2. Ultrasonication-assisted deep eutectic solvent extraction of flavonoids from pretreated fenugreek and their antidiabetic & hypo-lipidemic potential

- Abstract
The effects of germination pretreatments on fenugreek seeds were analyzed by employing an ultrasound-assisted deep eutectic solvent (UAE-DES) extraction process. The germinated and UAE-DES extracted fenugreek sample of day 3 (GFE3) was found to be the most effective based on the results of flavonoid contents, antioxidant properties, and antibacterial potential. The results of HPLC-DAD analysis also suggested GFE3as an effective pretreatment with the highest extraction yields of kaempferol (5.33 ± 0.26 mg/gm d.w.), rutin (9.06 ± 0.44 mg/gm d.w.) and quercetin (25.69 ± 0.44 mg/gm d.w.) among all treatments. The results of simulated digestion showed that a major portion of flavonoid contents were lost during gastric digestion due to poor stability values and a maximum TFC release of 60.23% was recorded after intestinal digestion for GFE3. Subsequently, GFE3exhibited good inhibition properties against α-amylase (86.53 ± 4.24%), α-glucosidase (95.67 ± 4.68%) and pancreatic lipase enzymes (82.73 ± 4.05%). Hence, GFE3can be used as a potential pretreatment for enhanced antioxidant activity and improved glycemic, lipid and digestive responses, which can be further used in food, nutraceutical and pharmaceutical industries.

ABSTRACTThe world is currently facing a global challenge against neoplastic diseases. Chemotherapy, hormonal therapy, surgery, and radiation therapy are some approaches used to treat cancer. However, these treatments are frequently causing side effects in patients, such as multidrug resistance, fever, weakness, and allergy, among others side effects. As a result, current research has focused on phytochemical compounds isolated from plants to treat deadly cancers. Plants are excellent resources of bioactive molecules, and many natural molecules have exceptional anticancer properties. They produce diverse anticancer derivatives such as alkaloids, terpenoids, flavonoids, pigments, and tannins, which have powerful anticancer activities against various cancer cell lines and animal models. Because of their safety, eco-friendly, and cost-effective nature, research communities have recently focused on various phytochemical bioactive molecules. Ursolic acid (UA) and its derivative compounds have anti-inflammatory, anticancer, apoptosis induction, anti-carcinogenic, and anti-breast cancer proliferation properties. Ursolic acid (UA) can improve the clinical management of human cancer because it inhibits cancer cell viability and proliferation, preventing tumour angiogenesis and metastatic activity. Therefore, the present article focuses on numerous bioactivities of Ursolic acid (UA), which can inhibit cancer cell production, mechanism of action, and modulation of anticancer properties via regulating various cellular processes.

5. Combined effect of ipilimumab and nivolumab improves oncology endpoints in metastatic melanoma patients

- Abstract
ABSTRACTMetastatic melanoma has less frequency, but considered as the most dreaded cancer. The combination of nivolumab & ipilimumab is proving their mettle in treating metastatic melanoma. The patients when administered with the combination of nivolumab & ipilimumab have shown improved median progression free survival, objective response rate and overall survival rate compared with nivolumab and ipilimumab monotherapy. The combination shrinks the tumor cells by attacking different checkpoints viz. CTLA-4 and PD-L1, respectively. The combination treatment reveals reduced disease progression and suggests nivolumab’s non-cross resistant nature. The median progression free survival in “nivolumab plus ipilimumab” group has shown an increase of 66.7% and 296.6% in comparison to nivolumab and ipilimumab monotherapy. The other parameter viz. objective response rate improvement is equivalent to almost 14% and 38.6% when compared to nivolumab and ipilimumab monotherapy, respectively.

The 2015 LancetCommission on global surgery identified surgery and anaesthesia as indispensable parts of holistic health-care systems. However, COVID-19 exposed the fragility of planned surgical services around the world, which have also been neglected in pandemic recovery planning. This study aimed to develop and validate a novel index to support local elective surgical system strengthening and address growing backlogs.

- Abstract
In‐situ interfacial polymerization technique is developed to prepare monovalent anion‐permselective membranes (MAPMs). The introduction of multi‐hydroxyl (OH) and cationic groups in the base membrane provide suitable routes towards the monovalent anion transport. Only trimesoyl chloride is employed to generate a crosslinked polyester layer on membrane surface. The ISIP transforms the surface of anion exchange membranes into a dense and negatively charged layer to promote the ion permselectivity. The modified membranes show a high limiting current density and low membrane resistance of 4.7 Ω cm2. The resultant membranes exhibit a high Cl−flux of 6.3 × 10−8mol cm−2s−1and a high permselectivity of ~60 for Cl−/SO42−. The findings demonstrate the ISIP technique having great potentials for the fabrication of highly permselective MAPMs.

9. Mechanical properties of Ti-6Al-4V thin walls fabricated by laser powder bed fusion

- Abstract
Laser powder bed fusion (PBF-LB/M) has recently garnered considerable attention for fabricating thin walls owing to its potential for producing lightweight structures. However, knowledge of the mechanical properties of thin walls with high surface-to-volume ratios remains insufficient. Specifically, no studies have been conducted for thin walls with a thickness less than 400 µm, making the size effect unclear. Therefore, the main objective of this work is to reveal the effect of thicknesses on the mechanical properties of Ti-6Al-4V samples as thin as 195 µm. To achieve this, the fabrication process was optimized to maximize the aspect ratio and properties of samples. In addition, the microstructure and surface roughness were examined to understand the size effect. The results demonstrated that the ultimate tensile strength of the thin-wall samples was similar to that of bulk Ti-6Al-4V samples fabricated via PBF-LB/M, regardless of the thickness. By contrast, the elongation of the thin-wall samples decreased with thickness, indicating a significant size effect. This size effect on elongation can be attributed to the strong influence of surface and internal defects.

10. Low voltage tunability of PVDF-based nanocomposites by interface polarization induced by TiO2-coated BST core-shell nanoparticle

- Abstract
Polymer-based flexible materials can be slated as successors of conventional inorganic ferroelectrics to induce tunable dielectric properties in microwave devices because of their advantageous characteristics, such as lightweight, flexibility, easy processibility, and eco-friendliness. Ferroelectric polymers require a high external field to induce dielectric tunability, which can be a problem. Another problem is the dielectric mismatch and incompatibility between ceramic and polymer, which hinders the ceramic-polymer composites to induce tunable dielectric properties. Herin, titanium oxide-barium strontium titanate (TO@BST) core-shell nanoparticle are reinforced into the poly (vinylidene fluoride) (PVDF) matrix to fabricate flexible nanocomposite with tunable dielectric properties. The core-shell nanoparticle-based nanocomposite produced low voltage dielectric tunability (12.03 % at 33 kV/mm) and excellent tunable dielectric efficiency (TuE, 0.8 %) while maintaining low dielectric loss (tanδ, 0.024). Moreover, TO@BST-based nanocomposite exhibited a remarkable 91 % improvement in dielectric constant compared to pure PVDF and a 25.7 % improvement compared to BST-based nanocomposite. The dielectric breakdown strength (Eb) of the nanocomposite filled with 40 wt% TO@BST nanoparticle improved by 45 % (114.36 kV/mm) compared to pure BST-based nanocomposite (78.68 kV/mm). This work provides valuable insights into designing novel flexible nanocomposites with low voltage tunability and high breakdown strength at high filler loading.

The main focus of the current article is to investigate the two-dimensional boundary layer flow of non-Newtonian fluid induced by a linear stretching surface in the streamwise direction. The flow features of non-Newtonian fluid (i.e. shear-thinning and thickening nature of the fluids) are explored with Carreau fluid model. In addition, thermophoresis, Brownian motion, thermal radiation (with Rosseland approximation), binary mixture and activation enthalpy are also incorporated in fluid model for better analysis of thermal and solutal properties. The mathematical modelling of under consider physical problem yields nonlinear partial differential system. The governing mathematical system is first simplified with scaling transformations for the high Reynolds number and then transformed into non-dimensional ordinary differential system by using the similarity variables. Since the governing mathematical system comprised on nonlinear boundary value problem, thus shooting method is utilized for the numerical solution of the fluid flow governing equations. The obtained results followed the qualitative manners of the previously reported data. The computed results for important physical quantities (i.e. velocity, temperature, and concentration) are presented through graphs and then interpreted physically. It is observed that the boundary layer thickness increased for shear thickening fluids (n > 1) while it is reduced for shear thinning fluids (n < 1). Also, the temperature of shear thinning fluid is higher than shear thickening fluid. In addition, heat generation and thermal radiations enhance the thermal boundary layer. Thermophoretic viscosity effect reduces the concentration profile significantly in the flow domain. • The laminar boundary-flows over heated linear stretching surface is computed for Carreau model. • For large Reynolds number the basic flow may be determined numerically in terms of a similarity variable. • Activation enthalpy and radiative heat flux are considered under the energy equation. • Resulting in enhanced heat transfer properties of the fluid [ABSTRACT FROM AUTHOR]

- Abstract
IMPORTANCE: DCVAC/PCa is an active cellular immunotherapy designed to initiate an immune response against prostate cancer. OBJECTIVE: To evaluate the efficacy and safety of DCVAC/PCa plus chemotherapy followed by DCVAC/PCa maintenance treatment in patients with metastatic castration-resistant prostate cancer (mCRPC). DESIGN, SETTING, AND PARTICIPANTS: The VIABLE double-blind, parallel-group, placebo-controlled, phase 3 randomized clinical trial enrolled patients with mCRPC among 177 hospital clinics in the US and Europe between June 2014 and November 2017. Data analyses were performed from December 2019 to July 2020. INTERVENTIONS: Eligible patients were randomized (2:1) to receive DCVAC/PCa (add-on and maintenance) or placebo, both in combination with chemotherapy (docetaxel plus prednisone). The stratification was applied according to geographical region (US or non-US), prior therapy (abiraterone, enzalutamide, or neither), and Eastern Cooperative Oncology Group performance status (0-1 or 2). DCVAC/PCa or placebo was administered subcutaneously every 3 to 4 weeks (up to 15 doses). MAIN OUTCOMES AND MEASURES: The primary outcome was overall survival (OS), defined as the time from randomization until death due to any cause, in all randomized patients. Survival was compared using 2-sided log-rank test stratified by geographical region, prior therapy with abiraterone and/or enzalutamide, and Eastern Cooperative Oncology Group performance status. RESULTS: A total of 1182 men with mCRPC (median [range] age, 68 [46-89] years) were randomized to receive DCVAC/PCa (n = 787) or placebo (n = 395). Of these, 610 (81.8%) started DCVAC/PCa, and 376 (98.4%) started placebo. There was no difference in OS between the DCVAC/PCa and placebo groups in all randomized patients (median OS, 23.9 months [95% CI, 21.6-25.3] vs 24.3 months [95% CI, 22.6-26.0]; hazard ratio, 1.04; 95% CI, 0.90-1.21; P = .60). No differences in the secondary efficacy end points (radiological progression-free survival, time to prostate-specific antigen progression, or skeletal-related events) were observed. Treatment-emergent adverse events related to DCVAC/PCa or placebo occurred in 69 of 749 (9.2%) and 48 of 379 (12.7%) patients, respectively. The most common treatment-emergent adverse events (DCVAC/PCa [n = 749] vs placebo [n = 379]) were fatigue (271 [36.2%] vs 152 [40.1%]), alopecia (222 [29.6%] vs 130 [34.3%]), and diarrhea (206 [27.5%] vs 117 [30.9%]). CONCLUSIONS AND RELEVANCE: In this phase 3 randomized clinical trial, DCVAC/PCa combined with docetaxel plus prednisone and continued as maintenance treatment did not extend OS in patients with mCRPC and was well tolerated. 14. Massive Saddle Pulmonary Embolism in a Preterm Neonate With Successful Emergent Open Embolectomy

- Abstract
Pulmonary embolism in the neonate is a rare, life-threatening emergency. Risk factors for neonatal pulmonary embolism (PE) include sepsis, asphyxia, prematurity, and vascular catheterisation. We report the case of a preterm neonate with a massive saddle pulmonary thrombosis of unidentified etiology. Prompt diagnosis by cardiology allowed an emergent lifesaving open surgical thrombectomy, underscoring the importance of efficient multidisciplinary teamwork. Pediatric health-care professionals must be aware of this rare entity when initial oxygen desaturation management fails, even when obvious risk factors for PE are not apparent. We emphasise the importance of seamless multidisciplinary communication and proactive surgical consultation.

- Abstract
To develop a consensus statement on the use of lung ultrasound (LUS) in the assessment of symptomatic general medical inpatients with known or suspected coronavirus disease 2019 (COVID‐19). Our LUS expert panel consisted of 14 multidisciplinary international experts. Experts voted in 3 rounds on the strength of 26 recommendations as “strong,” “weak,” or “do not recommend.” For recommendations that reached consensus for do not recommend, a fourth round was conducted to determine the strength of those recommendations, with 2 additional recommendations considered. Of the 26 recommendations, experts reached consensus on 6 in the first round, 13 in the second, and 7 in the third. Four recommendations were removed because of redundancy. In the fourth round, experts considered 4 recommendations that reached consensus for do not recommend and 2 additional scenarios; consensus was reached for 4 of these. Our final recommendations consist of 24 consensus statements; for 2 of these, the strength of the recommendations did not reach consensus. In symptomatic medical inpatients with known or suspected COVID‐19, we recommend the use of LUS to: (1) support the diagnosis of pneumonitis but not diagnose COVID‐19, (2) rule out concerning ultrasound features, (3) monitor patients with a change in the clinical status, and (4) avoid unnecessary additional imaging for patients whose pretest probability of an alternative or superimposed diagnosis is low. We do not recommend the use of LUS to guide admission and discharge decisions. We do not recommend routine serial LUS in patients without a change in their clinical condition.

Background: The global health emergency due to SARS-CoV-2 causing the COVID-19 pandemic emphasized the scientific community to intensify their research work for its therapeutic solution. In this study, Indian traditional spices owing to various medicinal properties were tested in silico for their inhibitory activity against SARS-CoV-2 proteins. SARS-CoV-2 spike proteins (SP) and main proteases (M
pro ) play a significant role in infection development were considered as potential drug targets. Methods: A total of 75 phytochemicals present in traditional Indian spices retrieved from the published literature and Dr. Duke’s Phytochemical and Ethnobotanical Database, were docked with Mpro (PDB IDs: 6YNQ), and the SP (PDB IDs: 6LXT and 6YOR). Results: Through the screening process, 75 retrieved phytochemicals were docked with spike protein (PDB IDs: 6LXT and 6YOR) and main protease (PDB ID: 6YNQ) of SARS-CoV-2. Among them, myricetin, a flavonoid (rank score: 6LXT: -11.72383; 6YOR: -9.87943; 6YNQ: -11.68164) from Allium sativum L and Isovitexin, an example of flavone (rank score: 6LXT: -12.14922; 6YOR: -10.19443; 6YNQ: -12.60603) from Pimpinella anisumL were the most potent ligands against SP and Mpro of SARS-CoV-2. Whereas, Astragalin from Crocus sativus L.; Rutin from Illicium verum, Oxyguttiferone from Garcinia cambogia; Scopolin from Apium graveolens L, Luteolin from Salvia officinalis, Emodin, Aloe-emodin from Cinnamomum zeylanicium and Apigenin from Allium sativum L showed better inhibition against Mpro than SP of SARS-CoV-2. The amino acid residues like SER, LYS, ASP and TYR were found playing important role in protein-ligand interactions via hydrogen bonding and Vander Waals forces. Conclusion: Optimal use of traditional spices in our daily meals may help fight against COVID-19. This study also paves the path for herbal drug formulation against SARS-CoV-2 after wet lab validation.- Published

Background: The main proteases (Mpro) and Spike Proteins (SP) of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) play a major role in viral infection development by producing several non-structural proteins (nsPs) and penetrating the host cells, respectively. In this study, the potential of in silico molecular docking-based drug repositioning approach was exploited for identifying the inhibitors of M
pro and SP of SARS-CoV-2. Methods: A total of 196 compounds, including various US-FDA-approved drugs, vitamins, and their analogs, were docked with Mpro (PDB IDs: 6YB7 and 6Y84), and the top six ligands were further tested for ADME properties, followed by docking with SP (PDB IDs: 6LXT and 6W41). Results: Out of 196 compounds, binding energy (DE) of Silybin B (6YB7: DE: -11.20 kcal/mol; 6Y84: DE: - 10.18 kcal/mol; 6LXT: DE: -10.47 kcal/mol; 6W41: DE: -10.96 kcal/mol) and Cianidanol (6YB7: DE: -8.85 kcal/mol; 6LXT: DE: -9.36 kcal/mol; 6Y84: DE: -10.02 kcal/mol; 6W41: DE: -9.52 kcal/mol) demonstrated better binding and ADME properties compared with the currently endeavored drugs like Hydroxychloroquine and Lopinavir. Additionally, Elliptinone, Diospyirin, SCHEMBL94263, and Fiboflavin have shown encouraging results. Fiboflavin, an immunity booster, was found to inhibit both the Mpro and spike protein of SARSCoV- 2. It was observed that amino acid residues MET6, ALA7, PHE8, PRO9, ASP295, GLY302, VAL303, and THR304 play significant roles in protein-ligand interactions through hydrogen bonds and Vander Waals forces. Conclusion: Silybin B and Cianidanol showed excellent binding and ADME properties compared with the currently endeavored drugs and can be exploited as therapeutic options against SARS-CoV-2 infection after experimental validation and clinical trials.- Published

Background: Osteoporosis is a skeletal disorder characterized by compromised bone strength, resulting in increased fracture risk. Patients with prostate cancer may have multiple risk factors contributing to bone fragility: advanced age, hypogonadism, and long-term use of androgen-deprivation therapy. Despite absence of metastatic disease, patients with nonmetastatic castrate-resistant prostate cancer receiving newer androgen receptor inhibitors can experience decreased bone mineral density. A systematic approach to bone health care has been hampered by a simplistic view that does not account for heterogeneity among prostate cancer patients or treatments they receive. This review aims to raise awareness in oncology and urology communities regarding the complexity of bone health, and to provide a framework for management strategies for patients with nonmetastatic castrate-resistant prostate cancer receiving androgen receptor inhibitor treatment. Methods: We searched peer-reviewed literature on the PubMed database using key words “androgen-deprivation therapy,” “androgen receptor inhibitors,” “bone,” “bone complications,” and “nonmetastatic prostate cancer” from 2000 to present. Results: We discuss how androgen inhibition affects bone health in patients with nonmetastatic castrate-resistant prostate cancer. We present data from phase 3 trials on the three approved androgen receptor inhibitors with regard to effects on bone. Finally, we present management strategies for maintenance of bone health. Conclusions: In patients with nonmetastatic castrate-resistant prostate cancer, aging, and antiandrogen therapy contribute to bone fragility. Newer androgen receptor inhibitors were associated with falls or fractures in a small subset of patients. Management guidelines include regular assessment of bone density, nutritional guidance, and use of antiresorptive bone health agents when warranted.

AbstractThe widespread antigenic changes lead to the emergence of a new type of coronavirus (CoV) called as severe acute respiratory syndrome (SARS)-CoV-2 that is immunologically different from the previous circulating species. Angiotensin-converting enzyme-2 (ACE-2) is one of the most important receptors on the cell membrane of the host cells (HCs) which its interaction with spike protein (SP) with a furin-cleavage site results in the SARS-CoV-2 invasion. Hence, in this review, we presented an overview on the interaction of ACE-2 and furin with SP. As several kinds of CoVs, from various genera, have at their S1/S2 binding site a preserved site, we further surveyed the role of furin cleavage site (FCS) on the life cycle of the CoV. Furthermore, we discussed that the small molecular inhibitors can limit the interaction of ACE-2 and furin with SP and can be used as potential therapeutic platforms to combat the spreading CoV epidemic. Finally, some ongoing challenges and future prospects for the development of potential drugs to promote targeting specific activities of the CoV were reviewed. In conclusion, this review may pave the way for providing useful information about different compounds involved in improving the effectiveness of CoV vaccine or drugs with minimum toxicity against human health.Communicated by Ramaswamy H. 21. The effect of umbilical cord milking on cerebral blood flow in very preterm infants: a randomized controlled study

Objective: To compare the effect of umbilical cord milking (UCM) vs. early cord clamping (ECC) on cerebral blood flow (CBF). Method: Preterm infants <31 weeks’ gestation were randomized to receive UCM or ECC at birth. Blood flow velocities and resistive & pulsatility indices of middle and anterior cerebral arteries were measured at 4–6 and 10–12?h after birth as an estimate of CBF. Results: Randomization allocated 37 infants to UCM and 36 to ECC. Maternal and antenatal variables were similar. There were no significant differences between groups in middle or anterior CBF velocities and resistive indices at either study time point. CBF variables were not correlated with mean blood pressure, systemic blood flow, or intraventricular hemorrhage. Conclusions: In very preterm infants, UCM compared with ECC was not shown to change CBF indices during the first 12?h of age or correlate with other hemodynamic measures or with intraventricular hemorrhage. 22. Medical oncology referral and systemic therapy of patients with advanced stage urothelial carcinoma

Aim:To understand physician visit patterns among patients with stage IV (including nonmetastatic [M0] and metastatic [M1] disease) urothelial carcinoma (UC) and understand factors associated with a timely referral to a medical oncologist and systemic treatment. Patients & methods:Retrospective analysis of Surveillance, Epidemiology and End Results-Medicare data. Results:First physician encounter was with a urologist (M0: 69%; M1: 53%) or primary care physician ([PCP]; M0: 19%, M1: 25%) for the majority of patients around UC diagnosis. After the index urologist encounter, most patients had a subsequent medical oncologist visit at a median of 52 days (M0: 69.5 days, M1: 33 days). In an adjusted model, older age, index PCP visit, higher comorbidities and M0 disease were negatively associated with a medical oncologist referral. Among those referred to a medical oncologist, older age, Hispanic or non-Hispanic Black race and not being married were negatively associated with subsequent chemotherapy receipt (p < 0.05). Conclusion:Many patients with advanced UC encounter multiple specialists during their disease course. Older patients or those with a first UC-related encounter with a PCP are less likely to be referred to medical oncology. Once referred to medical oncology, social determinants, including race and marital status, are relevant predictors of receiving chemotherapy.

- Abstract
Purpose: African Americans experience greater prostate cancer risk and mortality than do Caucasians. An analysis of pooled phase III data suggested differences in overall survival (OS) between African American and Caucasian men receiving sipuleucel-T. We explored this in PROCEED (NCT01306890), an FDA-requested registry in over 1900 patients with metastatic castration-resistant prostate cancer (mCRPC) treated with sipuleucel-T. Patients and methods: OS for patients who received ≥1 sipuleucel-T infusion was compared between African American and Caucasian men using an all patientset and a baseline prostate-specific antigen (PSA)-matchedset (two Caucasians to every one African American with baseline PSAs within 10% of each other). Univariable and multivariable analyses were conducted. Survival data were examined using Kaplan–Meier and Cox proportional hazard methodologies. Results: Median follow-up was 46.6 months. Overall survival differed between African American and Caucasian men with hazard ratios (HR) of 0.81 (95% confidence interval [CI]: 0.68–0.97, P= 0.03) in the all patientset and 0.70 (95% CI: 0.57–0.86, P< 0.001) in the PSA-matchedset. Median OS was longer in African Americans than in Caucasian men for both analysis sets, e.g., 35.3 and 25.8 months, respectively, in the PSA-matchedset. Similar results were observed in the all patientset. Differences were larger when treatment began at lower baseline PSA; curves were more similar among patients with higher baseline PSA. In patients with baseline PSA below the median, the HR was 0.52 (95% CI: 0.37–0.72, P< 0.001), with median OS of 54.3 versus 33.4 months. Known prognostic factors and African American race (multivariable analyses; HR: 0.60, 95% CI: 0.48–0.74, P< 0.001) were independently associated with OS. Use of post-sipuleucel-T anticancer interventions was balanced between races. Conclusion: In this exploratory analysis of a registry including nearly 12% African American men with mCRPC, OS was significantly different between African Americans and Caucasians, indicating further research is warranted.

Current investigation elaborates the impacts of Brownian motion and thermophoretic force on electrically conducting Prandtl-Eyring nanofluid flow yielded by stretched surface. Buongiorno nano-model is used to trace the heat and mass transfer characteristics in the flow regime. The mathematical formulation concerning to the adopted physical parameters is modeled in the form of complex partial differential structure. Boundary layer theory is obliged to reduce non-linearity of subsequent equations by truncating higher order terms. To facilitate the computation process, the governing problem in partial differential form is converted into dimensionless ordinary differential expressions. Numerical solution for attained boundary value problem is procured by R-K-Fehlberg methodology. The consequences of flow governing parameters on interested physical quantities (momentum, heat, concentration) are depicted in graphical manner while tabular representation is used to demonstrate the variations in wall drag coefficient, wall thermal flux and particles concentration flux. The computed results show that the presence of magnetic field is not favorable for fluid momentum, albeit, both Brownian motion and thermophoresis phenomenon surges the thermal energy of fluid. Besides these, concentration profile increases versus Brownian motion while thermophoresis phenomenon has reverse impacts on it. Surface drag force enriched by magnifying the magnetic field intensity, furthermore, the surface heat flux shows reduction versus Brownian motion and thermophoresis parameters. In addition, the surface mass flux shows increasing trend versus all governing parameters.

Purpose: The purpose of current flow configuration is to spotlights the thermophysical aspects of magnetohydrodynamics (MHD) viscoinelastic fluid flow over a stretching surface. Design/methodology/approach: The fluid momentum problem is mathematically formulated by using the Prandtl–Eyring constitutive law. Also, the non-Fourier heat flux model is considered to disclose the heat transfer characteristics. The governing problem contains the nonlinear partial differential equations with appropriate boundary conditions. To facilitate the computation process, the governing problem is transmuted into dimensionless form via appropriate group of scaling transforms. The numerical technique shooting method is used to solve dimensionless boundary value problem. Findings: The expressions for dimensionless velocity and temperature are found and investigated under different parametric conditions. The important features of fluid flow near the wall, i.e. wall friction factor and wall heat flux, are deliberated by altering the pertinent parameters. The impacts of governing parameters are highlighted in graphical as well as tabular manner against focused physical quantities (velocity, temperature, wall friction factor and wall heat flux). A comparison is presented to justify the computed results, it can be noticed that present results have quite resemblance with previous literature which led to confidence on the present computations. Purpose: The purpose of the current flow configurations is to bring to attention the thermophysical aspects of magnetohydrodynamics (MHD) Williamson nanofluid flow under the effects of Joule heating, nonlinear thermal radiation, variable thermal coefficient and activation energy past a rotating stretchable surface. Design/methodology/approach: A mathematical model is examined to study the heat and mass transport analysis of steady MHD Williamson fluid flow past a rotating stretchable surface. Impact of activation energy with newly introduced variable diffusion coefficient at the mass equation is considered. The transport phenomenon is modeled by using highly nonlinear PDEs which are then reduced into dimensionless form by using similarity transformation. The resulting equations are then solved with the aid of fifth-order Fehlberg method. Findings: The rotating fluid, heat and mass transport effects are analyzed for different values of parameters on velocity, energy and diffusion distributions. Parameters like the rotation parameter, Hartmann number and Weissenberg number control the flow field. In addition, the solar radiation, Joule heating, Prandtl number, thermal conductivity, concentration diffusion coefficient and activation energy control the temperature and concentration profiles inside the stretching surface. It can be analyzed that for higher values of thermal conductivity, Eckret number and solar radiation parameter the temperature profile increases, whereas opposite behavior is noticed for Prandtl number. Moreover, for increasing values of temperature difference parameter and thermal diffusion coefficient, the concentration profile shows reducing behavior. Originality/value: This paper is useful for researchers working in mathematical and theoretical physics. Moreover, numerical results are very useful in industry and daily-use processes. 27. Real-world incidence and burden of adverse events among non-metastatic prostate cancer patients treated with secondary hormonal therapies following androgen deprivation therapy

AbstractAims:To describe the incidence and identify prognostic factors of central nervous system (CNS) adverse events (AEs) and any AEs (CNS, skin rash, or fracture) and evaluate the healthcare resource utilization (HCRU), direct medical costs, and therapy discontinuation associated with these AEs among non-metastatic prostate cancer (nmPC) patients who received secondary hormone therapies.Methods and results:nmPC patients who had initiated secondary hormonal therapy with enzalutamide, bicalutamide, or abiraterone ≥1 year after androgen deprivation therapy (ADT) were identified in the MarketScan database. Survival analyses were used to describe the incidence of CNS or any AEs. Annual HCRU and costs were compared across patient groups (CNS AE vs no CNS AE; any AE vs no AE) using propensity score weighted generalized linear models. Multivariate Cox proportional hazards models were used to identify AE predictors and compare risks of discontinuation.Results:The analysis included 532 patients who initiated secondary hormonal therapies, among whom 201 (38%) and 244 (46%) experienced a CNS AE and any AE, respectively. Median times to CNS AE and any AE from therapy initiation were 17.90 and 11.00 months, respectively. Predictors of any AE were any AE in the baseline period (≤6 months before starting therapy), Charlson Comorbidity Index (CCI) score (1 vs 0), surgical castration, and older age. Predictors of CNS AEs were CNS AE in the baseline period and CCI score (1 vs 0). CNS and any AEs were associated with significantly higher HCRU. CNS AEs were associated with significantly higher incremental total medical costs ($18,522). CNS AEs and any AEs significantly increased therapy discontinuation risk by 48% and 38%, respectively.Conclusions:AEs increase the economic burden and therapy discontinuation among nmPC patients receiving secondary hormonal therapies subsequent to ADTs. These patients should be carefully evaluated for AEs to reduce therapy discontinuation, HCRU, and direct medical costs.

Anaerobic ammonium oxidation (anammox) is efficient and cost-effective for treating high-strength ammonia wastewater, but the organics in wastewater will affect its stability. To address this challenge, it is crucial to gain a deep understanding of the inhibitory effects and mechanisms of organics stress on anammox bacteria. The review provided a comprehensive classification of organics and evaluated their specific effects on the anammox system according to their respective characteristics. Based on the micro to macro perspective, the “molecule-cell-ecology” inhibitory mechanism of organics on anammox bacteria was proposed. The molecular observation systematically summarized the binding process and action sites of organics with anammox bacteria. At the cellular observation, the mechanisms of organics effects on extracellular polymeric substances, membranes, and anammoxosome of anammox bacteria were also expounded. At the ecological observation, the dynamic changes in coexisting populations and their role in organics transformation were further discussed. Further revelations on response mechanisms and inhibition mitigation strategies were proposed to broaden the applicability of anammox systems for organic wastewater. This review offered a multidimensional understanding of the organics inhibitory mechanism of anammox bacteria and provided a theoretical foundation for anammox systems.

Basic cardiac point-of-care ultrasound (POCUS)/focused cardiac ultrasound (FoCUS) often reveals incidental findings. Consider a patient with dyspnea in whom FoCUS reveals left ventricular (LV) hypertrophy, calcified valves, and pericardial effusion but “normal” LV systolic function. Such findings may trigger comprehensive echocardiography. Although advanced cardiac POCUS cannot replace comprehensive echocardiography, its role at the bedside is evolving. This systematic review identified studies suggesting that a sufficiently trained practitioner can leverage advanced techniques to evaluate conditions such as hypertrophic cardiomyopathy (4), LV diastolic dysfunction (6), severe valvular heart disease (6), and tamponade (8). This article discusses the use of advanced cardiac POCUS in these conditions.

This paper concerns the characteristics of heat and mass transfer in upper convected Maxwell fluid flow over a linear stretching sheet with solar radiation, viscous desperation and temperature based viscosity. After boundary layer approximation, the governing equations are achieved (namely Maxwell, upper convected material derivative, thermal and concentration diffusions). By using the self-similarity transformations the governing PDEs are converted into nonlinear ODEs and solved by RK-4 method in combination with Newton Raphson (shooting technique). The effects of developed parameters on velocity, temperature, concentration, fraction factor, heat and mass diffusions are exemplified through graphs and tabular form and are deliberated in detail. Numerical values of fraction factor, heat and mass transfer rates with several parameters are computed and examined. It is noticed that the temperature is more impactable for higher values of radiative heat transport, thermal conductivity and viscous dissipation. The comparison data for some limiting case are acquired and are originated to be in good agreement with previously published articles.

31. Overall survival, costs, and healthcare resource use by line of therapy in Medicare patients with newly diagnosed metastatic urothelial carcinoma

AbstractAims:Medicare patients with metastatic or surgically unresectable urothelial carcinoma (mUC) often receive platinum-based chemotherapy as first line of therapy (LOT), but invariably progress, requiring additional LOTs and healthcare resource use (HCRU). To better understand the evolving mUC treatment landscape, the economic burden of chemotherapy-based mUC treatments among US Medicare patients was estimated.Methods:Newly diagnosed Medicare patients with mUC were identified from the Surveillance, Epidemiology, and End Results (SEER)-Medicare database. Patients were followed from diagnosis to death, disenrollment, or end of study to characterize LOTs (first [LOT1], second [LOT2], and third or greater [LOT3+]). Kaplan-Meier methods were used to estimate overall survival (OS) by LOT. HCRU and mean costs were reported over the follow-up period, LOT duration, and maximum LOT received.Results:Among 1,873 eligible patients with mUC (median age = 77 years; median follow-up = 7.5 months), 1,035 (55%) received no chemotherapy. Among chemotherapy-treated patients, 61% had LOT1 only, 25% had LOT1 and LOT2 only, and 14% had LOT3+. Median OS was 8.1 months, range was 4.3 (untreated) to 29.8 (LOT3+) months. HCRU frequency increased with additional LOTs. Mean cumulative per-patient cost was $82,912 for all patients, increasing with additional LOTs (untreated = $57,207; LOT1 = $99,213; LOT2 = $125,190; LOT3+ = $163,884). Mean per patient per month cost was $18,827 for all patients, decreasing with increasing number of LOTs received (untreated = $27,211; LOT1 = $9,601; LOT2 = $7,325; LOT3+ = $6,017).Limitations:Potential for treatment misclassification when using the algorithm defining LOTs and non-generalizability of results to younger patients.Conclusions:Over 50% of Medicare patients with mUC received no chemotherapy. Among chemotherapy-treated patients, most received only one LOT. Additional LOTs led to higher mean costs and HCRU, but as patients were followed longer, monthly costs decreased. As treatments evolve to include immuno-oncology agents, these findings provide a clinically relevant economic benchmark for mUC treatment across different traditional LOTs.

Ultra-high-purity propylene glycol monomethyl ether acetate (PGMEA) is required as a solvent to meet the stringent requirements of the electronic-grade semiconductor industry. Here, a comparative study of two process intensification configurations—a coupling reaction and separation in the transesterification route for PGMEA production—is presented. Reactive distillation (RD), despite being technically feasible for the transesterification reaction, poses severe challenges owing to its operational/design limitations. For instance, the homogeneous catalyst sodium methoxide is insoluble in the reaction mixture and deposited on the reactive packing surface of the RD column. An intensified configuration of the side-reactor column (SRC) configuration is analyzed as an alternative to mitigate the operational limitations of RD. An economic evaluation is conducted for both configurations, showing that the SRC configuration can match the performance of an RD column with only a 5% increase in total annual cost.

34. Cord milking versus immediate clamping in preterm infants: a randomised controlled trial

ObjectiveTo investigate whether umbilical cord milking (UCM) at birth improves systemic blood flow and short-term outcomes, as compared with immediate cord clamping (ICC).DesignRandomised clinical trial.SettingSingle tertiary care centre.PatientsInfants born to eligible women presenting in preterm labour between 24 and 31 weeks’ gestation.InterventionsUCM three times at birth or ICC.Outcome measuresPrimary outcome included systemic blood flow as represented by echo-derived superior vena cava(SVC) flow at 4–6 hours after birth. The echocardiographer and interpreter were blinded to the randomisation. Secondary outcomes included cardiac output, neonatal morbidities and mortality. Analysis was by intention to treat.ResultsA total of 73 infants were randomised (37 to UCM and 36 to ICC). Mean (SD) gestational age was 27 (2) weeks and mean (SD) birth weight was 1040 (283) g. Haemoglobin on admission was higher in the UCM than in the ICC group (16.1 vs 15.0 g/L), p=0.049 (mean difference 1.1, 95% CI 0.003 to 2.2). No statistically significant differences were found between groups in SVC flow at 4–6 hours (88.9±37.8 and 107.3±60.1 mL/kg/min), p=0.13 (mean difference −18.4, 95% CI −41.7 to 5.0 mL/kg/min) or at 10–12 hours of age (102.5±41.8 and 90.6±28.4 mL/kg/min), p=0.17 (mean difference 12.0, 95% CI −4.7 to 28.7 mL/kg/min), cardiac output or neonatal morbidities.ConclusionsCord milking was not shown to improve functional cardiac outcomes, neonatal morbidity or mortality. More research is needed before routine cord milking can be recommended for very preterm infants.Trial registrationNCT01487187.

35. Tapping the energy and exergy benefits of channeling liquid air energy system in the hydrogen liquefaction process

- Abstract
Hydrogen's low volumetric energy density challenges its role as an energy vector, especially from bulk storage and transportation viewpoint. Similar to natural gas, hydrogen liquefaction is considered the solution and way forward. The energy-intensive process typically uses liquid nitrogen as a refrigerant in the precooling cycle; however, alternative candidates have also been studied. Liquid air, which has already drawn attention as a standalone cryogenic energy-storage system, can also be a potential candidate. The discharge half-cycle of a liquid-air energy storage system is integrated as the refrigerant stream in the precooling section of the hydrogen liquefaction process. The studied scenario is part of a larger integral scheme involving multiple processes. Aspen HYSYS® v12.1 was used for the design and performance analysis of this unique concept. A process-knowledge-inspired approach was employed to optimize the design variables and realize the maximum potential benefits of the proposed integration. As a result, the total refrigerant requirement was reduced by approximately 46 % compared with the base process. Furthermore, the flexibility brought about by using equilibrium reactors helped optimize the cycle temperatures and pressures. Consequently, 42.1 % (7.88 kWh/kgLH2) less energy was consumed in the liquefaction process. The high exergy efficiency of 84.29 % indicated excellent integration and optimization. Cryogenic heat exchangers were identified as the primary sources of exergy destruction, and the coefficient of performance of the integrated process was 0.155. The concept presented and the approach adopted in this study are expected to have far-reaching outcomes and benefit the hydrogen energy network and hydrogen economy perspectives.

37. Genistein Induces Alterations of Epigenetic Modulatory Signatures in Human Cervical Cancer Cells

Introduction: Epidemiological studies indicate that diet rich in fruits and vegetables is associated with decreased cancer risk thereby indicating that dietary polyphenols can be potential chemo-preventive agents. The reversible nature of epigenetic modifications makes them a favorable target for cancer prevention. Polyphenols have been shown to reverse aberrant epigenetic patterns by targeting the regulatory enzymes, DNA methyltransferases (DNMTs) and histone deacetylases (HDACs). In vitro and in silico studies of DNMTs and HDACs were planned to examine genistein’s role as a natural epigenetic modifier in human cervical cancer cells, HeLa. Methods: Expression of the tumour suppressor genes (TSGs) [MGMT, RARβ, p21, E-cadherin, DAPK1] as well the methylation status of their promoters were examined alongwith the activity levels of DNMT and HDAC enzymes after treatment with genistein. Expression of DNMTs and HDACs was also studied. In-silico studies were performed to determine the interaction of genistein with DNMTs and HDACs. Results: Genistein treatment significantly reduced the expression and enzymatic activity of both DNMTs and HDACs in a time-dependent way. Molecular modeling data suggest that genistein can interact with various members of DNMT and HDAC families and support genistein mediated inhibition of their activity. Timedependent exposure of genistein reversed the promoter region methylation of the TSGs and re-established their expression. Conclusions: In this study, we find that genistein is able to reinstate the expression of the TSGs studied by inhibiting the action of DNMTs and HDACs. This shows that genistein could be an important arsenal in the development of epigenetic based cancer therapy.

We report a case of recurrent strokes in a healthy teenager after complete closure of atrial septal defect with Gore Septal Occluder (W.L. Gore and Associates, Newark, DE) device. The disk of the device produced a friction injury to the left atrial endocardium promoting thrombus formation with subsequent embolization to the brain requiring surgical intervention.

The receptor tyrosine kinase HER3 has emerged as a therapeutic target in ovarian, prostate, breast, lung, and other cancers due to its ability to potently activate the PI3K/Akt pathway, especially via dimerization with HER2, as well as for its role in mediating drug resistance. Enhanced efficacy of HER3-targeted therapeutics would therefore benefit a wide range of patients. This study evaluated the potential of multivalent presentation, through protein engineering, to enhance the effectiveness of HER3-targeted affibodies as alternatives to monoclonal antibody therapeutics. Assessment of multivalent affibodies on a variety of cancer cell lines revealed their broad ability to improve inhibition of Neuregulin (NRG)-induced HER3 and Akt phosphorylation compared to monovalent analogues. Engineered multivalency also promoted enhanced cancer cell growth inhibition by affibodies as single agents and as part of combination therapy approaches. Mechanistic investigations revealed that engineered multivalency enhanced affibody-mediated HER3 downregulation in multiple cancer cell types. Overall, these results highlight the promise of engineered multivalency as a general strategy for enhanced efficacy of HER3-targeted therapeutics against a variety of cancers.

The aim of the present analysis is to examine the effects of slip boundary conditions and mixed convection flow of Williamson fluid over a stretching cylinder. The boundary layer partial differential equations are transformed into ordinary differential equations by using group theory transformations. The required ordinary differential equations are solved numerically by using implicit finite difference method known as Keller box method. The influence of dimensionless physical parameters on velocity and temperature profile as well as skin friction coefficient and local Nusselt number are presented graphically. Comparison has been made to the previous literature in order to check the accuracy of the method.

- Abstract
Background: Colorectal cancer (CRC) consensus molecular subtypes (CMS) have different immunological, stromal cell, and clinicopathological characteristics. Single-cell characterization of CMS subtype tumor microenvironments is required to elucidate mechanisms of tumor and stroma cell contributions to pathogenesis which may advance subtype-specific therapeutic development. We interrogate racially diverse human CRC samples and analyze multiple independent external cohorts for a total of 487,829 single cells enabling high-resolution depiction of the cellular diversity and heterogeneity within the tumor and microenvironmental cells. Results: Tumor cells recapitulate individual CMS subgroups yet exhibit significant intratumoral CMS heterogeneity. Both CMS1 microsatellite instability (MSI-H) CRCs and microsatellite stable (MSS) CRC demonstrate similar pathway activations at the tumor epithelial level. However, CD8+ cytotoxic T cell phenotype infiltration in MSI-H CRCs may explain why these tumors respond to immune checkpoint inhibitors. Cellular transcriptomic profiles in CRC exist in a tumor immune stromal continuum in contrast to discrete subtypes proposed by studies utilizing bulk transcriptomics. We note a dichotomy in tumor microenvironments across CMS subgroups exists by which patients with high cancer-associated fibroblasts (CAFs) and C1Q+TAM content exhibit poor outcomes, providing a higher level of personalization and precision than would distinct subtypes. Additionally, we discover CAF subtypes known to be associated with immunotherapy resistance. Conclusions: Distinct CAFs and C1Q+ TAMs are sufficient to explain CMS predictive ability and a simpler signature based on these cellular phenotypes could stratify CRC patient prognosis with greater precision. Therapeutically targeting specific CAF subtypes and C1Q + TAMs may promote immunotherapy responses in CRC patients.

ABSTRACTThe global dissemination and increasing incidence of carbapenem-resistant, Gram-negative organisms have resulted in acute public health concerns. Here, we present a retrospective multicenter study on molecular characterization of metallo-β-lactamase (MBL)-producing clinical Escherichia coliisolates recovered from extraintestinal infections in two hospitals in Pune, India. We screened a large sample size of 510 E. coliisolates for MBL production wherein we profiled their molecular determinants, antimicrobial resistance phenotypes, functional virulence properties, genomic features, and transmission dynamics. Approximately 8% of these isolates were MBL producers, the majority of which were of the NDM-1 (69%) type, followed by NDM-5 (19%), NDM-4 (5.5%), and NDM-7 (5.5%). MBL producers were resistant to all antibiotics tested except for colistin, fosfomycin, and chloramphenicol, which were effective to various extents. Plasmids were found to be an effective means of dissemination of NDM genes and other resistance traits. All MBL producers adhered to and invaded bladder epithelial (T24) cells and demonstrated significant serum resistance. Genomic analysis of MBL-producing E. coliisolates revealed higher resistance but a moderate virulence gene repertoire. A subset of NDM-1-positive E. coliisolates was identified as dominant sequence type 101 (ST101) while two strains belonging to ST167 and ST405 harbored NDM-5. A majority of MBL-producing E. colistrains revealed unique genotypes, suggesting that they were clonally unrelated. Overall, the coexistence of virulence and carbapenem resistance in clinical E. coliisolates is of serious concern. Moreover, the emergence of NDM-1 among the globally dominant E. coliST101 isolates warrants stringent surveillance and control measures.

Purpose – The purpose of this paper is to examine the Sisko fluid model over a stretching cylinder with heat transfer and magnetohydrodynamics. Design/methodology/approach – The boundary layer approach is employed to simplify the governing equations. Suitable similarity transformations are used to transform the governing partial differential equations into ordinary differential equations. In order to solve this system of ordinary differential equations numerically, shooting method in conjunction with Runge-Kutta-Fehlberg method is used. Findings – The effects of physical parameters involved in velocity and temperature profiles are shown through graphs. It is observed that Sisko fluid parameter and curvature parameter enhances fluid velocity while motion of fluid is retarded by increasing magnetic field strength. Additionally temperature of fluid raise with curvature parameter while it fall down for larger values of Prandtl number. Skin friction coefficient and Nusselt number are computed and presented in graphs and tables for further analysis. It can be seen that curvature parameter increases both skin friction and Nusselt number while magnetic field and Prandtl number decayed skin friction and Nusselt number, respectively. Also Sisko parameter enlarges skin friction coefficient. The accuracy of solution is verified by comparing it with existing literature. ABSTRACTEscherichia colisequence type 131 (ST131) is a pandemic clone associated with multidrug-resistant, extraintestinal infections, attributable to the presence of the CTX-M-15 extended-spectrum β-lactamase gene and mutations entailing fluoroquinolone resistance. Studies on subclones within E. coliST131 are critically required for targeting and implementation of successful control efforts. Our study comprehensively analyzed the genomic and functional attributes of the H30-Rx subclonal strains NA097 and NA114, belonging to the ST131 lineage. We carried out whole-genome sequencing, comparative analysis, phenotypic virulence assays, and profiling of the antibacterial responses of THP1 cells infected with these subclones. Phylogenomic analysis suggested that the strains were clonal in nature and confined entirely to a single clade. Comparative genomic analysis revealed that the virulence and resistance repertoires were comparable among the H30-Rx ST131 strains except for the commensal ST131 strain SE15. Similarly, seven phage-specific regions were found to be strongly associated with the H30-Rx strains but were largely absent in the genome of SE15. Phenotypic analysis confirmed the virulence and resistance similarities between the two strains. However, NA097 was found to be more robust than NA114 in terms of virulence gene carriage (draoperon), invasion ability (P< 0.05), and antimicrobial resistance (streptomycin resistance). RT2gene expression profiling revealed generic upregulation of key proinflammatory responses in THP1 cells, irrespective of ST131 lineage status. In conclusion, our study provides comprehensive, genome-inferred insights into the biology and immunological properties of ST131 strains and suggests clonal diversification of genomic and phenotypic features within the H30-Rx subclone of E. coliST131.

In the era of personalized medicine, oncologists seek to tailor treatments for their patients based on indicators of differential response to treatment. We first provide a conceptual explanation of heterogeneity of treatment effect using prostate cancer as a case study and review studies that test whether Gleason score has a role in modifying the effectiveness of androgen deprivation therapy in men newly diagnosed with metastatic prostate cancer.
