2. Management of pregnancies with anti-K alloantibodies and the predictive value of anti-K titration testing

- Abstract
Anti-KEL1 antigen (also referred to as anti-Kell, or anti-K) alloimmunisation is the second most common cause of severe haemolytic disease of the fetus and newborn, after anti-rhesus D antigen, and can cause substantial fetal morbidity and mortality. Both fetal erythropoietic suppression and haemolysis contribute to anaemia. Typically, once a clinically significant alloantibody is identified during pregnancy, antibody titration is performed as a screening test to predict the risk of anaemia and the need for maternal–fetal medicine referral. The titre is a semiquantitative laboratory method based on the underlying principle that increased maternal antibody concentrations are associated with an increased risk of fetal anaemia. Because some studies report that anti-K alloantibodies can lead to severe anaemia even at a low antibody titration, guidelines are inconsistent with respect to the role of titration testing. Some experts recommend maternal–fetal medicine referral and middle cerebral artery Doppler ultrasound without titration testing or with the use of a very low cutoff titre. This Viewpoint evaluates management for pregnancies affected by anti-K alloantibodies and highlights literature regarding the predictive value of anti-K titration testing.

3. Addressing Cardiovascular Risk in People With Psoriatic Disease Using a Care Coordinator Model: A Qualitative Analysis of a Pilot Study

- Abstract
Background:Risk for cardiovascular events is elevated in people with psoriatic disease. Our team developed a care coordination model to assist in managing cardiovascular risk in people with psoriatic disease. We piloted this model and study procedures prior to launching a fully powered prospective clinical trial. Objective:This study used qualitative methods to gather feedback on the care coordinator model from constituents (clinicians, patients, and care coordinators) who participated in the pilot trial to optimize the approach. Methods:We conducted 42 total interviews with people with psoriatic disease, referring clinicians (dermatologists and rheumatologists), primary care providers, and care coordinators who participated in the pilot study. A rapid qualitative analysis approach was used. Results:Perceptions of the care coordinator model were highly positive. Participants noted that the model raised their awareness of cardiovascular risk in people with psoriatic disease. They found it easy to follow the recommendations provided by the care coordinator. Participants identified areas for improvement related to eligibility criteria, increased personalization of materials and goal-setting, and clarification regarding next steps and responsibilities for follow-up after patients concluded participation. Additional feedback highlighted concerns about the intervention content overly focusing on statin medication therapy. Conclusion:Constituent recommendations gleaned via interviews were incorporated into the care coordinator model and adjustments were made to trial procedures. Insights from these interviews may also be relevant to those seeking to close care gaps for identification and treatment of cardiovascular risk in people with psoriatic disease using other interventions.

4. In Situ Annealing Effect on Thermally Co-Evaporated CsPbI2Br Thin Films Studied via Spectroscopic Ellipsometry

- Abstract
All-inorganic cesium lead halide perovskites possess excellent thermal stability, a feature that renders them highly favorable for optoelectronic applications with an elevated thermal budget. Employing a coevaporation approach for their deposition holds promise for manufacturing at an industrial level, owing to improvements in device scalability and reproducibility. For unlocking the full potential of vacuum-evaporated perovskite thin films, it is crucial to delve deeper into their crystallization process, which, as a solid-state reaction, has been less investigated compared to the crystallization process of, most commonly used, solution-based methods. In this work, we employ spectroscopic ellipsometry, a nondestructive, high speed, and high accuracy characterization method, to study the real time annealing effect on thermally coevaporated CsPbI2Br thin films in a temperature range between 25 and 300 °C. We achieve this by developing a singular dynamic model that can be fitted in real time as a function of temperature, providing insights into how thermal annealing influences the perovskite film’s morphology and optical constants. Based on the latter, we derive the temperature dependence of the thermo-optic coefficient and Urbach energy as well as analyze the interband transition energies via critical point analysis. We demonstrate that the γ- to β-phase transition can be identified through a pronounced shift in the bandgap energy, whereas the β- to α-phase transition can be discerned by a sharp increase in the film’s roughness. We corroborate the obtained fit results with additional in- and ex situ measurements, such as in situ grazing incidence wide-angle X-ray scattering, atomic force microscopy, reflectance/transmittance, and profilometry.

6. Narratieve coherentie over de intieme partner tijdens de zwangerschap: relatie met kindermishandeling en PTSS-symptomen

Kindermishandeling is gerelateerd aan de verstoring van de narratieve coherentie over hechte relaties. In deze pilotstudie introduceren we een kort, nieuw meetinstrument voor narratieve coherentie over de intieme partner en hierin hebben we onderzocht of symptomen van een posttraumatische stressstoornis (PTSS) de relatie tussen kindermishandeling en narratieve coherentie over de partner tijdens de zwangerschap medieert. De deelnemersgroep bestond uit 101 etnisch diverse, zwangere vrouwen met een laag inkomen (62 % leefde onder de armoedegrens; 37 % Latijns-Amerikaans, 22 % Afrikaans-Amerikaans, 20 % wit, 22 % anders), met een hoge score voor kindermishandeling. Zij gaven een Five-Minute Speech Sample van hun partner (d.w.z. de vader van de baby) en deze is door twee onafhankelijke codeurs beoordeeld op narratieve coherentie en prenatale geuite emoties (zoals negatief en warm affect) over de partner. De deelnemers hebben ook gestandaardiseerde vragenlijsten ingevuld over kindermishandeling, steun van de partner en prenatale symptomen van PTSS en depressie. Narratieve coherentie over de partner via de Five-Minute Speech Sample laat convergente validiteit zien met steun van de partner en geuite emoties over de partner. PTSS-symptomen mediëren de relatie tussen kindermishandeling en prenatale narratieve coherentie over de partner, maar symptomen van depressie niet. De Five-Minute Speech Sample biedt een korte en valide strategie voor het meten van narratieve coherentie over de partner. PTSS-symptomen en narratieve coherentie over de partner zijn allebei mogelijke interventiedoelen voor het bevorderen van gezond psychisch en relationeel functioneren tijdens de zwangerschap.

Dupilumab is a monoclonal antibody that targets the interleukin (IL)-4 receptor alpha subunit, thus blocking the effects of IL-4 and IL-13, and has shown efficacy in treating various conditions including asthma, atopic dermatitis, eosinophilic esophagitis, and others. Because of its immune modulatory effects, clinical trials that studied dupilumab did not allow patients to receive live vaccines during the clinical trials because of an abundance of caution, and thus package inserts recommend that patients who are being treated with dupilumab should avoid live vaccines. Because dupilumab is now approved for use in patients from 6 months of age for the treatment of atopic dermatitis, this reported contraindication is now posing a clinical dilemma for patients and clinicians.

- Abstract
This review summarizes new research developments and clinical practice recommendations for the diagnosis and management of anaphylaxis presented in the Joint Task Force on Practice Parameters 2023 Anaphylaxis practice parameter Update. It is intended to serve as a high-level summary of the 2023 practice parameter, which makes clinically impactful recommendations based on evidence that has emerged since the 2015 practice parameter. We invite clinicians to explore the full 2023 practice parameter to understand the research methods and underlying evidence that have informed the recommendations summarized here. There are new and evolving diagnostic criteria for anaphylaxis, rules for defining elevated tryptase levels, and recognition of signs and symptoms particular to infants and toddlers. The administration of epinephrine should not be used as a surrogate to diagnose anaphylaxis. Risk factors for anaphylaxis should be assessed on a case-by-case basis. Patient counseling and shared decision-making are essential to support patients' treatment decisions and capacity to manage the risk of anaphylaxis at home and in other community settings. Activation of emergency medical services after home epinephrine administration may not be required in all cases, and patients should be engaged in shared decision-making to determine when home management may be appropriate.

- Abstract
Semaglutide, a GLP-1 receptor agonist, reduces the risk of major adverse cardiovascular events (MACE) in people with overweight or obesity, but the effects of this drug on outcomes in patients with atherosclerotic cardiovascular disease and heart failure are unknown. We report a prespecified analysis of the effect of once-weekly subcutaneous semaglutide 2·4 mg on ischaemic and heart failure cardiovascular outcomes. We aimed to investigate if semaglutide was beneficial in patients with atherosclerotic cardiovascular disease with a history of heart failure compared with placebo; if there was a difference in outcome in patients designated as having heart failure with preserved ejection fraction compared with heart failure with reduced ejection fraction; and if the efficacy and safety of semaglutide in patients with heart failure was related to baseline characteristics or subtype of heart failure.

- Abstract
The objective of this study was to compare the effectiveness of a second tumor necrosis factor inhibitor (TNFi) versus a non‐TNFi biologic following discontinuation of a TNFi for patients with polyarticular‐course juvenile idiopathic arthritis (pJIA). Using the Childhood Arthritis and Rheumatology Research Alliance Registry, patients with pJIA who started receiving a second biologic following a first TNFi were identified. Patients were required to have no active uveitis on the index date and a visit six months after the index date. Outcome measures included Clinical Juvenile Arthritis Disease Activity Score with a maximum of 10 active joints (cJADAS10), cJADAS10 inactive disease (ID; ≤2.5) and cJADAS10 minimal disease activity (MiDA; ≤5). Multiple imputation was used to account for missing data. Adjusted odds ratios (aORs) were calculated using propensity score quintiles to compare outcomes at six months following second biologic initiation. There were 216 patients included, 84% initially received etanercept, and most patients stopped receiving it because of its ineffectiveness (74%). A total of 183 (85%) started receiving a second TNFi, and 33 (15%) started receiving a non‐TNFi. Adalimumab was the most common second biologic received (71% overall, 84% of second TNFi), and tocilizumab was the most common non‐TNFi second biologic received (9% overall, 58% of non‐TNFi). There was no difference between receiving TNFi versus non‐TNFi in cJADAS10 ID (29% vs 25%; aOR 1.23, 95% confidence interval [CI] 0.47–3.20) or at least MiDA (43% vs 39%; aOR 1.11, 95% CI 0.47–2.62) at six months. Most patients with pJIA started receiving TNFi rather than non‐TNFi as their second biologic, and there were no differences in disease activity at six months.

11. Identifying Essential Content in the Introduction to Adapted Physical Education Class

- Abstract
ABSTRACTPhysical education teachers often teach students with disabilities despite feeling ill-prepared to and do so. Most physical education teacher education programs in the US and abroad only include one single course in adapted physical education (APE) which predicts physical educators’ lack of preparation to support students with disabilities. Furthermore, instructors of this one APE course (in PETE programs) may not be professionally trained in the area of APE. Thus, it is paramount that the content and instruction provided in this one APE course is optimized, yet limited consensus exists as to what that content should include. The purpose of this study was to determine the most important topics to include in the Introduction to Adapted Physical Educationclass for physical education majors enrolled in teacher training programs. In this research, we conducted a three-phase descriptive/analytic study to obtain consensus regarding the most important adapted physical education topics needed for future physical educators to better support teaching students with disabilities. In phase one, 200 general and adapted physical educators provided their top 10 topics to be taught in this Intro to APE class in one open-ended question. In phase two, four APE “experts” created themes from the 2000 responses occurring in step one. Finally, in phase three, 15 APE “experts” rated the themes from 1 (least) to 5 (most) important. The top-rated topics are included in our results. Future research and implications are discussed.

12. The Physical Activity Self-Advocacy Inventory for Adolescents with and without Disabilities: A Delphi Study

ABSTRACTSelf-advocacy is a critical predictor of actual physical activity participation for children and adolescents with disabilities. Despite its reference within national standards for health and physical education, few practitioners are purported to promote self-advocacy among their students warranting the need for evaluation. However, no self-advocacy inventory exists for children and adolescents with robust psychometrics properties. The purpose of this study was to investigate the content and face validity for the Physical Activity Self-Advocacy Inventory (PASAI) within children and adolescents with and without disabilities. Experts in the fields of health and physical education (N =12) participated in this Delphi study. Once experts established the content/face validity for the PASAI, children and adolescents (N =18) provided feedback on the PASAI to further refine the questions and support comprehension/relevance for the target population. After the first round of questions, experts’ average feedback was a 3.99 out of 5.00 across all items. Due to low scores and based upon experts’ suggestions, four questions were deleted, and one question was added. After round two, experts’ average ratings increased to 4.8 out of 5.00. The PASAI appears to have reasonable content/face validity properties per expert- and population-based feedback. Future research should vet additional psychometric properties of PASAI scores.

13. Histologic Features of Mycobacterial Spindle Cell Pseudotumors

Mycobacterial spindle cell pseudotumors (MSPs) are a rare and diagnostically challenging manifestation of non-tuberculous mycobacterial (NTM) infections. Proper recognition of these pseudotumors is important because they are treatable and benign. In this study, we evaluated the morphologic patterns of MSPs to improve their pathologic identification. Clinical and morphologic features of 14 MSPs were analyzed. Histologic factors evaluated included the architectural growth pattern of spindled or epithelioid macrophages, granulomas and their location within the lesion, neutrophilic microabscesses, multinucleated giant cells, necrosis, and effacement of background tissue. The composition of inflammatory infiltrates, organism density by acid-fast staining, and stromal changes were also assessed. In addition, 8 of 14 cases underwent molecular microbiology identification by a clinical amplicon-sequencing assay for non-tuberculous mycobacteria. MSP sites included 2 bowel, 10 lymph nodes, 1 liver, and 1 extremity. Cases with available clinical history (n=10) all occurred in immunocompromised patients. All demonstrated effacement of normal structures with spindled cells arranged in a storiform or fascicular architectural pattern. In addition, all cases showed lymphocytic inflammation, with prominent concurrent neutrophilic inflammation in 50% (7/14) of cases. Other morphologic findings included foamy histiocytes (64%, 9/14), peripherally situated granulomas (21%, 3/14), and neutrophilic microabscesses (21%, 3/14). All tested cases were positive for NTM by PCR methods. Mycobacterium aviumwas the most commonly isolated pathogen (6/8). Mycobacterial spindle cell pseudotumors show predominantly spindled morphology that may be mistaken as a neoplasm. Surgical pathologists who evaluate lymph nodes, soft tissue, and gastrointestinal tissues should be aware of this spindled tumefactive phenomenon in the setting of immunocompromised patients. Recognition of key morphologic features of neutrophilic inflammation, peripheral granulomas, or foamy histiocytes within a spindled lesion can help guide the pathologist to a correct diagnosis of an inflammatory process secondary to infection rather than a spindle cell neoplasm. Accurate diagnosis to facilitate appropriate antimicrobial and/or surgical therapy requires a comprehensive evaluation combining clinical, histopathologic, and microbiological findings.

- Abstract
This practice parameter update focuses on 7 areas in which there are new evidence and new recommendations. Diagnostic criteria for anaphylaxis have been revised, and patterns of anaphylaxis are defined. Measurement of serum tryptase is important for diagnosis of anaphylaxis and to identify underlying mast cell disorders. In infants and toddlers, age-specific symptoms may differ from older children and adults, patient age is not correlated with reaction severity, and anaphylaxis is unlikely to be the initial reaction to an allergen on first exposure. Different community settings for anaphylaxis require specific measures for prevention and treatment of anaphylaxis. Optimal prescribing and use of epinephrine autoinjector devices require specific counseling and training of patients and caregivers, including when and how to administer the epinephrine autoinjector and whether and when to call 911. If epinephrine is used promptly, immediate activation of emergency medical services may not be required if the patient experiences a prompt, complete, and durable response. For most medical indications, the risk of stopping or changing beta-blocker or angiotensin-converting enzyme inhibitor medication may exceed the risk of more severe anaphylaxis if the medication is continued, especially in patients with insect sting anaphylaxis. Evaluation for mastocytosis, including a bone marrow biopsy, should be considered for adult patients with severe insect sting anaphylaxis or recurrent idiopathic anaphylaxis. After perioperative anaphylaxis, repeat anesthesia may proceed in the context of shared decision-making and based on the history and results of diagnostic evaluation with skin tests or in vitro tests when available, and supervised challenge when necessary.

- Abstract
Cancer cells edit gene expression to evade immunosurveillance. However, genome-wide studies of gene editing during early tumorigenesis are lacking. Here we used single-cell RNA sequencing in a breast cancer genetically engineered mouse model (GEMM) to identify edited genes without bias. Late tumors repressed antitumor immunity genes, reducing infiltrating immune cells and tumor–immune cell communications. Innate immune genes, especially interferon-stimulated genes, dominated the list of downregulated tumor genes, while genes that regulate cell-intrinsic malignancy were mostly unedited. Naive and activated CD8+T cells in early tumors were replaced with exhausted or precursor-exhausted cells in late tumors. Repression of immune genes was reversed by inhibiting DNA methylation using low-dose decitabine, which suppressed tumor growth and restored immune control, increasing the number, functionality and memory of tumor-infiltrating lymphocytes and reducing the number of myeloid suppressor cells. Decitabine induced important interferon, pyroptosis and necroptosis genes, inflammatory cell death and immune control in GEMM and implanted breast and melanoma tumors.

19. What Are We Spending on Special Education in the U.S.?

- Abstract
This brief summarizes data from a 1994-95 survey of the states on special education expenditures in the U.S. and analyzes other extant cost data that adjust for inflation and consider rising special education enrollments. The study estimates that the 1995-96 national expenditure for special education was about $32.6 billion (compared to $19.3 billion in 1987-88), the increase being primarily due to continued rising special education enrollments. The survey found considerable variability in expenditures per student among the 24 reporting states (from $2,758 in Indiana to $8,509 in Connecticut). Half of the surveyed states were unable to report the statewide cost of their special education programs and only 13 reported a high degree of confidence in their responses. Additionally, the relative shares of federal, state, and local expenditures were found to vary enormously by state. A table details special education expenditures of the 24 reporting states. A second table compares results of using alternative ways of estimating expenditures on special education and other special needs programs. These take into account such factors as the percentage of children with special needs, the ratio of expenditure on each special needs program to regular education, and marginal expenditure (i.e., excess over regular education) per child by program. The paper concludes that more uniform, refined data are required for accurate estimates of special education expenditures. (DB)

This brief guide presents principles and suggestions to help individuals who are deaf-blind enjoy and benefit from participation in recreational activities. Principles consider the need to: (1) start with the individual and determine what he/she is interested in, focusing on the selection of safe, age-appropriate activities; (2) research the individual's communication patterns and cooperatively develop a recreation plan with both short- and long-term objectives; (3) maintain interest through establishing a specific time period for trying out a new activity, selecting the proper time of day to regularly schedule the activity, and modifying the activity when necessary; and (4) identify effective teaching strategies. Effective teaching strategies include orientation of the individual to the playing area, use of braille or feeling and imitation, and facilitation of the individual's choice-making. - Abstract
This paper is devoted to educational reform strategy and tactics relating to the National Education Association (NEA) and the American Federation of Teachers (AFT), the nation's largest teacher unions. As is widely recognized, the teacher unions are the major obstacle to market oriented educational reforms, such as school choice or contracting out instruction to the private sector. They are also the main opposition to lowering the age of compulsory schooling, home schooling, or other reforms that would shrink the market for public education. Essentially, it is argued that it is now feasible to eliminate NEA/AFT veto power over state educational policy. This objective can be achieved in a few years, even without repeal of the state bargaining laws which have been the foundation of teacher union membership, revenues, and political power in most states. The argument is that legislative changes in the nonbargaining states and amendments to the bargaining laws in ways that are thoroughly justified on public policy grounds, apart from their implications for union political influence, will suffice. Indeed, in some cases, the amendments proposed would merely apply the safeguards for union members under the National Labor Relations Act (NLRA) to state bargaining statutes which govern school district labor relations. Partly for this reason, the changes cannot reasonably be characterized as "extremist" or some such pejorative term that suggests a narrow ideological approach to the issues of union power. (Author)

22. Perceptions of Diversity Training Needs in High Tech Business.

- Abstract
A study questioned 12 human resource personnel, corporation managers, and diversity trainers about their perceptions of diversity training needs in Pacific Northwest high tech organizations. The overarching research questions for the study were as follows: (1) What are the most frequently reported diversity training needs among human resource personnel and company managers currently working in Pacific Northwest high tech corporations? (2) What are the diversity training needs in high tech corporations reported by professional diversity trainers? All responses were analyzed using an analytical inductive approach. Emergent themes were identified and categorized. Results indicated that all participants expressed a need for common understanding of the concept "diversity" and"diversity training." Also, most participants felt a need to develop more extensive cultural diversity programs, though human resource personnel and managers expressed the need to obtain upper management support if diversity training was to become company policy. Finally, trainers voiced the belief that organizations look for more "quick-fix" approaches to diversity training rather than long term changes in organizational culture. These results are not necessarily generalizable. A study gathering data from a wider and more inclusive sample from other than Pacific Northwest high tech companies is in order. Also, findings from other studies comparing perceptions of other employees (e.g., top executives, line staff) could provide diversity trainers and high tech corporations with more complete information about employee needs and diversity trainer assessment of their clients' needs. 43. Canadian tertiary care pediatric massive hemorrhage protocols: a survey and comprehensive national review

Purpose: Hemorrhage is the leading cause of pediatric death in trauma and cardiac arrest during surgery. Adult studies report improved patient outcomes using massive hemorrhage protocols (MHPs). Little is known about pediatric MHP adoption in Canada. Methods: After waived research ethics approval, we conducted a survey of Canadian pediatric tertiary care hospitals to study MHP activations. Transfusion medicine directors provided hospital/patient demographic and MHP activation data. The authors extracted pediatric-specific MHP data from requested policy/procedure documents according to seven predefined MHP domains based on the literature. We also surveyed educational and audit tools. The analysis only included MHPs with pediatric-specific content. Results: The survey included 18 sites (100% response rate). Only 13/18 hospitals had pediatric-specific MHP content: eight were dedicated pediatric hospitals, two were combined pediatric/obstetrical hospitals, and three were combined pediatric/adult hospitals. Trauma was the most common indication for MHP activation (54%), typically based on a specific blood volume anticipated/transfused over time (10/13 sites). Transport container content was variable. Plasma and platelets were usually not in the first container. There was little emphasis on balanced plasma/platelet to red-blood-cell ratios, and most sites (12/13) rapidly incorporated laboratory-guided goal-directed transfusion. Transfusion thresholds were consistent with recent guidelines. All protocols used tranexamic acid and eight sites used an audit tool. Discussion/Conclusion: Pediatric MHP content was highly variable. Activation demographics suggest underuse in nontrauma settings. Our findings highlight the need for a consensus definition for pediatric massive hemorrhage, a validated pediatric MHP activation tool, and prospective assessment of blood component ratios. A national pediatric MHP activation repository would allow for quality improvement metrics.

- Abstract
Paediatric multisystem inflammatory syndrome temporally associated with SARS-CoV-2 (PIMS-TS), also known as multisystem inflammatory syndrome in children (MIS-C) emerged in April, 2020. The paediatric comparisons within the RECOVERY trial aimed to assess the effect of intravenous immunoglobulin or corticosteroids compared with usual care on duration of hospital stay for children with PIMS-TS and to compare tocilizumab (anti-IL-6 receptor monoclonal antibody) or anakinra (anti-IL-1 receptor antagonist) with usual care for those with inflammation refractory to initial treatment.

- Abstract
Animals can learn about sources of danger while minimizing their own risk by observing how others respond to threats. However, the distinct neural mechanisms by which threats are learned through social observation (known as observational fear learning1–4(OFL)) to generate behavioural responses specific to such threats remain poorly understood. The dorsomedial prefrontal cortex (dmPFC) performs several key functions that may underlie OFL, including processing of social information and disambiguation of threat cues5–11. Here we show that dmPFC is recruited and required for OFL in mice. Using cellular-resolution microendoscopic calcium imaging, we demonstrate that dmPFC neurons code for observational fear and do so in a manner that is distinct from direct experience. We find that dmPFC neuronal activity predicts upcoming switches between freezing and moving state elicited by threat. By combining neuronal circuit mapping, calcium imaging, electrophysiological recordings and optogenetics, we show that dmPFC projections to the midbrain periaqueductal grey (PAG) constrain observer freezing, and that amygdalar and hippocampal inputs to dmPFC opposingly modulate observer freezing. Together our findings reveal that dmPFC neurons compute a distinct code for observational fear and coordinate long-range neural circuits to select behavioural responses.

47. Anodic Cyclizations, Densely Functionalized Synthetic Building Blocks, and the Importance of Recent Mechanistic Observations

- Abstract
Anodic cyclization reactions can provide a versatile method for converting newly obtained chiral lactols to densely functionalized cyclic building blocks. The method works by first converting the lactol into an electron-rich olefin and then oxidatively generating a radical cation that is trapped by a nucleophile. Historically, such reactions have benefited from the use of less polar radical cations when the trapping nucleophile is a heteroatom and more polar radical cations when the reaction forms C–C bonds. This forced one to optimize underperforming reactions by resynthesizing the substrate. Here, we show that by taking advantage of methods that serve to drive a reversible initial cyclization reaction toward the product, this dichotomy and need to manipulate the substrate can be avoided. Two such methods were utilized: a faster second oxidation step and a mediated electrolysis. Both led to successful cyclizations using a polar radical cation and heteroatom nucleophiles.

48. Hearing parents learning American Sign Language with their deaf children: a mixed-methods survey

- Abstract
Hearing parents with deaf children face difficult decisions about what language(s) to use with their child. Sign languages such as American Sign Language (ASL) are fully accessible to deaf children, yet most hearing parents are not proficient in ASL prior to having a deaf child. Parents are often discouraged from learning ASL based in part on an assumption that it will be too difficult, yet there is little evidence supporting this claim. In this mixed-methods study, we surveyed hearing parents of deaf children (n= 100) who had learned ASL to learn more about their experiences. In their survey responses, parents identified a range of resources that supported their ASL learning as well as frequent barriers. Parents identified strongly with belief statements indicating the importance of ASL and affirmed that learning ASL is attainable for hearing parents. We discuss the implications of this study for parents who are considering ASL as a language choice and for the professionals who guide them.

49. How to diagnose IgE-mediated food allergy

- Abstract
Immunoglobulin E (IgE)-mediated food allergy is an immune response, typically to a food protein. Accurate diagnosis reduces unnecessary dietary restrictions and economic and psychological burden on patients and caregivers but relies on a rigorous clinical history, specific IgE diagnostic tests and, where needed, oral food challenge. Increased awareness is needed around which patients to test for IgE-mediated food allergy, as well as terms commonly associated with IgE-mediated food allergy testing, in order to optimise patient diagnosis and management. Herein, we describe approaches to diagnosis of IgE-mediated food allergy, appropriate interpretation of results and risks of overtesting.

50. Unequal social vulnerability to Hurricane Sandy flood exposure

- Abstract
Disparities exist in post-disaster flooding exposure and vulnerable populations bear a disproportionate impact of this exposure. We describe the unequal burden of flooding in a cohort of New York residents following Hurricane Sandy and assess whether the likelihood of flooding was distributed equally according to socioeconomic demographics, and whether this likelihood differed when analyzing self-reported or FEMA flood exposure measures. Residents of New York City and Long Island completed a self-administered survey 1.5–4.0 years after the storm. Multivariable logistic regressions were performed to determine the relationship between sociodemographic characteristics and flood exposure. Participants (n= 1231) residing in areas of the lowest two quartiles of median household income experienced flooding the most often (FEMA/self-reported: <$40,298: 65.3%/42.0%, $40,298–$67,188: 43.3%/32.1%), and these areas contained the highest proportions of non-White participants (<$40,298: 39.1%, $40,298–$67,188: 36.6%) and those with ≤high school education (<$40,298: 35.5%, $40,298–$67,188: 33.6%). Both self-report (p< 0.05) and FEMA (p< 0.05) flood measures indicated that older participants were more likely to live in a household exposed to flooding, while those living in higher-income areas had decreased likelihood of flooding (p< 0.0001). Socioeconomic and age disparities were present in exposure to flooding during Hurricane Sandy. Future disaster preparedness responses must understand flooding from an environmental justice perspective to create effective strategies that minimize disproportionate exposure and its adverse outcomes.
