This article reports findings from a national assessment of drug problems and antidrug initiatives in small cities and towns in the United States. It examines the importance of drug problems in small jurisdictions, compared with other problems, the type of drug problems reported, the 'most important' antidrug programs, and certain other issues. Implications for drug policy are discussed.

Individuals at risk of rheumatoid arthritis (RA) demonstrate systemic autoimmunity in the form of anti-citrullinated peptide antibodies (ACPA). MicroRNAs (miRNAs) are implicated in established RA. This study aimed to (1) compare miRNA expression between healthy individuals and those at risk of and those that develop RA, (2) evaluate the change in expression of miRNA from “at-risk” to early RA and (3) explore whether these miRNAs could inform a signature predictive of progression from “at-risk” to RA. We performed global profiling of 754 miRNAs per patient on a matched serum sample cohort of 12 anti-cyclic citrullinated peptide (CCP) + “at-risk” individuals that progressed to RA. Each individual had a serum sample from baseline and at time of detection of synovitis, forming the matched element. Healthy controls were also studied. miRNAs with a fold difference/fold change of four in expression level met our primary criterion for selection as candidate miRNAs. Validation of the miRNAs of interest was conducted using custom miRNA array cards on matched samples (baseline and follow up) in 24 CCP+ individuals; 12 RA progressors and 12 RA non-progressors. We report on the first study to use matched serum samples and a comprehensive miRNA array approach to identify in particular, three miRNAs (miR-22, miR-486-3p, and miR-382) associated with progression from systemic autoimmunity to RA inflammation. MiR-22 demonstrated significant fold difference between progressors and non-progressors indicating a potential biomarker role for at-risk individuals. This first study using a cohort with matched serum samples provides important mechanistic insights in the transition from systemic autoimmunity to inflammatory disease for future investigation, and with further evaluation, might also serve as a predictive biomarker.

The iconic helical structure of DNA is stabilized by the solvation environment, where a change in the hydration state can lead to dramatic changes to the DNA structure. X-ray diffraction experiments at cryogenic temperatures have shown crystallographic water molecules in the minor groove of DNA, which has led to the notion of a spine of hydration of DNA. Here, chiral nonlinear vibrational spectroscopy of two DNA sequences shows that not only do such structural water molecules exist in solution at ambient conditions but that they form a chiral superstructure: a chiral spine of hydration. This is the first observation of a chiral water superstructure templated by a biomolecule. While the biological relevance of a chiral spine of hydration is unknown, the method provides a direct way to interrogate the properties of the hydration environment of DNA and water in biological settings without the use of labels.

AbstractIntroduction:The aim of the current study was to assess the impact of an intervention aimed at communicating the negative reinforcement explanation for smoking, a set of ideas derived from popular self-help books, upon participants' cognitions and urges to smoke. Methods: Smokers (n= 205) undergoing standard stop-smoking treatment were randomised to receive either the experimental intervention, a brief intervention aimed at communicating the explanation or a control intervention, a video on the health risks of smoking. Outcomes were participants' acceptance of the negative reinforcement explanation for smoking, positive outcome expectations for smoking, self-efficacy and urges to smoke reported at one week post-cessation. Results: Post-cessation urges to smoke were similar in the two groups (Adjusted expt. group mean = 2.50, Control group mean = 2.75, F(1,60) = 0.98, p= .33). Other cognitive measures were also unchanged. Conclusions: The brief cognitive intervention offered as an adjunct to standard care failed to reduce urges to smoke or alter smokers' cognitions. Changing smokers' cognitions may be as challenging as changing their behaviours. Suggestions are provided for further research.

Planning robotic radiosurgery treatments for multiple (n > 3) metastatic brain lesions is challenging due to the need of satisfying a large number of dose-volume constraints and the requirement of prescribing different dose levels to individual targets. In this study, we developed a sequential two-step optimization technique to improve the planning quality of such treatments. In contrast to the conventional approach of where all targets are simultaneously planned, we have developed a two-step optimization method. In this method, the first step was to create treatment plans for individual targets. In the second step, the 3D dose matrices associated with each plan were exported to Dicom-RT digital files and subsequently optimized. For the optimization, a singular-value-decomposition (SVD) algorithm was implemented to minimize the dose interferences among different targets. Finally, we compared the optimized treatment plans with the treatment plans created using the conventional method to determine the effectiveness of the new method. Large improvements in target dose distributions as well as normal brain sparing were found for the two-step optimization treatment plans as compared with the conventional treatment plans. The two-step optimization significantly lowered the volume of normal brain receiving relatively low doses. For example, the normal brain volume receiving 12-Gy was reduced by averaged 42% (range 34%–47%) with the two-step optimization. Such improvements generally enlarged with increasing number of targets being treated regardless of target sizes. Of note, normal brain dose was found to increase non-linearly with increasing number of targets. In summary, a two-step optimization technique is demonstrated to significantly improve the treatment plan quality as well as reduce the planning effort for multi-target robotic radiosurgery.

In spinal muscular atrophy (SMA), weakness, decreased endurance, and fatigue limit mobility. Scales have been developed to measure function across the wide spectrum of disease severity. However, these scales typically are observer dependent, and scores are based on sums across Likert-scaled items. The Six-Minute Walk Test (6MWT) is an objective, easily administered, and standardized evaluation of functional exercise capacity that has been proven reliable in other neurologic disorders and in children.

To investigate the incidence of and risk factors for cognitive impairment in a large, well-defined clinical trial cohort of patients with early Parkinson disease (PD).

The large intestinal mucosa contains immunological structures that may potentially serve as a site for induction of mucosal immunity against infections. Adenovirus (Ad), which is effective in gene transfer to epithelia, may be an ideal antigen delivery system for vaccination at the large intestinal mucosa. To investigate this potential, we immunized mice with recombinant replication-deficient Ad through a single intracolorectal (ICR) administration. Effective transfer of encoded genes was found in both the epithelial layer and lamina propria of the colorectal mucosa. Dendritic cells were able to transfer antigen to the draining lymph nodes, where antigen-specific CD8+T cells were primed. Functional antigen-specific CD8+T cells and IgA-specific antibodies were detected during the effector phase in the large intestine. Compared to other immunization routes (intranasal, subcutaneous), ICR immunization induced stronger colorectal immune responses and more potent protection against rectal challenge with pathogenic viruses. Further, this immunization strategy provided vaginal protection, more potent than that induced by vaccination in the nose or skin. Therefore, large intestine mucosal immunization using Ad represents an effective vaccination strategy against virus infection at both rectal and vaginal mucosal tissue sites.Mucosal Immunology (2008) 1, 78–88. doi:10.1038/mi.2007.3

To determine the rate of disease progression in Charcot-Marie-Tooth disease type 1A (CMT1A).

Surgery is an effective, high-cost procedure used increasingly to treat refractory epilepsy. For surgery to be cost-effective, long-term cost savings from reduced health care use should provide some offset to the initial costs of evaluation and surgery. There is little information about how health care costs are affected by evaluation and surgery.

Phenol reaction cascades are commonly used in nature to create crosslinked materials that perform mechanical functions. These processes are mimicked by electrochemically initiating a reaction cascade to examine if the mechanical properties of a biopolymer film can be predictably altered. Specifically, thin films (≈ 30–45 μm) of the polysaccharide chitosan are cast onto gold‐coated silicon wafers, the chitosan‐coated wafers are immersed in catechol‐containing solutions, and the phenol is anodically oxidized. The product of this oxidation is highly reactive and undergoes reaction with chitosan chains adjacent to the anode. After reaction, the flexible chitosan film can be peeled from the wafer. Chemical and physical evidence support the conclusion that electrochemically initiated reactions crosslink chitosan. When gold is patterned onto the wafer, the electrochemical crosslinking reactions are spatially localized and impart anisotropic mechanical properties to the chitosan film. Further, deswelling of chitosan films can reversibly transduce environmental stimuli into contractile forces. Films patterned to have spatial variations in crosslinking respond to such environmental stimuli by undergoing reversible changes in shape. These results suggest the potential to enlist electrochemically initiated reaction cascades to engineer chitosan films for actuator functions.

To investigate whether serially assessed epidermal nerve fiber (ENF) density and quantitative sensory thresholds (QSTs) are associated with the clinical transition from HIV infection with no neuropathy or asymptomatic neuropathy to symptomatic distal sensory neuropathy (SDSP).

HIV infection is associated with a painful distal sensory polyneuropathy (DSP) that can severely limit the quality of life of affected subjects. The pathogenesis of DSP is unknown, although both HIV proteins and products of immune activation triggered by HIV infection have been implicated.

To investigate predictors of survival in Parkinson disease (PD).

To evaluate whether baseline levels of plasma and CSF HIV RNA, tumor necrosis factor alpha (TNF), monocyte chemoattractant protein-1 (MCP-1), matrix metalloproteinase-2 (MMP-2), or macrophage colony stimulating factor (M-CSF) are predictors of incident HIV-associated dementia (HIVD) in a cohort with advanced HIV infection.

A number of distinct subtypes of neuroblastoma exist with different genetic abnormalities that are predicative of outcome. Whole chromosome gains are usually associated with low stage disease and favourable outcome, whereas loss of 1p, 3p and 11q, unbalanced gain of 17q and MYCN amplification (MNA) are indicative of high stage disease and unfavourable prognosis. Although MNA and loss of 11q appear to represent two distinct genetic subtypes of advanced stage neuroblastoma, a detailed understanding of how these subtypes differ in terms of global gene expression is still lacking. We have used metaphase comparative genomic hybridization (CGH) analysis in combination with oligonucleotide technology to identify patterns of gene expression that correlate with specific genomic imbalances found in primary neuroblastic tumours and cell lines. The tumours analysed in this manner included a ganglioneuroma, along with various ganglioneuroblastoma and neuroblastoma of different stages and histopathological classifications. Oligonucleotide microarray-based gene expression profile analysis was performed with Affymetrix HU133A arrays representing ∼14 500 unique genes. The oligonucleotide microarray results were subsequently validated by quantitative real-time PCR, immunohistochemical staining, and by comparison of specific gene expression patterns with published results. Hierarchical clustering of gene expression data distinguished tumours on the basis of stage, differentiation and genetic abnormalities. A number of genes were identified whose patterns of expression were highly correlated with 11q loss; supporting the concept that loss of 11q represents a distinct genetic subtype of neuroblastoma. The implications of these results in the process of neuroblastoma development and progression are discussed.

Fibrin sealants are a type of soft tissue adhesive that employs biochemical reactions from the late stages of the blood coagulation cascade. Intrinsic to these adhesives are a structural protein and a transglutaminase crosslinking enzyme. We are investigating an alternative biomimetic adhesive based on gelatin and a calcium-independent microbial transglutaminase (mTG). Rheological measurements show that mTG catalyzes the conversion of gelatin solutions into hydrogels, and gel times are on the order of minutes depending on the gelatin type and concentration. Tensile static and dynamic loading of the adhesive hydrogels in bulk form demonstrated that the Young's modulus ranged from 15 to 120 kPa, and these bulk properties were comparable to those reported for hydrogels obtained from fibrin-based sealants. Lap-shear adhesion tests of porcine tissue were performed using a newly published American Society for Testing and Materials (ASTM) standard for tissue adhesives. The gelatin-mTG adhesive bound the opposing tissues together with ultimate adhesive strengths of 12−23 kPa which were significantly higher than the strength observed for fibrin sealants. Even after failure, strands of the gelatin-mTG adhesive remained attached to both of the opposing tissues. These results suggest that gelatin-mTG adhesives may offer the benefits of fibrin sealants without the need for blood products.

We report an enzyme-based method for the in situ entrapment of cells within a biopolymeric hydrogel matrix. Specifically, we used a calcium-independent microbial transglutaminase that is known to cross-link proteins and observed that it catalyzes the formation of gels from a pre-gel solution containing 10% gelatin and E. coli cells. Hydrogel formation occurs 2−3 h after adding transglutaminase, and no additional external intervention is required to initiate gel formation. The in situ entrapped cells grow rapidly and to high cell densities within the gelatin hydrogel. Additionally, the entrapped cells respond to isopropylthiogalactoside induction. The cross-linked gelatin network can be rapidly hydrolyzed (within 1 h) by the protease, proteinase K. Treatment of the network by this protease releases the entrapped E. coli cells. These cells appear unharmed by proteinase K; they can grow and be induced after protease treatment. The ability to in situ entrap, grow, and release cells under mild conditions provides unique opportunities for a range of applications and should be especially useful for microfluidic biosensor systems.

Self-assembled nanoporous materials have potential for many applications. Here we report unusual dendritic crystals from self-assembled nanoporous silicates and continuous morphological change of such materials through chemical means. We used hybrid organic silane and quaternary ammonium surfactants to prepare large dendritic nanoporous crystals. These self-assembled crystals have a cubic structure (Pm3m), 4-fold symmetry, and are oriented with the 〈100〉 direction perpendicular to the surface. Remarkably, the morphology of the crystals can change from highly branched, square-shaped dendrites to less branched structures, to nonbranched crosses, and finally to regular compact crystals when the Cl^- counterions on the surfactant were gradually replaced by Br^- while keeping the total surfactant concentration constant. The unusual dendritic crystals and their continuous morphology change suggest the importance to control diffusion and surface reactivity for self-assembled crystal growth under nonequilibrium conditions. The open dendritic nanoporous structures may be useful for sensing and other applications.

CPI-1189 is a compound with antioxidant properties that blocks tumor necrosis factor- (TNF) effects in animal models. It has neuroprotective properties in model systems for HIV-associated neurotoxicity and thus is a candidate for neuroprotective therapy in humans with HIV-associated CNS disease.

Tourette syndrome (TS) and related tic disorders are commonly associated with obsessive-compulsive disorder (OCD) and attention deficit hyperactivity disorder (ADHD). It has been argued, however, that any observed association between TS and these and other psychopathologies may be due to ascertainment bias in that individuals with multiple problems are more likely to be referred for medical evaluation.

To assess the incidence of and risk factors for distal sensory polyneuropathy (DSP) in a cohort of HIV-infected subjects.

We present a detailed next-to-leading order (NLO) leading twist QCD analysis of deeply virtual Compton scattering (DVCS) observables, for several different input scenarios, in the $\overline{\mbox{MS}}$scheme. We discuss the size of the NLO effects and the behavior of the observables in skewedness $\zeta$, momentum transfer,t, and photon virtuality, $q^2=-Q^2$. We present results on the amplitude level for unpolarized and longitudinally polarized lepton probes, and unpolarized and longitudinally polarized proton targets. We make predictions for various asymmetries and for the DVCS cross section and compare with the available data.

Scanning probe microscopy has been used for the analysis of a wear surface from a component removed from a heavy duty diesel engine. This component is analysed and compared to a wear surface that has been generated in a rig that simulates engine conditions. It was found that the wear surface from the engine component is highly pitted in the regions where contact pressures are exceedingly high, whereas regions further away from the high pressure points are covered with an anti-wear film that resembles the films formed in the wear rig. These results are confirmed by both scanning force microscopy images and nanoindentation results.

Abstract.: An analysis of Deeply Virtual Compton Scattering (DVCS) is made within the colour dipole model. We compare and contrast two models for the dipole cross-section which have been successful in describing structure function data. Both models agree with the available cross section data on DVCS from HERA. We give predictions for various azimuthal angle asymmetries in HERA kinematics and for the DVCS cross section in the THERA region.

Laminin 5 functions to promote cell-matrix adhesion and therefore is hypothesized to abrogate apoptosis initiated through the loss of epithelial cell contact with extracellular matrix. Laminin 5 levels are decreased in epithelial cells cultured in a hypoxic environment. Exposure of epithelial cells to hypoxia may induce apoptotic pathways transmitted through changes in mitochondrial membrane potential. Using an apoptosis assay based on mitochondrial membrane integrity, the effect of hypoxia (2% oxygen) on human corneal epithelial cell viability was determined. Both a virally transformed corneal epithelial cell line and third passage corneal epithelial cells were resistant to hypoxia-mediated apoptosis for up to 5 days in culture. However, at 7 days in culture, a statistically significant increase in apoptosis was noted in hypoxic corneal epithelial cells compared to normoxic (20% oxygen) controls. Increased apoptosis in hypoxic epithelium at 7 days in culture correlated with decreased deposition of laminin 5 into the extracellular matrix, as determined by western blot analysis and immunofluorescence microscopy. Additionally, the extracellular processing of the alpha3 and gamma2 chains of laminin 5 was negatively impacted by corneal epithelial cell exposure to hypoxia for 7 days. Treatment of human corneal epithelial cells cultured in 20% oxygen with function-inhibiting antibodies to laminin 5 for 2 or 3 days resulted in a statistically significant decrease in proliferation, and concomitant increase in apoptosis, compared with untreated normoxic controls. Based on these results, it appears that mechanisms of hypoxia-mediated apoptosis in human corneal epithelial cells may be initiated by the loss of processed laminin 5 in the extracellular matrix or by the loss of laminin 5-epithelial cell communication and transmitted through mitochondria.

Animal and human studies suggest that 2-adrenergic agonists exert anabolic effects on muscles, inducing and preventing atrophy after a variety of insults. Based on data from an open-label trial of albuterol in 15 patients with facioscapulohumeral dystrophy (FSHD), the authors conducted a randomized, double-blind, placebo-controlled trial of sustained-release albuterol in this disease.

Based on the knowledge that Tourette’s syndrome (TS) is associated with several clinical features that can impair school function and growing evidence that the disorder is much more common than previously thought, the authors hypothesized that TS and related tic disorders would be associated with school problems in the childhood population at large.

The functionalization of carbon electrodes with aryl films can be achieved via the electrochemical reduction of the corresponding diazonium salt. We have previously shown that this deposition procedure will produce multilayer films on ordered graphite under certain conditions. We examine here the formation of multilayer films on glassy carbon (GC) electrodes by potential step electrolysis of diazonium salts for longer periods of time. The deposition of diethylaniline (DEA) is tracked with the use of infrared reflectance absorption spectroscopy and scanning force microscopy. DEA is continually deposited over a 30 min potential step and the film thickness approaches 20 nm. Phenylacetic acid and nitrophenyl films that are 15−25 nm thick can also be deposited in this way. We also find that, despite the presence of a relatively thick DEA film on a GC electrode, electron transfer to benchmark redox systems is not completely blocked. We attribute the compromised blocking to a high density of defects in the film structure.

To study the development of freezing of gait in PD.

Epidemiological, cross-sectional, and prospective studies strongly suggest that exercise has beneficial effects on bone mass in premenopausal women. We prospectively compared the effects of resistance or aerobic exercises on regional bone mass in premenopausal active duty military women ranging in age from 19 to 40 years. Subjects were assigned, by preference, to a resistance exercise group or an aerobic exercise group and instructed to exercise at least 30 minutes per day, three times per week, for a period of 1 year. Bone mineral density (BMD) was measured by dual photon absorptiometry at the lumbar spine and femoral neck and by single photon absorptiometry at the mid radius at baseline, 6 months, and 12 months. BMD increments during the study were statistically significant at all sites in both exercise groups. Comparisons between the groups showed that after 12 months, BMD increased similarly in the lumbar spine (2.2% resistance vs. 1.8% aerobics, p = not significant) but more in the resistance group in the femoral neck (5.0% vs. 2.7%, p < 0.001) and the mid radius (7.8% vs. 6.7%, p < 0.05). Both resistance and aerobic exercises increase regional bone mass, particularly cortical bone mass, in premenopausal women. Resistance work appears to have a slightly greater effect on cortical bone than aerobics alone. A combination of aerobics and resistance exercises, therefore, may be a useful strategy for increasing peak bone mass in premenopausal women.

Abstract.: As a consequence of QCD factorization theorems, a wide variety of inclusive and exclusive cross sections may be formulated in terms of a universal colour dipole cross section at small x. It is well known that for small transverse size dipoles this cross section is related to the leading-log gluon density. Using the measured pion-proton cross section as a guide, we suggest a reasonable extrapolation of the dipole cross section to the large transverse size region. We point out that the observed magnitude and small x rise of the gluon density from conventional fits implies that the DGLAP approximation has a restricted region of applicability. We found that ‘higher twist’ or unitarity corrections are required in, or close to, the HERA kinematic region, even for small ‘perturbative’ dipoles for scattering at central impact parameters. This means that the usual perturbative leading twist description, for moderate virtualities, GeV, has rather large ‘higher twist’ corrections at small x. In addition, for these virtualities, we also find sizeable contributions from large non-perturbative dipoles ( fm) to , and also to . This also leads to deviations from the standard leading twist DGLAP results, at small x and moderate . Our model also describes the low data very well without any further tuning. We generalize the Gribov unitarity limit for the structure functions of a hadron target to account for the blackening of the interaction at central impact parameters and to include scattering at peripheral impact parameters which dominate at extremely large energies.

Nanoscale Pt structures were prepared on highly ordered pyrolytic graphite (HOPG) by chemical vapor deposition. The organometallic compound Pt(COD)(CH3)2 (1) (COD is 1,5-cyclooctadiene) was reduced by hydrogen gas to effect the deposition of Pt. The deposited Pt was characterized by cyclic voltammetry and by tapping mode scanning force microscopy. It was found that Pt initially nucleates at defect sites on HOPG. These nucleated sites then catalyze the hydrogenation of 1 to further deposit Pt adatoms. At low coverages, we observe substrate defect decoration by “nanobead” type structures. Structures that emanate from defect sites onto the basal plane form at higher coverages.
