La obesidad es una enfermedad multifactorial, crónica, progresiva y recidivante considerada un problema de salud pública a nivel mundial y un importante determinante de discapacidad y muerte. En España su prevalencia actual en población adulta se sitúa en el 24%, y se estima su incremento hasta el 37% en 2035. La obesidad aumenta la probabilidad de diversas enfermedades vinculadas a una mayor mortalidad, como diabetes, enfermedades cardiovasculares, hiperlipidemia, hipertensión arterial, enfermedad del hígado graso no alcohólico, varios tipos de cáncer o apnea obstructiva del sueño. Por otra parte, aunque la incidencia de enfermedad inflamatoria intestinal (EII) se está estabilizando en los países occidentales, su prevalencia supera ya el 0,3%. Paralelamente a lo que ocurre en la población general, la prevalencia actual de obesidad en pacientes adultos con EII se estima en el 15-40%. La obesidad en pacientes con EII podría comportar, además de su impacto sobre la discapacidad y la mortalidad ya conocido, una peor evolución de la EII y una peor respuesta a los tratamientos. El objetivo del presente documento, elaborado en colaboración por cuatro sociedades científicas implicadas en la atención médica de la obesidad grave y la EII, es establecer unas recomendaciones claras y concisas sobre las posibilidades terapéuticas de la obesidad grave en pacientes con EII. En el documento se establecen recomendaciones generales sobre el tratamiento dietético, farmacológico, endoscópico y quirúrgico de la obesidad grave en pacientes con EII, así como la evaluación pre y postratamiento.

Neuromyelitis optica spectrum disorder (NMOSD) is characterised by recurrent attacks of optic neuritis and transverse myelitis. The purpose of this work was to identify the incidence and prevalence of NMOSD and its clinical characteristics in the population treated for demyelinating diseases in Western Mexico.

In this study, the application of Plasma Electrolytic Oxidation (PEO) technique was investigated for fabricating composite coatings on magnesium (Mg) substrates aimed at modulating their corrosion resistance, an important characteristic for biodegradable orthopedic implants. The coatings were manufactured using an electrolytic solution containing MgO and TiO2oxides, varying mass ratios, at currents of 30 mA and 200 mA. X-ray diffraction patterns confirmed the presence of TiO2and MgO phases in the coatings, demonstrating the PEO capability to modulate the chemical composition by adjusting the mass ratio of oxides in the electrolytic solution. Microstructural analysis revealed decreased surface roughness with increasing TiO2concentration. Corrosion tests indicated a significant improvement in corrosion resistance for all coatings compared to uncoated Mg substrate. In particularly, coatings with higher TiO2concentrations, and those applied at a current density of 30 mA exhibited, enhanced corrosion resistance.

Lower extremity deep venous thrombosis (DVT) is estimated to occur in 1 in 1000 persons annually in adult populations, with prevalence predicted to double by the year 2050. While acute DVT and pulmonary embolism are a major cause of cardiovascular morbidity and mortality, the long-term prognosis for patients with venous thromboembolism is in part determined by the development of post-thrombotic syndrome (PTS), which occurs in up to 50% of patients. PTS refers to a chronic syndrome complex, invariably characterized by intractable edema, pain, stasis dermatitis, and venous stasis ulceration when severe.

Organic diradicals are highly promising candidates as future components in molecular electronic and spintronic devices because of their low spin–orbit coupling. To advance toward final circuit realizations, a thorough knowledge of the behavior of diradicals within a single-molecule junction framework is imperative. In this work, we have measured for the first time the single-molecule conductance of a neutral open-shell diradical compound, a [2,1-b] isomer of indenofluorene (IF). Our results reveal that the conductance of the [2,1-b] isomer is about 1 order of magnitude higher than that of the corresponding closed-shell regioisomer [1,2-b] IF. This is significant, as it fundamentally demonstrates the possibility of forming stable single-molecule junctions using neutral diradical compounds which are also highly conducting. This opens up a new approach to the development of externally addressable spintronic devices operable at room temperature.

The IBA’s Section on Energy, Environment, Natural Resources and Infrastructure Law is advised by the Academic Advisory Group (AAG), a group of leading academics from around the world. Every two years, the AAG publishes a book about a key topic of interest to the entire Section. This year’s book focuses on the legal issues involved with the clean energy transition. In response to the climate emergency, several countries, corporations and other actors worldwide have announced programmes aimed at reducing global emissions of greenhouse gases that contribute to climate change to net zero by the year 2060 or earlier. While the need for a clean energy transition is clear, incoherently designed transition programmes could produce complex environmental, social and governance risks, including legal liability and protracted disputes. At the same time, the growing rush for minerals needed to manufacture clean energy technologies raises fundamental questions. Most crucial is how to ensure the exploration and development of energy transition minerals in a manner that does not exacerbate resource conflicts, resource nationalism, human rights violations, protectionism, energy insecurity, social exclusions and inequity, especially in conflict-affected and high-risk regions. These scenarios raise the need for net-zero and climate-aligned natural resource contracts, legislation and investment agreements to drive a just and inclusive energy transition. With presentations from some of the expert contributors to the latest AAG book, Net Zero and Natural Resources Law, this panel session reflected upon innovative legal drafting, contract negotiation and policymaking approaches needed to address the wide range of resource sovereignty, security and solidarity questions posed by the net zero transition.

Hypothalamic hamartomas are congenital lesions that typically present with gelastic seizures, refractory epilepsy, neurodevelopmental delay, and severe cognitive impairment. Surgical procedures have been reported to be effective in removing the hamartomas, however, they are associated with significant morbidity. Therefore, it is not considered a safe therapeutic modality. Image‐guided robotic radiosurgery (CyberKnife® Radiosurgery System) has been shown to provide good outcomes without lasting complications. This series of cases describes the clinical, radiological, radiotherapeutic, and postsurgical outcomes of five patients with epileptic encephalopathies secondary to hypothalamic hamartomas who were treated with CyberKnife®. All patients exhibited refractory epilepsy with gelastic seizures and were unsuitable candidates for surgical resection The prescribed dose ranged between 16 and 25 Gy, delivered in a single fraction for four patients and five fractions for one patient while adhering strictly to visual pathway constraints. After radiosurgery, four patients maintained seizure control (one with an Engel class Ia, three with an Engel class 1d), and another presented sporadic, nondisabling gelastic seizures (with an Engel class IIa). After 24–26 months of follow‐up, in three patients, their intelligence quotient scores increased. No complications were reported. This report suggests that Cyberknife may be a good option for treating hypothalamic hamartoma, particularly in cases where other noninvasive alternatives are unavailable. Nevertheless, additional studies are essential in order to evaluate the effectiveness of the technique in these cases.
