12. A health economic evaluation of the unnecessary ordering of coagulation studies and blood grouping prior to elective surgery

Abstract

Unnecessary pathology tests add significant financial burden to health care expenditures while offering limited benefit to patients. Current guidelines do not support indiscriminate ordering of preoperative coagulation studies and ABO blood typing. We sought to estimate the incidence and financial cost of the indiscriminate ordering of these investigations in our institution. A single centre retrospective electronic chart review was performed in patients who underwent preoperative coagulation studies or ABO blood typing prior to elective surgery over a ten-month period. Using local evidence-based guidelines, only 9% of coagulation studies and 75% of ABO blood typing studies were indicated. The estimated cost of unnecessary tests in our cohort of 2688 patients was approximately AUD35,500. Interventions such as directed education and clinician feedback should be considered to combat the high incidence of indiscriminate ordering of preoperative coagulation studies and ABO blood typing.

Abstract

Low levels of testosterone or dihydrotestosterone (DHT) appear to be associated with AIDS wasting syndrome. Wasting is the loss of 10% or more of body weight, often accompanied by fatigue, diarrhea, loss of muscle mass, and depression. Researchers analyzed the sex hormone levels of 24 men with AIDS wasting syndrome. Forty-two percent had low DHT levels, and 12% had low testosterone and DHT levels. Men with depressed DHT, a precursor of testosterone, lost an average of 9 kg, compared to 6 kg among men with normal hormone levels. DHT therapy may reverse some wasting symptoms.

Bonser Abstract: Objectives: To determine the predictive value of quantitative evaluation of myocardial viability on changes in left ventricular function, exercise capacity, and quality of life after coronary artery bypass grafting in patients with ischemic heart failure (congestive heart failure, New York Heart Association class [greater than or equal to] III) with and without angina. Methods: Thirty-five patients, 14 with congestive heart failure and angina (CHF-angina) and 21 with congestive heart failure without angina (CHF-no angina) were studied at baseline and 6 months after coronary bypass grafting. Left ventricular function was evaluated with transthoracic echocardiography and radionuclide ventriculography. Myocardial viability was assessed with [.sup.18F]-2-fluoro-2-deoxy-d-glucose using positron emission tomography. Peak aerobic capacity (peak oxygen consumption) and anaerobic threshold were assessed with treadmill exercise test and quality of life with a questionnaire. Results: A total of 286 of 336 dysfunctional left ventricular segments were viable. There were two perioperative deaths (5.7%) and three late deaths. Left ventricular ejection fraction increased from 23% [+ or -] 7% to 32% [+ or -] 9% (p < 0.0001), and a linear correlation was found between the number of viable segments and the changes in ejection fraction (r = 0.65; p = 0.0001). Receiver operating characteristics curve identified eight viable segments as the best predictor for increase of ejection fraction more than 5 percentage points. Peak oxygen consumption increased from 15 [+ or -] 4 to 22 [+ or -] 5 ml/kg per minute (p < 0.0001). Preoperatively, anaerobic threshold was identified in one patient from the CHF-angina group and in all from the CHF-no angina group and increased from 13 [+ or -] 4 to 19 [+ or -] 4 ml/kg per minute (p < 0.0001). Quality of life scores improved significantly in both groups. No correlation was found between the amount of viable dysfunctional myocardium and changes in exercise capacity or quality of life. Conclusions: In patients with postischemic congestive heart failure the amount of viable myocardium dictates the degree of improvement in left ventricular function after revascularization. Abstract

The Lancet's editor-in-chief, Richard Horton, offers a brief op-ed about the culture of the medical-education system and its negative impact on the mental health and welfare of its graduates year after year. He seems to have awakened to the problems some twenty years ago, and writes that they have only grown more severe as the years roll on. He argues that the education environment is failing the students because it continues to prop up failing healthcare systems feeding on the new graduates. Academic deans, he writes, seem oblivious or indifferent to the problem. He reports that between 60% and 99% of graduates are already feeling burnt out, exhausted, depressed, and distressed when they enter their new careers! Then, in their new jobs they feel as if they are a minor cog in a very large machine. They feel minimally effective and highly insignificant. Horton calls it an "epidemic of injury" foisted upon medicine's most precious opportunity, "each new generation of medical students," and he ends the article with the challenge: "Who will act?".
