- Abstract
ABSTRACT:In 1974, under the authorship of the Institute for Industrial Education (IIE), a booklet was published to report and reflect on the 'Durban strikes – 1973'. Through the book, I returned to introduce the events, their context and importance in changing apartheid South Africa at an event to reflect on the strikes 50 years later, in January 2023 at the Durban University of Technology. A summary introduction to the strikes is followed by a description of the book itself, the context, process and participants: my brief for the 2023 event. In conclusion, I reflect on the very different world and country 50 years later, and the relevance to the present of what had been hoped for in 1973.

29. Two successive predatory attacks on the Late Ordovician (Sandbian) gastropod Deaechospira elliptica

- Abstract
Well-preserved Deaechospira elliptica from the Sandbian of Estonia shows two successive scars. The traces resemble Caedichnus spiralis which is a common predatory trace in gastropod shells. The injuries possibly caused by cephalopods to Paleozoic brachiopods are morphologically similar to the shell damage in D. elliptica, likely caused by cephalopods. The alternative candidates for the trace makers include large durophagous arthropods. The differences in scar morphology suggest that D. elliptica survived attacks by two different species of durophagous predators. D. elliptica was likely a valuable target for the durophagous predators in the Sandbian of Estonia.

30. Data-Driven Design of Novel Polymer Excipients for Pharmaceutical Amorphous Solid Dispersions

- Abstract
About 90% of active pharmaceutical ingredients (APIs) in the oral drug delivery system pipeline have poor aqueous solubility and low bioavailability. To address this problem, amorphous solid dispersions (ASDs) embed hydrophobic APIs within polymer excipients to prevent drug crystallization, improve solubility, and increase bioavailability. There are a limited number of commercial polymer excipients, and the structure–function relationships which lead to successful ASD formulations are not well-documented. There are, however, certain solid-state ASD characteristics that inform ASD performance. One characteristic shared by successful ASDs is a high glass transition temperature (Tg), which correlates with higher shelf stability and decreased drug crystallization. We aim to identify how polymer features such as side chain geometry, backbone methylation, and hydrophilic–lipophilic balance impact Tgto design copolymers capable of forming high-TgASDs. We tested a library of 50 ASD formulations (18 previously studied and 32 newly synthesized) of the model drug probucol with copolymers synthesized through automated photoinduced electron/energy transfer-reversible addition–fragmentation chain-transfer (PET-RAFT) polymerization. A machine learning (ML) algorithm was trained on the Tgdata to identify the major factors influencing Tg, including backbone methylation and nonlinear side chain geometry. In both polymer alone and probucol-loaded ASDs, a Random Forest Regressor captured structure–function trends in the data set and accurately predicted Tgwith an average R2> 0.83 across a 10-fold cross validation. This ML model will be used to predict novel copolymers to design ASDs with high Tg, a crucial factor in predicting ASD success.

ABSTRACTIntroductionAntiplatelets and oral anticoagulants are commonly used to treat patients with various cardiovascular and cerebrovascular diseases. However, the primary concern for clinicians remains the risk of bleeding, thus necessitating the development of new therapies. Milvexian is a new anticoagulant that inhibits factor XIa, preventing the pathological formation of thrombi without increasing bleeding risk.Areas coveredThis drug evaluation examines the pharmacokinetic properties of milvexian and provides information on its pharmacodynamics and clinical efficacy in treating some cerebrovascular conditions.Expert opinionMilvexian shows a good pharmacokinetic profile with low renal elimination rates, justifying its use in patients with a high degree of renal impairment, and without relevant drug-drug interactions. In patients affected by acute non-cardioembolic ischemic stroke or high-risk transient ischemic stroke, milvexian, in addition to dual antiplatelet therapy, seems to have a positive efficacy profile without any safety concerns, especially in terms of intracranial hemorrhage. Two phase 3 trials are ongoing to investigate the efficacy and safety of milvexian for preventing cardioembolic and non-cardioembolic ischemic stroke.

- Abstract
Introduction: Mechanical thrombectomy (MT) is the standard treatment for acute ischemic stroke (AIS) due to anterior large vessel occlusion (LVO). Despite successful recanalization, some patients remain disabled after 3 months. Mechanisms that can cause futile recanalization (FR) are still largely unknown. We investigated if stress hyperglycemia might be associated with FR.Patients and methods: This is a retrospective analysis of consecutive patients with successful recanalization treated in four participating centers between January 2021 and December 2022. According to the modified Rankin scale (mRS) status at 3 months, patients were divided into two groups: FR, if mRS score >2, and useful recanalization (UR), if mRS score ⩽2. Stress hyperglycemia was estimated by the glucose-to-glycated hemoglobin ratio (GAR) index.Results: A total of 691 subjects were included. At 3 months, 403 patients (58.3%) were included in the FR group, while the remaining 288 patients (41.7%) were included in the UR group. At the multivariate analysis, variables independently associated with FR were the following: age (OR 1.04, 95% CI 1.02–1.06, p< 0.001), GAR index (OR 1.08, 95% CI 1.03–1.14, p= 0.003), NIHSS at admission (OR 1.16, 95% CI 1.11–1.22; p< 0.001), and procedure length (OR 1.01, 95% CI 1.00–1.02; p= 0.009). We observed that the model combining age, GAR index, NIHSS at admission, and procedure length had good predictive accuracy (AUC 0.78, 95% CI 0.74–0.81).Conclusions: Stress hyperglycemia predicts FR in patients with successful recanalization after MT. Further studies should explore if managing stress hyperglycemia may reduce futile recanalization. Additionally, we recommend paying close attention to AIS patients with a GAR index greater than 24.8 who exhibit a high risk of FR.

- Abstract
One of the main hallmarks of Alzheimer’s Disease is the formation of ß-amyloid plaques, whose formation may be enhanced by metal binding or the appearance of familial mutations. In the present study, the simultaneous effect of familial mutations (E22Q, E22G, E22K, and D23N) and binding to metal ions (Cu(II) or Al(III)) is studied at the Aß42monomeric and fibrillar levels. With the application of GaMD and MD simulations, it is observed that the effects of metal binding and mutations differ in the monomeric and fibrillar forms. In the monomeric structures, without metal binding, all mutations reduce the amount of a-helix and increase, in some cases, the ß-sheet content. In the presence of Cu(II) and Al(III) metal ions, the peptide becomes less flexible, and the ß-sheet content decreases in favor of forming a-helix motifs that stabilize the system through interhelical contacts. Regarding the fibrillar structures, mutations decrease the opening of the fiber in the vertical axis, thereby stabilizing the S-shaped structure of the fiber. This effect is, in general, enhanced upon metal binding. These results may explain the different Aß42aggregation patterns observed in familial mutations.

ABSTRACTIntroductionThe drug most frequently used for thrombolysis in cases of acute ischemic stroke (AIS) is alteplase. However, there is moderate-to-high-quality evidence that tenecteplase has similar or higher efficacy and safety. With improved pharmacokinetic properties over alteplase, tenecteplase could be a significant advantage in treating AIS.Areas CoveredAfter conducting an extensive search on Scopus and PubMed, this manuscript reviews and compares the pharmacokinetic properties of alteplase and tenecteplase. Additionally, it provides information on pharmacodynamics, clinical efficacy, safety, tolerability, and drug–drug interactions.Expert opinionThe pharmacokinetic profile of alteplase and tenecteplase is derived from studies in patients with acute myocardial infarction. Thanks to its pharmacokinetic properties, tenecteplase is the drug closest to being the ideal fibrinolytic for AIS. Its longer half-life enables a single-bolus administration, which is particularly useful in emergencies. Tenecteplase has proven to have a good efficacy and safety profile in randomized clinical trials. Although we are awaiting the results of the ongoing phase 3 randomized clinical trials, we believe that tenecteplase has the potential to revolutionize the treatment of AIS through thrombolysis.

49. Development of a Triplex Endpoint PCR Assay for the Detection of SARS-CoV-2: Insights on Cost-Efficiency and Method Design

- Abstract
Introduction:Lower respiratory tract infections, including COVID-19, have a substantial global impact, making the development of diagnostic tests crucial. The study aimed to develop a new, accurate, fast, and cost-effective PCR-based detection method for SARS-CoV-2, applicable in limited settings and capable of detecting all current variants and potential future pathogens. Methods:The study was conducted between 2020 and 2022 at the molecular biology department of Mures County Clinical Hospital (MCCH), Romania. Initially, pharyngeal and nasal secretions were collected and processed using the real-time qRT-PCR method for routine COVID-19 diagnosis. Ninety-two samples were randomly selected to develop the assay, including samples from different infection periods and negative controls. Complementary DNA was prepared from the selected samples, and the presence and integrity of the extracted RNA were evaluated by amplifying the GAPDH housekeeping gene. Primers for three specific viral genes (N, ORF1ab, and S) were designed, and their efficiency was evaluated using endpoint PCR and sequencing. Finally, the method was optimized and implemented as a one-step triplex PCR assay for routine diagnostic use. Results:The molecular laboratory at the MCCH analyzed a total of 41,818 samples between June 2020 and December 2022. Among these samples, 26.15% tested positive for SARS-CoV-2, while 70.9% were negative and 2.95% were inconclusive or invalid. Three peaks of positive tests were observed in November 2020, April 2021, and February 2022. The study selected 92 preserved RNA samples for triplex PCR assay development, validating the primers’ specificity and confirming the quality of the nucleic acids. The comparative analysis showed the efficiency and accuracy of the endpoint reverse transcription triplex PCR method (RT-PCR), indicating its potential as a cost-effective alternative to real-time reverse transcription PCR (qRT-PCR) in low-income countries with limited infrastructure for COVID-19 testing. Conclusion:This method has the potential to facilitate large-scale diagnosis of SARS-CoV-2 infections, allowing for rapid and appropriate therapeutic management and ongoing monitoring of patients. Additionally, the method can be easily adapted for the detection of other pathogens.

50. Promotion of Socio-Environmental Sustainability by Water Utilities: Optimal Budget’s Estimation and Results’ Analysis of Varying Company Size

- Abstract
Sustainable management of water resources cannot ignore environmental aspects, let alone social and economic ones. To curb water waste, and groundwater pollution and promote equitable access to the resource, water utilities are required to implement a series of initiatives to promote environmental and social sustainability. In this paper, an optimization model is proposed to define the budget that water utilities should allocate to promote socio-environmental sustainability, optimally conveying their message and achieving the right trade-off between capital invested and audience reached. The optimization problem is solved using a nonlinear programming model. The application of the model shows that both big and small water companies should invest an amount that can increase the visibility of sustainability’s promotional message by 20%. However, this amount is about 0.2% of turnover for large utilities and about 17% for small utilities. The model can be a useful tool for both budgeting and incentivizing sustainable practices.
