Efficacy and costs of empirical antibacterial therapy in febrile neutropenic patients are important issues. Several strategies have been reported to be similarly effective: monotherapy with cefepime, ceftazidime or a carbapenem or duotherapy with an antipseudomonal β-lactam antibiotic or ceftriaxone in combination with an aminoglycoside. Piperacillin-tazobactam monotherapy is promising, but its role in this setting still has to be defined.Of 212 consecutive febrile episodes in 130 neutropenic patients with hematological malignancies randomized to receive either piperacillin-tazobactam (4.5 g every 8 h; group A) or ceftriaxone (2 g once daily plus gentamicin 5 mg/kg once daily; group B), 183 episodes (98 group A, 85 group B) were evaluable for response.Defervescence within 72 h without modification of the antibiotic therapy was achieved in 56/98 episodes (57.1%) in group A and in 30/85 (35.3%) in group B (P=0.0047). If fever persisted, teicoplanin plus gentamicin (group A) or teicoplanin plus ciprofloxacin (group B) were added. All patients still febrile then received meropenem, teicoplanin and amphotericin B. With these modifications of antibiotic therapy, 89.8% of patients in group A had responded at 21 days but only 71.8% in group B (P=0.005). The mean total antibiotic drug cost in group A was only 39.4% of that in group B (€445 versus €1129;P=0.010).Piperacillin-tazobactam monotherapy is significantly more effective and cost-efficient than ceftriaxone plus gentamicin as first-line therapy in febrile neutropenic patients with hematological malignancies.

Abstract. Ultrasound guidance for percutaneous puncture of the internal jugular vein provides many advantages over the classic landmark-guided technique, particularly in complicated cases (e.g. thrombocytopenia, obesity, dyspnea). The present prospective investigation involved analysis of 493 punctures and provides patient- and operator-dependent variables with respect to the impact on puncture success and the complication rate. These 493 punctures of the internal jugular vein were performed using identical puncturing equipment and a standardized two-operator catheterization technique and were prospectively recorded on the hematology-oncology ward of a university hospital. Alongside success rates, the frequency and nature of complications, patient-inherent risk variables (obesity, thrombocytopenia, patient cooperation, vein diameter, etc.) and the individual experience of the physician performing the puncture and ultrasound were analyzed with respect to possible impact on success and complication rate. Internal jugular vein cannulation was successful in 94.5% of all patients. Catheter placement was successful at the first attempt in 87.6% of cases. Arterial fail punctures occurred in 1.4% of the patients and local hematoma in a further 4.3%. Among the patient-dependent variables, only poor patient compliance and a maximum vein diameter smaller than 7 mm showed a negative influence on the success rate. The experience of the physician carrying out the puncture influenced neither the success rate nor the complication rate. In contrast, both failure and complication rates were significantly lower when the physician guiding the sonographic probe was familiar with the method. Ultrasound-guided cannulation of the internal jugular vein provides safe central venous access with high success rates and low complication rates. Difficulties due to patient-inherent risk factors (e.g. thrombocytopenia, obesity, dyspnea) can be managed well using ultrasonographic guidance. The success rate achieved and the frequency of complications are decisively influenced not by the experience of the physician performing the puncture, but by the experience of the physician acting as sonographer.

Alkylene-bis(monophenylphosphinechalcogenides), Ph(H)P(E)(CH2)n(E)P(H)Ph (E = S, n = 2-4, 6 and E = Se, n = 2), were prepared by treating the appropriate alkylene-bis(monophenylphosphines) with sulfur or selenium, respectively. The diastereomers of the ethylene-bis(monophenylphosphine chalcogenides) (E = O, S, Se) reveal different 31P chemical shifts with increasing differences between those of the meso form and those of the racemic form going from the oxide to the selenide. The 13C NMR spectra of the diastereomeric pairs show only insignificant differences. Analysis of these spectra and of the 13C satellites of 31P spectra allowed us to obtain 3J(P - P) between the chemically equivalent phosphorus atoms. The 1H{31P} spectra of the ethylene protons of the meso form and the racemate are distinctly different. Analysis of these spectra made it possible to assign the diastereomers as well as the chemical shifts of the diastereotopic CH2 protons.The trans conformation of the phosphorus-containing groups could be proved to predominate in all the diastereomers investigated.Alkylen-bis(monophenylphosphinchalkogenide), Ph(H)P(E)(CH2)n(E)P(H)Ph (E = S, n = 2, 3, 4, 6 und E = Se, n = 2) wurden aus den entsprechenden Alkylen-bis(monophenylphosphinen) und Schwefel bzw. Selen dargestellt. Die Diastereomere der Ethylen-bis(monophenylphosphinchalkogenide) (E = O, S, Se) zeigen unterschiedliche 31P-chemische Verschiebungen, wobei die Differenzen zwischen meso-Form und Racemat vom Oxid zum Selenid zunehmen. In den 13C-Spektren wersen die Diastereomerenpaare nur sehr geringfügige Unterschiede auf. Durch Analyse dieser Spektren bzw. aus den 13C-Satelliten der 31P-Spektren konnten die 3J(P - P)-Werte zwischen den chemisch äquivalenten Phosphoratomen bestimmt werden.Besonders deutlich unterscheiden sich meso-Form und Racemat im 1H {31P}-Spektrum der Ethylen-protonen. Die Analyse dieser Spektren führte zur Zuordnung der Diastereomeren sowie der chemischen Verschiebungen der diastereotopen CH2-Protonen. Es konnte nachgewiesen werden, daß in allen untersuchten Diastereomeren die trans-Konformation der Phosphorgruppierungen vorherrscht.

Zusammenfassung Bei gesteigertem Zerfall von Erythrozyten wird eine blutgerinnungsaktive Substanz frei (Erythrozytin), die in der Lage ist, die Thromboplastinbildung anzuregen. Auf die Bedeutung dieses Vorganges für die Entstehung von Thrombosen bei hämolytischen Anämien wird hingewiesen. Therapeutische Möglichkeiten der Erythrozytinanwendung zeichnen sich bei Fällen ab, die durch eine verminderte Plättchenfaktor-3-Aktivität charakterisiert sind. Diesbezügliche Untersuchungen sind noch nicht abgeschlossen.

Zusammenfassung Es wird über einen Patienten berichtet, bei dem eine Agranulozytose durch das Medikament Privin® ausgelöst worden war; das Vorliegen allergischer Leukozyten-Antikörper konnte durch verschiedene Versuchsanordnungen gesichert werden. Die einzelnen Nachweisverfahren werden beschrieben. Der Antikörpernachweis gelang noch fünf Wochen nach der zytopenischen Krise. Dies wird als Hinweis auf die von einigen Autoren angezweifelte Existenz stabiler antileukozytärer Antikörper angesehen.

Zusammenfassung Es wird über ein tierexperimentelles Modell berichtet, mit dem es beim Kaninchen gelingt, das Krankheitsbild der serogenen hämolytischen Anämie zu erzeugen. Als Antigen dienten körpereigene Erythrozyten, die einer Hitze-einwirkung ausgesetzt worden waren. Ihre intravenöse Injektion bewirkte eine Produktion von antierythrozytären 7 S-Globulinen in der Milz, die in jedem Falle auch charakteristische histologische Veränderungen zeigte.

Zusammenfassung Bei 7 Patienten mit einer chronischen Leberparenchymschädigung und einer begleitenden hämolytischen Anämie konnte nachgewiesen werden, daß zwischen beiden ein ursächlicher Zusammenhang besteht. Infolge der gestörten Entgiftungsfunktion der Leber gegenüber Arzneimitteln kommt es bei häufiger Verabreichung zu einer Retention im Serum. Mit der Retention geht das Auftreten einer Hämolyse parallel. Jede weitere Zufuhr des betreffenden Arzneimittels bewirkt einen erneuten hämolytischen Schub mit Vermehrung von nichtkonjugiertem Bilirubin im Serum. Daraus resultiert die Forderung nach sparsamster Verwendung von Arzneimitteln bei der chronischen Hepatopathie.
