Currently, orthopaedic manual physical therapy (OMPT) lacks a description of practice that reflects contemporary thinking and embraces advances across the scientific, clinical, and educational arms of the profession. The absence of a clear definition of OMPT reduces understanding of the approach across health care professions and potentially limits OMPT from inclusion in scientific reviews and clinical practice guidelines. For example, it is often incorrectly classified as passive care or incorrectly contrasted with exercise-therapy approaches. This perspective aims to provide clinicians, researchers, and stakeholders a modern definition of OMPT that improves the understanding of this approach both inside and outside the physical therapist profession. The authors also aim to outline the unique and essential aspects of advanced OMPT training with the corresponding examination and treatment competencies. This definition of practice and illustration of its defining characteristics is necessary to improve the understanding of this approach and to help classify it correctly for study in the scientific literature. This perspective provides a current definition and conceptual model of OMPT, defining the distinguishing characteristics and key elements of this systematic and active patient-centered approach to improve understanding and help classify it correctly for study in the scientific literature. Purpose: The United States Opioid Epidemic was declared a public health emergency in 2017 and previous studies found American patients are prescribed larger amounts of opioids compared to other Western countries. Our objective was to investigate differences in opioid consumption, postoperative pain, and satisfaction with pain management in patients undergoing a robot-assisted radical prostatectomy in Denmark and the United States. Materials and Method: Patients undergoing robot-assisted radical prostatectomy were prospectively enrolled between September 2018 to November 2018 in Denmark and between August 2019 to September 2020 in the United States. Patients completed a questionnaire assessing pain expectations the day before surgery and the validated 'Brief Pain Inventory' questionnaire on postoperative days 1, 2, 3, and 7. Opioid and non-opioid analgesic consumption was recorded during the first postoperative week. Categorical variables were analyzed using a Chi-square test. Results: A total of 45 Danish patients and 41 American patients completed follow-up. Danish patients consumed a median of 15 mg [inter-quartile range (IQR) 0 to 30mg] of morphine sulfate equivalent, whereas American patients consumed a median of 37.5 mg (IQR 0 to 45mg) morphine sulfate equivalent (p = 0.02). There was no significant difference in postoperative experience of pain between the groups. On postoperative day 7, 91.1% of Danish patients were satisfied with pain management compared with 97.5% of American patients (p = 0.20). Conclusion: Treating postoperative pain after robot-assisted radical prostatectomy is feasible with a low amount of opioid use without compromising safety and satisfactory pain control. Objective. The purpose of this study was to understand the lesbian, gay, bisexual, transgender, queer, intersex, agender, and other gender and sexually diverse identities (LGBTQIA+) health care experience and associated cultural competence from the physical therapist perspective (physical therapist and physical therapist assistant). Methods.An exploratory qualitative approach implementing semi-structured focus groups and private interviews was utilized. To further anonymity, researchers allowed subjects to keep their camera off on Zoom. An interview protocol included questions guided by Campinha-Bacote domains of cultural competence (cultural awareness, skill, knowledge, encounter, and desire) to collect individual experiences, stories, discussions, thoughts, and opinions. Physical therapist clinicians were recruited from the clinical education affiliation lists of Regis University and Thomas Jefferson University. Seventy-one practicing physical therapists from the USA agreed to be part of the study. Results. Themes were organized using the Social Ecological Model Framework. Themes are in parentheses following each level of the Social Ecological Model and include intrapersonal level (psychological stress and implicit and explicit biases), interpersonal (acceptance and competency), organizational (experience), community (advocacy), and society and policy (explicit biases and policy). Conclusion. Cultural competence in physical therapy is influenced by intrapersonal, interpersonal, organizational, community, and social and policy factors. Themes of psychological stress, limited awareness, decreased acceptance, and competency as well as limited exposure and experience, and a lack of advocacy and broader societal and policy issues prevent adequate LGBTQIA+ cultural competency of physical therapist providers. Further research in the physical therapist profession is needed to elaborate on the student, educator, and patient perspectives and how this information informs the LGBTQIA+ cultural competence of clinicians. Impact. This project may have a significant impact on suggestions for the delivery of content for health profession education to best impact health equity goals and save lives. Implementation of this content may have a direct impact on health disparities in LGBTQIA+ populations by reducing stigma and discrimination from health care providers, thus improving quality of health care and decreasing rates of patient mortality for LGBTQIA+ individuals. The current mixed methods, embedded study evaluated the use of an online tool (Care to Plan [CtP]) that generates tailored service and support recommendations for dementia caregivers as well as information that can connect dementia caregiver users to recommended resources. Forty-three dementia caregivers identified in a health care system were randomly assigned to receive either CtP or usual care. The embedded, mixed methods design resulted in no quantitative, direct evidence for CtP's efficacy. Qualitative data revealed important insights into facilitators of and barriers to CtP use. Qualitative data also demonstrated that CtP helped caregivers obtain a better awareness of their needs and experiences. Clinicians may find CtP useful as an initial tool to spur memory care/case management and facilitate conversations about caregivers' needs and resources to meet individualized challenges. 'Liberal' theories of the origin of property rights, which see property rights as something that emerged independently of state action, retain considerable explanatory value. The paper begins with a discussion of the principal features of David Hume's account of the origins of property. The paper then engages with historical scholarship that shows that (1) it is a mistake to see the medieval English hierarchy of administration as implying a hierarchy of property rights ultimately derived from the Crown's 'title,' and (2) the idea that communal medieval agricultural practices were super-imposed upon a pre-existing system of individual property rights is plausible. AbstractThis paper provides an overview of ongoing work aimed at developing spent nuclear fuel (SNF) canister deposition models. Currently, it is known that stainless steel canisters are susceptible to chloride-induced stress corrosion cracking (CISCC). However, the rate of CISCC degradation and the likelihood that it could lead to a through-wall crack is unknown. While it is currently unknown if there is a threshold chloride surface concentration for CISCC initiation, it can be assumed that the onset and progress of material degradation will depend on the local contaminant concentration, the properties of the contaminant species, and the synergistic effects when multiple contaminants are present.This study uses well-developed computational fluid dynamics and particle tracking tools and applies them to SNF storage to determine the rate of deposition on canisters. Understanding the rate of deposition on SNF canisters could be important for making canister aging management predictions. This study is a part of an ongoing effort funded by the U.S. Department of Energy, Office of Nuclear Energy, Office of Spent Fuel and Waste Science and Technology, which is tasked with doing research relevant to enhancing the technical basis for ensuring the safe extended storage and subsequent transport of SNF.This work is being presented to demonstrate a potentially useful technique for SNF canister vendors, utilities, regulators, and stakeholders to utilize and further develop for their own designs and site-specific studies.

• Sodium germanate glasses permanently densify upon hot compression. • Shear modulus features surprising non-monotonic variation upon increasing pressure. • Only very minor changes in Ge-O coordination are observed. • Shear modulus trend is mostly attributed to decrease in edge-sharing with pressure. • Pressure treatment also induces smaller and more elliptical Ge-O rings. Unlike traditional silicate glasses, germanate glasses often feature non-monotonic variations in material properties (e.g., elastic moduli and glass transition temperature) with varying chemical composition, temperature, and pressure. However, the underlying atomic-scale structural origins remain poorly understood. This is because, in most oxide glasses, the structural changes are quantified through solid-state NMR spectroscopy, but unfortunately the only NMR active germanium isotope (73Ge) has very unfavorable NMR properties. Here, we circumvent this problem by using high-energy X-ray and neutron total scattering coupled with ab initio molecular dynamics simulations as input for Reverse Monte Carlo modeling. In detail, we study the structure and properties of two sodium germanate glasses (10Na 2 O-90GeO 2 and 20Na 2 O-80GeO 2) subjected to permanent densification through hot compression up to 2 GPa at the glass transition temperature. While density as well as Young's and bulk modulus increase with pressure as expected, shear modulus first increases and then decreases slightly at higher pressures. The refined atomistic structure models suggest that the glasses feature a distribution of 4, 5, and 6 coordinated Ge with a majority of 4 and 5 coordinated species. Only minor changes in the Ge–O coordination occur upon hot compression, but a notable transformation of edge- to corner-sharing Ge-polyhedra is found. This anomalous polyhedral packing causes a lower number of angular constraints upon higher pressure treatment, explaining the non-monotonic trend of shear modulus with pressure. We also find that the rings become smaller and less circular upon compression, contributing to the volumetric compaction. These findings may aid the future design of germanate glasses with tailored properties and the general understanding of structure-property relations in oxide glasses. We analyze the link between 'too big to fail' (TBTF) and moral hazard using a natural experiment from an epoch of unregulated commercial banking in Denmark. In 1908 the country faced a large banking shock where the creditors of distressed commercial banks received a bailout by the government for the first time in Danish history. Due to a fortuitous combination of circumstances, banks continued to operate in an unregulated environment for more than a decade after the bailout. By considering a sample from a pre-regulation epoch, we isolate the TBTF effect. Our empirical analysis shows that TBTF banks significantly reduced post-bailout capital ratios compared to other banks.
