Obtaining arrays of single nanoparticles with three-dimensional complex shapes is still an open challenge. Current nanolithography methods do not allow for the preparation of nanoparticles with complex features like nanostars. In this work, we investigate the optical printing of gold nanostars of different sizes as a function of laser wavelength and power. We found that tuning the laser to the main resonances of the nanostars in the near-infrared makes it possible to avoid nanoparticles reshaping due to plasmonic heating, enabling their deposition at the single particle level and in ordered arrays.

BackgroundThe long-term impact of cancer treatment is associated with respiratory dysfunction and physical fitness impairment. Although inspiratory muscle training (IMT) has been shown as an effective exercise therapy in cancer survivors, there is no evidence on the optimal dose, application moment nor specific population effects of this intervention. The main objective of this meta-analysis is to analyse the effects of IMT on pulmonary function, physical fitness and quality of life (QoL) in cancer survivors.MethodsThis systematic review and meta-analysis was preregistered in the International Prospective Register of Systematic Reviews (PROSPERO) register and conducted according to the Preferred Reporting for Systematic Reviews and Meta-analysis statement. We used a Bayesian multilevel random-effects meta-analysis model to pool the data. Multilevel metaregression models were used to examine the conditional effects of our covariates. Convergence and model fit were evaluated through specific model parameters. Sensitivity analyses removing influential cases and using a frequentist approach were carried out.ResultsPooled data showed that IMT intervention is effective to improve pulmonary function (standardised mean difference=0.53, 95% credible interval 0.13 to 0.94, SE=0.19). However, IMT did not present statistically significant results on physical fitness and QoL. Metaregression analyses found that the type of cancer, the moment of application and the evaluation tool used had significant moderation effects on pulmonary function.ConclusionIMT could be an important part in the management of side effects suffered by cancer survivors. Considering the current evidence, this intervention is highly recommended in patients diagnosed with oesophageal and lung cancers. IMT may provide superior benefits before the biological treatment and after the surgery.PROSPERO registration number304909.

Background: Non-alcoholic fatty liver disease (NAFLD) is now considered the most common chronic liver disease worldwide. NAFLD is related to changes in lipid metabolism and is characterized by the increase or accumulation of fat in hepatocytes that may progress to nonalcoholic steatohepatitis (NASH), which leads to the appearance of inflammatory processes. Treatment consists of changes in diet, physical activity, and weight control; however, these disorders represent a health problem and require the development of novel alternatives to treatment and prevention. NAFLD/NASH are strongly associated with other disorders, such as metabolic syndrome (MetS); in fact, NAFLD is considered the hepatic manifestation of MetS. These disorders are related to other components of MetS, including dyslipidemia, which is characterized by an imbalance in blood cholesterol and triglyceride levels. Prebiotics and probiotics benefit from treating and preventing several ailments, including liver diseases. Specifically, in dyslipidemia, NAFLD, and NASH, probiotics play a fundamental role in conducting the biotransformation of primary bile acids into secondary bile acids, which generally have important activity as immunomodulators and metabolism regulators. The mechanisms of action of pre and probiotics involve the activity of bile acid receptors, such as FXR and TGR-5, and the events resulting from their activation. Therefore, prebiotics and probiotics may be reasonable options to prevent and treat metabolic- related liver diseases.

El estudio tiene como objetivo describir las características demográficas, clínicas, funcionales y los desenlaces de los pacientes adultos mayores hospitalizados en la unidad de agudos del Hospital Universitario San Ignacio (HUSI).

Chronic kidney disease of unknown etiology (CKDUE) is one of the main global causes of kidney failure. Genetic studies may identify an etiology in these patients, but few studies have implemented genetic testing of CKDUE in a population-based series of patients, which was the focus of the GENSEN Study.

Cost‐effective miniaturized electrodes that maintain a high electroactive surface area (ESA) are needed for the widespread deployment of point‐of‐care sensors. Cost‐effective methods are recently developed to fabricate nanoroughened microstructured gold electrodes (NR‐MSEs) with ultrahigh ESA. In this work, the effectiveness of NR‐MSEs for bioelectrochemical enzymatic sensors is evaluated. A glucose sensor is constructed by first casting onto NR‐MSEs a solution containing reduced graphene oxide decorated with gold nanoparticles, glucose oxidase, and glutaraldehyde, followed by a solution containing ferrocene, and a layer of chitosan to prevent the leakage of sensor components. A urea biosensor is also fabricated using Nafion as a cationic exchanger for the electropolymerization of polyaniline, followed by the deposition of a composite containing urease, bovine serum albumin, and glutaraldehyde. The limit of quantification for both biosensors is below clinically relevant concentrations of the analytes in biofluids, 0.67 mmfor glucose and 1.70 mmfor urea. The sensors exhibit excellent performance in complex matrixes (human blood serum and wine for glucose and human blood serum and urine for urea), with recovery for spiked analytes in the range of 92–108%. It is anticipated that NR‐MSEs will expedite the development of highly sensitive bioelectrochemical sensors for use in resource‐limited settings. González–Martínez and collaborators present the use of hierarchically structured electrodes, fabricated through simple and cost‐effective benchtop techniques, for the implementation of enzymatic electrochemical sensors. The implementation for the detection of glucose and urea demonstrates the high sensitivity, reproducibility, and reusability of the sensors, paving the way for the use of nanoroughened microstructured electrodes in sensors for resource limited settings.

Human-robot collaborative systems have emerged as a solution that aims to satisfy the current demand of highly personalized products by combining the skills of humans and robots within a shared workspace. However, the safety of human operators is of major concern when located next to a robotic companion. The objective of this article is to present a general outlook of safety assurance practices in current collaborative systems of the manufacturing industry. In this sense, the integration of safety standards and technologies is analyzed. A favoring tilt to techno-centered practices is spotted, where policies designed for rigid automation are being used for collaborative systems alike. This brings additional risks to the wellbeing of human operators. Consequently, a human-centered approach is highlighted as the ultimate level of safety compliance.

A geometric approach to Sundman infinitesimal time-reparametrisation is given and some of its applications are used to illustrate the general theory. Cardiovascular diseases are a leading cause of mortality and pose a significant burden on healthcare systems worldwide. Despite remarkable progress in medical research, the development of effective cardiovascular drugs has been hindered by high failure rates and escalating costs. One contributing factor is the limited availability of mature cardiomyocytes (CMs) for accurate disease modeling and drug screening. Human induced pluripotent stem cell‐derived CMs offer a promising source of CMs; however, their immature phenotype presents challenges in translational applications. This review focuses on the road to achieving mature CMs by summarizing the major differences between immature and mature CMs, discussing the importance of adult‐like CMs for drug discovery, highlighting the limitations of current strategies, and exploring potential solutions using electro‐mechano active polymer‐based scaffolds based on conductive polymers. However, critical considerations such as the trade‐off between 3D systems and nutrient exchange, biocompatibility, degradation, cell adhesion, longevity, and integration into wider systems must be carefully evaluated. Continued advancements in these areas will contribute to a better understanding of cardiac diseases, improved drug discovery, and the development of personalized treatment strategies for patients with cardiovascular disorders. Cardiovascular diseases impose a substantial global burden, underscoring the necessity for effective drug development strategies reliant on the maturation of cardiomyocytes. This review explores the potential of cardiomyocytes derived from human induced pluripotent stem cells, focusing on strategies for achieving adult‐like maturity. Innovative electro‐mechano‐active polymer‐based scaffolds (EMAPS), based on conductive polymers, emerge as a promising platform in this highly urgent quest.

Lung adenosquamous carcinomas, cell line H125, with an expression of human epidermal growth factor receptor 1 (HER1), were cultured on two scaffolds of poly(-caprolactone) produced through electrospinning, one with a nanometric size and the other with a micrometric size. Cytotoxicity, adhesion and proliferation assays demonstrated the non-cytotoxicity of the scaffolds and the capacity of the cells to grow on them. Tumorlike morphology of cells, HER1 expression and the initial formation of invadopodia were detected through fluorescence microscopy. In the nanometric scaffolds, spheroid formation of cancerous cells was observed. Cells cultured in nanometric scaffolds presented higher resistance to cetuximab than those cultured in micrometric ones and also than cells of two-dimensional cultures. This work demonstrated the potentialities of poly(-caprolactone) scaffolds for lung adenosquamous carcinoma cultures and is an initial step for further studies on the growth of cancer cell spheroids on electrospun nanofibers.

Diabetic nephropathy (DN) is one of the most frequent complications in patients with diabetes mellitus (DM) and its diagnosis is usually established on clinical grounds. However, kidney involvement in some diabetic patients can be due to other causes, and renal biopsy might be needed to exclude them. The aim of our study was to establish the clinical and analytical data that predict DN and no-diabetic renal disease (NDRD), and to develop a predictive model (score) to confirm or dismiss DN.

La nefropatía diabética (ND) es una complicación frecuente de la diabetes mellitus (DM), y su diagnóstico suele ser clínico. Sin embargo, en numerosas ocasiones la enfermedad renal que presentan los pacientes diabéticos es debida a otras causas cuyo diagnóstico es histológico. El objetivo del estudio fue determinar los datos clínicos y analíticos predictores de ND y enfermedad renal no diabética (ERND), y elaborar un modelo predictivo (score)para confirmar o descartar ND.

Groundwater is an important resource that can help in climate change adaptation. However, the pollution of these aquifers with nitrate is a widespread problem of growing concern. Biological denitrification using inorganic electron donors shows significant advantages in treating nitrate-polluted groundwater where organic matter presence is negligible. However, mass transfer limitations and secondary contamination seem to be the major hinderance to spread the use of these technologies. This could be solved by the use of bioelectrochemical systems (BES), which emerge as an attractive technology to solve these problems due to the reported low energy demand and high denitrification rates. However, technical and operational issues must be considered to replicate these results at full-scale. This review summarizes the biological basis of autotrophic denitrification and the key aspects of its application in bioelectrochemical systems. In addition, an estimation of the capital costs required for the implementation of a BES considering different population sizes and initial nitrate concentration in the groundwater is made.

Abstract

Background: Liver ailments are among the leading causes of death; they originate from viral infections, chronic alcoholism, and autoimmune illnesses, which may chronically be precursors of cirrhosis; furthermore, metabolic syndrome may worsen those hepatopathies or cause Non-alcoholic Fatty Liver Disease (NAFLD) that may advance to non-alcoholic steatohepatitis (NASH). Cirrhosis is the late-stage liver disease and can proceed to hepatocellular carcinoma (HCC). Pharmacological treatment options for liver diseases, cirrhosis, and HCC, are limited, expensive, and not wholly effective. The use of medicinal herbs and functional foods is growing around the world as natural resources of bioactive compounds that would set the basis for the development of new drugs. Review and Conclusion: Plant and food-derived sterols and triterpenoids (TTP) possess antioxidant, metabolic-regulating, immunomodulatory, and anti-inflammatory activities, as well as they are recognized as anticancer agents, suggesting their application strongly as an alternative therapy in some chronic diseases. Thus, it is interesting to review current reports about them as hepatoprotective agents, but also because they structurally resemble cholesterol, sexual hormones, corticosteroids and bile acids due to the presence of the steroid nucleus, so they all can share pharmacological properties through activating nuclear and membrane receptors. Therefore, sterols and TTP appear as a feasible option for the prevention and treatment of chronic metabolic-related liver diseases, cirrhosis, and HCC.

Esophageal perforation constitutes a surgical emergency. Despite its gravity, no single strategy has been described as sufficient to deal with most situations to date. The aim of this study was to assess the etiology, management, and outcome of esophageal perforation over a 28-year period, to characterize optimal treatment options in this severe disease. A retrospective clinical review of all patients treated for esophageal perforation at Ramón y Cajal Hospital between January 1987 and December 2015 was performed (n = 57). Iatrogenic injury was the most frequent cause of esophageal perforation (n = 32). Abdominal esophagus was the main location (23 patients; 40.4%). Eight patients (14%) were managed with antibiotics and parenteral nutrition. In seven patients (12.3%), an endoscopic stent was implanted. Surgical therapy was performed in 38 patients (66.7%). Morbidity and 90-day mortality rates were 61.4 and 28 per cent, respectively. Five patients were reoperated (8.8%). Median hospital stay was 23.5 days. The mortality rate was higher among patients with spontaneous and tumoral perforation (54.5 and 100%; P= 0.009), delayed diagnosis (>24 hours; P= 0.0001), and abdominal/thoracic location (37.5%; P= 0.05). No statistical differences were found between surgical and conservative/endoscopic management (31% vs20%; P= 0.205) although hospital staying was longer in surgical group (36.30 days vs15.63 days; P= 0.029). Esophageal perforation was associated with high morbidity and mortality rates. Global outcomes depend on etiology, site of perforation, and delay in diagnosis. An individualized approach for each patient should be chosen to prevent septic complications of this potentially fatal disease.

Presentamos un estudio caso-control de tumores no definitorios de sida (TNDS) en una cohorte de pacientes infectados por el VIH en la que valoramos las tasas de incidencia, supervivencia y factores pronósticos de mortalidad.

Background: Colorectal cancer (CRC) is the second and third most frequent cancer in women and men, respectively; indeed, CRC is placed as the fourth world's most deadly cancer (after lung, liver, and stomach cancer). The incidence of CRC is strongly influenced by nutrition and the high fat/high carbohydrate Western-style diet. CRC is one of the most intensively studied cancer types, partly because of its high prevalence, but also because of the existence of its precursor lesions, tubular or villous adenomas, and more recently serrated adenomas. The morphological steps in the adenomacarcinoma sequence have been elucidated at a molecular level, which allow the identification of the genes responsible for CRC. Review and Conclusions: The main aim of this review is to provide data regarding the pathophysiological characteristics, molecular mechanisms as well as carcinogenic and chemopreventive agents for CRC, with emphasis on evidence supporting their efficacy. These compounds may modulate multiple signaling pathways involved in cell proliferation and apoptosis in transformed cells, they also enhance the host immune system and favor an effective treatment. Despite promising results from experimental studies, only a limited number of these compounds have been tested in clinical trials. The mechanistic spectrum and specificity of the action of phytochemicals represent a complex and evolving field of research.

In calcific aortic valve disease (CAVD), activated T lymphocytes localize with osteoclast regions; however, the functional consequences of this association remain unknown. We hypothesized that CD8+T cells modulate calcification in CAVD. CAVD valves (n = 52) dissected into noncalcified and calcified portions were subjected to mRNA extraction, real-time quantitative PCR, enzyme-linked immunosorbent assay, and immunohistochemical analyses. Compared with noncalcified portions, calcified regions exhibited elevated transcripts for CD8, interferon (IFN)-γ, CXCL9, Perforin 1, Granzyme B, and heat shock protein 60. Osteoclast-associated receptor activator of NK-κB ligand (RANKL), tartrate-resistant acid phosphatase (TRAP), and osteoclast-associated receptor increased significantly. The stimulation of tissue with phorbol-12-myristate-13-acetate and ionomycin, recapitulating CAVD microenvironment, resulted in IFN-γ release. Real-time quantitative PCR detected mRNAs for CD8+T-cell activation (Perforin 1, Granzyme B). In stimulated versus unstimulated organoid cultures, elevated IFN-γ reduced the mRNAs encoding for RANKL, TRAP, and Cathepsin K. Molecular imaging showed increased calcium signal intensity in stimulated versus unstimulated parts. CD14+monocytes treated either with recombinant human IFN-γ or with conditioned media-derived IFN-γ exhibited low levels of Cathepsin K, TRAP, RANK, and tumor necrosis factor receptor-associated factor 6 mRNAs, whereas concentrations of the T-cell co-activators CD80 and CD86 increased in parallel with reduced osteoclast resorptive function, effects abrogated by neutralizing anti–IFN-γ antibodies. CD8+cell–derived IFN-γ suppresses osteoclast function and may thus favor calcification in CAVD.

In the past decade, the surprising magnetic behavior of gold nanoparticles has been reported. This unexpected property is mainly attributed both to size and surface effects. Mesoporous thin films are ideal matrices for metallic nanoparticles inclusion, because of their highly accessible and tailorable pore systems that lead to completely tunable chemical environments. Exploiting these features, we synthesized Au nanoparticles within mesoporous titania thin films (film thickness of ∼150 nm and pore diameter of ∼5 nm), and we studied their magnetic properties under confinement. Here, we present the results of the magnetization as a function of temperature and magnetic field for this system, which are consistent with the previously reported for free (unconfined) thiol-capped gold nanoparticles. The successful inclusion of stable magnetic Au nanoparticles within transparent mesoporous thin films opens the gates for the application of these nanocomposites in two-dimensional (2D) microdevices technology and magneto-optical devices.

To analyze the impact of introducing omeprazole in the drug formulary of the Hospital de Barbanza on prescriptions made in hospital and out-of-hospital (Outpatient Units and Primary Care) for all Proton Pump Inhibitors (PPIs).

Background. Hypertension and type 2 diabetes (T2D) are the most prevalent noncommunicable diseases in Mexico and worldwide. According to international practice management guidelines, the principal chronic management therapy is daily oral medication. Aim. We aim to describe the trends of antihypertensive, antidiabetic, and nonsteroidal anti-inflammatory (NSAID) drugs use among the Mexican adult population from 2004–2018. Methods. We analyzed data from the Health Workers Cohort Study (HWCS) for males and females aged >18 years. We calculated the prevalence of chronic diseases and utilization for every kind of antihypertensive, antidiabetic, and NSAIDs (measured by self-reported utilization) at baseline and two follow-ups (2004, 2010, and 2017). Trends were analyzed using Fisher’s exact test. Results. Hypertension prevalence increased from 19.8 to 30.3%, higher than T2D prevalence from 7.0 to 12.8% through fourteen years of follow-up. Like the self-reported dual therapy, the proportion of patients using beta-blockers and angiotensin II receptor blockers increased. Regarding T2D, the prevalence of metformin utilization increased to 83.9%. The utilization of common NSAIDs, mainly for muscular pain, remained around 13 to 16%. Conclusions. Our findings showed a changing prevalence of drug utilization for hypertension and T2D between 2004 and 2018 and consistent use of NSAIDs in the adult Mexican population.

Paper has emerged as an excellent alternative to create environmentally benign disposable electrochemical sensing devices. The critical step to fabricating electrochemical sensors is making paper conductive. In this work, paper-based electrodes with a high electroactive surface area (ESA) were fabricated using a simple electroless deposition technique. The polymerization time of a polydopamine adhesion layer and the gold salt concentration during the electroless deposition step were optimized to obtain uniformly conductive paper-based electrodes. The optimization of these fabrication parameters was key to obtaining the highest ESA possible. Roughening factors (Rf) of 7.2 and 2.3 were obtained when cyclic voltammetry was done in sulfuric acid and potassium ferricyanide, respectively, demonstrating a surface prone to fast electron transfer. As a proof of concept, mercury detection was done through anodic stripping, achieving a limit of quantification (LOQ) of 0.9 ppb. By changing the metal deposition conditions, the roughness of the metalized papers could also be tuned for their use as surface-enhanced Raman scattering (SERS) sensors. Metallized papers with the highest SERS signal for thiophenol detection yielded a LOQ of 10 ppb. We anticipate that this method of fabricating nanostructured paper-based electrodes can accelerate the development of simple, cost-effective, and highly sensitive electrochemical and SERS sensing platforms.

En el área sanitaria de Santiago de Compostela, el subgrupo terapéutico “otros antipsicóticos” era el quinto en mayor gasto extrahospitalario en el año 2013, correspondiendo más de la mitad a la risperidona y paliperidona parenterales de acción prolongada. La implantación de un programa de seguimiento farmacoterapéutico basado en la gestión por procesos y en la coordinación de actuaciones entre profesionales sanitarios de ambos niveles asistenciales supuso, en 12 meses un ahorro para la organización de 636.391,01 €, para el paciente de 16.767,36 € en aportaciones y 9.008 desplazamientos a la oficina de farmacia.
