BackgroundAdolescents and young adults with risk factors for opioid misuse and opioid use disorder are at elevated risk for overdose. We examined prior non-fatal overdose experiences among at-risk adolescents/young adults to inform prevention efforts.MethodsAdolescents/young adults (ages 16–30) in two US emergency departments self-reporting past year opioid misuse or opioid use plus a misuse risk factor completed a baseline survey as part of an ongoing randomised controlled trial. We describe baseline factors associated with (a) overall non-fatal overdose experiences and (b) groups based on substance(s) used during the worst overdose experience.ResultsAmong 771 participants (27.9% male), 40.7% reported a non-fatal overdose experience. Compared with those without a prior overdose experience, those with prior overdose experience(s) were less likely to be heterosexual, and more likely to report a prior suicide attempt and greater peer substance misuse. Regarding the worst overdose experience, substance(s) included: 36.6% alcohol only, 28.0% alcohol and cannabis, 22.6% alcohol with other substance(s) and 12.7% other substance(s) only (eg, opioids). Compared with the alcohol only group, the alcohol and cannabis group were younger and less likely to be heterosexual; the alcohol with other substance(s) group were older and had greater peer substance misuse; and the other substance(s) only group were more likely to be male, receive public assistance, screen positive for anxiety and less likely to be heterosexual.ConclusionsAmong at-risk adolescents/young adults, findings support the need for tailored overdose prevention efforts based on substance(s) used, with consideration of sexuality, mental health and peer substance use.Trial registration numberNCT04550715.

Objective: Maternal depression is a consistent and well-replicated risk factor for child psychopathology. The authors examined the changes in psychiatric symptoms and global functioning in children of depressed women 1 year following the initiation of treatment for maternal major depressive disorder. Method: Participants were 1) 151 women with maternal major depression who were enrolled in the Sequenced Treatment Alternatives to Relieve Depression (STAR*D) study and 2) their eligible offspring who, along with the mother, participated in the child STAR*D (STAR*D-Child) study (mother-child pairs: N=151). The STAR*D study was a multisite study designed to determine the comparative effectiveness and acceptability of various treatment options for adult outpatients with nonpsychotic major depressive disorder. The STAR*D-Child study examined children of depressed women at baseline and involved periodic follow-ups for 1 year after the initiation of treatment for maternal major depressive disorder to ascertain the following data: 1) whether changes in children's psychiatric symptoms were associated with changes in the severity of maternal depression and 2) whether outcomes differed among the offspring of women who did and did not remit (mother-child pairs with follow-up data: N=123). Children's psychiatric symptoms in the STAR*D-Child study were assessed using the Schedule for Affective Disorders and Schizophrenia for School-Age Children--Present and Lifetime Version (K-SADS-PL), and maternal depression severity in the STAR*D study was assessed by an independent clinician, using the 17-item Hamilton Depression Rating Scale (HAM-D). Results: During the year following the initiation of treatment, maternal depression severity and children's psychiatric symptoms continued to decrease over time. Decreases in the number of children's psychiatric symptoms were significantly associated with decreases in maternal depression severity. When children's outcomes were examined separately, a statistically significant decrease in symptoms was evident in the offspring of women who remitted early (i.e., within the first 3 months after the initiation of treatment for maternal depression) or late (i.e., over the 1-year follow-up interval) but not in the offspring of nonremitting women. Conclusions: Continued efforts to treat maternal depression until remission is achieved are associated with decreased psychiatric symptoms and improved functioning in the offspring.

An interprofessional approach from healthcare professionals can assist the woman with opioid use disorder (OUD) to become free of using opioids during pregnancy and beyond. These vulnerable women and their newborns need extended community support. The purpose of this article is to provide foundational information and standards that support the collaboration of community professionals in providing healthcare and treatment options for the woman with OUD to promote the family unit remaining together, support bonding, and encourage lactation/breastfeeding. Women with OUD need guidance from Christian nurses and the community as they transition to sobriety, motherhood, and breastfeeding.

The ideas of Hannah Arendt about private and public space and her metaphor of politics as storytelling are used as the foundation for a discussion of the role of public administrators in the construction of public space. Issues addressed include the creation of space fro people to come together to talk and make decisions about what they will do next, and the concept of what exactly is public space, especially in contemporary times that are radically different to the ancient Greek era from which many of the concepts of public and political space sprung forth.

The supposed dichotomy between theory and practice is, like other dichotomies we have known, a false one. It is rooted in a paradigm in which theory is conceptualized as lawlike generalizations that cumulatively constitute an ever-growing body of knowledge. Practitioners/experts are supposedly then able to improve managerial effectiveness by applying such knowledge to practice. When public administration theory fails to produce relevant lawlike generalizations and instead produces abstract conjecture, a gap between theory and practice is perceived to exist. The authors propose practical theory wherein practice and theory are mutually constitutive. This study examined bereavement‐related risk markers (number of deaths, cause of death, and relationship to deceased) of mental and behavioral health problems (suicidal thoughts or behaviors, self‐injury, depression, posttraumatic stress, and substance use) in a national sample of clinic‐referred bereaved adolescents. Participants included 1281 bereaved youth aged 12–21 years (M=15, SD=1.8; 62.1% female), from the National Child Traumatic Stress Network Core Data Set. Generalized linear mixed‐effects regression models controlling for demographics and other traumas revealed that youth bereaved by multiple deaths had higher posttraumatic stress scores than youth bereaved by a single death (Estimated difference ±SE=3.36 ± 1.11,p=0.003). Youth bereaved by suicide were more likely to report experiencing suicidal thoughts or behaviors (AOR=1.68, p=0.049) and alcohol use (AOR=2.33, p<0.001) than youth bereaved by natural causes. Youth bereaved by homicide were at greater risk for substance use than youth bereaved by natural death (AOR=1.76, p=0.02). Compared to parentally bereaved youth, youth who lost a peer were more likely to use alcohol (AOR=2.32, p=0.02) or other substances (AOR=2.41, p=0.01); in contrast, parentally bereaved youth were more likely to experience depression compared to those who experienced the death of an adult relative or unrelated adult (range of AOR: 0.40 to 0.64, p‐values<0.05). These bereavement‐related contextual factors can serve as early markers of mental and behavioral health problems among bereaved youth. Polymeric differences of plastic debris were assessed across four compartments of the Main Hawaiian Islands (sea surface, windward beaches, leeward beaches, and seafloor) to better describe sources and fate. Plastic debris pieces (n= 4671) were collected from 11 beaches, three sea surface tows, and three seafloor dives. Fourier transform infrared spectroscopy identified the polymers of 3551 pieces. Significant differences (p< 0.05) in concentration, types, polymer composition, and weathering were found among four compartments. Windward beaches had 1–2 orders of magnitude more plastic pollution (g/m2) than leeward beaches, despite smaller human populations on windward sides. Sea surface and windward beaches were dominated by severely weathered, less dense floating polymers (polyethylene and polypropylene comprised 92.7 and 93.5% on average, respectively, of the total debris mass), while leeward beaches and the seafloor debris consisted of less weathered and more dense sinking polymers (e.g., 41.0 and 44.7% of total mass consisted of the sum of polystyrene, nylon, cellulose acetate, polyethylene terephthalate, and additive-masked debris). These results are some of the first to provide evidence of polymeric stratification in the marine environment and emphasize that the majority of marine debris in Hawaii is floating in from distant sources rather than from Hawaii’s residents or tourists.

IMPORTANCE: Firearm injuries are the second leading cause of death among US children and adolescents. Because of the lack of resources allocated to firearm injury prevention during the past 25 years, research has lagged behind other areas of injury prevention. Identifying timely and important research questions regarding firearm injury prevention is a critical step for reducing pediatric mortality. OBJECTIVE: The Firearm Safety Among Children and Teens (FACTS) Consortium, a National Institute for Child Health and Human Development–funded group of scientists and stakeholders, was formed in 2017 to develop research resources for the field, including a pediatric-specific research agenda for firearm injury prevention to assist future researchers and funders, as well as to inform cross-disciplinary evidence-based research on this critical injury prevention topic. EVIDENCE REVIEW: A nominal group technique process was used, including 4 key steps (idea generation, round-robin, clarification, and voting and consensus). During idea generation, stakeholders and workgroups generated initial research agenda topics after conducting scoping reviews of the literature to identify existing gaps in knowledge. Agenda topics were refined through 6 rounds of discussion and survey feedback (ie, round-robin, and clarification steps). Final voting (using a 5-point Likert scale) was conducted to achieve consensus (≥70% of consortium ranking items at 4 or 5 priority for inclusion) around key research priorities for the next 5 years of research in this field. Final agenda questions were reviewed by both the stakeholder group and an external panel of research experts not affiliated with the FACTS Consortium. Feedback was integrated and the final set of agenda items was ratified by the entire FACTS Consortium. FINDINGS: Overall, 26 priority agenda items with examples of specific research questions were identified across 5 major thematic areas, including epidemiology and risk and protective factors, primary prevention, secondary prevention and sequelae, cross-cutting prevention factors, policy, and data enhancement. CONCLUSIONS AND RELEVANCE: These priority agenda items, when taken together, define a comprehensive pediatric-specific firearm injury prevention research agenda that will guide research resource allocation within this field during the next 5 years.

Despite the high prevalence of psychiatric emergency (PE) visits for attempted suicide and nonsuicidal self-injury (NSSI) among adolescents, we have limited information about assessment tools that are helpful in predicting subsequent risk for suicide attempts among adolescents in PE settings. This study examined the predictive validity of a highly promising instrument, the Columbia-Suicide Severity Rating Scale (C-SSRS).

An online survey of public administration graduate students in the United States1was conducted in early 2010 to measure students’ views on public participation. In addition, document analysis was used to investigate MPA curricula and the extent to which NASPAA-accredited programs were teaching courses in this area. Survey results2indicate that most respondents (a) believe public participation is important to good governance, (b) see themselves as facilitators of public participation, and (c) are interested in learning more about the subject. Analysis of MPA-accredited program curricula showed few graduate-level public administration programs offering courses on public participation and citizen engagement. The findings point to the opportunity, and need, for greater emphasis on public participation in MPA curricula.

The objective of this study was to investigate adolescent and parent attitudes toward screening adolescents for suicide risk and other mental health problems in the emergency department (ED).

This autobiographical essay addresses the question: How do the needs of students of working-class origins differ from those of their counterparts from more privileged backgrounds? As one of the invisible differences in the United States, class pervades everything we do, and we are mostly unaware of it. Readers are encouraged to examine their own presumptions about social class, including their suppositions about access to resources and how these can differ based on one’s family of origin. In addition to suggestions on how to address social class in the classroom, readers are encouraged to raise their own consciousness about class in order to reach out to students from working-class backgrounds.

Adolescent mental health service utilization has received increasing attention recently, with epidemiological studies revealing rates of service use that fall far below rates of psychopathology. Our primary aim in this article is to review and integrate current conceptions of the function that parents can serve in obtaining mental health services for adolescents. The review highlights the importance of developing a process‐oriented model of service seeking. The initial parental help‐seeking steps of gaining awareness of adolescents' distress and identifying specific emotional or behavioral problems receive particular attention. We stress that only by focusing on the role of parents and attending to specific developmental influences can the phenomenon of adolescent mental health service utilization be truly understood.

Fluoxetine, a selective serotonin reuptake inhibitor, is gaining increased acceptance in the treatment of adolescent depression. Generally safe and well tolerated by adults, fluoxetine has been reported to induce mania. The cases of five depressed adolescents, 14–16 years of age, who developed mania during pharmacotherapy with fluoxetine, are reported here. Apparent risk factors for the development of mania or hypomania during fluoxetine pharmacotherapy in this population were the combination of attention-deficit hyperactivity disorder and affective instability; major depression with psychotic features; a family history of affective disorder, especially bipolar disorder; and a diagnosis of bipolar disorder. Further study is needed to determine the optimal dosage and to identify risk factors that increase individual vulnerability to fluoxetine induced mania in adolescents.

Classroom peer perceptions of 18 teacher-nominated hyperactive and 18 teacher-nominated active but normal elementary school-age boys, as revealed in two sociometric measures (Bower's Class Play, like-dislike nomination), were compared. Results indicated that hyperactives were significantly different from actives on all sociometric measures in that they were perceived more negatively. Peer communication patterns also were assessed. The communicative content, communicative quality, and attention to task instructions of hyperactive boys were compared to those of comparison boys. Limited support was found for the earlier finding that hyperactive boys were less likely than comparison boys to modulate behavior in response to changing external cues and to respond to subtle social learning opportunities. The communication task was found to be highly engaging for both groups of boys, and results suggest that hyperactive children may not lack the interpersonal skills necessary for referential communication, although they may be unable to use them consistently in all settings. The need to continue the study of peer relations of hyperactive children in naturalistic settings is stressed.

The present study assessed teacher and peer perceptions of boys classified according to DSM-III criteria for Attention Deficit with Hyperactivity (ADH) and Attention Deficit without Hyperactivity (AD). Teachers completed SNAP checklists (Pelham, Note 1) on all boys in their classrooms (219 boys). Twelve percent fit ADH criteria; less than 5% fit AD criteria. Teacher perceptions were assessed with the Conners Teacher Rating Scale. ADH and AD groups did not differ on the Inattention factor, although the AD group was rated significantly lower than the ADH group on Conduct Problems and Hyperactivity. These profiles were expected on the basis of DSM-III descriptions of ADH and AD; the ADH group seemed to resemble previous research samples of hyperactive children. Classroom peer perceptions, assessed with like-dislike nominations, did not discriminate between diagnostic groups. Both groups were perceived more negatively than the Comparison group, suggesting that both groups are “at risk”.
