- Abstract
AbstractWe present a rare case of a 40-year-old nulliparous lady, with no past medical or surgical history, who was diagnosed with metastatic squamous cell carcinoma of the right ovary that originated from a mature cystic ovarian teratoma. Our patient underwent debulking total abdominal hysterectomy, bilateral salpingo-oophorectomy and partial omentectomy followed by postoperative carboplatin and paclitaxel chemotherapy. Rapid disease progression ensued, complicated by severe parathyroid hormone-related protein-induced hypercalcaemia resistant to medical therapy. The patient was treated in a palliative manner and died five months after her diagnosis.

3. Mental health attitudes in Malta: a cross-sectional survey exploring the knowledge and perceptions of general practitioner trainees

Aims and methodThis study aimed to assess current levels of knowledge, opinions and attitudes regarding mental health among the local cohort of general practitioner trainees (n= 45) working in Malta. A questionnaire adapted from the Mental Health Literacy Scale was used. Data were analysed using one-way analysis of variance and Pearson correlation tests.ResultsAll participants had scores equal to or more than the mean score in their knowledge and confidence assessments; 51% of the participants achieved the maximum score for a very positive attitude towards mental health, with such scores found particularly among female trainees. Increased levels of knowledge are associated with a more positive attitude, which can in turn lead to greater acceptance and reduce stigma.Clinical implicationsKnowledge is a powerful tool for reducing stigma and improving the doctor–patient relationship, indicating that regular training initiatives are necessary to equip budding general practitioner specialists with the necessary skills and confidence.

5. Effect of glucagon like peptide-1 receptor agonist exenatide, used as an intracranial pressure lowering agent, on cognition in Idiopathic Intracranial Hypertension

- Abstract
Background: Cognitive function can be affected in conditions with raised intracranial pressure (ICP) such as idiopathic intracranial hypertension (IIH). Drugs used off label to treat raised ICP also have cognitive side effects, underscoring the unmet need for effective therapeutics which reduce ICP without worsening cognition. The Glucagon Like Peptide-1 (GLP-1) receptor agonist, exenatide, has been shown to significantly reduce ICP in IIH, therefore this study aimed to determine the effects of exenatide on cognition in IIH. Methods: This was an exploratory study of the IIH:Pressure trial (ISTCRN 12678718). Women with IIH and telemetric ICP monitors (n = 15) were treated with exenatide (n = 7) or placebo (n = 8) for 12 weeks. Cognitive function was tested using the National Institute of Health Toolbox Cognitive Battery at baseline and 12 weeks. Results: Cognitive performance was impaired in fluid intelligence ((T-score of 50 = population mean), mean (SD) 37.20 (9.87)), attention (33.93 (7.15)) and executive function (38.07 (14.61)). After 12-weeks there was no evidence that exenatide compromised cognition (no differences between exenatide and placebo). Cognition improved in exenatide treated patients in fluid intelligence (baseline 38.4 (8.2), 12 weeks 52.9 (6.6), p= 0.0005), processing speed (baseline 43.7 (9.4), 12 weeks 58.4 (10.4), p= 0.0058) and episodic memory (baseline 49.4 (5.3), 12 weeks 62.1 (13.2), p= 0.0315). Conclusions: In patients with raised ICP due to IIH, exenatide, a drug emerging as an ICP lowering agent, does not adversely impact cognition. This is encouraging and has potential to be relevant when considering prescribing choices to lower ICP.

- Abstract
Globally, more than 1 billion people with disabilities are disproportionately and differentially at risk from the climate crisis. Yet there is a notable absence of climate policy, programming, and research at the intersection of disability and climate change. Advancing climate justice urgently requires accelerated disability-inclusive climate action. We present pivotal research recommendations and guidance to advance disability-inclusive climate research and responses identified by a global interdisciplinary group of experts in disability, climate change, sustainable development, public health, environmental justice, humanitarianism, gender, Indigeneity, mental health, law, and planetary health. Climate-resilient development is a framework for enabling universal sustainable development. Advancing inclusive climate-resilient development requires a disability human rights approach that deepens understanding of how societal choices and actions—characterised by meaningful participation, inclusion, knowledge diversity in decision making, and co-design by and with people with disabilities and their representative organisations—build collective climate resilience benefiting disability communities and society at large while advancing planetary health.

Aim: Diabesity, the co-occurrence of diabetes and obesity, presents a global health crisis. Understanding its prevalence, associated risk factors, and mortality outcomes is crucial for effective public health interventions. This study aims to investigate the prevalence of diabesity and diabetes, assess associated risk factors, and analyze mortality outcomes over a 7-year period in the diabetogenic country of Malta. Subject and methods: A nationwide health examination survey (2014–16) was conducted involving 3947 adults aged 18–70 years. Sociodemographic data, anthropometric measurements, and blood samples were collected. Relationships between different adiposity indices were explored. Mortality data was obtained by cross-referencing with the national mortality register. Statistical analyses included chi-square tests, logistic regression, and Cox proportional hazard models. Results: Prevalence of obesity was 34.08%, diabetes 10.31%, and diabesity 5.78%. Sociodemographic characteristics were similar across all three cohorts. Multivariable regression identified increasing age (OR 1.10 CI95% 1.07–1.12; p≤ 0.001), male gender (OR 0.53 CI95% 0.30–0.93; p= 0.03), and low educational level (OR 2.19 CI95% 1.39–3.45; p= 0.001) as significant predictors of diabesity. Only diabetes showed a significant increase in mortality risk (HR 3.15 CI95% 1.31–7.62; p= 0.02) after adjustment, with gender (HR 3.17 CI95% 1.20–8.37) and body adiposity index (HR 1.08 CI95% 1.01–1.16) also significant (p≤ 0.05). Conclusion: Diabesity represents a substantial public health challenge in Malta, with implications for mortality outcomes. Targeted interventions addressing sociodemographic disparities and promoting healthy lifestyles are essential to mitigate its impact. The findings underscore the need for comprehensive healthcare strategies and policy initiatives to combat diabesity and reduce associated mortality rates.

8. Are the European Commission’s Business and Consumer Survey Results Coincident Indicators for Maltese Economic Activity?

- Abstract
The European Commission’s business and consumer surveys are the most extensive regular surveys of Maltese firms and households. The Economic Sentiment Indicator (ESI) for Malta is closely correlated with real GDP growth, particularly when one focuses on the first vintage of national accounts data. This suggests that the opinions expressed by economic agents are partly driven by news prevailing at the time. The sectoral confidence indicators that underpin the ESI are quite highly correlated, with construction sentiment being the most synchronised with sentiment in other sectors. In general, sectoral expectations on future activity appear to be less strongly correlated to changes in national accounts sectoral value added than survey responses to planned employment changes are to observed changes in sectoral employment. Maltese household economic expectations appear to be mostly reflective of current conditions and could be useful to forecast variables that are issued with some time lag, like real GDP.

- Abstract
BackgroundThe practical application of ‘virtual’ (computed) fractional flow reserve (vFFR) based on invasive coronary angiogram (ICA) images is unknown. The objective of this cohort study was to investigate the potential of vFFR to guide the management of unselected patients undergoing ICA. The hypothesis was that it changes management in >10% of cases.MethodsvFFR was computed using the Sheffield VIRTUheart system, at five hospitals in the North of England, on ‘all-comers’ undergoing ICA for non-ST-elevation myocardial infarction acute coronary syndrome (ACS) and chronic coronary syndrome (CCS). The cardiologists’ management plan (optimal medical therapy, percutaneous coronary intervention (PCI), coronary artery bypass surgery or ‘more information required’) and confidence level were recorded after ICA, and again after vFFR disclosure.Results517 patients were screened; 320 were recruited: 208 with ACS and 112 with CCS. The median vFFR was 0.82 (0.70–0.91). vFFR disclosure did not change the mean number of significantly stenosed vessels per patient (1.16 (±0.96) visually and 1.18 (±0.92) with vFFR (p=0.79)). A change in intended management following vFFR disclosure occurred in 22% of all patients; in the ACS cohort, there was a 62% increase in the number planned for medical management, and in the CCS cohort, there was a 31% increase in the number planned for PCI. In all patients, vFFR disclosure increased physician confidence from 8 of 10 (7.33–9) to 9 of 10 (8–10) (p<0.001).ConclusionThe addition of vFFR to ICA changed intended management strategy in 22% of patients, provided a detailed and specific ‘all-in-one’ anatomical and physiological assessment of coronary artery disease, and was accompanied by augmentation of the operator’s confidence in the treatment strategy.

- Abstract
High-resolution X-ray spectroscopy in the sub-nanosecond to femtosecond time range requires ultrashort X-ray pulses and a spectral X-ray flux considerably larger than that presently available. X-ray free-electron laser (XFEL) radiation from hard X-ray self-seeding (HXRSS) setups has been demonstrated in the past and offers the necessary peak flux properties. So far, these systems could not provide high repetition rates enabling a high average flux. We report the results for a cascaded HXRSS system installed at the European XFEL, currently the only operating high-repetition-rate hard X-ray XFEL facility worldwide. A high repetition rate, combined with HXRSS, allows the generation of millijoule-level pulses in the photon energy range of 6–14 keV with a bandwidth of around 1 eV (corresponding to about 1 mJ eV–1peak spectral density) at the rate of ten trains per second, each train including hundreds of pulses arriving at a megahertz repetition rate. At 2.25 MHz repetition rate and photon energies in the 6–7 keV range, we observed and characterized the heat-load effects on the HXRSS crystals, substantially altering the spectra of subsequent X-ray pulses. We demonstrated that our cascaded self-seeding scheme reduces this detrimental effect to below the detection level. This opens up exciting new possibilities in a wide range of scientific fields employing ultrafast X-ray spectroscopy, scattering and imaging techniques.

- Abstract
Introduction: We assessed best available data on access and delivery of acute stroke unit (SU) care, intravenous thrombolysis (IVT) and endovascular treatment (EVT) in the European region in 2019 and 2020.Patients and methods: We compared national data per number of inhabitants and per 100 annual incident first-ever ischaemic strokes (AIIS) in 46 countries. Population estimates and ischaemic stroke incidence were based on United Nations data and the Global Burden of Disease Report 2019, respectively.Results: The estimated mean number of acute SUs in 2019 was 3.68 (95% CI: 2.90–4.45) per one million inhabitants (MIH) with 7/44 countries having less than one SU per one MIH. The estimated mean annual number of IVTs was 21.03 (95% CI: 15.63–26.43) per 100,000 and 17.14% (95% CI: 12.98–21.30) of the AIIS in 2019, with highest country rates at 79.19 and 52.66%, respectively, and 15 countries delivering less than 10 IVT per 100,000. The estimated mean annual number of EVTs in 2019 was 7.87 (95% CI: 5.96–9.77) per 100,000 and 6.91% (95% CI: 5.15–8.67) of AIIS, with 11 countries delivering less than 1.5 EVT per 100,000. Rates of SUs, IVT and EVT were stable in 2020. There was an increase in mean rates of SUs, IVT and EVT compared to similar data from 2016.Conclusion: Although there was an increase in reperfusion treatment rates in many countries between 2016 and 2019, this was halted in 2020. There are persistent major inequalities in acute stroke treatment in the European region. Tailored strategies directed to the most vulnerable regions should be prioritised.

Ethereum is a distributed blockchain platform, serving as an ecosystem for smart contracts: full-fledged intercommunicating programs that capture the transaction logic of an account. A gas limit caps the execution of an Ethereum smart contract: instructions, when executed, consume gas, and the execution proceeds as long as gas is available. Gas-focused vulnerabilities permit an attacker to force key contract functionality to run out of gas-effectively performing a permanent denial-of-service attack on the contract. Such vulnerabilities are among the hardest for programmers to protect against, as out-of-gas behavior may be uncommon in nonattack scenarios and reasoning about these vulnerabilities is nontrivial. In this paper, we identify gas-focused vulnerabilities and present MadMax: a static program analysis technique that automatically detects gas-focused vulnerabilities with very high confidence. MadMax combines a smart contract decompiler and semantic queries in Datalog. Our approach captures high-level program modeling concepts (such as "dynamic data structure storage" and "safely resumable loops") and delivers high precision and scalability. MadMax analyzes the entirety of smart contracts in the Ethereum blockchain in just 10 hours and flags vulnerabilities in contracts with a monetary value in billions of dollars. Manual inspection of a sample of flagged contracts shows that 81% of the sampled warnings do indeed lead to vulnerabilities. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Idiopathic intracranial hypertension (IIH) is characterized by increased intracranial pressure occurring predominantly in women with obesity. The pathogenesis is not understood. We have applied untargeted metabolomic analysis using ultrahigh-performance liquid chromatography–mass spectrometry to characterize the cerebrospinal fluid (CSF) and serum in IIH compared to control subjects. Methods and findings: Samples were collected from IIH patients (n= 66) with active disease at baseline and again at 12 months following therapeutic weight loss. Control samples were collected from gender- and weight-matched healthy controls (n= 20). We identified annotated metabolites in CSF, formylpyruvate and maleylpyruvate/fumarylpyruvate, which were present at lower concentrations in IIH compared to control subjects and returned to values observed in controls following weight loss. These metabolites showed the opposite trend in serum at baseline. Multiple amino acid metabolic pathways and lipid classes were perturbed in serum and CSF in IIH alone. Serum lipid metabolite pathways were significantly increased in IIH. Conclusions: We observed a number of differential metabolic pathways related to amino acid, lipid, and acylpyruvate metabolism, in IIH compared to controls. These pathways were associated with clinical measures and normalized with disease remission. Perturbation of these metabolic pathways provides initial understanding of disease dysregulation in IIH.

- Abstract
Resonant oscillators with stable frequencies and large quality factors help us to keep track of time with high precision. Examples range from quartz crystal oscillators in wristwatches to atomic oscillators in atomic clocks, which are, at present, our most precise time measurement devices1. The search for more stable and convenient reference oscillators is continuing2–6. Nuclear oscillators are better than atomic oscillators because of their naturally higher quality factors and higher resilience against external perturbations7–9. One of the most promising cases is an ultra-narrow nuclear resonance transition in 45Sc between the ground state and the 12.4-keV isomeric state with a long lifetime of 0.47 s (ref. 10). The scientific potential of 45Sc was realized long ago, but applications require 45Sc resonant excitation, which in turn requires accelerator-driven, high-brightness X-ray sources11that have become available only recently. Here we report on resonant X-ray excitation of the 45Sc isomeric state by irradiation of Sc-metal foil with 12.4-keV photon pulses from a state-of-the-art X-ray free-electron laser and subsequent detection of nuclear decay products. Simultaneously, the transition energy was determined as 12,389.59+0.12syst±0.15stateVwith an uncertainty that is two orders of magnitude smaller than the previously known values. These advancements enable the application of this isomer in extreme metrology, nuclear clock technology, ultra-high-precision spectroscopy and similar applications.

22. Simultaneous tasks as a contributory factor to maritime accidents: A socio-cultural approach

- Abstract
Analyses of accident investigation reports show that attempting to simultaneously perform tasks onboard ships to manage workload has led to many accidents at sea. These accident reports point out that seafarers fail to follow procedures as expected by shipping companies when conducting tasks simultaneously. Only a handful of studies that examined such discrepancies between work as imagined by shipping companies and work as actually done by seafarers exist. This study aims to examine why seafarers engage in simultaneous tasks, which may lead to accidents. In an exploratory qualitative study, face-to-face interviews were conducted with selective and targeted seafarers previously involved in maritime accidents. The collected data were analysed using interpretive and descriptive qualitative methods with a socio-cultural approach. The findings of this study show that the substantial regulatory changes over the years in the maritime industry have resulted in increased occupational workload for seafarers. Some of these regulatory barriers designed to improve safety at sea prompted seafarers to engage in simultaneous tasks to perform excessive paperwork and compensate for insufficient crewing. The research also revealed how a poor speak-up culture onboard might encourage simultaneous tasks leading to accidents at sea. The analysis shows that reasons for engaging in simultaneous tasks at sea are varied and complex. Therefore, complex intervention efforts to discourage seafarers from engaging in simultaneous tasks is needed to mitigate accidents at sea.

23. Incentivization matters: a meta-perspective on dictator games

- Abstract
Dictator game experiments come in three flavors: plain vanilla with strictly dichotomous separation of dictator and recipient roles, an interactive alternative whereby every subject acts in both roles, and a variant thereof with role uncertainty. We add information regarding which of these three protocols was used to data from the leading meta-study by Engel (Exp Econ 14(4):583–610, 2011) and investigate how these variations matter. Our meta-regressions suggest that interactive protocols with role duality compared with standard protocols, in addition to being relevant as a control for other effects, render subjects’ giving less generous but more efficiency-oriented. Our results help organize existing findings in the field and indicate sources of confounds.

28. A comparative assessment of attitudes and hesitancy for influenza vis-à-vis COVID-19 vaccination among healthcare students and professionals in Malta

Introduction: Vaccine hesitancy has been a public health threat for decades, even among healthcare professionals. The study explored vaccination attitudes and hesitancy with regard to seasonal influenza and the anticipated Covid-19 vaccines among healthcare professionals and students. Methods: Anonymous questionnaires were distributed through electronic mail to healthcare professionals working in Malta’s Health Division, at University of Malta, and in community pharmacies, and to members of Malta College of Family Doctors. Results: The response rate was 18.60% (n= 1802). The percentages of those who took the influenza vaccine last year (2019) and who will take it this year (2020) increased across professionals, highest for family doctors and lowest for trainees in family medicine. The proportion of those likelier to take the Covid-19 vaccine was directly related to likelihood of taking influenza vaccination. Both family doctor trainees and nurses showed the highest Covid-19 vaccine hesitancy. Concerns pertaining to COVID-19 vaccination were mostly related to insufficient knowledge, although pharmacists (community, academic and students) were concerned about the rapid approval of the vaccine. Conclusion: An increase in influenza vaccine uptake is expected in view of the current Covid-19 pandemic. Of concern is the hesitancy towards Covid-19 vaccine particular within healthcare professionals, a cohort expected to be insightful and potential role models for the community. The associated hesitancy for both COVID-19 and influenza vaccination infers an intrinsic degree of vaccine hesitancy, which brings forward the need for healthcare professionals to be engaged in education, vaccination policies and administration in order to ensure compliance.

Practice-led research can sometimes develop discourses that are not always consistent with the grammatical logics of academic language. However, practice-led research can reproduce and/or explain what happens when an individual encounters things and events in the world. This dynamic may thus open up innovative ways of codifying and authenticating knowledge gained from the performance of everyday life that might otherwise remain inexplicable (or seem irrelevant or disconnected within the existing structures and grammars of scientific discourse). Such practice-led research can lead to new forms of expression in order to understand the individual's subjective experience. Thus, while practice-led research may challenge (and sometimes upturn) established methods of logic and rational argument, it also enables a researcher to develop explanations of events and encounters in the worm that may otherwise not be accessible to them. Creative work can also make the impact of scientific research available to those who may not have a thorough working knowledge of scientific and academic discourses in the relevant discipline. The paper discusses these issues while focusing on a creative project/website developed through practice-led research.

In this paper, we present stylized facts about exchange rate fluctuations and intervention behavior in the Exchange Rate Mechanism I (ERM I), in particular in light of the recent literature on multilateral target zone models. We estimate bilateral exchange rate distributions of the maximum spot rate deviations of six ERM I currencies, explicitly taking the multilateral setting of ERM I into account. In a further analysis, we estimate short-term reaction functions for the central banks of Belgium, Denmark, France, Ireland, Portugal, and Spain by applying a Tobit analysis. The period under review is from August 1993 to April 1998. We use daily exchange rate and intervention data. The exchange rate position in the band (deviation of the deutschemark (DEM) spot rates from the DEM central parity) significantly induces intervention activity. This paper reports the preliminary validation of the satisfaction and perceived usefulness questionnaires for evaluating smoking cessation interventions among individuals with diabetes.

45. Ecological Characterization of Mosquitoes (Diptera: Culicidae) at the Southern Coast of Mar Chiquita Lake, Argentina

- Abstract
In the southern coast of Mar Chiquita Lake, central Argentina, mosquitoes affect public health and community livelihood, since they transmit pathogens to human beings causing diseases such as malaria, filariasis, encephalitis, yellow fever, and dengue, among others, and have a negative effect on cattle farming as well. To characterize the structure of the mosquito assemblage of the region, we determined the species composition and diversity, the temporal distribution of different species, and the patterns of species richness, abundance, and diversity across seasons. We collected adult mosquitoes over a two-year period (October 2004–September 2006) by means of CDC light traps baited with CO2from 18:00 to 08:00 h during the warm season (October–April) and from 12:00 h to 18:00 h in the cold season (May–September). A total of 71,501 individuals from 30 species were collected, with CulexLinnaeus and AedesMeigen genera representing more than 98% of collected specimens (61.5% and 37.3%, respectively). The higher values of richness and abundance of Culicidae were registered in warm seasons compared to cold seasons. Chao1estimates suggested that more than 90% of the species were detected in all seasons. Mosquito abundance distribution fit the logarithmic series and log-normal models. Aedes albifasciatus(Macquart), Ae. scapularis(Rondani), Culex interforDyar, Cx. saltanensisDyar, and Cx. dolosus(Lynch Arribálzaga), vectors incriminated in arbovirus transmission, were abundant year-round, with Cx. saltanensisand Cx. dolosusmost prevalent in cold seasons. Further studies are needed to assess the role of these species in arbovirus transmission in this region of central Argentina.

46. Development and Pilot of a Process for Regularly Sharing Summary Patient Safety Data

- Abstract
Objective: Dissemination of patient safety data is key to understanding safety events and improving the quality of patient care. However, there is limited guidance on how psychiatry residency programs can create a supportive environment in which to disclose and discuss such information. The authors developed and piloted a resident-led Patient Safety Presentation process at an Accreditation Council for Graduate Medical Education–accredited psychiatry residency program, sharing patient safety data while enhancing residents’ education and engagement in patient safety. Methods: From September 2020 through February 2021, the authors convened a workgroup of psychiatry residents and faculty members to devise and conduct the presentation process. The process consisted of an introductory hour-long training of residents in patient safety concepts, followed a week later by the presentation by two psychiatry residents. The authors evaluated the pilot presentation process using pre- and post-presentation resident surveys. Results: The introductory training and the Patient Safety Presentation were included into the didactic schedules of all 32 program residents. Twenty (62.5%) and 17 (53.1%) residents completed the pre- and post-presentation surveys, respectively. Improvements were seen in residents’ knowledge regarding the medical center’s patient safety practices and perspectives on patient safety practices. On the post-presentation survey, all 17 residents reported overall satisfaction with the presentation. Conclusions: The piloted Patient Safety Presentation process increased psychiatry residents’ knowledge of and engagement in patient safety. The development and pilot of the presentation process serve as an illustrative case study for other residency programs that are aspiring to grow this aspect of their curriculum.
