1. Protocolo diagnóstico de la colestasis

Abstract: El concepto de colestasis hace referencia a la dificultad o detención del flujo biliar. Es un problema clínico frecuente que puede ser la manifestación de múltiples enfermedades. Se diferencia entre colestasis intrahepática cuando existe una alteración dentro del parénquima hepático, y colestasis extrahepática si la obstrucción se localiza a nivel de colédoco. La anamnesis y la exploración física minuciosa son imprescindibles para poder orientar el diagnóstico. La ecografía abdominal es la primera prueba de imagen a realizar. Ante una ecografía normal o no concluyente, se debe ampliar el estudio con otras pruebas de imagen más específicas. Si existe sospecha de malignidad, la tomografía computarizada abdominal debe ser el primer procedimiento diagnóstico. La ecoendoscopia oral y la colangiografía mediante resonancia magnética son exploraciones con una alta rentabilidad diagnóstica equiparable entre sí. La colangiopancreatografía retrógrada endoscópica permite una visualización directa de la vía biliar con la posibilidad de realizar terapéutica.

2. Litiasis biliar

Abstract: La litiasis biliar o colelitiasis se define como la existencia de cálculos dentro de la vesícula biliar. Es una patología común, siendo la mayor parte de los casos asintomáticos sin necesidad de tratamiento. Las presentaciones clínicas típicas incluyen el cólico biliar, la coledocolitiasis, la colecistitis y la colangitis aguda. El síntoma común a todas las formas clínicas suele ser el dolor abdominal localizado en el hipocondrio derecho. El cólico biliar y la coledocolitiasis típicamente cursan con colestasis, mientras que la colangitis y la colecistitis aguda también asocian signos de infección como el aumento de reactantes de fase aguda. Para el diagnóstico se emplean parámetros clínicos, analíticos y radiológicos, siendo la ecografía abdominal la prueba más sensible para la localización de las litiasis. Con respecto al tratamiento, difiere en función del cuadro clínico. El cólico biliar no complicado precisa de analgesia en el momento agudo y colecistectomía programada. La coledocolitiasis tiene indicación de drenaje de la vía biliar y, en caso de colangitis aguda asociada, precisa antibioterapia. En los cuadros de colecistitis aguda está indicada la colecistectomía precoz, así como el tratamiento con antibióticos.

3. Protocolo diagnóstico de la rectitis (proctitis)

Abstract: La rectitis (inflamación del recto) puede estar causada por numerosas condiciones. Algunas de las causas más comunes de rectitis incluyen la enfermedad inflamatoria intestinal (colitis ulcerosa y enfermedad de Crohn), las enfermedades infecciosas de transmisión sexual (como la sífilis, la gonorrea y la clamidiasis), la radioterapia en la región pélvica, el trauma rectal, el prolapso rectal o la más infrecuente rectitis isquémica. El diagnóstico de la rectitis debe incluir un examen físico, pruebas de laboratorio como análisis de sangre y heces, así como pruebas de imagen como una colonoscopia o una rectosigmoidoscopia, ya que permiten observar la mucosa rectal y obtener muestras para análisis histológico y microbiológico. El tratamiento de la rectitis generalmente depende de la causa subyacente. Esto puede incluir medicamentos antiinflamatorios, antibióticos, antivirales, modificaciones en la dieta y cambios en el estilo de vida. 5. Experience-based transition to robotic surgery in an experienced program in minimally invasive hepatobiliary surgery

Abstract: Background: The adoption of robotic techniques in liver surgery introduces significant challenges for their safe integration within hepatobiliary surgery units. This study is designed to investigate the complexities associated with establishing a robotic surgery program. Methods: Data on robotic hepatobiliary surgeries were prospectively collected from October 2021 to October 2023. Historical cohorts from the institutional experiences for comparison were hand-assisted (HALS) and purely laparoscopic procedures (PLS). Inverse probability of treatment weighting and propensity score matching were employed to compare outcomes between PLS and robotic resections. The learning curve for robotic surgeries was evaluated by the cumulative sum method. Results: In this study, 454 patients were enrolled (113 robotic surgeries, 157 HALS, and 184 PLS). The posterosuperior segments resections were significantly higher in the robotic group (47.8%) compared to PLS (31.5%) and HALS (35.7%). There were no conversions in the robotic group, in PLS 2.7% and HALS 3.8%. The degree of difficulty according to the median of the IWATE score and IMM score was significantly higher in the robot group (p< 0.001 and p= 0.008, respectively). No significant differences in short-term outcomes were observed between robotic procedures and PLS in a matched subset of patients. Operative efficiency and blood loss improved significantly after the 75th robotic surgery patient, with high-difficulty cases (IWATE ≥ 10) incorporated from the beginning. Conclusion: This study suggests that robotic liver surgery in units with prior experience in minimally invasive liver surgery offers benefits, such as a lower conversion rate and a higher rate of successful difficult resections.

6. Individualized prediction models in ADHD: a systematic review and meta-regression

Abstract: There have been increasing efforts to develop prediction models supporting personalised detection, prediction, or treatment of ADHD. We overviewed the current status of prediction science in ADHD by: (1) systematically reviewing and appraising available prediction models; (2) quantitatively assessing factors impacting the performance of published models. We did a PRISMA/CHARMS/TRIPOD-compliant systematic review (PROSPERO: CRD42023387502), searching, until 20/12/2023, studies reporting internally and/or externally validated diagnostic/prognostic/treatment-response prediction models in ADHD. Using meta-regressions, we explored the impact of factors affecting the area under the curve (AUC) of the models. We assessed the study risk of bias with the Prediction Model Risk of Bias Assessment Tool (PROBAST). From 7764 identified records, 100 prediction models were included (88% diagnostic, 5% prognostic, and 7% treatment-response). Of these, 96% and 7% were internally and externally validated, respectively. None was implemented in clinical practice. Only 8% of the models were deemed at low risk of bias; 67% were considered at high risk of bias. Clinical, neuroimaging, and cognitive predictors were used in 35%, 31%, and 27% of the studies, respectively. The performance of ADHD prediction models was increased in those models including, compared to those models not including, clinical predictors (β = 6.54, p = 0.007). Type of validation, age range, type of model, number of predictors, study quality, and other type of predictors did not alter the AUC. Several prediction models have been developed to support the diagnosis of ADHD. However, efforts to predict outcomes or treatment response have been limited, and none of the available models is ready for implementation into clinical practice. The use of clinical predictors, which may be combined with other type of predictors, seems to improve the performance of the models. A new generation of research should address these gaps by conducting high quality, replicable, and externally validated models, followed by implementation research.

7. The human role to guarantee an ethical AI in healthcare: a five-facts approach

Abstract: With the emergence of AI systems to assist clinical decision-making, several ethical dilemmas are brought to the general attention. AI systems are claimed to be the solution for many high-skilled medical tasks where machines can potentially surpass human ability as for example in identifying normal and abnormal chest X-rays. However, there are also warns that AI tools could be the basis for a human replacement that can risk dehumanisation in medicine. In recent years, important proposals in the domain of AI ethics in healthcare have identified main ethical issues, as for example fairness, autonomy, transparency, and responsibility. The human warranty, which implies human evaluation of the AI procedures, has been described to lower the ethical risks. However, as relevant these works have been, translating principles into action has proved challenging as existing codes were mostly a description of principles. There is a great need to produce how-toproposals that are specific enough to be action-guiding. We present five human-focussed facts designed into a framework of human action for an ethical AI in healthcare. Through the factors, we examine the role of medical practitioners, patients, and developers in designing, implementing, and using AI in a responsible manner that preserves human dignity. The facts encompass a range of ethical concerns that were commonly found in relevant literature. Given that it is crucial to bring as many perspectives as possible to the field, this work contributes to translate principles into human action to guarantee an ethical AI in health.

11. Long-term oncological outcomes for HALS/Hybrid vs pure laparoscopic approach in colorectal liver metastases: a propensity score matched analysis

Abstract: Background: Studies comparing hand-assisted laparoscopic (HALS)/Hybrid and pure laparoscopic (PLS) resection for colorectal cancer liver metastasis have focused on short-term results,while long-term oncological outcomes remain understudied. Methods: We established a multi-institutional retrospective cohort study from four centers with experience in minimally invasive surgery between 2004 and 2020. Primary endpoints were overall survival (OS) and disease-free survival (DFS). Other endpoints analyzed were intraoperative and postoperative outcomes. Propensity score matching (PSM) was used to minimize baseline differences. Results: A total of 219 HALS/Hybrid (57.8%) and 160 PLS (42.2%) patients were included. After PSM, 155 patients remained in each group. Operative time (182 vs. 248 min, p= 0.012), use of intraoperative ablation (12.3 vs. 4.5%, p= 0.024), positive resection margin (4.5 vs 13.2%, p= 0.012), and pringle time (21 vs. 37 min, p= 0.001) were higher in PLS group. DFS at 1, 3, 5, and 7 years in HALS/Hybrid and PLS groups were 65.4%, 39.3%, 37.5%, and 36.3% vs. 64.9%, 38.0%, 33.1%, and 33.1%, respectively (p= 0.84). OS at 1, 3, 5, and 7 years in HALS/Hybrid and PLS groups were 94.5%, 71.4%, 54.3%, and 46.0% vs. 96.0%, 68.5%, 51.2%, and 41.2%, respectively (p= 0.73). Conclusion: Our study suggests no differences in long-term oncologic outcomes between the two techniques. We discovered that longer total operative, pringle time, higher rates of intraoperative ablation, and positive resection margins were associated with PLS. These differences in favor of HALS/Hybrid could be due to a shorter learning curve and a greater ability to control hemorrhage.

Abstract: ABSTRACTDuring the last decade, the policy efforts aiming to tackle the NEET situation (young people who neither study nor work) have increased dramatically in the European Union. Meanwhile, many studies challenged those policies, showing how they failed due to lack of understanding the NEETs. Thus, we analyse the factors that influence the appearance, modification and elimination of a NEET identity. The results reveal the presence of a shared NEET identity in the countries with high NEET rates, despite the considerable differences in the countries’ and NEETs’ characteristics. The NEET rate stands out as the key factor influencing the NEET identity, showing that this identity depends more on economic cycles than on the specific country or its culture.

14. Opioid-sparing anesthesia and patient-reported outcomes after open gynecologic surgery: a historical cohort study

Abstract: Purpose: Dexmedetomidine and ketamine may be administered intraoperatively as continuous infusions to provide opioid-sparing anesthesia. Recent evidence has yielded controversial results regarding the impact of opioid-free anesthesia on postoperative complications, and there is a gap in knowledge regarding patient-reported outcomes (PROs). This study aimed to determine the impact of opioid-sparing anesthesia and opioid-based anesthesia on PROs among gynecologic patients within an enhanced recovery after surgery (ERAS) program. Methods: We formed a single-center historical cohort from patients enrolled in another study who underwent open gynecologic surgery on an ERAS program from November 2014 to December 2020 (n= 2,095). We identified two cohorts based on the type of balanced anesthesia administered: 1) opioid-sparing anesthesia defined as the continuous infusion of dexmedetomidine and ketamine (adjuvants) during surgery or 2) opioid-based anesthesia (no adjuvants). We measured the quality of postoperative recovery using the MD Anderson Symptom Inventory (MDASI), a 29-item validated tool that was administered preoperatively, daily while admitted, and weekly after discharge until week 6. The primary outcome was interference with walking. We matched both cohorts and used a multilevel linear mixed-effect model to evaluate the effect of opioid-sparing anesthesia on the primary outcome. Results: In total, 498 patients were eligible (159 in the opioid-sparing anesthesia cohort and 339 in the opioid-based anesthesia cohort), of whom 149 matched pairs were included in the final analysis. Longitudinal assessment showed no significant or clinically important difference in interference with walking (P= 0.99), general activity (P= 0.99), or other PROs between cohorts. Median [interquartile range (IQR)] intraoperative opioid administration (expressed as morphine milligram equivalents [MME]) among matched patients in the opioid-sparing anesthesia cohort was 30 [25–55] mg vs58 [8–70] mg in the opioid-based anesthesia cohort (P< 0.01). Patients in the opioid-sparing anesthesia cohort had a lower opioid consumption in the postanesthesia care unit than those in the opioid-based anesthesia cohort (MME, 3 [0–10] mg vs5 [0–15] mg; P< 0.01), but there was no significant difference between cohorts in total postoperative opioid consumption (MME, 23 [0–94] mg vs35 [13–95] mg P= 0.053). Conclusions: In this single-center historical cohort study, opioid-sparing anesthesia had no significant or clinically important effects on interference with walking or other PROs in patients undergoing gynecologic surgery compared with opioid-based anesthesia. Opioid-sparing anesthesia was associated with less short-term opioid consumption than opioid-based anesthesia.

16. Originator recombinant human follitropin alfa versus recombinant human follitropin alfa biosimilars in Spain: A cost-effectiveness analysis of assisted reproductive technology related to fresh embryo transfers

Abstract: This study compared the cost per live birth and cost-effectiveness of the originator recombinant human follicle-stimulating hormone follitropin alfa (r-hFSH-alfa) and r-hFSH-alfa biosimilars for ovarian stimulation prior to assisted reproductive technology treatment in Spain. A decision tree model was developed, comprising pregnancy and live birth for one treatment cycle with fresh embryo transfer. Clinical inputs were based on a recent meta-analysis by Chua et al. [4]. Cost inputs were extracted from publicly available Spanish sources. The costs per live birth were lower with originator r-hFSH-alfa (€18,138) versus r-hFSH-alfa biosimilars (€20,377). The incremental cost-effectiveness ratio was €7208 for originator r-hFSH-alfa versus biosimilars. Drug acquisition costs for originator r-hFSH-alfa represented 10.5% of total costs in the base case analysis, and 6.2% in a treatment cycle resulting in live birth with one fresh embryo transfer. Results from the sensitivity analyses confirmed the robustness of the findings.

Abstract: Chronic use of Angiotensin-converting enzyme (ACE) inhibitors (ACEi) and aldosterone-receptor blockers (ARB) is not associated with worse outcomes in patients with COVID-19. However, evidence on the impact of their discontinuation during hospital admission is scarce. Our aim was to determine whether withdrawal of ACEi, ARB and mineralocorticoid receptor antagonists (MRA) is associated with all-cause mortality in a real-life large cohort of patients with SARS-CoV-2 infection.

21. Bioavailability Study of a Transdermal Patch Formulation of Rivastigmine Compared with Exelon in Healthy Subjects

Abstract: Background and Objectives: Rivastigmine is a reversible cholinesterase inhibitor indicated for the treatment of all stages of Alzheimer’s disease (AD). Transdermal patch formulation allows smooth and continuous drug delivery. Its tolerability, efficacy and convenience of use increase treatment compliance. This study was designed to evaluate the bioavailability and to assess the bioequivalence of two rivastigmine transdermal patches at steady state (RIV-TDS Test Product versus Exelon Marketed Reference Product), with a release rate of 13.3 mg/24 h, after multiple patch applications. As secondary objectives, safety, patch adhesion and skin irritation were evaluated. Methods: This was an open-label, randomized, balanced, two-period, two-sequence, cross-over study of healthy adults (n= 31). The treatment period consisted of two 5-day study periods during which consecutive daily application of the investigational patches with a release rate of 13.3 mg/24 h rivastigmine took place. Serial blood samples were collected to measure plasma concentrations. Adhesion and skin irritation assessments were performed after application of patches. Results: Point estimates and 90% confidence intervals of pharmacokinetic parameters at steady state, viz. area under the plasma concentration versus time curve from dosing time to the end of the dosing interval τ(profile day) at steady state [AUC0–τ,ss] (97.4; 88.8–106.9), maximum plasma concentration within the dosing interval τ(profile day) at steady state [Cmax,ss] (99.6; 90.4–109.7) and trough plasma concentration at the end of the dosing interval τ(profile day) at steady state [Cτ,ss] (96.8; 86.2–108.9), demonstrated that both patches were bioequivalent. Evaluation of patch adhesion showed better skin adherence for RIV-TDS as well as dermal response scores (skin tolerability after removal). Conclusions: For both products, bioequivalence was shown and systemic tolerability was in accordance with the safety profile of the drug substance.

23. Weaknesses and strengths of online marketing websites

Abstract: Purpose: Websites are the most important element of company strategy in the digital space. Therefore, establishing strategic management of online business is essential to improve firm connectedness and competitiveness and to achieve global company reach. This paper aims to propose the analysis of technical and content quality of websites to identify the main weaknesses and strengths of online business. Design/methodology/approach: An innovative measurement instrument called IWebQEI is designed to measure web quality level. This instrument is validated with data from 104 international companies. The results are used to verify whether there are quality differences between informative and e-commerce websites. Findings: The main findings indicate that e-commerce websites achieve greater levels of technical and content quality than informative sites, and companies implementing e-commerce pay more attention to the content quality dimension. In contrast, companies using an informative website are more focused on the technical quality dimension. Based on these results, practical insights are offered to improve the strategic management of e-commerce. Originality/value: Few studies have focused on analysing the technical and content quality of websites to identify the main weaknesses and strengths of online business. The results offer important theoretical and practical contributions for companies on how to manage their website to improve firm connectivity and competitiveness. Propósito: Los sitios web son el elemento más importante de la estrategia digital de la compañia. Una gestión estratégica del negocio online es esencial para mejorar su conectividad, competitividad y para conseguir su alcance global. Este trabajo propone el análisis de la calidad técnica y de contenido de sitios web para identificar las principales debilidades y fortalezas del negocio online. Metodología: Para medir el nivel de calidad web, se ha diseñado un innovador instrumento de medición, denominado IWebQEI. Este instrumento se valida con datos de 104 empresas internacionales. Los resultados se utilizan para comprobar si existen diferencias de calidad entre las webs informativas y los e-commerce. Resultados: Las resultados indican que los e-commerce alcanzan mayores niveles de calidad técnica y de contenido que las webs informativas, y las empresas que implementan un e-commerce prestan más atención a la calidad de contenido. En cambio, las empresas que implementan una web informativa se centran más en la calidad técnica. A partir de estos resultados se ofrecen ideas prácticas para mejorar la gestión estratégica del e-commerce. Originalidad: Pocos estudios analizan la calidad técnica y de contenido de los sitios web para identificar las principales debilidades y fortalezas del negocio online. Abstract: SummaryThe Portuguese millipede Ommatoiulus moreleti(Lucas, 1860) is widespread by commerce in Atlantic Islands, Australia, and South Africa, besides being regarded as a pest infesting plantations of vegetables and fruits. To date, the species has never been recorded in South America. In this study, we report for the first time the occurrence of O. moreletiin Brazil, with adults and immatures collected in the municipality of Campos do Jordão, São Paulo state. A maximum entropy model was used to explore the predicted distribution of the species in South America. The results showed a highly suitable area in the continent (AUC = 0.987), mainly in the Atlantic Forest, and regions of Argentina, Chile, and Uruguay. Considering percent contribution, the annual mean temperature (= 34%) and the temperature seasonality (= 33.4%) were the main variables which influenced the modeling. The potential risks of invasion of O. moreletimust be highlighted, including possible competition with native millipedes, its being a significant agricultural pest, and its successful invasion into new habitat without natural enemies.

Abstract: For many years, psychiatrists have tried to understand factors involved in response to medications or psychotherapies, in order to personalize their treatment choices. There is now a broad and growing interest in the idea that we can develop models to personalize treatment decisions using new statistical approaches from the field of machine learning and applying them to larger volumes of data. In this pursuit, there has been a paradigm shift away from experimental studies to confirm or refute specific hypotheses towards a focus on the overall explanatory power of a predictive model when tested on new, unseen datasets. In this paper, we review key studies using machine learning to predict treatment outcomes in psychiatry, ranging from medications and psychotherapies to digital interventions and neurobiological treatments. Next, we focus on some new sources of data that are being used for the development of predictive models based on machine learning, such as electronic health records, smartphone and social media data, and on the potential utility of data from genetics, electrophysiology, neuroimaging and cognitive testing. Finally, we discuss how far the field has come towards implementing prediction tools in real‐world clinical practice. Relatively few retrospective studies to‐date include appropriate external validation procedures, and there are even fewer prospective studies testing the clinical feasibility and effectiveness of predictive models. Applications of machine learning in psychiatry face some of the same ethical challenges posed by these techniques in other areas of medicine or computer science, which we discuss here. In short, machine learning is a nascent but important approach to improve the effectiveness of mental health care, and several prospective clinical studies suggest that it may be working already.

34. Combination of chemotherapy with BRAF inhibitors results in effective eradication of malignant melanoma by preventing ATM-dependent DNA repair

Abstract: Invasive malignant melanoma (MM) is an aggressive tumor with no curative therapy in advanced stages. Chemotherapy has not demonstrated its efficacy in MM and current treatment for tumors carrying the most frequent BRAFV600E mutation consists of BRAF inhibitors alone or in combination with MAPK pathway inhibitors. We previously found that BRAF inhibition prevents activation of the DNA-damage repair (DDR) pathway in colorectal cancer thus potentiating the effect of chemotherapy. We now show that different chemotherapy agents inflict DNA damage in MM cells, which is efficiently repaired, associated with activation of the ATM-dependent DDR machinery. Pharmacologic inhibition of BRAF impairs ATM and DDR activation in these cells, leading to sustained DNA damage. Combination treatments involving DNA-damaging agents and BRAF inhibitors increase tumor cell death in vitro and in vivo, and impede MM regrowth after treatment cessation. We propose to reconsider the use of chemotherapy in combination with BRAF inhibitors for MM treatment.

Abstract: AbstractThis article describes an intervention program to improve writing skills, intended to be a useful tool for everyday remedial work in primary school. The design was based on an initial review of the approaches most often used for writing intervention and identification of the main concerns for such intervention; namely, the use of direct and explicit instructions, the importance of feedback, recommended levels of intervention, the main content to consider, and use of motivating activities to get the student engaged or even to enjoy the intervention. All these considerations guided decision making about the appropriate features that would ensure an effective intervention in children with general and specific writing difficulties. The intervention program follows evidence-based practice strategies and may be useful for educational professionals in primary school, acknowledging that the program needs to be experimentally evaluated.

37. Writing intervention: A comprehensive program for primary school children

Abstract: This article describes an intervention program to improve writing skills, intended to be a useful tool for everyday remedial work in primary school. The design was based on an initial review of the approaches most often used for writing intervention and identification of the main concerns for such intervention; namely, the use of direct and explicit instructions, the importance of feedback, recommended levels of intervention, the main content to consider, and use of motivating activities to get the student engaged or even to enjoy the intervention. All these considerations guided decision making about the appropriate features that would ensure an effective intervention in children with general and specific writing difficulties. The intervention program follows evidence-based practice strategies and may be useful for educational professionals in primary school, acknowledging that the program needs to be experimentally evaluated.

Abstract: SummaryThe millipede Oxidus gracilis(C. L. Koch, 1847) is a cosmopolitan introduced species largely recorded in the southeast region of Brazil. The species is considered an agricultural pest with records in poly- or monocultures, and its influence on native communities of plants and invertebrates is still unknown in Brazil. From this perspective, a maximum entropy model was used to explore the predicted distribution of O. gracilisin Brazil. The results showed a highly suitable area in Atlantic Forest (AUC = 0.949 ± 0.02). Considering percent contribution, annual mean temperature (= 54.9) was the main variable which has influenced the modeling. The potentially suitable areas recovered in Atlantic Forest suggest that attention should be given to its possible invasion and the potential damage of O. gracilisto natural communities.

Durante las fases inicial y pico de la epidemia, los fármacos modificadores del curso de la EM caracterizados por ser inmunosupresores administrados en pulsos (TIP), vieron pospuesta su administración debido a la incertidumbre sobre su influencia en la infección, principalmente en contagiados/contagiosos asintomáticos/presintomáticos. El objetivo de este trabajo es presentar un algoritmo basado en criterios de seguridad que permita reanudar los TIP durante la fase de desescalado.

46. Outcomes of ulnar shortening osteotomy using a new compression plate

Abstract: Ulnar impaction syndrome (UIS) is a frequent cause of ulnar-sided wrist pain. Several open and arthroscopic surgery techniques have been described. Ulnar shortening osteotomy (USO) is often the method of choice. It has the advantage of leveling the distal radioulnar joint extra-articularly while maintaining the integrity of the soft tissues that stabilize the joint. This retrospective study reports the short-term outcomes of 20 patients who underwent surgery between 2013 and 2015. USO was performed with a locked plate and a new compression device (Alians®, Newclip™, Haute Goulaine, France). Preoperative and postoperative ulnar variance were measured on wrist X-rays in neutral pronation-supination. Functional outcomes were evaluated with the QuickDASH score and the pain on VAS (visual analog scale). Postoperative range of motion and grip strength were compared to those of the contralateral wrist. The average follow-up was 16 months (4 to 28 months). Preoperative ulnar variance was positive with an average of 4mm (1–11mm). The average length of the final ulna shortening was 3.5mm (1.5–6mm); the mean time to union was 4 months (3–12 months). The average QuickDASH was 12 and pain had decreased 2.4mm on the VAS. The results achieved with this new compression plate designed specifically for USO are similar to those described in the literature. This kind of device has an easily reproducible technique and can reduce the operative time.

47. Customer functional value creation through a sustainable entrepreneurial orientation approach

Abstract: AbstractThis paper advances a theoretical model to empirically test firms’ behaviour regarding sustainable entrepreneurship, enhancing what researchers have recently proposed at a solely conceptual level; this entails sustainable entrepreneurship being understood as a discipline that reliably allows organizations to successfully respond to sustainable development and market requirements. The authors suggest an integrated approach of dynamic-capabilities, S-D logic and product-service system views, which highlights the managerial predisposition to adopt a strategic position that fosters value in use (instead of regular property value), according to the current school of thoughts engaged with innovation and sustainable development. In this regard, this paper investigates structural influences (direct and indirect) of sustainable entrepreneurial orientation and customer functional value creation in firm performance. Hypotheses are tested using the partial least squares method on a multisectoral sample of 210 companies randomly stratified with proportional allocation. The findings indicate that a sustainable entrepreneurial orientation not only has a positive and direct relationship with firm performance, but also that this relationship improves when it is mediated by customer functional value creation. Further research pathways are provided.
