techniques.

Abstract.: Since the introduction of helical scanners, CT angiography (CTA) has achieved an essential role in many vascular applications that were previously managed with conventional angiography. The performance of CTA is based on the accurate selection of collimation width, pitch, reconstruction spacing and scan delay, which must be modulated on the basis of the clinical issue. However, the major improvement of CT has been provided by the recent implementation of many post-processing techniques, such as multiplanar reformatting, shaded surface display, maximum intensity projections, 3D perspectives of surface and volume rendering, which simulate virtual intravascular endoscopy. The integration of the potentialities of the scanner and of the image processing techniques permitted improvement of: (a) the evaluation of aneurysms, dissection and vascular anomalies involving the thoracic aorta; (b) carotid artery stenosis; (c) aneurysms of abdominal aorta; (d) renal artery stenosis; (e) follow-up of renal artery stenting; and (f) acute or chronic pulmonary embolism. Our experience has shown that the assessment of arterial pathologies with CTA requires the integration of 3D post-processing techniques in most applications.

β-endorphin (β-EP) levels increase in maternal plasma starting from the 10th to the 12th week of pregnancy and reach a peak during labor and at delivery. Respiratory autogenous training (RAT) has been acclaimed as one of the most effective non-pharmacological methods of obtaining hypoalgesia and relaxing perineal muscles during labor. In order to study the effects of the RAT method in both maternal and fetal β-EP levels during labor, 28 pregnant women were enrolled in this study. Fourteen attended a RAT program, while the others (control group) did not. β-EP levels in both groups were measured in maternal plasma at early and late labor, at delivery and on the 4th day of the puerperium and in umbilical cord blood at birth. From late labor on, β-EP maternal plasma levels increased significantly less in the RAT group than in the control group. The gap in the β-EP levels between both groups was still significant at puerperium and in umbilical cord blood. In conclusion, the RAT method favorably modulates the secretion of β-EP during labor and at delivery. This result supports the effectiveness of the RAT method in reducing both maternal and fetal stress during labor.

Abstract: Virtual endoscopy enables the creation of endoluminal views of the aorta and its branches by processing spiral computed tomographic (CT) images, thereby allowing the preoperative and postoperative evaluations of abdominal aortic aneurysms, aneurysms of the splenic, celiac, and common iliac arteries, and renal artery stenoses. Moreover, it is helpful for verifying the position of stents and endoprostheses from within the aortic lumen. This method is a promising addition to spiral CT.

To evaluate the feasibility of magnetic resonance (MR) virtual endoscopy of the pancreaticobiliary tract by using MR cholangiopancreatography (MRCP) data sets as source images, we retrospectively reviewed MRCP data sets of 120 patients with Navigator software (GE/Medical Systems, Milwaukee, WI) that allowed display of inner views by surface rendering the internal wall of the bile ducts with simulated light and shadow.

WHEN dissolved in water, the natural slimes of most of the fishes we have tested have a remarkable capacity to decrease greatly the friction of water when flowing in the turbulent state. In one species, a dilute solution of its slime was found to reduce the friction of water by as much as 65.9%; reductions of 57% to 63% were frequently obtained with the slimes of other species. These measurements have led us to the view that the slimes of many (but not all) fishes assist in locomotion by reducing the drag or resistance which water offers to their bodies.

Metal leaf (gold, silver or their alloys) glass tesserae began to be used in wall mosaics in the first century AD (the first examples are in Rome) and their use has been uninterrupted up to day. The metal leaf could be obtained from circulating coins, jewellery or refining. According to various techniques that have changed over the centuries, the leaf was hot fixed between two glass layers. From an archaeological point of view, it is interesting to know when and where these tesserae were made, if they were new made or if they were reused tesserae recovered from earlier dismantled mosaics. The determination of the glass composition of the tesserae is not of great help in this connection, for the same kind of glass was used over long periods. Available information is still scanter for glasses produced between the 1st to 8th centuries when the batch of raw materials (a natural soda called natron and a silica-lime sand) was melted in large tank furnaces and chunks of raw glass were transported all over the Mediterranean to be remelted and shaped into manufacts in small pot furnaces. The SEM-EDS analysis is proposed in this study as a useful tool to investigate the composition of both the glass and the gold alloy in leaf tesserae from mosaics of the 1st - 9th centuries. The comparison of the composition of the gold leaf of the tesserae with that of circulating gold coins (for which an important analytical data base is available), adds further information to the glass analysis, allowing us to improve the dating of the tesserae and increase the knowledge that may result from scientific analyses. The results demonstrate that good quantitative analyses of the metal leaf can be performed and that metal leaves made of pure gold or gold-silver alloys were used.
