Hematopoietic stem cell transplantation-associated thrombotic microangiopathy (HSCT-TMA) is a severe and potentially life-threatening complication. HSCT-TMA is often underdiagnosed due to multifactorial pathophysiology and a historic lack of standard diagnostic criteria. Identification of the multi-hit hypothesis and the key role of the complement system, particularly the lectin pathway of complement, has led to development of treatments targeting the underlying pathogenesis of HSCT-TMA. Additional research is ongoing to investigate the efficacy and safety of these targeted therapies in patients with HSCT-TMA. Advanced practice providers (APPs; nurse practitioners and physician assistants) and pharmacists are critical members of the multidisciplinary HSCT team and ensure management of patients throughout the continuum of care. Additionally, pharmacists and APPs can improve patient care through medication management of complex regimens; transplant education for patients, staff, and trainees; evidence-based protocol and clinical guideline development; assessment and reporting of transplant-related outcomes; and quality improvement initiatives to improve outcomes. Understanding the presentation, prognosis, pathophysiology, and treatment options for HSCT-TMA can improve each of these efforts.

Is it the weather? Journal of Banking and Finance 32 (4), 526-540], in which the authors challenge our previous finding that stock market returns exhibit seasonal patterns consistent with the influence of seasonal affective disorder on investor risk aversion. We find that we cannot replicate the authors' findings, even after corresponding with them. Furthermore, we document several problems with their methodology, including misspecification of their economic model, misspecification of their econometric model, and use of inappropriate data. While we agree that seasonal affective disorder is not an explanation for all variation in equity markets, we do maintain that careful analysis leads to economically and statistically significant evidence of the effect we originally documented. The United States Advanced Television Systems Committee has created a standard for terrestrial broadcast of digital HDTV signals that specifies the transmittal of 12 separate, interleaved trellis encoded data streams. This paper describes an area-efficient, low latency delay, single trellis decoder that decodes all 12 separate data streams and provides seamless transitions between the two different expected operating conditions- with and without NTSC co-channel interference.

The article deals with the issue of personalization of Internet services, employing web mining. Personalization technology involves software that learns patterns, habits, and preferences. On the Internet, its use is primarily in systems that support electronic-business. Observational personalization is a category of personalization that attempts to elude the need for users to divulge any personal information. There are three principal components to observational personalization; analytics, representation, and deployment. Web mining provides tools to analyze web log data in a user-centric manner such as segmentation, profiling and clickstream discovery. The success of personalization on the web depends on the ability of the personalization community to promote responsible use of the technology by e-business. The article presents a foray into articles dealing with the issue of deploying Web mining in personalization technology.

Objective: This study tested the hypothesis that psycbiatric comorbidity measured in medical and surgical general hospital inpatients predicts increased readmissions and days spent rehospitalized at the same hospital up to 4 years after discharge. Method: A convenience sample of 273 medical/surgical inpatients aged 18 years and older were given psychological tests during their third to fifth hospital days on medical and surgical units from June 1, 1985, through June 30, 1986. The main outcome measure was the number of medical/surgical readmissions and number of days rehospitalized during a 4-year follow-up at the same institution. Results: Compared to the rest of the study group, the cognitively impaired patients (according to the Mini-Mental State examination) averaged twice as many rehospitalizations and three times as many days rehospitalized at 6-month follow-up and twice as many days rehospitalized at 2-year follow-up. Compared to the rest of the group, the patients who were depressed or who had high interpersonal sensitivity scores at the index admission spent twice as many days rehospitalized during the 4-year follow-up, while the patients with high hostility scores had almost twice as many readmissions. These results remained statistically significant after separate partialing out of the effects of severity of functional impairment, age, cognitive impairment, and number of admissions or days spent hospitalized before the index admission. Conclusions: Psychiatric comorbidity, previously well documented as contributing to increased length of stay in the general hospital, is associated with increased hospital utilization for at least 4 years after discharge.

Increasing global competition is diminishing the relevance of 20th century management paradigms. Organizational structures built around functional hierarchies and traditional approaches to the management of processes, information and human resources are being supplanted by new models of higher performing organizations. Teams are taking the place of hierarchies, the mechanisms for directing organization effort are shifting away from top-down control towards empowerment and goal-alignment, while processes are now the key organizing unit of companies rather than functions. Traditional companies that want to be transformed into higher performing organizations should subscribe to several principles of change leadership. An innovative analytic approach to business process redesign is presented. A steering committee identifies the critical business process for maximum effectivity and sets imrovement targets for successful process redesign. The process-evaluation team evaluates the existing process through baseline sets and benchmarking. An initial process vision is developed which requires the use of visioning workshops for mindset shifts and breakthrough thinking. Problem-solving concepts are then developed and initiated that results in a consolidated plan for process improvement. Line management is brought in for the implementation of the process change to monitor targets and timetables and to handle change accountabilities. Finally, team-oriented organizational behavior is incorporated into the organization for developing participative management and continuous improvement methodology.

Fundamental, continual change is necessary to ensure business survival. Concurrent engineering brings about and sustains change by using cross-functional teams. In this set-up, teams consisting of people from various disciplines interact to evaluate the design of a new product and its manufacturing and support processes. Trade-off decisions are quickly made. Computer-based tools support this interaction and results in a horizontally integrated workforce which is more effective in making design decisions. A framework to support this approach to change is described.

Psychological factors that may prolong hospital stay were evaluated by studying 278 inpatients in a teaching hospital. A prospective study design was utilized, which statistically controlled for variables previously linked to longer hospitalizations, such as age. The patients, who were from four medical floors, four surgical floors and one gynecology floor, were evaluated during their third to fifth hospital day, and given a battery of tests to assess psychological and physical symptoms, depression, organic mental state (e.g. dementia) and anxiety. Nurses rated each patient's degree of physical impairment. The patients ranged in age from 18 to 83 years. They were primarily female (54 percent), Caucasian (84 percent) and married (67 percent). Data analysis revealed that longer stay was significantly related to dementia, depression and anxiety. Greater physical impairment was significantly related to both longer hospital stay and mental state organicity, depression, paranoia and obsessive-compulsive traits. Patients who had been admitted on an emergency basis also had significantly longer hospital stays than patients who were admitted on an elective basis. The finding that depression, anxiety and mental state organicity during the first week of hospitalization predicted longer hospital stays indicates a need for screening approaches and early therapeutic interventions. The inclusion of psychiatric consultation-liaison services in future research endeavors is recommended. (Consumer Summary produced by Reliance Medical Information, Inc.)

Three new abietane-type diterpenoids, plectranthroyleanones A-C (1-3), together with five known compounds (4-8) were isolated from the methanol extract of the whole plant of Plectranthus africanus using column chromatography techniques. The structures of the new compounds were elucidated using a combination of 1D and 2D NMR and HRESIMS analyses. Compound 1 exhibited weak activities with minimal inhibitory concentration values of 75 µg/mL against grampositive bacteria, Bacillus subtilis and Staphylococcus aureus, and 150 µg/mL against two gram-negative bacteria, Pseudomonas aeruginosa and Klebsiella pneumoniae, respectively, while 2 and 3 had moderate antibacterial activity against K. pneumoniae with a minimal inhibitory concentration value of 37.5 µg/mL.
