AimsThe aim of this study was to investigate the association between the type of operation used to treat a trochanteric fracture of the hip and 30-day mortality.Patients and MethodsData on 82 990 patients from the National Hip Fracture Database were analyzed using generalized linear models with incremental case-mix adjustment for patient, non-surgical and surgical characteristics, and socioeconomic factors.ResultsThe use of short and long intramedullary nails was associated with an increase in 30-day mortality (adjusted odds ratio (OR) 1.125, 95% confidence interval (CI) 1.040 to 1.218; p = 0.004) compared with the use of sliding hip screws (12.5% increase). If this were causative, it would represent 98 excess deaths over the four-year period of the study and one excess death would be caused by treating 112 patients with an intramedullary nail rather than a sliding hip screw.ConclusionThere is a 12.5% increase in the risk of 30-day mortality associated with the use of an intramedullary nail compared with a sliding hip screw in the treatment of a trochanteric fractures of the hip.

Bis(1‘-organylthiolatoferrocenylene)disulfanes, RS-fc-SS-fc-SR type I ligands (R = Me, ^tBu), have been synthesized by cleavage of 1,2,3-trithia[3]ferrocenophane with organolithium reagents, RLi. The disulfanes have been successfully cleaved using lithium triethylborohydride and then reacted with dibromo-organyl species (Br-X-Br) to produce a novel series of sterically hindered ligands, RS-fc-S-X-S-fc-SR type II ligands {(1) R = ^tBu; X = CH2CH2CH2, CH2C5H3NCH2, CH2C10H6CH2; (2) R = Me; X = CH2CH2CH2, CH2C5H3NCH2, CH2C10H6CH2}. Both type I and type II ligands have been structurally characterized, and their coordination chemistry has been investigated by reaction with the late transition metal reagents Cu(MeCN)4PF6 and Pd(cod)Cl2. The resulting Cu(I) and Pd(II) complexes have been fully characterized, showing some unusual reactivity and a preponderance for a bis-bidentate chelating mode.

Abstract:  The aim of this study was to examine mechanisms of growth in different tissue compartments of the ventricular myocardium of prenatal human hearts. To this end, stereological methods were applied in order to estimate tissue volumes and total numbers of myocyte, connective tissue and endothelial nuclei in hearts collected after death at between 16 and 35 weeks of gestation. Volumes of tissue compartments were obtained after multiplying volume densities (estimated by test-point counting) by ventricular volumes (estimated from ventricular mass and tissue density). Absolute numbers of nuclei were calculated in similar fashion from corresponding nuclear packing densities (estimated using physical disectors). The volumes of all three tissue compartments increased linearly over the period of gestation examined, and in each case, the increase in tissue volume appeared to be due entirely to proliferation. Numbers of all three types of nuclei increased linearly whilst tissue volumes per nucleus remained constant. The net rate of production of myocyte nuclei was 35×107 per week (2.1 million nuclei per hour). The net rate of production of connective tissue nuclei was 12×107 per week (0.7 million nuclei per hour) and that for endothelial cell nuclei was 5.1×107 per week (0.3 million nuclei per hour). Predictions are made about the postnatal ages at which adult ratios of different nuclear types might be attained.
