6. Low dose paracetamol for management of patent ductus arteriosus in very preterm infants: a randomised non-inferiority trial

ObjectiveTo compare the efficacy of low dose-short course intravenous paracetamol with a conventional dose regimen for early targeted closure of patent ductus arteriosus (PDA).DesignSingle-centre, double-blinded, active controlled, randomised non-inferiority trial.SettingLevel III neonatal intensive care unit in Western India.PatientsPreterm infants <30 weeks of gestation requiring mechanical ventilation, or continuous positive airway pressure with FiO2≥0.35 and diagnosed with a haemodynamically significant PDA (hsPDA) at 18–24 hours of postnatal age.InterventionsLow dose (10 mg/kg/dose 6 hourly for 72 hours) versus conventional dose (15 mg/kg/dose 6 hourly for 120 hours) intravenous paracetamol treatment.Main outcome measuresComparison of the rates of ductal closure at completion of sixth postnatal day, using a prespecified non-inferiority margin of 20%.ResultsA total of 102 infants were enrolled. The median gestational age and birth weight of the included infants were 26.4 weeks and 830 g. At completion of the sixth postnatal day, closure of PDA was achieved in 92% of infants in the low dose group as compared with 94% of those in the standard dose group (risk difference: −1.6%, 95% CI: −11.6% to 8.5%, p=0.38). The rates of rescue therapies, adverse effects and other neonatal morbidities were comparable in both groups.ConclusionIn very preterm infants on significant respiratory support, low dose-short course intravenous paracetamol treatment was non-inferior to a conventional dosing regime of paracetamol for closure of hsPDA in the first week of postnatal age. Larger studies with narrow margins of non-inferiority are required to confirm our findings.Trial registration numberCTRI/2017/10/010012.

The 2015 LancetCommission on global surgery identified surgery and anaesthesia as indispensable parts of holistic health-care systems. However, COVID-19 exposed the fragility of planned surgical services around the world, which have also been neglected in pandemic recovery planning. This study aimed to develop and validate a novel index to support local elective surgical system strengthening and address growing backlogs.

Strain analysis of transthoracic echocardiography (TTE) is a sensitive tool to detect myocardial dysfunction in those affected by COVID-19. Consideration of preexisting cardiovascular disease is important in detecting changes related to COVID-19. We sought to assess serial TTE changes in patients recovered from COVID-19 compared with baseline, pre-COVID-19 exams, with a focus on left and right ventricular longitudinal strain.

12. Performance analysis of a novel ejector-assisted condenser outlet split dual-evaporator refrigeration system

- Abstract
The performance of an ejector-assisted condenser outlet split dual-evaporator cycle is compared with a conventional dual-evaporator cycle albeit consisting a pressure reducing valve. The cycles do not employ any separator due to its inability to efficiently separate the liquid and the vapor phases. The comparison of both the cycles has been made for the same cooling capacity in low-temperature evaporator and unit flow rate of R134a and R1234yf as refrigerants. The impacts of changing the operating temperatures of evaporator and condenser have been examined in the current investigation. The study reveals that with the increase in temperature of the high-temperature evaporator, the cooling capacity of the high-temperature evaporator yields, while that of the low-temperature evaporator plummets in both the cycles. Further, the compressor work is allayed in the ejector-assisted cycle; thus, the COP is enhanced considerably. The percentage COP improvement over the basic cycle is obtained from 14.7 to 17.53% for the refrigerant R1234yf and from 14.45 to 17.32% for R134a; however, the COP of both the cycles with R12134yf is slightly lower than with R134a. The ejector has been modeled assuming a constant pressure theory. The observed trend indicates that the entrainment ratio is improved with the rise in the temperature of low-temperature evaporator, whereas it is decreased with the rise in the temperature of high-temperature evaporator.

14. Impact of COVID-19 on Orthopaedic Care and Practice: A Rapid Review

- Abstract
Introduction: The COVID-19 pandemic has led to a large body of literature regarding the impact of COVID-19 on orthopaedic care and practice. This rapid review aims to synthesize this published literature to give the orthopaedic fraternity an overview about the best practices that need to be followed during this period. Methodology: A rapid review was conducted following the preferred reporting items for systematic reviews and meta-analyses (PRISMA) statement for rapid reviews on the impact of COVID-19 on orthopaedic care and practice. A Pubmed search was done to identify all literature related to the impact of COVID-19 on orthopaedic care and practice, published between December 2019 and October 2020 using a predefined search strategy. The final review included 375 peer-reviewed articles addressing the objectives. Results: The majority of articles were expert opinions ( 37.1%) and narrative reviews (13.1%). There were 17.3% retrospective studies and 2.1% prospective studies with only one randomized control trial and ten systematic reviews. 83.8% of articles had levels of evidence IV and V, 79.5% of the articles were published in core-orthopaedic journals. Maximum publications were from the United States of America (31.7%), followed by India (11.5%). European countries together contributed to 32.0% of all publications. Conclusion: COVID-19 has had a significant impact on all aspects of orthopaedic care and practice. The pandemic has affected outpatient clinics, emergency and elective surgery, rehabilitation, resident training, personnel management, use of personal protective equipment, telemedicine and all sub-specialities of orthopaedics. Orthopaedic practice will require the incorporation of new technologies, restructuring of health systems and reorganizing of training programs for optimal patient care. There would also be a need for frequent review of emerging literature to provide evidence-based guidelines to the orthopaedic fraternity which will not only help in mitigation of transmission of disease but also ensure continuity of optimum patient care.

- Abstract
Brain injury is highly associated with preterm birth. Complications of prematurity, including spontaneous or necrotizing enterocolitis (NEC)-associated intestinal perforations, are linked to lifelong neurologic impairment, yet the mechanisms are poorly understood. Early diagnosis of preterm brain injuries remains a significant challenge. Here, we identified subventricular zone echogenicity (SVE) on cranial ultrasound in preterm infants following intestinal perforations. The development of SVE was significantly associated with motor impairment at 2 years. SVE was replicated in a neonatal mouse model of intestinal perforation. Examination of the murine echogenic subventricular zone (SVZ) revealed NLRP3-inflammasome assembly in multiciliated FoxJ1+ependymal cells and a loss of the ependymal border in this postnatal stem cell niche. These data suggest a mechanism of preterm brain injury localized to the SVZ that has not been adequately considered. Ultrasound detection of SVE may serve as an early biomarker for neurodevelopmental impairment after inflammatory disease in preterm infants.

16. Thermodynamic analysis of ejector-assisted vapour compression–absorption hybrid refrigeration system

ABSTRACTThe vapour compression–absorption hybrid refrigeration system (VCAHRS) is a potential alternative refrigeration technology as it economises the use of energy resources. The main issues observed with VCAHRS are low overall COP and large size of system which is mainly due to the poor performance of absorption section. Absorption section, while assisted by an ejector, can improve the overall performance of VCAHRS. It is shown that at low generator temperatures, the ejector-assisted absorption section provides better performance than the conventional absorption system adapted in VCAHRS. Simulation results show that the overall COP of VCAHRS is improved by 22.6%, whereas the COP improvement of 25.2% is observed in the absorption section. The total irreversibility of the system is reduced by 38.8% using ejector at optimum generator temperature which is found to be 92.1°C while varying it from 117.9°C. Therefore, ejector-assisted VCAHRS also permits the utilisation of low temperature heat source.

- Abstract
Music genre classification has its own popularity index in the present times. Machine learning can play an important role in the music streaming task. This research article proposes a machine learning based model for the classification of music genre. The evaluation of the proposed model is carried out while considering different music genres as in blues, metal, pop, country, classical, disco, jazz and hip-hop. Different audio features utilized in this study include MFCC (Mel Frequency Spectral Coefficients), Delta, Delta-Delta and temporal aspects for processing the data. The implementation of the proposed model has been done in the Python language. The results of the proposed model reveal an accuracy SVM accuracy of 95%. The proposed algorithm has been compared with existing algorithms and the proposed algorithm performs better than the existing ones in terms of accuracy.

ObjectiveTo conduct a systematic review and meta-analysis of the efficacy and safety of umbilical cord milking in preterm infants.DesignRandomised controlled trials comparing umbilical cord milking with delayed cord clamping/immediate cord clamping in preterm infants were identified by searching databases, clinical trial registries and reference list of relevant studies in November 2019. Fixed effects model was used to pool the data on various clinically relevant outcomes.Main outcome measuresMortality and morbidities in preterm neonates.ResultsNineteen studies (2014 preterm infants) were included. Five studies (n=922) compared cord milking with delayed cord clamping, whereas 14 studies (n=1092) compared milking with immediate cord clamping. Cord milking, as opposed to delayed cord clamping, significantly increased the risk of intraventricular haemorrhage (grade III or more) (risk ratio (RR): 1.95 (95% CI 1.01 to 3.76), p=0.05). When compared with immediate cord clamping, cord milking reduced the need for packed RBC transfusions (RR:0.56 (95% CI 0.43 to 0.73), p<0.001). There was limited information on long-term neurodevelopmental outcomes. The grade of evidence was moderate or low for the various outcomes analysed.ConclusionUmbilical cord milking, when compared with delayed cord clamping, significantly increased the risk of severe intraventricular haemorrhage in preterm infants, especially at lower gestational ages. Cord milking, when compared with immediate cord clamping, reduced the need for packed RBC transfusions but did not improve clinical outcomes. Hence, cord milking cannot be considered as placental transfusion strategy in preterm infants based on the currently available evidence.

- Abstract
UBE2N, a Lys63 ubiquitin–conjugating enzyme, plays critical roles in embryogenesis and immune system development and function. However, its roles in adult epithelial tissue homeostasis and pathogenesis are unclear. We generated conditional mouse models that deleted Ube2nin skin cells in a temporally and spatially controlled manner. We found that Ube2nknockout in the adult skin keratinocytes induced a range of inflammatory skin defects characteristic of psoriatic and actinic keratosis. These included inflammation, epidermal and dermal thickening, parakeratosis, and increased immune cell infiltration as well as signs of edema and blistering. Single-cell transcriptomic analyses and RT-qPCR showed that Ube2n-knockout keratinocytes expressed elevated myeloid cell chemoattractants such as Cxcl1and Cxcl2and decreased the homeostatic T lymphocyte chemoattractant Ccl27a. Consistently, the infiltrating immune cells were predominantly myeloid-derived cells, including neutrophils and M1-like macrophages, which expressed high levels of inflammatory cytokines such as Il1βand Il24.Pharmacological blockade of the IL-1 receptor associated kinases (IRAK1/4) alleviated inflammation, epidermal and dermal thickening, and immune infiltration of the Ube2n-mutant skin. Together, these findings highlight a key role of keratinocyte UBE2N in maintenance of epidermal homeostasis and skin immunity and identify IRAK1/4 as potential therapeutic target for inflammatory skin disorders.

- Abstract
The performance of a dual evaporator cycle using ejector is compared with a conventional cycle employing pressure reducing valve. In both the systems, high temperature evaporator is considered as a flooded evaporator, thus a separator is employed after the high temperature evaporator. However, low temperature evaporator is a kind of conventional dry evaporator. The comparison of both systems, i.e., conventional and ejector assisted, is done for the same cooling capacities and same dryness fraction at the exit of high temperature evaporator with R134a, R152a, and R1234yf refrigerants. The effects of varying the states of refrigerant at the exit of flooded evaporator, and temperatures of both the evaporators and the condenser are analyzed using Engineering Equation Solver. It is found that the compressor work is reduced in both the cycles with the rise in low temperature evaporator temperature; however, a little variation is observed in the total cooling effect. The cooling effect in high temperature evaporator is increased with the increase in dryness fraction at the exit of the high temperature flooded evaporator, but it is decreased in low temperature evaporator.

24. Ahmed valve implantation in childhood glaucoma associated with Sturge–Weber syndrome: our experience

Purpose: To evaluate the efficacy and safety of Ahmed valve implantation in childhood glaucoma associated with Sturge–Weber syndrome (SWS). Methods: A retrospective chart review of patients who underwent primary Ahmed valve implantation in childhood glaucoma associated with SWS. The outcome measures were success rate (Intraocular pressure (IOP) >5 and <21?mm Hg without additional surgical maneuver), and operative complications. Results: Twenty-four eyes of 18 patients were analyzed. Mean IOP (26.21?±?3.52 to 12.27?±?1.27?mm Hg) and mean number of anti-glaucoma medicines (3.92?±?0.28 to 1.5?±?1) reduced significantly after the surgery at the mean follow-up of 2.12?±?0.87 years (P?=?<?0.05). The cumulative probability of success rate was 75% (Confidence Interval 95%, 60 to 100%) by Kaplan–Meier survival curve analysis. Failed eyes [6 (25%)], hyphema [4 (16.67%)], hypotony [3 (12.5%)], choroidal detachment [3 (12.5%)], tube–cornea touch [2 (8.33%), and visually significant cataract [4 (16.67%)] were the complications observed in post-operative period. Conclusion: Primary Ahmed valve implantation may provide a safe and effective option in the management of childhood glaucoma associated with SWS.

Purpose: To evaluate the outcome of combined Ahmed glaucoma valve (AGV) and phacoemulsification with posterior chamber intraocular lens implantation under infliximab in refractory uveitic glaucoma (UG).Methods: In this prospective interventional case series, 26 eyes of 26 patients with refractory UG underwent surgery under intravenous infliximab. The success rate was defined as intraocular pressure (IOP) 5 to 21 mm Hg with or without antiglaucoma medications (AGM), without additional glaucoma surgical intervention.Results: The mean IOP (37.8 ± 11.86 to 12.2 ± 2.8 mm Hg; p<0.0001) and mean number of AGM (3.4 ± 1.2 to 0.4 ± 0.1; p<0.001) were significantly reduced after surgery at 2 years. Kaplan-Meier survival analysis showed a cumulative probability of success for IOP control of 92% at 2 years of follow-up.Conclusions: Combined AGV and phacoemulsification is an effective treatment for controlling refractory UG with complicated cataract under infliximab.

- Abstract
White matter injuries (WMIs) are the leading cause of neurologic impairment in infants born premature. There are no treatment options available. The most common forms of WMIs in infants occur prior to the onset of normal myelination, making its pathophysiology distinctive, thus requiring a tailored approach to treatment. Neonates present a unique opportunity to repair WMIs due to a transient abundance of neural stem/progenitor cells (NSPCs) present in the germinal matrix with oligodendrogenic potential. We identified an endogenous oxysterol, 20-αHydroxycholesterol (20HC), in human maternal breast milk that induces oligodendrogenesis through a sonic hedgehog (shh), Gli-dependent mechanism. Following WMI in neonatal mice, injection of 20HC induced subventricular zone-derived oligodendrogenesis and improved myelination in the periventricular white matter, resulting in improved motor outcomes. Targeting the oligodendrogenic potential of postnatal NSPCs in neonates with WMIs may be further developed into a novel approach to mitigate this devastating complication of preterm birth.

30. Phacoemulsification at High-Altitude Himalayan Region: Comparison

Purpose To compare phacoemulsification parameters at different high-altitude regions as well as between peristaltic and Venturi-based machines.Methods In this prospective, nonrandomized clinical study, 160 eyes of 160 patients with senile cataract underwent phacoemulsification using either peristaltic or Venturi system at a high-altitude Himalayan region (>10,000 feet). Patients (n = 200, including 100 each with either peristaltic or Venturi system) operated at mean altitude of 1115 feet (Delhi) were included as controls (group 1). At Leh (11,203 feet), 110 patients were operated with peristaltic (62) or Venturi (48) system (group 2), whereas 50 patients (group 3) (peristaltic = 37; Venturi = 13) were operated either with peristaltic (37) or Venturi (13) system at Tangtse (14,106 feet). Intraoperative parameters–i.e., bottle height (BH), vacuum (V), and flow rate (FR)–were compared for different phacoemulsification steps–i.e., central chopping (CC), segment removal (SR), epinucleus removal (ER), and cortex removal (CR)–between all groups and between peristaltic and Venturi pump-based machines in each group.Results Mean BH, V, and FR for CC, SR, ER, and CR significantly increased with the increment in altitude of surgery (p = 0.000). Venturi and peristaltic-based phacoemulsification showed higher values of the mean BH and V, respectively, for CC, SR, ER, and CR at Leh as well as Tangtse.Conclusions At the high-altitude region, the higher setting of BH, FR, and V is required in phacoemulsification.

31. Implementation of Simulation-Based Virtual Sensors Using Radiance and Java

- Abstract
Building performance simulation is traditionally used to support the building design process. However, it can also be used during building operation phase by providing relevant data to building automation systems. We refer to monitoring and metering data generated by computational simulation (virtual) models as data from virtual sensors. Virtual sensors, if reliably and effectively incorporated, can expand the reach of real sensors. A continuous supply of virtual sensor data can support near real-time representation of both primary environmental variables (e.g., air temperature, relative humidity) as well as complex performance indicators (e.g., thermal comfort indices, visual glare). Such information, especially in large multi-zone buildings, would be otherwise expensive to obtain from real sensors due to high capital costs of the sensors, their installation, networking, and maintenance. Virtual sensors can increase the resolution of sensory information, as they imply little monetary expenditures and are not limited in view of numbers and location.Conventional building energy simulation uses a model of the building and creates boundary conditions using historic weather data and predefined schedules for lighting and equipment load. This method is suitable for evaluation of building designs but not suitable for monitoring and control processes during building operation phase, as such processes usually require actual real-time information regarding boundary conditions. Thus, capability to automatically update simulation model's boundary conditions in real-time using monitored data is required. Toward this end, a supervisory software application is developed, in Java programming language, to implement and illustrate the concept of virtual sensors using Radiance simulation program. This application creates virtual illuminance sensors in Radiance model and then automates the process of regularly updating simulation model's external and internal boundary conditions (solar irradiance, state of electrical lighting elements) by using real-time monitored data from on-site weather station and building management system (BMS). As a result, the simulation kernel can continuously generate virtual sensor readings that can be utilized by building monitoring and/or control systems. This paper discusses the general architecture of the application, to further illustrate the concept of virtual sensors. Moreover, it discusses the accuracy of virtual illuminance sensors by comparing their output with monitored illuminance data in realistic test scenarios.

33. Origins of the specificity of inhibitor P218 toward wild-type and mutant PfDHFR: a molecular dynamics analysis

Molecular dynamics simulations were performed to evaluate the origin of the antimalarial effect of the lead compound P218. The simulations of the ligand in the cavities of wild-type, mutant Plasmodium falciparumDihydrofolate Reductase (PfDHFR) and the human DHFR revealed the differences in the atomic-level interactions and also provided explanation for the specificity of this ligand toward PfDHFR. The binding free energy estimation using Molecular Mechanics Poisson-Boltzmann Surface Area method revealed that P218 has higher binding affinity (~ −30 to −35 kcal/mol) toward PfDHFR (both in wild-type and mutant forms) than human DHFR (~ −22 kcal/mol), corroborating the experimental observations. Intermolecular hydrogen bonding analysis of the trajectories showed that P218 formed two stable hydrogen bonds with human DHFR (Ile7 and Glu30), wild-type and double-mutant PfDHFR’s (Asp54 and Arg122), while it formed three stable hydrogen bonds with quadruple-mutant PfDHFR (Asp54, Arg59, and Arg122). Additionally, P218 binding in PfDHFR is stabilized by hydrogen bonds with residues Ile14 and Ile164. It was found that mutant residues do not reduce the binding affinity of P218 to PfDHFR, in contrast, Cys59Arg mutation strongly favors inhibitor binding to quadruple-mutant PfDHFR. The atomistic-level details explored in this work will be highly useful for the design of non-resistant novel PfDHFR inhibitors as antimalarial agents.

The authors report on the electrochromic (EC) behaviour of the environmentally friendly water-soluble polythiophene sodium poly (2-(3-thienyl) ethoxy-4-butylsulfonate) (PTEBS) solid-state devices. Films are fabricated on indium tin oxide substrates and are composed of bilayers of PTEBS and poly (allylamine hydrochloride) using the ionic self-assembled multilayer approach. The self-assembled films showed high stability and demonstrated EC switching from orange to green with a contrast of 61% and colouration efficiency of −238 cm2/C in 0·1 M NaClO4(aq) at 735 nm. 80-bilayer solid-state devices have an optical contrast of 38% at 735 nm, and 1-cm2active area devices show fast t90%switching times of 125 ms for coloration and 225 ms for decoloration with applied voltage of 2 V for long-term switching of more than 50,000 cycles.

In the present study, the performance of vapour compression–absorption cascaded refrigeration system (VCACRS) is studied with modified Gouy–Stodola equation and the results are compared with the classical approach of exergy evaluation. The quantitative difference in two approaches is presented which necessitates the need to employ modified approach. The expressions are formulated for effective temperature and real irreversible loss for different components of VCACRS. The overall performance of system can be improved by increasing the evaporator and generator temperature and by decreasing the absorber, cascade condenser and condenser temperature. Coefficient of structural bonds (CSB) analysis shows evaporator is relatively more sensitive to change in its temperature as compared with the other components.

37. Uridine monophosphate kinase as potential target for tuberculosis: From target to lead identification

Mycobacterium tuberculosis (Mtb) is a causative agent of tuberculosis (TB) disease, which has affected approximately 2 billion people worldwide. Due to the emergence of resistance towards the existing drugs, discovery of new anti-TB drugs is an important global healthcare challenge. To address this problem, there is an urgent need to identify new drug targets in Mtb. In the present study, the subtractive genomics approach has been employed for the identification of new drug targets against TB. Screening the Mtb proteome using the Database of Essential Genes (DEG) and human proteome resulted in the identification of 60 key proteins which have no eukaryotic counterparts. Critical analysis of these proteins using Kyoto Encyclopedia of Genes and Genomes (KEGG) metabolic pathways database revealed uridine monophosphate kinase (UMPK) enzyme as a potential drug target for developing novel anti-TB drugs. Homology model of Mtb-UMPK was constructed for the first time on the basis of the crystal structure of E. coli-UMPK, in order to understand its structure-function relationships, and which would in turn facilitate to perform structure-based inhibitor design. Furthermore, the structural similarity search was carried out using physiological inhibitor UTP of Mtb-UMPK to virtually screen ZINC database. Retrieved hits were further screened by implementing several filters like ADME and toxicity followed by molecular docking. Finally, on the basis of the Glide docking score and the mode of binding, 6 putative leads were identified as inhibitors of this enzyme which can potentially emerge as future drugs for the treatment of TB.

Understanding how semiconductor quantum dots (QDs) engage in photoinduced energy transfer with carbon allotropes is necessary for enhanced performance in solar cells and other optoelectronic devices along with the potential to create new types of (bio)sensors. Here, we systematically investigate energy transfer interactions between C60fullerenes and four different QDs, composed of CdSe/ZnS (type I) and CdSe/CdS/ZnS (quasi type II), with emission maxima ranging from 530 to 630 nm. C60-pyrrolidine tris-acid was first coupled to the N-terminus of a hexahistidine-terminated peptide viacarbodiimide chemistry to yield a C60-labeled peptide (pepC60). This peptide provided the critical means to achieve ratiometric self-assembly of the QD-(pepC60) nanoheterostructures by exploiting metal affinity coordination to the QD surface. Controlled QD-(pepC60)Nbioconjugates were prepared by discretely increasing the ratio (N) of pepC60assembled per QD in mixtures of dimethyl sulfoxide and buffer; this mixed organic/aqueous approach helped alleviate issues of C60solubility. An extensive set of control experiments were initially performed to verify the specific and ratiometric nature of QD-(pepC60)Nassembly. Photoinitiated energy transfer in these hybrid organic–inorganic systems was then interrogated using steady-state and time-resolved fluorescence along with ultrafast transient absorption spectroscopy. Coordination of pepC60to the QD results in QD PL quenching that directly tracks with the number of peptides displayed around the QD. A detailed photophysical analysis suggests a competition between electron transfer and Förster resonance energy transfer from the QD to the C60that is dependent upon a complex interplay of pepC60ratio per QD, the presence of underlying spectral overlap, and contributions from QD size. These results highlight several important factors that must be considered when designing QD-donor/C60-acceptor systems for potential optoelectronic and biosensing applications.

In clinical diagnostics, homogeneous time-resolved (TR) FRET immunoassays are used for fast and highly sensitive detection of biomarkers in serum samples. The most common immunoassay format is based on europium chelate or cryptate donors and allophycocyanin acceptors. Replacing europium donors with terbium complexes and the acceptors with QDs offers large photophysical advantages for multiplexed diagnostics, because the Tb-complex can be used as FRET donor for QD acceptors of different colors. Water-soluble and biocompatible QDs are commercially available or can be synthesized in the laboratory using many available recipes from the literature. Apart from the semiconductor material composition, an important aspect of choosing the right QD for TR-FRET assays is the thickness of the QD coating, which will influence the photophysical properties and long-term stability as well as the donor–acceptor distance and FRET efficiency. Here we present a detailed time-resolved spectroscopic study of three different QDs with an emission maximum around 605 nm for their application as FRET acceptors (using a common Tb donor) in TR-bioassays: (i) Invitrogen/Life Technologies Qdot605, (ii) eBioscience eFluorNC605 and iii) ter-polymer stabilized CdSe/CdS/ZnS QDs synthesized in our laboratories. All FRET systems are very stable and possess large Förster distances (7.4–9.1 nm), high FRET efficiencies (0.63–0.80) and low detection limits (0.06–2.0 pM) within the FRET-bioassays. Shapes, sizes and the biotin/QD ratio of the biocompatible QDs could be determined directly in the solution phase bioassays at subnanomolar concentrations. Both commercial amphiphilic polymer/lipid encapsulated QDs and self-made ligand-exchanged QDs provide extremely low detection limits for highly sensitive TR-FRET bioassays.

This article features the application of electrochromic (EC) devices for displays and the efforts made to improve the switching speed and contrast performance of these devices. The article primarily focuses on the layer-by-layer (LbL) assembly deposition technique to deposit thin films of EC materials ranging from organic, inorganic and conducting polymers. Device sizes were varied for comparison of the performance of the lab-made prototype device with the commercially available “small pixel” size displays. EC devices fabricated by LbL assembly, as shown with poly(3,4-ethylenedioxythiophene) results, obtained with a switching speed of less than 30 ms and make EC flat-panel displays possible by adjusting film thickness, device size and type of material. The high contrast value (84%) for ruthenium purple suggests that its LbL films can be used for low-power consumption displays where contrast, and not fastest switching, is of prime importance.

The ability to predict outcomes following a kidney transplant is limited by the complex physiologic decline of kidney failure, a latent factor that is difficult to capture using conventional comorbidity assessment. The frailty phenotype is a recently described inflammatory state of increased vulnerability to stressors resulting from decreased physiologic reserve and dysregulation of multiple physiologic systems. We hypothesized that frailty would be associated with delayed graft function, based on putative associations between inflammatory cytokines and graft dysfunction. We prospectively measured frailty in 183 kidney transplant recipients between December 2008 and April 2010. Independent associations between frailty and delayed graft function were analyzed using modified Poisson regression. Preoperative frailty was independently associated with a 1.94-fold increased risk for delayed graft function (95% CI, 1.13-3.36; P = .02). The assessment of frailty may provide further insights into the pathophysiology of allograft dysfunction and may improve our ability to preoperatively risk-stratify kidney transplant recipients.

Effective biological application of nanocrystalline semiconductor quantum dots continues to be hampered by the lack of easily implemented and widely applicable labeling chemistries. Here, we introduce two new orthogonal nanocrystal bioconjugation chemistries that overcome many of the labeling issues associated with currently utilized approaches. These chemistries specifically target either (1) the ubiquitous amines found on proteins or (2) thiols present in either antibody hinge regions or recombinantly introduced into other proteins to facilitate site-specific labeling. The amine chemistry incorporates aniline-catalyzed hydrazone bond formation, while the sulfhydryl chemistry utilizes nanocrystals displaying surface activated maleimide groups. Both reactive chemistries are rapidly implemented, yielding purified nanocrystal–protein bioconjugates in as little as 3 h. Following initial characterization of the nanocrystal materials, the wide applicability and strong multiplexing potential of these chemistries are demonstrated in an array of applications including immunoassays, immunolabeling in both cellular and tissue samples, in vivocellular uptake, and flow cytometry. Side-by-side comparison of the immunolabeled cells suggested a functional equivalence between results generated with the amine and thiol-labeled antibody–nanocrystal bioconjugates in that format. Three-color labeling was achieved in the cellular uptake format, with no significant toxicity observed while simultaneous five-color labeling of different epitopes was demonstrated for the immunolabeled tissue sample. Novel labeling applications are also facilitated by these chemistries, as highlighted by the ability to directly label cellular membranes in adherent cell cultures with the thiol-reactive chemistry.

- Abstract
A PV was synthesized by polymerization of N,N′-bis(δ‐aminopropyl)‐4,4′‐bipyridinium bromide hydrobromide (APD) and isophthaloyl chloride (ISP). The PV was completely soluble in water as well as in organic solvents. The spectroelectrochemical and EC properties of the resulting ionic self‐assembled multilayers of PV/poly‐(2‐acrylamido‐2‐methylpropanesulfonic acid) (PAMPs) were examined by cyclic voltammetry, FT‐IR spectroscopy, UV‐vis spectroscopy, optical switching and current density measurements. Solid state devices made of PV films sandwiched with PANI as a counter electrode have switching times of 100–250 ms. 40 Bilayer films of PV/PAMPs show high contrast (ΔT = 61%) in 0.1 M NaClO4 liquid electrolyte solution and CE as high as 57 cm2 · C−1, one of the highest reported so far for any bipyridinium salt system.

Truncus arteriosus (TA) or common arterial trunk is a congenital cardiac anomaly having high association with arch anomalies such as right aortic arch or aortic arch interruption. However, TA with double aortic arch (DAA) is a rare occurrence. We report a case of TA with DAA where the diagnosis of DAA was missed initially.

- Abstract
FTIR and FT-Raman spectra of 5-methyluracil (thymine) were recorded in the regions 4004000 cm−1 and 504000 cm−1, respectively. The observed wavenumbers were analysed and assigned to different normal modes of vibration of the molecule. Ab initio and density functional calculations were performed to support the assignments of the observed wavenumbers. The geometry structure was determined and a comparison with the uracil molecule was made. Good reproducibility of the experimental wavenumbers was obtained at the B3LYP level and the percentage error was very small, less than 1% in most cases. Copyright © 2000 John Wiley & Sons, Ltd.

FT Raman study of 5-fluorouracil, an anticarcinogenic drug, was carried out. Ab initio and density functional computations of the vibrational spectrum, the molecular geometry, the atomic charges, the total energy, the zero point energy, the rotational constants, the room temperature entropy, (translational, rotational and vibrational) and dipole moment were carried out. The observed Raman wavenumbers were analysed in light of the computed vibrational spectrum. Thermodynamic functions, namely, the enthalpy, the entropy, the heat capacity and the free energy were also computed in the temperature range 2001000 K using the rigid-rotor harmonic oscillator approximation. For this purpose the fundamental wavenumbers were taken from the present FT-Raman spectrum and the structural parameters were taken from the present theoretically computed geometry. Introduction:We aim to elucidate the incidence of major complications related to right heart catheterization (RHC) and ascertain if there are unique risks with right ventricular biopsy.Methods:Diagnostic RHC procedures from 1/1/2002 through 12/31/2013 were identified using the clinical scheduling system at Mayo Clinic, Rochester. Major complications were identified through electronic records using International Classification of Diseases, Ninth Revision billing codes. All events were recorded and adjudicated. Analysis was performed on four separate patient categories (RHC/RV angiography alone, RV biopsy alone, multiple RHC procedures, and concurrent RHC/LHC procedures) and the overall group. Primary outcomes are described in Table 1. Outcomes were measured within 7 days of the procedure, except for death (same day events) and tricuspid regurgitation (variable time endpoints).Results:There were 17,696 RHC procedures (5,556 RHC/RV angiography, 3,846 RV biopsy alone, 776 multiple RHC procedures, and 7,518 combined RHC/LHC). Patient population had a mean age of 56.5 years (15.3) and was predominantly male (58.5%). Same-day death related to RHC procedure occurred in 1 procedure or 0.6 of 10,000 RHCs. There was no significant difference in the risk of complications across RHC procedure categories except for worsening tricuspid regurgitation, which was significantly (p<0.001) more prevalent with RV biopsy (392 cases, 6.0%) than without (136 cases, 1.2%).Conclusion:The incidence of major complications related to right heart catheterization is rare. Worsening tricuspid regurgitation is the most frequently identified change with a significantly higher prevalence with biopsies.

49. Abstract 11274: All-Cause Mortality Association of Right Ventricular Free Wall Longitudinal Strain and Strain Rate in Acute Pulmonary Embolism Patients

Introduction:Right ventricle (RV) dysfunction linked with acute Pulmonary embolism (PE) is associated with significant mortality. Several studies have documented use of RV free wall strain (RVS) for assessing mortality, but RV free wall systolic (RVSRs) and early diastolic strain rate (RVSRe) have not been explored.Hypothesis:The study aims to assess the RV function in longitudinal domain of contractility using 2 D speckle tracking echocardiography and evaluate if it was able to predict all-cause mortality at 30 days.Methods:This is a single center prospective study done between November 2012 to June 2020. 293 adult patients with acute PE having echocardiogram performed within 24 hours were enrolled and baseline comorbidities were recorded. 2 D Speckle tracking echocardiography was performed offline. Multiple logistic regression analyses and ROC were performed to evaluate if RV Strain (RVS) and strain rate (systolic-SRs, early diastolic-SRe) were able to predict all-cause mortality at 30 days.Results:Mean age was 65 ± 15 years, 60.7 % males and 41% had coronary artery disease, 29% had cancer and 11% had COPD. Mean free wall (FW) RVS was -19.7±7.7%, FW RV SRs was -0.9±0.4 s-1, FW RV SRe was 0.8±0.3 s-1. (Figure A) All-cause mortality was present in 11 patients (3.8%) at 30 days, 17 (5.8%) at 3 months and 29 (9.9%) at 6 months. In the Univariate analysis, FW RVS and FW RVSRs were significantly associated with 30 days all-cause, but after adjusting for age, cancer, and COPD, only FW RVSRs remains significant (p=0.02) with an AUC to predict 30-day all-cause mortality of 0.73 (95% CI 0.54-0.88) a cut-off value of -0.7 had a sensitivity of 73% and specificity of 64% (Figure B)Conclusions:The study demonstrated that RV Free wall SRs measured by 2 D STE is a good prognostic indicator for 30 days all-cause mortality in patients with acute PE. This novel modality could be used for risk stratification in the setting of acute PE.

Introduction:Subclinical cardiovascular involvement in COVID-19 patients has not been well described. 2D-Speckle Tracking Echocardiography derived global longitudinal strain (GLS) and systolic and early diastolic strain rate (SRs, SRe) measurements are more sensitive than standard echocardiographic parameters to diagnose subclinical mechanical dysfunction in patients with normal Left Ventricle Ejection Fraction (LVEF).Hypothesis:Evaluate subclinical myocardial mechanical function and reserve in active COVID-19 systemic disease patients with normal LVEF vs controls and between survivors and non survivors of COVID-19 in the longitudinal domain of contractility.Methods & Results:166 adult patients with active COVID 19 having normal LVEF were included and compared to 89 healthy volunteers. Baseline parameters were recorded. Peak GLS, SRs and SRe, were measured offline. Mean age was 62.53 ± 18.96 years and 87 (52%) were males and Mean LVEF was 62±5%. COVID-19 patients had higher GLS compared to controls (-20.93%±0.30 vs. -18.48%±0.40; p value <0.0001), SRs was similar (-0.97 s-1±0.10 vs. -0.98 s-1±0.02, p value 0.2528) and SRe was lower (0.85 s-1±0.0 vs. 1.05 s-1±0.02; p value <0.0001) respectively. After adjusting for age and sex, GLS, SRs and SRe was significant (p<0.001). GLS, SRs and SRe were similar in survivors vs. non survivors (p NS), both groups had elevated biomarkers (cardiac troponin, NT-pro BNP, CRP), but non survivors had higher levels (3.22±0.18 vs 3.62±0.28, p = 0.2371, 5.86±0.19 vs 7.15±0.27, p = 0.0004, 3.75±0.14 vs 4.31±0.21, p = 0.0422, 7.64±0.17 vs 8.26±0.25, p=0.0487 respectively) (Table 1).Conclusions:In our cohort, COVID-19 patients had better systolic reserve and abnormal relaxation compared to controls. There was no significant difference in strain (GLS) and strain rate (SRs & SRe) amongst survivors and non survivors in the longitudinal domain of contractility despite of higher inflammatory biomarkers in non-survivors.
