2. Knowledge, attitudes, and practices of Jamaican healthcare workers on nosocomial infection control

- Abstract
Objectives The emergence of deadly infections such as methicillin-resistant Staphylococcus aureusand COVID-19 (SARS-CoV-2) highlights the need for effective infection control (IC) procedures in healthcare facilities. The aim of the study was to assess the knowledge, attitudes, and practices (KAP) of healthcare workers (HCWs) on IC procedures.Methods A cross-sectional survey was conducted among HCWs in western Jamaica from May to August 2015. A questionnaire based on the Jamaican Health Facilities Infection and Control Policies and Procedures Manual was used to collect data on staff training in IC, and their KAP of IC. Summary statistics, ANOVA, and multiple linear regression were applied for data analyses.Results Among 227 participants, the mean knowledge scores were statistically higher among the younger HCWs, Physicians/Nurses, and those in clinical units. Older HCWs, non-physicians/nurses, non-clinical unit workers, HCWs at Western Regional Hospital A, and those who received ongoing IC training scored higher on attitude. The practice scores were significantly correlated with the attitude scores but not the knowledge scores. The attitude score was a significant predictor for IC practices.Conclusions Six knowledge and three attitude variables were significant predictors of IC practices that the Western Regional Health Authority Officials can use to implement measures to improve IC practices among HCWs.

Adiposity can be measured using BMI (which is based on weight and height) as well as indices of abdominal adiposity. We examined the association between BMI and waist-to-height ratio (WHtR) within and across populations of different world regions and quantified how well these two metrics discriminate between people with and without hypertension.

5. Severe bradycardia at acute medical take – Think about myxoedema coma

- Abstract
We report two cases of myxoedema coma that presented to the acute medical take with severe bradycardia. These patients were initially misdiagnosed as bradyarrhythmia of primary cardiac origin. They were then diverted to the cardiology service at another district general hospital (DGH) for admissions. Both cases were subsequently diagnosed with myxoedema coma having screened thyroid function tests on arrival at the cardiology unit. Despite being treated for myxoedema coma, both patients unfortunately succumbed to the disease and later died in the hospital. These cases highlight that clinical suspicion and recognition of myxoedema coma remain significant challenges in a developed world despite readily available and highly sensitive thyroid hormone assays.

Sparsentan, a novel, non-immunosuppressive, single-molecule, dual endothelin angiotensin receptor antagonist, significantly reduced proteinuria versus irbesartan, an angiotensin II receptor blocker, at 36 weeks (primary endpoint) in patients with immunoglobulin A nephropathy in the phase 3 PROTECT trial's previously reported interim analysis. Here, we report kidney function and outcomes over 110 weeks from the double-blind final analysis.

- Abstract
This article designs three coordinated control schemes for a grid-connected wind energy conversion system based on a permanent magnet synchronous generator. The system includes a static synchronous compensator (STATCOM) and a supercapacitor energy storage (SCES) to maintain stable operation during different operating conditions. Coordinated control scheme-I (CCS-I) integrates the SCES into the DC-link via a bidirectional DC/DC converter to stabilize the voltage under varying wind speeds and grid disturbances. Coordinated control scheme-II (CCS-II) uses a STATCOM at the point of common coupling (PCC) to enhance the grid voltage stability during voltage sag or swell, where the SCES balances active power, and the grid side converter (GSC) provides reactive power support. The STATCOM is only activated when the PCC's reactive power requirement exceeds the GSC's support capacity. Coordinated control scheme-III (CCS-III) considers simultaneous disturbances on both the wind turbine and grid side to stabilize the DC-link and grid voltage. The proposed control schemes have been experimentally implemented with a dSpace MicroLabBox. Additionally a Lyapunov function based stability analysis of the system including the SCES is presented in the paper.

11. Characterization of selected phages for biocontrol of food-spoilage pseudomonads

- Abstract
Pseudomonasspp., such as P. fluorescensgroup, P. fragi, and P. putida, are the major psychrophilic spoilage bacteria in the food industry. Bacteriophages (phages) are a promising tool for controlling food-spoilage and food-poisoning bacteria; however, there are few reports on phages effective on food-spoilage bacteria such as Pseudomonasspp. In this study, 12 Pseudomonasphages were isolated from chicken and soil samples. Based on the host range and lytic activity at 30 °C and 4 °C and various combinations of phages, phages vB_PflP-PCS4 and vB_PflP-PCW2 were selected to prepare phage cocktails to control Pseudomonasspp. The phage cocktail consisting of vB_PflP-PCS4 and vB_PflP-PCW2 showed the strongest lytic activity and retarded regrowth of P. fluorescensand P. putidaat 30 °C, 8 °C, and 4 °C at a multiplicity of infection of 100. Nucleotide sequence analysis of the genomic DNA indicated that vB_PflP-PCS4 and vB_PflP-PCW2 phages were lytic phages of the Podoviridae family and lacked tRNA, toxin, or virulence genes. A novel endolysin gene was found in the genomic DNA of phage vB_PflP-PCS4. The results of this study suggest that the phage cocktail consisting of vB_PflP-PCS4 and vB_PflP-PCW2 is a promising tool for the biocontrol of psychrophilic food-spoilage pseudomonads during cold storage and distribution.

12. Sea Grant’s Community Engaged Internship: expanding participation and cultivating belonging in coastal and ocean sciences

- Abstract
Providing hands-on experiences to undergraduates is a common strategy to recruit and retain diverse students in geosciences. And yet, without equal attention to science identity cultivation and culturally responsive mentoring, a lack of diversity plagues scientific fields. Sea Grant’s Community Engaged Internship (CEI) program is a unique initiative that engages undergraduate students in place-based coastal and ocean science research, outreach, education, law and policy, and communication projects. Students are recruited from communities that are historically underrepresented and underserved, including local, Tribal, and Indigenous communities, into a multiple-month paid internship. Recruitment and selection strategies draw applicants from diverse cultural, ethnic, and socioeconomic backgrounds with unique lived experiences, skills, abilities, and interests. Interns work on community-focused projects that integrate traditional and local knowledge with Western ways of doing science. The program includes mentorship education and training for all participants, multi-level mentoring for students, a vibrant learning community, and extensive professional development and networking opportunities. Program evaluation results demonstrate that CEI has achieved its primary goal of increasing the recruitment and participation of students from underserved communities in coastal and ocean sciences. The evaluation also sheds light on attributes of the internship that are essential for its success, namely, engagement in community-based projects, culturally inclusive mentorship education, peer-to-peer and near-peer mentoring, and cohort-building. Competitive wages and stipends, professional development and training, network-building, and institutional support are also critical. Participants report numerous benefits including personal and professional gains from their participation in the program. To date, 183 students from all 34 Sea Grant programs have graduated from the Community Engaged Internship program.

- Abstract
This article presents the inclusive DC-link voltage control and protection scheme for the DC-link of the microgrid. The proposed inclusive scheme is proposed to control the DC-link voltage during load-switching or AC-side faults scenarios, and protect the DC-link when short-circuit fault emerges at DC-link. To examine the proposed inclusive scheme, the microgrid test system is created according to the IEEE-1547 standard in PSCAD/EMTDC based software system. Then the designed microgrid is examined under various fault scenarios to confirm the performance of the developed scheme. Results show that the presented control and protection scheme can maintain the DC-link voltage under faults on the AC or DC sides in grid-connected and islanding modes of operation. The proposed scheme has the ability to ensure the voltage regulation and protection of the DC-link of the microgrid under different operating conditions and faults.

14. Continuous Wavelet Transform and Deep Learning for Accurate AE Zone Detection in Laminated Composite Structures

- Abstract
Acoustic emission (AE) source localization is a crucial area of research, particularly in the context of laminated composite structures. Existing AE localization techniques based on the time difference of arrival are limited to homogeneous isotropic materials and are not suitable for the anisotropic characteristics of laminated composites. To address these limitations, this study introduces a novel approach using continuous wavelet transform (CWT) to transform AE signal waveforms captured by a sensor network into scalograms. These scalograms were then utilized to train deep convolutional neural network (CNN) models, resulting in exceptional prediction accuracy. The trained models achieved remarkable performance, with training, validation, and testing accuracy exceeding 97%, 95%, and 96%, respectively. To further validate the model, additional AE experimental data was collected and tested, yielding an accuracy of 95% with only two misclassifications out of 40 test data points. Moreover, a user-friendly web application was developed using the Streamlit open-source framework, enabling the practical deployment of this industrial-grade AE localization system without necessitating advanced AI skills, achieving a significant technology readiness level (TRL) for widespread utilization in engineering applications.

15. Lessons from the use of telemedicine in the austere military environment and the implications for deployed surgical teams

- Abstract
Over the last 20 years, there have been significant changes in UK surgical training. Civilian surgical training may no longer prepare military surgeons for the range of skills they require on operations. One method to address gaps in knowledge or experience is to use telemedicine to facilitate specialist consultations from UK-based specialists to deployed medical teams. In the UK Defence Medical Services (DMS), this capability is called real-time clinical support (RTCS). RTCS provides a direct audio-visual link from a deployed location anywhere in the world to a supporting medical specialist in the UK. RTCS is currently delivered via a combination of off-the-shelf hardware and commercially available software. This article will outline the current use of RTCS, with emphasis on deployed surgical teams in austere environments, and discuss the advantages and limitations of this capability. However, it must be emphasised that no technology can be a substitute for clinical training and experience. Although several limitations remain, the authors believe that RTCS offers potential benefits for the DMS and could be an important tool aiding deployed clinicians. It can also be argued that by engaging with the concept now, the DMS can shape future developments in this sphere.

- Abstract
Development of nanoscale multicomponent solid inorganic materials is often hindered by slow solid diffusion kinetics and poor precursor mixing in conventional solid-state synthesis. These shortcomings can be alleviated by combining nanosized precursor mixtures and low temperature reaction, which could reduce crystal growth and accelerate the solid diffusion at the same time. However, high throughput production of nanoparticle mixtures with tunable composition via conventional synthesis is very challenging. In this work, we demonstrate that ∼10 nm homogeneous mixing of sub-10 nm nanoparticles can be achieved via spark nanomixing at room temperature and pressure. Kinetically driven Spark Plasma Discharge nanoparticle generation and ambient processing conditions limit particle coarsening and agglomeration, resulting in sub-10 nm primary particles of as-deposited films. The intimate mixing of these nanosized precursor particles enables intraparticle diffusion and formation of Cu/Ni nanoalloy during subsequent low temperature annealing at 100 °C. We also discovered that cross-particle diffusion is promoted during the low-temperature sulfurization of Cu/Ag which tends to phase-segregate, eventually leading to the growth of sulfide nanocrystals and improved homogeneity. High elemental homogeneity, small diffusion path lengths, and high diffusibility synergically contribute to faster diffusion kinetics of sub-10 nm nanoparticle mixtures. The combination of ∼10 nm homogeneous precursors via spark nanomixing, low-temperature annealing, and a wide range of potentially compatible materials makes our approach a good candidate as a general platform toward accelerated solid state synthesis of nanomaterials.

IntroductionNon-alcoholic fatty liver disease (NAFLD) is the leading cause of chronic liver disease, with type 2 diabetes mellitus (T2DM) as a major predictor. Insulin resistance and chronic inflammation are key pathways in the pathogenesis of T2DM leading to NAFLD and vice versa, with the synergistic effect of NAFLD and T2DM increasing morbidity and mortality risks. This meta-analysis aims to quantify the prevalence of NAFLD and the prevalence of clinically significant and advanced fibrosis in people with T2DM.MethodsMEDLINE and Embase databases were searched from inception until 13 February 2023. The primary outcomes were the prevalence of NAFLD, non-alcoholic steatohepatitis (NASH) and fibrosis in people with T2DM. A generalised linear mixed model with Clopper-Pearson intervals was used for the analysis of proportions with sensitivity analysis conducted to explore heterogeneity between studies.Results156 studies met the inclusion criteria, and a pooled analysis of 1 832 125 patients determined that the prevalence rates of NAFLD and NASH in T2DM were 65.04% (95% CI 61.79% to 68.15%, I2=99.90%) and 31.55% (95% CI 17.12% to 50.70%, I2=97.70%), respectively. 35.54% (95% CI 19.56% to 55.56%, I2=100.00%) of individuals with T2DM with NAFLD had clinically significant fibrosis (F2–F4), while 14.95% (95% CI 11.03% to 19.95%, I2=99.00%) had advanced fibrosis (F3–F4).ConclusionThis study determined a high prevalence of NAFLD, NASH and fibrosis in people with T2DM. 27. Externalist argument against medical assistance in dying for psychiatric illness

- Abstract
Medical assistance in dying, which includes voluntary euthanasia and assisted suicide, is legally permissible in a number of jurisdictions, including the Netherlands, Belgium, Switzerland and Canada. Although medical assistance in dying is most commonly provided for suffering associated with terminal somatic illness, some jurisdictions have also offered it for severe and irremediable psychiatric illness. Meanwhile, recent work in the philosophy of psychiatry has led to a renewed understanding of psychiatric illness that emphasises the role of the relation between the person and the external environment in the constitution of mental disorder. In this paper, I argue that this externalist approach to mental disorder highlights an ethical challenge to the practice of medical assistance in dying for psychiatric illness. At the level of the clinical assessment, externalism draws attention to potential social and environmental interventions that might have otherwise been overlooked by the standard approach to mental disorder, which may confound the judgement that there is no further reasonable alternative that could alleviate the person’s suffering. At the level of the wider society, externalism underscores how social prejudices and structural barriers that contribute to psychiatric illness constrain the affordances available to people and result in them seeking medical assistance in dying when they otherwise might not have had under better social conditions.

- Abstract
The growing significance of migration on a global scale is generating increased scholarly interest in the subject, offering a fuller and more nuanced understanding of the nature and implications of different types of migration. However, research to date has presented divergent findings on the factors influencing the presence or absence of migration-induced conflicts, as most contributions are single-case studies focusing on different regions, levels of analysis, and measurements, whether applied to ‘objective’ or ‘subjective’ situations. We develop a framework to assess a wide spectrum of interactions between ‘local’ residents and ‘migrant populations’ by integrating the literature on ethnic conflict and considering the findings of studies that have investigated developed countries separately from developing countries; utilized a methodology specific to a particular discipline; or have focused on a single historical time frame or level of analysis. We propose a distinctive way of organizing and integrating the literature, incorporating three levels of analysis (independent variables) and a wide range of responses (dependent variables). We demonstrate that this framework enables researchers who employ one level of analysis at the expense of the others, or consider only one aspect or level of conflict, to assess the limitations and representativeness of their case studies; to identify gaps in operationalization and measurements of concepts and definitions; and to become aware of the trade-offs involved in their selection and scope of analysis.

- Abstract
Background: To fight the current coronavirus disease (COVID-19) pandemic, many countries have implemented various mitigation measures to contain the spread of the disease. By engaging with health service providers, the community’s participation in adherence to preventive measures is certainly required in the implementation of COVID-19 mitigation strategies. Therefore, this study aimed to assess the level of adherence to COVID-19 preventive measures and its associated factors among the residents, Yangon Region, Myanmar. Methods: A community-based cross-sectional study was carried out among 636 residents in Yangon Region, Myanmar, from October to December 2021. A multistage non-probability sampling method, purposively selected for three townships in Yangon Region and convenience sampling for 212 participants from each township, was applied and the data were collected by face-to-face interviews using structured and pretested questionnaires. Data were entered, coded, and analyzed using IBM SPSS version 25.0. Simple and multiple logistic regression analysis were performed to identify the significant variables of adherence to COVID-19 preventive measures. Results: As a level of adherence to COVID-19 preventive measures, the proportion of residents who had good adherence was 39.3% (95% CI 35.5–43.2%), moderate adherence was 37.6% (95% CI 33.8–41.5%), and poor adherence was 23.1% (95% CI 19.9–26.6%). The age group of 31–40 years (AOR: 3.13, 95% CI 1.62–6.05), 30 years and younger (AOR: 3.22, 95% CI 1.75–5.92), Burmese ethnicity (AOR: 2.52, 95% CI 1.44–4.39), own business (AOR: 3.19, 95% CI 1.15–8.87), high school education level and below (AOR: 1.64, 95% CI 1.02–2.69), less than 280.90 USD of monthly family income (AOR: 1.51, 95% CI 1.01–2.29), low knowledge about COVID-19 (AOR: 1.90, 95% CI 1.26–2.88) were significantly associated with poor adherence to COVID-19 preventive measures. Conclusions: In this study, nearly one-fourth of the residents were experiencing poor adherence to COVID-19 preventive measures. Therefore, building up the risk communication through the community using widely used mainstream media, the continuation of disease surveillance and announcement of updated information or advice for the public to increase awareness towards COVID-19, and enforcement to follow the recommended directions and regulations of health institutions are vital to consider for improving the adherence to preventive measures against COVID-19 among the residents.

- Abstract
Background: Excessive or inappropriate use of social media has been linked to disruptions in regular work, well-being, mental health, and overall reduction of quality of life. However, a limited number of studies documenting the impact of social media on health-related quality of life (HRQoL) are available globally. Aim: This study aimed to explore the perceived social media needs and their impact on the quality of life among the adult population of various selected countries. Methodology: A cross-sectional, quantitative design and analytical study utilized an online survey disseminated from November to December 2021. Results: A total of 6689 respondents from ten countries participated in the study. The largest number of respondents was from Malaysia (23.9%), followed by Bangladesh (15.5%), Georgia (14.8%), and Turkey (12.2%). The prevalence of social media users was over 90% in Austria, Georgia, Myanmar, Nigeria, and the Philippines. The majority of social media users were from the 18–24 age group. Multiple regression analysis showed that higher education level was positively correlated with all four domains of WHOQoL. In addition, the psychological health domain of quality of life was positively associated in all countries. Predictors among Social Media Needs, Affective Needs (β = -0.07), and Social Integrative Needs (β = 0.09) were significantly associated with psychological health. Conclusion: The study illuminates the positive correlation between higher education levels and improved life quality among social media users, highlighting an opportunity for policymakers to craft education-focused initiatives that enhance well-being. The findings call for strategic interventions to safeguard the mental health of the global social media populace, particularly those at educational and health disadvantages.

- Abstract
We assessed the trends, characteristics. and consequences of potentially avoidable hospitalizations (PAH) for hypertension in Switzerland, for period 1998 to 2018. Hospital discharge data from the Swiss federal office of statistics. Hospitalizations with hypertension as the main cause for admission were eligible. PAH for hypertension was defined according to the Organization for Economic Cooperation and Development criteria. Overall, data from 117’507 hospitalizations (62.1% women) was used. The number of PAH increased between 1998 (n=1685) and 2018 (n=3410), and their frequency among hospitalizations for hypertension increased from 36.9% in 2008 to 73.0% in 2018. Compared to non-PAH, patients with PAH for hypertension were younger, more frequently women (66.9% vs. 56.7%), non-Swiss nationals (15.9% vs. 10.9%), were more frequently admitted as an emergency (78.9% vs. 59.5%), and by the patient’s initiative (33.1% vs. 14.1%). Patients with PAH had also less comorbidities, as per the Charlson’s index. Patients with PAH for hypertension were more frequently hospitalized in a semi-private or private setting, stayed less frequently in the ICU (4.6% vs. 7.3%), were discharged more frequently home (91.4% vs. 73.0%), and had a shorter LOS than patients with non-PAH for hypertension: median and [interquartile range] 5 [3-8] vs. 9 [4-15] days. In 2018, the total costs of PAH were estimated at 16.5 million CHF, corresponding to a median cost of 4936 (4445-4961] CHF per stay. We conclude that in Switzerland, PAH represent a considerable fraction of hospitalizations for hypertension, and carry a non-negligible health cost.

ABSTRACTBy using individual-level survey data from 97 countries, we investigate the effect of informal institutions on external financing and its impact on entrepreneurship. We find that a culturally driven entrepreneurial environment allows entrepreneurs to obtain more debt, equity, and venture capital financing, and this, in turn, increases entrepreneurial activities. We further find that culturally driven entrepreneurial environment is more critical in determining entrepreneurship than formal institutional arrangements (such as investor protection). Our results provide evidence that cross-country variations in entrepreneurship can be explained by differences in cultural support to new venture financing across countries.

- Abstract
Boesenbergia rotunda (B. rotunda) extract-loaded hydrogel patches were fabricated to be an antibacterial dressing. The biological properties such as antioxidant properties, bacterial protection activity, and the effect on normal human fibroblast (NHF) viability were assessed compared with blank hydrogel patches. The antioxidant activity of B. rotunda extract-loaded hydrogel patches was 75.00 ± 0.79 % (at 4-h treatment time), whereas the blank hydrogel patch had low antioxidant activity. The extract-loaded patches exhibited antioxidant activity approximately 16.5 times higher than that of the blank patches. In addition, the qualitative antibacterial activity of the patches was studied by agar disc diffusion. The inhibition zone of the extract-loaded hydrogel discs against Staphylococcus aureus (S. aureus) was 7.33 ± 0.58 mm, whereas the inhibition zone of the discs against Escherichia coli (E. coli) could not be observed. Moreover, the quantitative antibacterial activity showed an inhibition effect of the extract-loaded hydrogels against both types of bacteria. After incubation for 24 h, the extract-loaded hydrogel patches inhibited the growth of S. aureus and E. coli at the percentage of 49.76 ± 3.82 and 20.13 ± 2.75%, respectively. The results from the infiltration test showed that the extract-loaded hydrogel patches could prevent bacteria contamination. In addition, no toxicity to NHF cells had been observed. Therefore, the B. rotunda extract-loaded hydrogel patches could be a candidate material as an antibacterial wound dressing.

Neutralizing monoclonal antibodies and nanobodies have shown promising results as potential therapeutic agents for COVID-19. Identifying such antibodies and nanobodies requires evaluating the neutralization activity of a large number of lead molecules viabiological assays, such as the virus neutralization test (VNT). These assays are typically time-consuming and demanding on-lab facilities. Here, we present a rapid and quantitative assay that evaluates the neutralizing efficacy of an antibody or nanobody within 1.5 h, does not require BSL-2 facilities, and consumes only 8 μL of a low concentration (ng/mL) sample for each assay run. We tested the human angiotensin-converting enzyme 2 (ACE2) binding inhibition efficacy of seven antibodies and eight nanobodies and verified that the IC50values of our assay are comparable with those from SARS-CoV-2 pseudovirus neutralization tests. We also found that our assay could evaluate the neutralizing efficacy against three widespread SARS-CoV-2 variants. We observed increased affinity of these variants for ACE2, including the β and γ variants. Finally, we demonstrated that our assay enables the rapid identification of an immune-evasive mutation of the SARS-CoV-2 spike protein, utilizing a set of nanobodies with known binding epitopes.

46. Defence Medical Services telemedicine now: a successful pilot of video consultations and instant messaging support to firm base training

Introduction: Telemedicine was pioneered in the Defence Medical Services (DMS) in 1998, since then the capabilities within the DMS have not advanced in step with advances in technology. We present our findings of a pilot of remote video consultation via Skype for MODNET during an arduous course held in the UK. Method: Combat medical technician sick parades were live streamed via Skype to a Defence Primary Healthcare Medical Centre and medical officer (MO) support was delivered remotely. This process was augmented by the use of Pando for still images of wounds and infection sites in order to enhance decision making and situational awareness. Results: Over a 3-week period, 34 consultations carried out during sick parade required the input from a remote MO, of those 34% required a prescription from an MO. None of the presentations required a face-to-face consultation, and all patients received MO-led care remotely. Conclusion: We have successfully demonstrated that video telemedicine consultations are safe, while simultaneously improving patient care, augmenting the distribution of medical assets and reducing costs.

Background and objectiveVancomycin pharmacokinetics have been described by both one- and two-compartment models. One-compartment models are widely used to predict the area under the curve (AUC), a useful parameter for determining the efficacy and safety of vancomycin, based on sparse data collected during therapeutic drug monitoring. It is uncertain whether AUCs from one-compartment models with sparsely sampled data can sufficiently represent the true AUC. This study aimed to compare AUC estimates from one- and two-compartment models using sparse data. The reliability of AUCs from models constructed with trough-only data was also assessed.MethodsA previously published robust model was used to simulate vancomycin concentration points at 15 min intervals in 100 patients. From these simulated data, the reference AUC (AUCref) was calculated and two depleted dataset versions (trough-only and peak-trough datasets) were also created. One- and two-compartment models were built from the depleted datasets with the use of NONMEM. Vancomycin 24-hour AUC was calculated from concentration–time profiles of each model by a linear trapezoidal formula at three different time periods: 0–24 hours (AUC0–24), 24–48 hours (AUC24–48) and 0–48 hours (AUCavg). The deviation of each of the AUCs from the AUCrefwas examined to assess the AUC predictability of models from sparse data. The difference in AUCs between one- and two-compartment models was analysed from statistical and clinical perspectives.ResultsWhen assessing the deviation of each AUC from the AUCref, the one-compartment model from both peak-trough and trough-only data could adequately represent the true AUC with no statistically significant differences. Two-compartment model from peak-trough data also provided similar AUC estimates with the AUCref. However, AUCs from the two-compartment model with trough-only data did not adequately represent the true AUC, with significant differences of 25.16% for AUC0–24, 15.92% for AUC24–48and 19.45% for AUCavg.ConclusionRegardless of statistically significant differences between AUCs from one- and two-compartment models, the level of difference was acceptable from the clinical perspective, being <17% in models from peak-trough data. Therefore, both one- and two-compartment models with sparse data having at least a pair of peak-trough data per patient could be reliable for predicting AUC. Furthermore, AUCs of the one-compartment model from trough-only data did not show a significant difference from the AUCref. Hence, one-compartment models developed from trough-only data could be useful for predicting AUC when models with rich data are not available for the intended population. However, it is suggested that the use of the two-compartment model built from trough-only data should be avoided.

- Abstract
Polyomavirus BK virus (BKPyV) infection is an important complication of kidney transplantation and allograft failure. The prevalence of viremia is 10%–15%, compared with BK-associated nephropathy (BKPyVAN) at 3%–5%. Given that there are no effective antiviral prophylaxis or treatment strategies for BKPyVAN, active screening to detect BKPyV viremia is recommended, particularly during the early posttransplant period. Immunosuppression reduction to allow viral clearance may avoid progression to severe and irreversible allograft damage. The frequency and duration of screening are highly variable between transplant centers because the evidence is reliant largely on observational data. While the primary treatment goals center on achieving viral clearance through immunosuppression reduction, prevention of subsequent acute rejection, premature graft loss, and return to dialysis remain as major challenges. Treatment strategies for BKPyV infection should be individualized to the recipient’s underlying immunological risk and severity of the allograft infection. Efficacy data for adjuvant therapies including intravenous immunoglobulin and cidofovir are sparse. Future well-powered and high-quality randomized controlled trials are needed to inform evidence-based clinical practice for the management of BKPy infection.

50. A Nonlinear Backstepping Control Scheme for Rapid Earth Fault Current Limiters in Resonant Grounded Power Distribution Systems: Applications for Mitigating Powerline Bushfires

- Abstract
This article presents a nonlinear backstepping control scheme for rapid earth fault current limiters (REFCLs) in resonant grounded power distribution systems to mitigate the severity of powerline bushfires. The main feature of the proposed control scheme is that it quickly eliminates both active and reactive components of the fault current to make it zero for reducing the chance of igniting bushfires. The nonlinear backstepping control scheme is employed on the dynamical model of a REFCL equipped with a T-type inverter. The desired tracking of the fault current is ensured with the proposed scheme by appropriately injecting the current to the neutral point. The performance of the controller is evaluated in terms of the fault current and faulty phase-to-ground voltage under different fault conditions while following the standard criteria for the practical operation. The fault current compensation capability of the proposed scheme is evaluated for both low and high impedance faults. Simulation results in software and processor-in-loop platforms clearly demonstrate the fault current is limited to a value much lower than its desired value of 0.5 A in less than 1 s, which means that the chance of igniting bushfires will be reduced with the proposed controller.
