A systematic review, conducted to ascertain the most effective local anaesthetic agent and volume for cataract surgery, found that 5mL 2% lidocaine (or 2% or 4% articaine) will provide sufficient and timely akinesia and analgesia for cataract surgery. Green tea catechins, especially (?)-epigallocatechin-3-gallate (EGCG), are known to regulate obesity and fat accumulation. We performed a kinetic analysis in a cell-free system to determine the mode of inhibition of glycerol-3-phosphate dehydrogenase (GPDH; EC 1.1.1.8) by EGCG. GPDH catalyzes the ?-nicotinamide adenine dinucleotide (NADH)-dependent reduction of dihydroxyacetone phosphate (DHAP) to yield glycerol-3-phosphate, which serves as one of the major precursors of triacylglycerols. We found that EGCG dose-dependently inhibited GPDH activity at a concentration of approximately 20??M for 50?% inhibition. The IC 50values of other green tea catechins, such as (?)-epicatechin, (?)-epicatechin-3-gallate, and (?)-epigallocatechin, were all above 100??M. This suggests a catechin type-dependent effect. Based on double-reciprocal plots of the kinetic data, EGCG was a noncompetitive inhibitor of the GPDH substrates, NADH and DHAP, with respective inhibition constants (Ki) of 18 and 31??M. OBJECTIVE--The aim of the current study was to evaluate the long-term effects of leptin on glucose metabolism, diabetes complications, and life span in an insulin-dependent diabetes model, the Akita mouse. RESEARCH DESIGN AND METHODS--We cross-mated Akita mice with leptin-expressing transgenic (LepTg) mice to produce Akita mice with physiological hyperleptinemia (LepTg:Akita). Metabolic parameters were monitored for 10 months. Pair-fed studies and glucose and insulin tolerance tests were performed. The pancreata and kidneys were analyzed histologically. The plasma levels and pancreatic contents of insulin and glucagon, the plasma levels of lipids and a marker of oxidative stress, and urinary albumin excretion were measured. Survival rates were calculated. RESULTS Akita mice began to exhibit severe hyperglycemia and hyperphagia as early as weaning. LepTg:Akita mice exhibited normoglycemia after an extended fast even at 10 months of age. The 6-h fasting blood glucose levels in LepTg:Akita mice remained about half the level of Akita mice throughout the study. Food intake in LepTg:Akita mice was suppressed to a level comparable to that in WT mice, but pair feeding did not affect blood glucose levels in Akita mice. LepTg:Akita mice maintained insulin hypersensitivity and displayed better glucose tolerance than did Akita mice throughout the follow-up. LepTg:Akita mice had normal levels of plasma glucagon, a marker of oxidative stress, and urinary albumin excretion rates. All of the LepTg:Akita mice survived for > 12 months, the median mortality time of Akita mice. CONCLUSIONS--These results indicate that leptin is therapeutically useful in the long-term treatment of insulin-deficient diabetes. OBJECTIVE--Metreleptin has been efficacious in improving metabolic control in patients with lipodystrophy, but its efficacy has not been tested in obese patients with type 2 diabetes. RESEARCH DESIGN AND METHODS--We studied the role of leptin in regulating the endocrine adaptation to long-term caloric deprivation and weight loss in obese diabetic subjects over 16 weeks in the context of a double-blinded, placebo-controlled, randomized trial. We then performed detailed interventional and mechanistic signaling studies in humans in vivo, ex vivo, and in vitro. RESULTS--In obese patients with diabetes, metreleptin administration for 16 weeks did not alter body weight or circulating inflammatory markers but reduced [HbA.sub.1c] marginally (8.01 ± 0.93-7.96 ± 1.12, P = 0.03). Total leptin, leptin-binding protein, and antileptin antibody levels increased, limiting free leptin availability and resulting in circulating free leptin levels of ~50 ng/mL. Consistent with clinical observations, all metreleptin signaling pathways studied in human adipose tissue and peripheral blood mononuclear cells were saturable at ~50 ng/mL, with no major differences in timing or magnitude of leptin-activated STAT3 phosphorylation in tissues from male versus female or obese versus lean humans in vivo, ex vivo, or in vitro. We also observed for the first time that endoplasmic reticulum (ER) stress in human primary adipocytes inhibits leptin signaling. CONCLUSIONS--In obese patients with diabetes, metreleptin administration did not alter body weight or circulating inflammatory markers but reduced [HbA.sub.1c] marginally. Purpose. The purpose of this qualitative study was to obtain in-depth information on dental hygienists' awareness and opinions of oral cancer, oral cancer examinations, and related factors. These findings were intended to supplement a previous statewide survey of Maryland dental hygienists on the subject. Methods. A professional focus group moderator conducted two focus groups in Maryland. Two types of focus groups were used--one face-to-face focus group session with 10 dental hygienists in the Baltimore are and one telephone focus group among 7 dental hygienists who practiced on the Eastern Shore. Criterion-purposeful sampling and qualitative content analysis were used. Results. Six major themes emerged from the focus groups: dental hygienists' lack of awareness of Maryland's oral cancer statistics, level of training to provide oral cancer examinations, provision of oral cancer examinations and barriers for not providing them, reactions to Maryland surveys of dental hygienists and dentists, assessment of oral cancer risk factors, and interest in additional training. Conclusions. The focus groups provided in-depth information about why oral cancer examinations are or are not provided on a routine basis, as well as ideas for providing updates on oral cancer prevention and early detection for dental hygienists. To estimate the probabilities of various factors at various time intervals for duration of exclusive breastfeeding, the present study was done at the well baby clinic. Mothers with infants below one year of age were selected and interviewed. The median duration of exclusive breastfeeding (EBF) was found 6 months. Relationship between duration of EBF and practice of breastfeeding (on demand or at fixed time) was studied and found statistically not significant (p>0.05) by log rank test. Other factors affecting EBF were also studied like literacy, type of family and monthly family income. Median duration of EBF was found more (7 months) for illiterate women than for literate (6 months) and statistically significant (p0.05). A statistically significant difference was observed while testing two different groups based on total family income. A cornual pregnancy is an ectopic pregnancy that develops in the interstitial portion of the fallopian tube invading through the uterine wall. Cornual pregnancies often rupture later than other tubal pregnancies because the myometrium is more distensible than the fallopian tube. Increased vascularity associated with interstitial ectopic pregnancies is more likely to result in a catastrophic hemorrhage and death. The mortality rate of interstitial pregnancy is more than twice that of other tubal pregnancies. We report a case where cornual pregnancy was diagnosed at 18 weeks of gestation after uterine rupture and profound hemorrhage occurred. The patient underwent hysterectomy. Close monitoring of pregnancies in these patients is important to prevent a deleterious delay in treatment of a cornual pregnancy. Purpose. The aim of this study was to determine the knowledge, opinions, and practices of dental hygienists in the areas of dental caries etiology and prevention. Methods. A pretested, validated 35-question survey instrument was mailed to a one percent national, stratified random sample of dental hygienists in the United States (n=960) in October 1996. Four complete mailings of the instrument resulted in a 67% response rate, of which 77% were useable (n=498). Respondents were asked if they agreed or disagreed with statements on caries etiology and preventive procedures, and also asked to rate the effectiveness of procedures for preventing dental caries in children and adults. Data were analyzed using descriptive statistics, t-tests, and ANOVA. Results. Overall level of knowledge of caries etiology and preventive procedures was low. More than 40% of subjects did not recognize that remineralization is the most important mechanism of action of fluoride, and fewer than 50% recognized that dental caries is a chronic infectious disease. Analysis of four factors thought to be related to knowledge and practice showed that younger graduates, more recent graduates, and ADHA members were more knowledgeable about the effectiveness of caries preventive procedures for children (p [is less than] .01). Although an overwhelming majority correctly agreed that adults benefit from fluoride and that root caries is an emerging problem, this knowledge was inconsistent with practice (p=.02). Fewer than 35% reported that they provide fluoride to adults of any age, or they wait until the disease is present. For children and adults who did receive fluoride treatments, a one-minute application of an APF gel or foam was most often provided. Conclusions. Correct information about the etiology and prevention of dental caries is a predisposing factor to making informed decisions. Overall, respondents overrated the effectiveness of flossing and toothbrushing while underrating the effectiveness of fluorides. In the majority of cases, efficiency rather than efficacy was given priority when providing a topical fluoride treatment. An improved understanding of dental caries etiology and the scientific evidence for appropriate caries preventive procedures will allow dental hygienists to prevent and manage this disease better.
