Die bettseitige Lungensonographie (LUS) dient in Notaufnahmen und auf Intensivstationen als wertvolles Instrument, um häufige Lungenerkrankungen zu diagnostizieren. Der Verlauf kann kontrolliert und die klinische Strategie geplant werden. Die LUS benötigt wenige Minuten und ist eine brauchbare Ergänzung zur physikalischen Untersuchung. Frakturen der Rippen und des Sternums können gut dargestellt werden. Minimale pleurale Flüssigkeitsansammlungen (Erguss, Hämatothorax) werden detektiert. Mittels LUS kann zwischen einem Lungenödem und einer Exazerbation einer chronisch-obstruktiven Lungenerkrankung differenziert werden. Entzündungen, wie Pleuritis und Pneumonie, sind besser darstellbar als im Röntgenthoraxbild, sodass LUS bei ambulant erworbener Pneumonie die Röntgenuntersuchung ersetzen könnte. Bei ventilatorassoziierten Pneumonien oder Atelektasen können diese dargestellt werden und es kann die Wiederbelüftung gut kontrolliert werden. Eine Herz-Lungen-Beinvenen-Triple-Sonographie entsprechend dem klinischen Status hilft, die Diagnose der Lungenembolie zu verbessern. Diese sollte ein Instrument für viele Ärzte sein, speziell in Notaufnahmen.

Zusammenfassung.Pneumonische Lungenkonsolidierungen stellen sich durch typische sonomorphologische Kriterien dar: echoarme Läsionen mit verzerrten Rändern, Bronchoaerogramme, reguläre Durchblutung und parapneumonische Ergüsse. Pneumonien können sonografisch bettseitig dargestellt werden. Die Wiederbelüftung korreliert gut mit dem klinischen Verlauf. In vier Metanalysen wurde die Lungensonografie (LUS) mit der Computertomografie (CT) evaluiert. Dabei zeigt sich für die LUS eine Sensitivität von 88–97 % und eine Spezifität von 90–96 %. Das Thoraxröntgen hingegen wies eine gepoolte Sensitivität von 77 % und eine Spezifität von 91 % auf. Daher sollte LUS das Thoraxröntgen in der bildgebenden Diagnostik der ambulant erworbenen Pneumonie ersetzen.

Zusammenfassung.Infizierte und septierte Pleuraergüsse sind kompliziert. Ultraschall ist die beste Methode, Septierungen zu entdecken. Unzureichend behandelte parapneumonische Ergüsse haben ein hohes Risiko für lange Hospitalisation und erhöhte Mortalität bei insuffizienter Pleuradrainage. Pleurale Verdickungen sind vielgestaltig, diffus oder umschrieben, knotig und meist echoarm. Die meisten Patienten mit Pleuritis zeigen typische Veränderungen. In malignen Pleuraergüssen sind oft Metastasen zu entdecken. Die Rolle der Sonografie beim Mesotheliom ist in Diskussion.

The aim of this prospective, multicenter study was to define the accuracy of lung ultrasound (LUS) in the diagnosis of community-acquired pneumonia (CAP).

Common pulmorary is the most unexpected mortal event in necropsy, antemortem correctly diagnosed in 28–39%. Clinical symptoms are non-specific, and chest radiography is not very helpful. Scintigraphic investigations, helical computed tomography and pulmonary angiography have a high diagnostic accuracy but they are not always available. With a suitable sonographic equipment, it is possible to visualize subpleural lesions bedside. Clinical studies and the sonographic appearance with pathologic correlation showed pulmonary infarctions in location, form and size exactly corresponding with pathological findings. Fresh reperfusable infarcts were homogenous and relatively hypoechoic. Older infarcts were well demarcated, mainly wedge shaped. Triangular pleural based lesions, more roughly structured, were observed with a hyperechoic reflex in the center corresponding to the bronchiole: a sign of segmental involvement. In massive central lung embolism, both fresh and old infarctions are found. An imminent larger embolism can be predicted, for instance, in a deep venous thrombosis of the lower limb, as proven by bedside antemortem ultrasound examination. With one ultrasound system, we can ‘kill 3 birds with one stone’: source, way and outcome of pulmonary embolism.

A 32-year-old woman underwent laparoscopic cholecystectomy during which there was severe bleeding from the bed of the gallbladder. As application of metallic clips to control the bleeding was not successful, the operation was converted to an open laparotomy. Cholecystectomy was successfully completed without further complications, and the post-operative course was uneventful and the patient was discharged. Eighteen months later, the patient complained of dyspnea. Plain radiograph and computed tomography of the thorax showed a metallic clip in the branch of the pulmonary artery supplying the posterior basal segment of the inferior lobe of the left lung. There was no connection between the patient’s symptoms and the clip embolism. Nevertheless, clip migration or embolism could cause severe complications. Therefore, metallic clips should not be used to stop bleeding from the gallbladder bed.
